Cavernous Sinus Syndrome Caused by Noninvasive Fungal Maxillary Sinusitis.

It is rare that cavernous sinus complications are caused by maxillary sinus lesions because the locations of these lesions are some distant from each other. The authors describe an unusual presentation that the primary lesion was located in the maxillary sinus and triggered cavernous sinus syndrome and optic nerve symptoms. The most likely possibility was that the infection traveled retrograde along the vascular plexus. Removal of maxillary sinus lesions and establishment ventilation may achieve source control.

Lemierre's syndrome with cavernous sinus thrombosis caused by dental infection.

Lemierre's syndrome is a rare multisystemic infection beginning in oropharynx commonly caused by oral anaerobic organisms and leading to internal jugular vein thrombosis with septic emboli. Here, we describe a 45-year-old woman with hypertension and unrecognised type 2 diabetes who presented to a community hospital with fever, double vision and septic shock. Examination showed neck pain aggravated by neck flexion, limited ocular movement of right lateral rectus, left medial rectus and left superior oblique and incomplete ptosis of the left eye. These symptoms were suggestive of bilateral caver

Cavernous sinus syndrome as the first manifestation of metastatic breast disease.

BACKGROUND: The cavernous sinus is a venous plexus crossed by vital neurovascular structures. Metastases to the region are uncommon and often associated with a headache, facial pain, or progressive neurological deficit in III, IV, and VI cranial nerves. The treatment options are surgery, including endoscopic approach, radiotherapy, radiosurgery, and chemotherapy. CASE DESCRIPTION: We report the case of a 26-year-old female with cavernous sinus syndrome due to breast cancer metastasis, who was subjected to chemotherapy with complete neurological recovery. A literature review was performed usin

Tolosa-Hunt syndrome: an arcane pathology of cavernous venous sinus.

Tolosa-Hunt syndrome, an idiopathic granulomatous inflammation of the cavernous sinus, is primarily a diagnosis of exclusion. The majority of patients present with unilateral orbital pain and features suggestive of paralysis of one or more of the cranial nerves passing through the cavernous sinus and/or superior orbital fissure. MRI of the head may show unilateral enhancement of the cavernous sinus and orbital apex. Treatment is with high-dose intravenous steroids followed by tapering oral steroids. Rapid amelioration of pain within 24-48 h supports this rare diagnosis. Resolution of neuropath

A case report of cavernous sinus syndrome in a patient with Takayasu's arteritis.

Neurologists must entertain a broad differential diagnosis when considering a patient with cavernous sinus syndrome, including neoplasm, trauma, vascular causes, inflammatory processes, and infections. We report the case of a 37-year-old woman initially diagnosed with cavernous sinus syndrome, where subsequent investigations revealed findings of Takayasu's arteritis, a large vessel vasculitis. The patient also tested positive for perinuclear antineutrophil cytoplasmic antibodies, suggesting the possibility of a vasculitic spectrum disorder although no clinical features of Wegener's granulomato

A Case of Pituitary Apoplexy and Cavernous Sinus Syndrome during Hemodialysis.

BACKGROUND: Pituitary apoplexy (PA) is a clinical syndrome of pituitary hemorrhage or infarction and can result in hypopituitarism as well as compression of adjacent brain structures. Visual loss occurs frequently, as a result of tumor expansion and compression of the optic chiasm and optic nerves. Additionally, with pituitary tumor invasion into the fixed space of the cavernous sinus, compression of multiple cranial nerves can result in cavernous sinus syndrome (CSS). We describe a case of an undiagnosed pituitary tumor manifesting as abrupt PA with CSS during hemodialysis (HD). Clinical Case

Intracranial germinoma manifesting as cavernous sinus syndrome--case report.

An 11-year-old girl presented with a rare case of neurohypophyseal germinoma manifesting as cavernous sinus syndrome with left abducens nerve palsy. Magnetic resonance (MR) imaging showed a mass located in the left cavernous sinus. Biopsy revealed an inflammatory reaction with lymphocytic infiltration. Postoperative MR imaging showed regression of the mass. Her eye movements were also restored. However, after 5 months, she developed right oculomotor and abducens nerve palsy, and MR imaging showed a large suprasellar mass invading the bilateral cavernous sinuses. The transcranial biopsy sample

Burkitt leukemia with numb chin syndrome and cavernous sinus involvement.

Cavernous sinus syndrome is a rare event. Non-Hodgkin lymphomas, are one possible cause. Neurological presentation of these lymphomas is also exceptional. We report the case of an 11-year-old boy that developed a right third cranial nerve palsy and numbness in the distribution of the right mental nerve, with normal CSF, and enlargement of cavernous sinus on the same side, who was diagnosed Burkitt leukemia.

A singular case of cavernous internal carotid artery aneurysm in patient with cavernous sinus syndrome and bacterial meningitis.

We report the uncommon case of an acute cavernous sinus syndrome in a patient who was consequently discovered to have both a cavernous internal carotid artery aneurysm and bacterial meningitis. Which came first, the chicken or the egg? Which of the two, the aneurysm or the meningitis, gave rise to the patient's symptoms? We briefly reviewed the literature of similar cases and tried to analyze the possible pathophysiological relationship between these findings. Moreover, this case highlights the importance of a multidisciplinary management of these patients to better decide between a medical an

Cavernous sinus syndrome in a patient with occult nasopharyngeal carcinoma: A diagnostic challenge.

Nasopharyngeal carcinoma(NPC) is an epithelial malignancy of the nasopharyngeal mucosa. It typically exhibits local infiltration with frequent skull base involvement and often presents with multiple cranial neuropathies. It can invade the cavernous sinus via the skull base and foramen ovals, thereby causing cavernous sinus syndrome (CSS). When NPC invades nerves, the mass in the nasopharyngeal cavity may not be obvious. We report a case of CSS presenting as painful ophthalmoplegia or nonspecific inflammation. While initial glucocorticoid therapy provided transient symptom relief, the patient d

Cavernous Sinus Syndrome in a Traumatic Dissecting Aneurysm of the Internal Carotid Artery.

We report a rare case of a missed intracavernous internal carotid artery dissecting aneurysm occurring as a complication of the base of skull fracture with severe brain injury causing acute cavernous sinus syndrome with permanent vision loss. A 31-year-old Myanmar lady had an alleged motor vehicle accident and suffered severe traumatic brain injury with multiple intracranial bleeds, multiple facial bone and base of skull fractures, and limb fractures. At one week post-trauma, she had severe right eye proptosis with vision loss, ophthalmoplegia, chemosis, and high intraocular pressure. She was

[Flow diverter treatment for dissecting aneurysm causing cavernous sinus syndrome].

Cavernous sinus syndrome is a combination of unilateral ophthalmoplegia, autonomic dysfunction and sensory loss of the ophthalmic or maxillary branch of the trigeminal nerve. In the case of cavernous sinus syndrome after a head injury, we should consider the possibility of a dissecting aneurysm. In addition to ruptured aneurysms, silent intracranial aneurysms must also be treated if they cause compression symptoms. In our case report, we present a 23-year-old female patient who lost consciousness during a fall, and immediately developed right-sided ptosis, ophthalmoplegia and loss of facial se

Case Report: Cavernous Sinus Syndrome as the Initial Presentation of Multiple Myeloma.

Multiple myeloma (MM) is the second most common hematologic malignancy and most common primary bone malignancy. Ocular manifestations of MM are extremely rare and may be the first presentation leading to diagnosis. Ophthalmologists routinely encounter cavernous sinus syndrome, and there is a wide range of possible etiologies. Here, we present a case of a patient presenting with diplopia, ptosis, and ophthalmoplegia found to have a cavernous sinus plasmacytoma with systemic workup consistent with MM. MM is a rare cause of cavernous sinus syndrome and should be considered in the setting of a sku
