Gastric emptying. Risk versus benefit in the treatment of acute poisoning.

This review examines the various clinical options used to elicit gastric emptying, viz. drug-induced emesis, mechanical pharyngeal stimulation, gastric lavage, and catharsis. Apomorphine and syrup of ipecac are the 2 drugs most frequently used for induction of emesis. Both agents act centrally and, in addition, syrup of ipecac has a peripheral action. Toxins ingested or foods previously eaten may inhibit or enhance emetic action by interfering with mediating and conducting mechanisms. Studies indicate that both syrup of ipecac and apomorphine are similarly effective in inducing emesis; however

Bowel preparation before colonoscopy in the pediatric patient: a randomized study.

Flexible fiberoptic colonoscopy is an established diagnostic procedure in the pediatric patient. The approach to the pediatric patient is, however, different from the approach to adults. Bowel preparation must be geared to the age, size, and clinical status of the patient. An effective, palatable, standardized bowel preparation has not been determined for the child undergoing a colonoscopy. This study systematically evaluated three components of the bowel preparation--cathartics, diet, and enema use. The bowel preparation of 140 children undergoing colonoscopy at The Children's Hospital of Phi

CT colonography: can we achieve an adequate bowel preparation without diet restriction?

OBJECTIVE: To evaluate if an adequate bowel preparation for CT colonography, can be achieved without diet restriction, using a reduced amount of cathartic agent and fecal tagging. To investigate the influence of patients' characteristics on bowel preparation and the impact on patients' compliance. METHODS: In total, 1446 outpatients scheduled for elective CT colonography were prospectively enrolled. All patients had the same bowel preparation based on a reduced amount of cathartic agent (120 g of macrogol in 1.5 l of water) the day before the exam and a fecal tagging agent (60 ml of hyperosmo

Mechanical bowel preparation in review.

Mechanical bowel preparation is necessary for colorectal surgery, colonoscopy, and diagnostic radiological studies. While the techniques of bowel cleansing employed for each of these procedures are similar, indication-specific methods of preparation vary. Peroral lavage, traditional cathartics and enemas, and per-rectal lavage are discussed in this overview.

Bowel preparation for gastrointestinal procedures.

Adequate colonic cleansing is essential for accurate and safe colonic procedures. Common preparations for cleansing include diet in combination with a cathartic agent, gut lavage, and phosphate preparations. The diet used with a cathartic consists of clear liquids or is designed to leave a minimal colonic fecal residue with laxatives. Gut lavage solutions wash out the colon in a safe and effective manner. Phosphate preparations offer an attractive alternative due to smaller volumes required for ingestion; however, electrolyte disturbances can occur. This review discusses the development and cl

Encopresis: a study of treatment alternatives and historical and behavioral characteristics.

The historical, demographic and pre/post treatment behavioral profiles of a Health Maintenance Organization's (HMO) encopresis patients were studied. The efficacy of two maintenance treatments were also compared. Management included an initial bowel evacuation, high fiber diet, behavior modification program, and random assignment to maintenance on oral laxatives or conditional rectal cathartics. Of the 136 patients evaluated, 61 completed treatment (oral 24; rectal 37). Significant behavioral problems were identified. However, the majority (65%) of patients were managed without psychosocial in

Clinical pharmacokinetics of drugs used in the treatment of gastrointestinal diseases (Part I).

Drug treatment of gastrointestinal diseases, which was previously limited to the use of antacids, anticholinergics, antispasmodics, cathartics and laxatives, has changed markedly over the past decade. Histamine H2-receptor antagonists (e.g. cimetidine, ranitidine and more recently famotidine and nizatidine) have revolutionised the treatment of peptic acid disorders, but their role is currently challenged by muscarinic-M1-receptor antagonists (e.g. pirenzepine), proton pump inhibitors (e.g. omeprazole), prostaglandin analogues and site-protective drugs (e.g. colloidal bismuth subcitrate and suc

Magnesium oxide induced metabolic alkalosis in cattle.

A study was designed to compare the metabolic alkalosis produced in cattle from the use of an antacid (magnesium oxide) and a saline cathartic (magnesium sulphate). Six, mature, normal cattle were treated orally with a magnesium oxide (MgO) product and one week later given a comparable cathartic dose of magnesium sulphate (MgSO(4)). The mean percent dry matter content of the cattle feces changed significantly (P<0.001) following administration of both MgO (15.6-8.1) and MgSO(4) (17.0-8.7) but there was no significant difference between treatments. The mean rumen pH values changed significantly

Review article: colon cleansing preparation for gastrointestinal procedures.

Adequate cleansing is essential for reliable diagnostic and surgical colon procedures. Accuracy and safety depend on good preparation. Patient compliance is enhanced by simplicity and well-tolerated methods. Several methods are available. Diet and cathartic regimens utilize clear liquids or diets designed to leave a minimal colonic residue. Laxatives, cathartics and enemas are employed. Gut lavage solutions are osmotically balanced electrolyte lavage products. Oral sodium phosphate solutions and tablets are available and are attractive because of good efficacy with a small volume of administra

[Side effects of laxatives].

The side effects of anthraquinones, diphenylmethane derivatives (e.g. bisacodyl, sodium picosulphate), saline laxatives, lactulose, and cisapride are discussed. When taken in recommended doses, no relevant side effects have been observed, except for hypermagnesemia following magnesium containing cathartics in renal insufficiency. This holds also true for the time of pregnancy and lactation. (Pseudo-)melanosis coli is characterized by pigment loaded macrophages without apparent functional consequences. It occurs mainly after anthraquinones, but to a lesser degree also after diphenylmethane deri

[Preparation of the colon for endoscopic examinations. A clinical study].

The paper gives a short critical bibliographic review of colonic preparation for endoscopy, focusing on the out-patient use of gastrointestinal endoscopy. A method of preparation based on dietary restrictions plus cathartics is proposed that allows the number of enemas used to be reduced to a minimum. The results obtained in 265 out-patients undergoing colonoscopy confirm the efficacy, practicality and tolerability of the method.

Fatal hyperphosphatemia following Fleet Phospo-Soda in a patient with colonic ileus.

A fatal case of hyperphosphatemia secondary to enteral administration of Fleet Phospo-Soda is presented. A 64-yr-old male admitted for theophylline toxicity was treated with activated charcoal and sorbitol, but subsequently developed colonic ileus. Two sequential doses of Phospo-Soda were administered to facilitate clearance of the charcoal; however, this resulted in marked hyperphosphatemia, hypocalcemia, acidemia, and other electrolyte abnormalities, followed by the patient's demise. This case is added to several other reports about the risks of injudicious use of sodium phosphate cathartics

Alkaline ingestions.

Alkaline ingestion is a potentially life-threatening problem that may confront the emergency physician. It is similar to many other toxins in that children and those who attempt suicide are its most common victims; however, implications in terms of initial stabilization and definitive care are quite distinct. Mucosal exposure to lye results in a quick, deep liquefactive necrosis. Consequently, blind nasotracheal intubation may result in the perforation of damaged tissues in the pharynx and trachea. For similar reasons, the blind passage of a nasogastric tube is contraindicated. The use of dilu

Acute poisoning: management protocol.

Management of the acutely poisoned patient should start with decontamination of the skin and irrigation of the eyes, if necessary, and assessment of cardiorespiratory status, neurologic status, and pupils and eye movement. If a definable toxic syndrome is present, the specific "antidote" should be given. If no such syndrome is apparent and the patient is comatose, 50 ml of 50% glucose and 0.4 mg of naloxone (Narcan) intravenously should be tried. General measures, applicable in either situation, include induction of emesis or lavage and administration of charcoal and cathartics.

Magnesium and Drugs Commonly Used in Chronic Kidney Disease.

As with other electrolytes, magnesium homeostasis depends on the balance between gastrointestinal absorption and kidney excretion. Certain drugs used commonly in patients with CKD can decrease gastrointestinal ingestion and kidney reclamation, and potentially cause hypomagnesemia. Other magnesium-containing drugs such as laxatives and cathartics can induce hypermagnesemia, particularly in those with impaired glomerular filtration and magnesium excretion. In this review, we will discuss the potential magnesium complications associated with a range of commonly encountered drugs in the care of CK

Contribution of sorbitol combined with activated charcoal in prevention of salicylate absorption.

The use of cathartics and activated charcoal in treating toxic ingestions has become a standard treatment modality. Sorbitol has been shown to be the most rapidly acting cathartic, but its therapeutic significance has been debated. Using a previously described aspirin overdose model, ten healthy volunteers participated in a crossover design study that investigated the effect of activated charcoal alone versus that of activated charcoal and sorbitol in preventing salicylate absorption. In phase 1 of the study, subjects consumed 2.5 g aspirin followed by 25 g activated charcoal one hour later. U

Zinc supplementation in oral rehydration solutions: experimental assessment and mechanisms of action.

BACKGROUND: Zinc deficiency is associated with chronic diarrhea. This condition is generally linked to an overproduction of nitric oxide (NO), which induces secretion and cellular damage as a free radical. Use of oral rehydration solutions (ORS) is an important part of diarrhea treatment, especially early in infancy and for patients with cholera. The presence of zinc in an ORS could be a positive factor in recovery from diarrheal disease. OBJECTIVE: This study was undertaken to determine whether zinc added to an ORS could regulate the synthesis of NO metabolites in the lumen of zinc deficient

[Comparative evaluation of a new colon preparation method prior to a double-contrast barium enema: toward discontinuation of the cleansing enema].

Evaluation of the efficacy and adequacy for double contrast barium enema of a recently described oral lavage solution associated with bisacodyl (Prepacol) and its comparison with a traditional method of colon cleansing (low-residue diet, cathartics and water enema) are reported. Two similar groups of 100 outpatients were compared. The new cleansing method presents several interesting advantages: time saving for the nurse of the radiology department and the patient himself (cleansing enema is no longer required), better patient acceptance, equal evacuation of fecal residue but significantly bet
