Curative catheter ablation for atrial fibrillation.

With the recent advances in the understanding of the mechanisms of atrial fibrillation, radiofrequency catheter ablation has emerged as an effective therapeutic modality for patients with atrial fibrillation. Techniques for catheter ablation evolved from elimination of triggers that often originate within the pulmonary veins and initiate atrial fibrillation, to additional left atrial ablation using a variety of approaches to also eliminate the mechanisms that play a role in perpetuation of atrial fibrillation. With the current ablation strategies, atrial fibrillation can be eliminated in appro

[Recent advances in radiofrequency catheter ablation].

Radiofrequency catheter ablation has been established as a first-line treatment of various paroxysmal tachycardias, and its developments are still ongoing. As recent advances of radiofrequency catheter ablation, we can point out the following issues: 1) transaortic approach for idiopathic ventricular tachycardia(VT) of LBBB-form with inferior axis, 2) new approach guided by mid-diastolic potential for verapamil-sensitive VT, 3) pulmonary vein(PV) isolation technique guided by PV ostial circular electrogram mapping for paroxysmal atrial fibrillation, 4) new ablation strategies for macro-reentry

Radiofrequency catheter ablation: indications and complications.

Radiofrequency catheter ablation was first described in pediatric patients in the early 1990s. Since then, multiple advances in the technology and understanding of radiofrequency ablation have allowed this technique to blossom into one of the most powerful therapeutic tools available to the pediatric electrophysiologist. This treatment has, in the majority of cases, replaced arrhythmia surgery as the definitive cure for most arrhythmias. Ablation therapy is commonly implemented as an elective procedure to treat paroxysmal reentrant supraventricular tachycardia. There are several advantages to

Radiofrequency catheter ablation for atrial fibrillation.

Until recently, catheter-based radiofrequency ablation for atrial fibrillation was limited to palliative approaches of either atrioventricular node ablation or modification. It is now recognized that at least a proportion of patients with paroxysmal atrial fibrillation may be suitable for curative ablation of an underlying single arrhythmogenic focus. With the intense interest in this area, a catheter-based cure involving endocardial linear lesion creation for patients with chronic or paroxysmal atrial fibrillation may not be far in the future.

Catheter ablation in competitive athletes: indication.

Some supraventricular tachyarrhythmias (SVT), particularly if paroxysmal and/or related to Wolff-Parkinson-White syndrome (WPW), may in some cases endanger an athlete's professional career due to hemodynamic consequences during athletic activity, which in some instances may be life-threatening. One must also take into account that in Italy the law makes antiarrhythmic drug treatment (AAD) incompatible with sport eligibility. For these reasons, the utilization of radiofrequency ablation (RFA) in athletes has different indications as opposed to the normal population, since the primary goal is "t

Radiofrequency catheter ablation for supraventricular tachycardia.

Radiofrequency catheter-mediated ablation is a recently developed technique of achieving cure of certain rhythm disorders, notably supraventricular tachycardia. In less than a decade, it has evolved from a theoretic concept to first line therapy for many patients, including those in the pediatric age group. The most common types of arrhythmias amenable to catheter ablation include supraventricular tachycardia due either to an accessory pathway or A-V nodal reentry and atrial fibrillation with a rapid ventricular response refractory to medical therapy. Before the development of the technique, t

Dipole Density Guided Catheter Ablation versus Conventional Substrate Modification for Repeat Catheter Ablation of Persistent Atrial Fibrillation.

AIMS: The optimal ablation strategy for recurrent persistent atrial fibrillation (persAF) after initially successful catheter ablation (CA) remains debatable. Dipole density (DD) guided CA using the AcQMap system has been proven to be feasible and effective in patients with persAF. So far, long-term outcome data for DD-guided CA in patients with recurrence of persAF are sparse. This study sought to assess long-term outcome data in patients undergoing a DD-guided CA for recurrence of persAF after previous CA in comparison to conventional repeat CA. METHODS AND RESULTS: Patients undergoing DD-g

Catheter Ablation of Idiopathic Ventricular Arrhythmias.

Ventricular arrhythmias (VA) are observed in the setting of structural heart disease. However, in a proportion of patients presenting with VT, the routine diagnostic modalities fail to demonstrate overt myocardial abnormality. These arrhythmias have been called idiopathic VAs. They consist of various subtypes that have been defined by their anatomic location of origin within the heart and/or their underlying mechanism. While the majority of patients are asymptomatic, some experience debilitating symptoms and may develop reversible ventricular dysfunction. Catheter ablation has been traditional

Epicardial Catheter Ablation of Ventricular Tachycardia.

Over the last two decades, epicardial catheter ablation has evolved into a practical approach for treatment of ventricular tachycardia (VT). There are certain considerations when performing this procedure. First, presence of epicardial fat can diminish peak-to-peak electrogram amplitude and also impede radiofrequency energy delivery. Hence, epicardial VT ablation should be performed with cooled-tip radiofrequency using reduced irrigation flow within a relatively 'dry' pericardial milieu. Furthermore, catheter orientation is key when performing epicardial ablation. Lastly, hemo-pericardium rema

Catheter Ablation of Arrhythmia During Pregnancy.

Cardiac arrhythmia as a complication of pregnancy can be problematic to maternal health and fetal life and development. Catheter ablation of tachyarrhythmias during pregnancy has been successfully performed in selected patients with limited experience. Techniques to limit maternal and fetal radiation exposure, including intracardiac echo and electroanatomic mapping systems, are particularly important in this setting. Specific accommodations are necessary in the care of the gravid patient during catheter ablation.

Catheter ablation of inappropriate sinus tachycardia.

Catheter ablation for inappropriate sinus tachycardia (IST) is recommended for patients symptomatic for palpitations and refractory to other treatments. The current approach consists in sinus node modification (SNM), achieved by ablation of the cranial part of the sinus node to eliminate faster sinus rates while trying to preserve chronotropic competence. This approach has a limited efficacy, with a very modest long-term clinical success. To overcome this, proper patient selection is crucial and an epicardial approach should always be considered. This brief review will discuss the current role

Catheter ablation of persistent atrial fibrillation.

Catheter ablation of atrial fibrillation (AF) is performed increasingly worldwide and with the development of new technologies the procedures have become safer and more effective after a single attempt, particularly with paroxysmal AF. However, success rates for persistent AF ablation remain far lower than paroxysmal AF and there is large variation in the strategies used worldwide. This review describes the background to persistent AF ablation, the different strategies used and their associated risks and benefits, developing technologies and the authors' perspective on the future of this rapid

Cryoballoon catheter ablation in atrial fibrillation.

Pulmonary vein isolation with catheter ablation is an effective treatment in patients with symptomatic atrial fibrillation refractory or intolerant to antiarrhythmic medications. The cryoballoon catheter was recently approved for this procedure. In this paper, the basics of cryothermal energy ablation are reviewed including its ability of creating homogenous lesion formation, minimal destruction to surrounding vasculature, preserved tissue integrity, and lower risk of thrombus formation. Also summarized here are the publications describing the clinical experience with the cryoballoon catheter
