Cost-effectiveness of early surgical fixation versus cast immobilization for adults with a scaphoid waist fracture: five-year follow-up of the Scaphoid Waist Internal Fixation for Fractures Trial.

AIMS: The conclusion of the one-year analysis of the Scaphoid Waist Internal Fixation for Fractures Trial (SWIFFT) was that initial cast immobilization with surgical fixation for nonunion was the optimal treatment for patients with a fracture of the waist of the scaphoid. However, there remained significant uncertainty about the medium-term outcomes and how these could influence the patient's health-related quality of life (HRQoL), care requirements, and costs. The aim of this study was to explore how additional data from the five-year follow-up influenced the cost-effectiveness of the treatme

Radiological comparison of two immobilization methods in the non-surgical treatment of distal radius fractures in the elderly: Single sugar-tong splint shows similar efficacy to long-arm cast.

BACKGROUND: This study aims to compare the effectiveness of the long-arm cast (LAC) and the single sugar-tong splint (SSTS) in the non-operative treatment of distal radius fractures in the geriatric population. METHODS: Patients consulted at the Orthopedics and Traumatology Department within the Emergency Department (ED) were reviewed through the electronic archives of a tertiary university hospital over a five-year period. The study included patients aged 65 years and older with a distal radius fracture who required reduction, had successful closed reduction, and had at least six weeks of X-

Surgical fixation with K-wires versus plaster casting in the treatment of dorsally displaced distal radius fractures: protocol for Distal Radius Acute Fracture Fixation Trial 2 (DRAFFT 2).

INTRODUCTION: Optimal management of distal radius fractures in adults remains controversial. Previous evidence and current clinical guidelines tell us that, if a closed reduction of a dorsally displaced fracture is possible, Kirschner wires (K-wires) are the preferred form of surgical fixation. However, the question remains whether there is any need to perform surgical fixation following a successful closed reduction, or is a simple plaster cast as effective? This is the protocol for a randomised controlled trial of manipulation and surgical fixation with K-wires versus manipulation and castin

Scaphoid Waist Internal Fixation for Fractures Trial (SWIFFT) protocol: a pragmatic multi-centre randomised controlled trial of cast treatment versus surgical fixation for the treatment of bi-cortical, minimally displaced fractures of the scaphoid waist in adults.

BACKGROUND: A scaphoid fracture is the most common type of carpal fracture affecting young active people. The optimal management of this fracture is uncertain. When treated with a cast, 88 to 90 % of these fractures unite; however, for the remaining 10-12 % the non-union almost invariably leads to arthritis. The alternative is surgery to fix the scaphoid with a screw at the outset. METHODS/DESIGN: We will conduct a randomised controlled trial (RCT) of 438 adult patients with a "clear" and "bicortical" scaphoid waist fracture on plain radiographs to evaluate the clinical effectiveness and cost

Reproducibility of the index of orthognathic functional treatment need scores derived from plaster study casts and their three-dimensional digital equivalents: a pilot study.

OBJECTIVE: To determine the reproducibility of Index of Orthognathic Functional Treatment Need (IOFTN) scores derived from plaster casts and their three-dimensional (3D) digital equivalents. DESIGN: Pilot study, prospective analytical. SETTING: UK hospital orthodontic department. PARTICIPANTS: Thirty casts and their digital equivalents, representing the pre-treatment malocclusions of patients requiring orthodontic-orthognathic surgical treatment, were scored by four clinicians using IOFTN. METHODS: Casts were scanned using a 3Shape digital scanner and 3D models produced using OrthoAnalyzer

The Ankle Injury Management (AIM) trial: a pragmatic, multicentre, equivalence randomised controlled trial and economic evaluation comparing close contact casting with open surgical reduction and internal fixation in the treatment of unstable ankle fractures in patients aged over 60 years.

BACKGROUND: Close contact casting (CCC) may offer an alternative to open reduction and internal fixation (ORIF) surgery for unstable ankle fractures in older adults. OBJECTIVES: We aimed to (1) determine if CCC for unstable ankle fractures in adults aged over 60 years resulted in equivalent clinical outcome compared with ORIF, (2) estimate cost-effectiveness to the NHS and society and (3) explore participant experiences. DESIGN: A pragmatic, multicentre, equivalence randomised controlled trial incorporating health economic evaluation and qualitative study. SETTING: Trauma and orthopaedic de

Ankle Injury Management (AIM): design of a pragmatic multi-centre equivalence randomised controlled trial comparing Close Contact Casting (CCC) to Open surgical Reduction and Internal Fixation (ORIF) in the treatment of unstable ankle fractures in patients over 60 years.

BACKGROUND: Ankle fractures account for 9% of all fractures with a quarter of these occurring in adults over 60 years. The short term disability and long-term consequences of this injury can be considerable. Current opinion favours open reduction and internal fixation (ORIF) over non-operative treatment (fracture manipulation and the application of a standard moulded cast) for older people. Both techniques are associated with complications but the limited published research indicates higher complication rates of fracture malunion (poor position at healing) with casting. The aim of this study i

Immobilization Versus Observation in Children With Toddler's Fractures: A Retrospective Review.

While long leg casts have been historically recommended for immobilization of toddler's fractures, short leg casts are often used by treating physicians. Others question whether any immobilization is necessary because of the internal stability provided by the thickened periosteum of pediatric bones. The purpose of this study is to review the results of toddler's fractures treated with long leg casts, short leg casts, or without immobilization. Eighty-five patients were included in a retrospective review of nondisplaced spiral or oblique tibial shaft fractures in children under 6 years old from

Flexible intramedullary nails with traction versus plaster cast for treating femoral shaft fractures in children: comparative retrospective study.

CONTEXT AND OBJECTIVE Femoral fractures are common in children between 2 and 12 years of age, and 75% of the lesions affect the femoral shaft. Traction followed by a plaster cast is universally accepted as conservative treatment. However, in some situations, a surgical approach is recommended. The objective here was to compare treatments for femoral shaft fractures using intramedullary nails (titanium elastic nails, TEN) versus traction and plaster casts in children. The hypothesis was that TEN might provide better treatment, with good clinical results in comparison with plaster casts. DESIGN

Bronchial casts in children with cardiopathies: the role of pulmonary lymphatic abnormalities.

Expectoration of bronchial casts, a condition also called plastic bronchitis, is very rare in children. Bronchial casts may be associated with bronchopulmonary diseases associated with mucus hypersecretion, bronchopulmonary bacterial infections, congenital and acquired cardiopathies, or pulmonary lymphatic abnormalities. A classification based on anatomy and pathology has been proposed which identifies an "acellular" group associated with congenital cardiopathies and palliative surgery. We report on 3 cases with bronchial casts associated with cardiopathy. Observations suggest that the formati

Complication Rates of Casts versus Bulky Dressings after Syndactyly Release.

BACKGROUND: The arms are commonly immobilized in a cast after syndactyly release. Despite this practice, there is little research examining complications from cast immobilization. This study compares the complication rates of congenital syndactyly release using casts and bulky soft dressings. METHODS: A retrospective review was performed of patients undergoing syndactyly release. Patients were included if they underwent a primary release of congenital syndactyly and excluded if they had a noncongenital cause. Data regarding surgical procedures, medical complications, and unplanned caregiver p

Effect of cast removal timing in the correction of idiopathic clubfoot by the Ponseti method.

Clubfoot correction by the Ponseti method is highly successful with an average of 5 casts in the majority of patients. However, early experience with the technique demonstrated that many cases required more than 5 casts for correction. The purpose of this study was to evaluate the effects of timing of cast removal before subsequent manipulation and casting in the correction of club-foot using the Ponseti method. We reviewed 90 patients (129 clubfeet) treated between December 2000 and June 2006. Two groups were evaluated. Group A: 44 patients (63 clubfeet) had their cast removed by the parents

Refractory type 1 plastic bronchitis in a child; case report.

BACKGROUND: Plastic bronchitis (PB) is a rare pediatric pulmonary condition characterized by the production of branching bronchial casts that cause partial or total obstruction of the bronchial lumen. CASE PRESENTATION: We describe a 13-year-old boy with a history of bronchial asthma and left lower lobectomy, with persistent cough and left-sided chest pain when he went to the emergency room. Chest radiography showed complete left lung opacity denoting total left lung collapse, and flexible bronchoscopy revealed cohesive casts totally occluding the left bronchus, with frequent recurrence that

Plastic bronchitis in children: a case series and review of the medical literature.

Plastic bronchitis is characterized by marked obstruction of the large airways by bronchial casts. We reviewed our experience and the literature to determine whether mortality rates are determined by underlying disease or cast type. We present 3 children with obstructive bronchial casts. One 3-year-old patient with Noonan's syndrome developed respiratory failure following surgery for tetralogy of Fallot requiring support with extracorporeal membrane oxygenation (ECMO) the first such case. There were 42 cases in the literature of children with plastic bronchitis. Casts may be divided into two t

Serial Casting Before Relapse Surgery: Investigating the Challenges of Relapsed Clubfoot Care.

BACKGROUND: Clubfoot is a common congenital condition typically treated in infancy using the Ponseti method. Despite successful initial treatment, relapse occurs in up to 40% of cases and often requires surgical management. While serial casting is widely used for initial correction, its utility before surgical relapse management remains unclear. This study evaluates whether presurgical casting is associated with a reduction in the number and complexity of surgical procedures for relapsed clubfoot in children under the age of 16 years. METHODS: A retrospective cohort audit conducted at a terti

Using 'cling film' to protect lower limb plaster casts in babies with club foot.

Clubfoot or congenital talipes equinovarus is a common condition affecting babies and non-surgical treatment involves serial manipulation and plaster casting for many weeks. The casts are susceptible to soiling during this time, which makes management and child care even more challenging. The authors report initial experience in a typical district general hospital clubfoot clinic where the parents of a baby used conventional cling film to cover the casts and provide a low-cost, effective and well-tolerated method of protection. Informal reports received from these parents were very positive an
