Journal of stroke and cerebrovascular diseases : the official journal of National Stroke Association2024eng
OBJECTIVES: Symptomatic carotid web is an increasingly recognized cause of acute ischemic stroke with a high risk of recurrent ischemic events despite aggressive medical interventions. Surgical interventions including transfemoral carotid artery stenting (TFCAS) and carotid endarterectomy have been described to reduce this risk, but transcarotid arterial revascularization (TCAR) has not been evaluated for this purpose.
MATERIALS AND METHODS: Patients with cerebral ischemia from carotid web underwent TCAR with flow reversal. Patients were monitored for periprocedural complications and assessed

Carotid web is a rare intraluminal parietal protrusion in the carotid artery. This vascular anomaly mostly occurs at the bifurcation causing turbulent flow, and is responsible of thrombi formation associated with embolic ischemic events. We report the case of a 35-year-old woman, with no medical history, who presented a recurrent middle cerebral artery (MCA) occlusion within twelve hours caused by a carotid bulb web. Although considered as a rare entity, carotid web is associated with the risk of recurrent strokes in the MCA territory and especially in the absence of the typically recognized r

Understanding the pathogenesis and the typical imaging features of carotid artery web, and accurately diagnosing the carotid artery web will help to implement targeted intervention for cryptogenic stroke and reduce the recurrence of stroke events. Carotid web (CW) can be defined as an endoluminal shelf-like projection often noted at the origin of the internal carotid artery just beyond the bifurcation. It is recognized as a possible cause of ischemic stroke in young adults. We describe here a case of ischemic stroke caused by a CW in a healthy man. Detection of this vascular abnormality requir

IMPORTANCE: A succession of research has explored the linkage between carotid webs (CaWs) and ischemic strokes. Imaging and pathologic analysis have defined CaW as an intimal variant of fibromuscular dysplasia, which appears as a shelflike lesion on the posterior aspect of the carotid bulb, more specifically at the origin of internal carotid artery. Reported findings of carotid webs in young patients with recurrent ischemic strokes without an otherwise determined cause have raised questions about the mechanism, natural history, and need for intervention. This review addresses the current under

BACKGROUND: Whether carotid artery web can be considered as a potential source of arterial thromboembolism in ischemic stroke remains uncertain.
AIMS: In a large sample of individuals with large intracranial artery occlusion, we compared the prevalence of carotid artery webs between patients with and without embolic stroke of undetermined source.
METHODS: In a single-center study of consecutive patients with anterior circulation ischemic stroke referred for mechanical thrombectomy, the presence of carotid artery web was systematically assessed by two independent readers. Thereafter, its prev

Here, we describe a case of a 67-year-old man who was transferred to our hospital with complaints of sudden upper right limb weakness and ataxia. Scattered acute cerebral infarction was found in the watershed zone between the left anterior cerebral artery and the middle cerebral artery territories. A shelf-like structure at the origin of the left carotid artery and a vulnerable plaque distal to the lesion was found. Symptomatic atherosclerotic plaque with concomitant carotid web was diagnosed. Carotid endarterectomy resulted in good revascularization. Hemodynamic changes associated with vascul

BACKGROUND: A carotid web (CaW) is a shelf-like lesion in the posterior aspect of the internal carotid bulb and represents an intimal variant of fibromuscular dysplasia. CaW has been associated with recurrent strokes and conventionally treated with surgical excision. We report a multicenter experience of stenting in patients with symptomatic CaWs.
METHODS: Retrospective review of consecutive patients admitted to 5 comprehensive stroke centers who were identified to have a symptomatic CaW and treated with carotid stenting. A symptomatic CaW was defined by the presence of a shelf-like/linear, s

Carotid web, a special vascular lesion, has been increasingly recognized as a significant etiology of ischemic stroke. Unlike traditional atherosclerosis, carotid web is a linear intraluminal protrusion with a free edge that oscillates with blood flow, leading to blood flow stagnation and thrombus formation. These pathophysiological features lead to the high risk of stroke recurrence and certain association with large-vessel occlusion. Given these, carotid artery revascularization has been recommended as an optional treatment for carotid web. We herein described a case of a patient with ischem

BACKGROUND AND PURPOSE: Carotid webs (CWs) are shelf-like fibrous intimal projections in the carotid bulb that perturb laminar flow, promote local stasis, and predispose to embolic stroke. While morphology correlates with events, in vivo hemodynamic markers of stasis are lacking. We hypothesized that multiphase CT angiography can quantify contrast persistence within web recess, and impaired washout may be associated with stroke/transient ischemic attack.
MATERIALS AND METHODS: We retrospectively reviewed head and neck CTA performed between January 2021 and June 2024 in adults whose radiology

INTRODUCTION: Carotid web (CW) refers to an atypical manifestation of fibromuscular dysplasia that affects the intimal layer of the internal carotid artery (ICA) and carotid bulb. CWs involve a shelf-like intraluminal projection within the ICA, increasing the risk of thrombus formation in young patients without known cardiovascular risk factors. The relative lack of literature regarding CWs may cause this pathology to go largely underdiagnosed. Hence, the goal of this study is to provide clarity regarding the demographic and clinical characteristics of CWs.
METHODS: A literature search was pe

We describe a case of an asymptomatic 70-year-old female patient on whom a carotid ultrasound examination was performed that showed intima-media thickening and a 4 mm long carotid web with a 50% web-to-bulb ratio. Spectral Doppler waveform demonstrated a turbulent flow pattern and a peak systolic velocity increase of 100% (velocity ratio = 2) when compared with the common carotid artery. Therefore, the patient seemed to be at risk of stroke, and antiaggregant treatment was suggested.

BACKGROUND: Carotid webs (CaWs) are associated with ischemic strokes in younger patients without degrees of stenosis that are traditionally considered clinically significant.
OBJECTIVE: To compare the hemodynamic parameters in the internal carotid artery (ICA) bulbar segment in patients with CaW with those in patients with atherosclerotic lesions using time-density curve (TDC) analysis of digital subtraction angiography (DSA) images.
METHODS: We retrospectively assessed DSA images of 47 carotid arteries in 41 adult patients who underwent ICA catheter angiography for evaluation after ischemic

OBJECTIVE: Carotid webs (CWs) have increasingly been recognized as a cause of recurrent ischemic stroke. However, the natural history and clinical course of CWs remain unclear. The authors aimed to clarify the prevalence, imaging features, and optimal treatment of CWs in a Japanese cohort study.
METHODS: A series of 444 consecutive Japanese patients who had undergone CTA of the head and neck between April 2011 and October 2016 was retrospectively reviewed. CW was diagnosed on CT angiograms as a membrane-like intraluminal filling defect along the posterior wall of the carotid bulb or the origi

A carotid web is a rare, focal vascular malformation at the posterior wall of the carotid bulb, which can be associated with acute ischemic stroke, especially in young patients. Also, a carotid web is associated with a high risk of stroke recurrence, despite optimal antithrombotic therapy. Our patient is a 41-year-old, previously healthy female, with a symptomatic carotid web on the right carotid artery, which led to acute embolic ischemic lesions in the right insular and frontal juxtacortical regions. She had recurrent episodes of mild left-sided hemiparesis and dysarthria, with complete spon

The carotid web (CW), an intimal variant of fibromuscular dysplasia (FMD), is recognized as a cause of stroke and with a high-risk of stroke recurrence. Other names are carotid-bulb atypical FMD, carotid (bulb) diaphragms and it is described like a shelf of tissue from the posterior wall of the carotid bulb. Here we present a case of a young patient with recurrent stroke, with no other risk factors that was diagnosed with a common CW.

INTRODUCTION: Described for 60 years under various names, the carotid web is a suspected cause of cryptogenic stroke, especially in young patients. The web creates an intraluminal protrusion that may contribute to turbulent flow and thrombus embolization into cerebral arteries. Although the carotid web has frequently been related to arterial fibrodysplasia, its natural history and pathological description remain unclear.
PATIENTS: Among all consecutive patients admitted to the stroke unit of Sainte-Anne Hospital and referred to the vascular surgery department from January 2015 to December 202

IntroductionCarotid Web (CaW) is an increasingly recognized etiology of ischemic stroke, and has been shown to be amenable to endovascular stenting. The technical complexity of stenting for CaW may be lower than for carotid atherosclerotic disease (CAD). We aimed to assess procedural characteristics of stenting for CaW as compared to CAD.MethodsWe retrospectively analyzed a cohort of consecutive patients at a single comprehensive stroke center from 2014 to 2021, who had undergone elective endovascular stent placement for symptomatic CAD or CaW.ResultsIn total, 118 patients underwent elective s

BACKGROUND: Determining the cause of acute ischemic stroke is crucial for patient management, particularly for preventing future stroke. In recent years, carotid web (CW), a non-atherosclerotic disorder of the carotid wall, has been found to be an underestimated source of cerebral emboli.
OBJECTIVE: The present study aimed to analyze the clinical, radiological, and pathological findings, along with the treatments performed in patients with CW and ipsilateral ischemic events.
METHODS: Patients with anterior circulation ischemic stroke or transient ischemic attack and ipsilateral CW were prosp

OBJECTIVE: To validate the carotid web (CW) risk stratification assessment described in previous works within a larger cohort of patients with symptomatic and incidentally found asymptomatic CWs.
METHODS: A retrospective analysis of our institution's electronic medical records identified all patients with a diagnosis of CW from 2017 to 2024. We included symptomatic patients and those with asymptomatic CWs, that is, incidentally found webs without history of stroke or transient ischemic attack. Patient charts were reviewed for demographics, imaging, comorbidities, and a diagnosis of stroke aft
