A case of transient diplopia and ophthalmoplegia following dental anesthesia.

Dental anesthesia is one of the most frequently performed medical procedures. Although the frequency of ocular complications is extremely low, these reactions can be highly alarming and may bring up medicolegal issues when they do occur. Dentists and oral surgeons should be well-informed of these adverse reactions and should be aware that both ophthalmologists and emergency physicians might be required to care for these patients.

Articaine - the best choice of local anesthetic in contemporary dentistry.

Local anesthesia forms the foundation of pain control techniques in clinical dentistry. Within the rich local anesthetic drugs available in dentistry for the prevention and management of pain 4% articaine solutions achieve highest level of anesthetic potency and lowest systemic toxicity in all clinical situations, prior to its superlative physicochemical characteristics and the pharmacological profile. These are - low lipid solubility, high plasma protein binding rate, fast metabolization, fast elimination half time; low blood level. Articaine inactivates in both ways: in the liver and the blo

Aspirations and solutions--a 20 year journey through dental local anaesthetics.

This paper describes clinical and laboratory investigations concerned with the delivery, comfort, systemic effects and efficacy of dental local anaesthesia. Factors influencing the aspirating ability of dental local anaesthetic delivery systems are discussed. The effects of adrenaline in dental local anaesthetic solutions on plasma potassium levels and on the transplanted heart are described. The use of an infiltration technique as an alternative to the inferior alveolar nerve block in the mandible is discussed.

A novel articaine gargle for upper endoscopy in adults: a randomized, double-blind and placebo-controlled trial.

OBJECTIVE: To assess whether a gargled articaine solution makes gastroscopy more tolerable for the patient. BACKGROUND: Gastroscopy is an unpleasant experience for most people. A drug that is easy to use, has a fast onset, enables quick ambulation, and causes minimal side-effects is needed to relieve gastroscopy-related procedural discomfort. STUDY: This double-blinded randomized, placebo-controlled study included 90 patients who underwent gastroscopy without sedation. The patients were allocated into three different treatment groups to receive 10 ml of either articaine 10 mg/ml, articaine 2

Transient glossopharyngeal nerve palsy due to mandibular nerve block.

Local anaesthesia in dental procedures is considered safe. Few cases of neurological complication have been reported, most of which involve the lingual or inferior alveolar nerve. We report a case of a woman in her late 40s who 30 min after administration of local anaesthetic in the right pterygomandibular space (articaine and mepivacaine) developed nasal speech and numbness in the area around external auditory meatus. Uvula was deviated to the left. No facial palsy or diplopia were observed. The symptoms subsided gradually after 3 hours. Clinical symptoms were attributed to glossopharyngeal n

Adding Dexmedetomidine to Articaine Increases the Latency of Thermal Antinociception in Rats.

Articaine is a low-toxicity local anesthetic that is widely used in dentistry. Typically, epinephrine is added to prolong the duration of articaine local anesthesia; however, epinephrine exhibits adverse effects. Low-dose dexmedetomidine (DEX), an α2-adrenoreceptor agonist, reportedly prolongs local anesthesia without notable adverse cardiovascular effects. The purpose of this study was to assess whether a combination of low-dose DEX and articaine would provide a low-toxicity local anesthetic option for dental procedures without adverse cardiovascular effects. Thus, this study investigated whe

Pulpal anesthesia in pediatric patients following supplemental mandibular buccal infiltration in vital permanent mandibular molars with deep caries.

OBJECTIVES: Inferior alveolar nerve block (IANB) does not always provide adequate pulpal anesthesia, and supplemental techniques have been investigated in adults. This study aimed to pre- and intraoperatively evaluate the success of pulpal anesthesia following supplemental mandibular buccal infiltration (SMBI) after failure of IANB in permanent mandibular molars with deep caries of pediatric patients. MATERIALS AND METHODS: Following IANB, preoperative pulpal anesthesia was assessed using the cold test, and success was defined when there was a negative response. In cases with failed IANB, SMB

Deterioration of Kearns-Sayre syndrome following articaine administration for local anesthesia.

Worsening of the neuromuscular manifestations in Kearns-Sayre syndrome after administering local anesthesia with articaine has not been reported. The authors describe a severe adverse reaction to local anesthesia with articaine for tooth extraction in a 28-year-old woman with Kearns-Sayre syndrome due to a 5.9-kb mitochondrial DNA deletion. The patient was subjected to local anesthesia with 1.5 mL (60 mg) articaine in the left submandibular nerve for tooth extraction. Five minutes after the injection the patient developed weakness of the limb muscles, extreme fatigue with increased desire to s

Comparative study of the anaesthetic efficacy of 4% articaine versus 2% lidocaine with adrenaline during extraction of mandibular molars using an inferior alveolar nerve blocking technique.

Various anaesthetic agents have been used in dentistry for the extraction of teeth. The most commonly used local anaesthetic agent is lidocaine hydrochloride. Recently, articaine hydrochloride came into existence because of its versatile properties and longer duration of action. Due to lack of study of effects of articaine on various systems, the present study is aimed to compare the anaesthetic efficacy of articaine and lidocaine with adrenaline during the extraction of mandibular molars. A total of 100 patients was randomly divided into two groups (50 each) and clinical variables such onset
