Tissue concentrations of cefepime in acute cholecystitis patients.

Cefepime is a new broad-spectrum cephalosporin with activity against Staphylococcus, Streptococcus, Pseudomonas, and the Enterobacteriaceae. The purpose of this study was to measure cefepime concentrations in plasma, peritoneal fluid, bile fluid and appendix tissue in patients undergoing elective cholecystectomy. Patients were randomly assigned to receive either cefepime, 2 g intravenously in phosphate buffer (IVPB) q 12 h or gentamicin 1.5 mg/kg IVPB q 8 h plus mezlocillin 4 g IVPB q 6 h. During surgery, gall bladder tissue, plasma, peritoneal fluid, and bile fluid samples were obtained at ap

Cefepime Pharmacokinetics in Patients Receiving Extracorporeal Membrane Oxygenation.

BACKGROUND: Data on the effect of extracorporeal membrane oxygenation (ECMO) on cefepime concentration and protein binding are limited. This study aimed to describe the pharmacokinetics of cefepime in patients receiving ECMO, with or without renal replacement therapy. METHODS: Participants receiving ECMO were prospectively enrolled in the thoracic and lung transplant intensive care unit at the University of Florida Health-Shands Hospital in Gainesville, FL. Serum samples for drug quantification were collected at 1, 2, 4, 6, and 8 hours after the completion of cefepime infusion, pre- and post-

Cefepime-Induced Seizures: The Overlooked Outpatient Adverse Reaction.

Cefepime is a 4th generation cephalosporin often used for its ability to cover gram-positives, gram negatives, anaerobic bacteria, and, most importantly, pseudomonas. Prior to initiation of cefepime, the medication is dosed based on the renal function to avoid a multitude of its toxicity profiles, including encephalopathy, aphasia, myoclonus, seizures, and nonconvulsive status epilepticus. These risks are increased in the presence of renal impairment. We present a case of a 65-year-old woman who had presented to the emergency department (ED) two weeks after initiation of outpatient IV cefepime

Severe cefepime-induced status epilepticus treated with haemofiltration.

Neurotoxicity caused by cefepime may occur predominantly in patients with impaired renal function. A case of a cefepime-induced non-convulsive status epilepticus (NCSE) is presented. A 65-year-old woman suffered a severe NCSE due to cefepime in the presence of acute renal failure, requiring coma induction with sodium thiopental. A serious interaction between valproic acid (VPA) and meropenem was also produced after changing cefepime to meropenem. Continuous veno-venous haemofiltration was employed to improve cefepime clearance, and the patient progressively regained her previous mental conditi

Activity of cefepime against nosocomial blood culture isolates.

Cefepime is a new aminothiazolylacetamido cephalosporin with a wider spectrum and greater potency than many currently available cephalosporins. Since the blood culture isolates from patients of the study centre in Saudi Arabia are significantly more resistant to antimicrobial agents in clinical practice, we evaluated the in-vitro activity of cefepime and six other beta-lactam antibodies against 390 and 273 isolates of Gram-negative and Gram-positive bacteria, respectively. Cefepime had a broad spectrum of activity against the Enterobacteriaceae (MIC50 < 0.12 mg/L), Pseudomonas aeruginosa, Acin

Assessment of human exposures of cefepime-taniborbactam against cefepime-resistant Enterobacterales and Pseudomonas aeruginosa in a 7-day hollow fiber infection model.

Taniborbactam is a novel cyclic boronate β-lactamase inhibitor that potentiates the in vitro activity of cefepime against Enterobacterales and Pseudomonas aeruginosa strains harboring serine and metallo-β-lactamases. Taniborbactam lacks intrinsic antibacterial activity. An in vitro hollow fiber infection model (HFIM) was used to evaluate bacterial kill and the potential for treatment-emergent resistance associated with the clinical cefepime-taniborbactam dose of 2-0.5 g every 8 h, administered as a 2 h infusion, for 7 days. Nine cefepime-resistant bacterial strains were studied among one Esche

Cefepime-Induced Generalized Fixed Drug Eruption With Morbilliform Rash.

Fixed drug eruption (FDE) is a cutaneous reaction that characteristically recurs in the same locations upon re-exposure to the offending drug(s). The typical presentation of FDEs is single or multiple violaceous plaques with hyperpigmentation due to inflammation. The causative agents for FDEs include antibiotics, nonsteroidal anti-inflammatory drugs (NSAIDs), acetaminophen, barbiturates, and anticonvulsants. We present an interesting case of a generalized fixed drug eruption secondary to cefepime that resolved with the cessation of the offending drug and the institution of antihistamines and t

Cefepime-Induced Neurotoxicity in a 74-Year-Old Woman.

Cefepime is a fourth-generation cephalosporin with anti-pseudomonal coverage. It has been known to cause neurotoxicity, especially in critically ill patients and those with renal impairment. This neurotoxicity is poorly characterized and under-recognized. We present a case of cefepime-induced neurotoxicity in a 74-year-old woman being treated for cellulitis and osteomyelitis. Symptoms were gradual in onset and included confusion, verbal perseveration, and myoclonus. EEG findings included generalized periodic discharges (GPD) and generalized rhythmic delta activity with admixed sharps (GRDA + S

Low-dosage cefepime as treatment for serious bacterial infections.

Since infection develops in significant numbers of hospitalized patients, the problem of resistance to third-generation cephalosporins is of increasing concern. We evaluated the efficacy of cefepime 1 g bd as treatment for acute, moderately severe bacterial infection in 239 hospitalized patients (mean age 60 years). Of these patients, 204 were evaluated clinically for urinary tract infection (UTI) (n = 90), lower respiratory tract infection (LRTI) (n = 70), skin and soft tissue infection (S/STI) (n = 12) and bacteraemia which was associated with either UTI or LRTI (n = 32) but not included in

Selection and characterization of cefepime-resistant gram-negative bacteria.

The NCTC type strains and four clinical isolates of Enterobacter cloacae, Citrobacter freundii, Serratia marcescens, Morganella morganii, Providencia stuartii and Pseudomonas aeruginosa were exposed, in agar, to cefepime at 3, 5, and 10 x MIC and a breakpoint concentration of 16 mg/L. Mutants were selected at a frequency of approximately 10(-8) that had decreased susceptibility to cefepime and cefpirome, and species-dependent resistance to other beta-lactams. Any putative mutant with a greater than four-fold increase in the MIC was examined to determine its beta-lactamase expression and outer

In vivo pharmacokinetic/pharmacodynamic evaluation of cefepime/taniborbactam combination against cefepime-non-susceptible Enterobacterales and Pseudomonas aeruginosa in a murine pneumonia model.

BACKGROUND: Cefepime/taniborbactam is a cephalosporin/bicyclic boronate β-lactamase inhibitor combination in clinical development for nosocomial pneumonia due to MDR Gram-negative bacteria. A murine pneumonia model was used to characterize cefepime/taniborbactam in vivo pharmacodynamics against Enterobacterales and Pseudomonas aeruginosa strains. METHODS: Clinical cefepime-non-susceptible Enterobacterales and P. aeruginosa strains expressing serine carbapenemases and/or other cefepime-hydrolysing β-lactamases with cefepime/taniborbactam combination MICs of 0.12-16 mg/L were used. Delayed-Onset Cefepime Neurotoxicity in a Patient With Epilepsy and ESRD.

Cefepime is associated with central nervous system toxicity, particularly in patients with renal impairment. Cefepime-induced neurotoxicity (CIN) can manifest as seizures, encephalopathy, and other neurologic disturbances, often mimicking other conditions in complex patients. In this case report, we report a 42-year-old woman with end-stage renal disease on hemodialysis, systemic lupus erythematosus, and epilepsy who presented with recurrent seizures after four months of indefinite cefepime therapy for chronic Pseudomonas aeruginosa catheter-related bacteremia. Despite therapeutic clobazam lev

Cefepime and Mortality: A Systematic Review and Bayesian Meta-Analysis.

IMPORTANCE: Cefepime is recommended for the treatment of febrile neutropenia and gram-negative infections at moderate risk of ampicillin resistance locus C β-lactamase production. Concerns regarding cefepime's safety have been raised in the literature without firm conclusions. OBJECTIVE: To conduct a systematic review and bayesian random-effects meta-analysis of all randomized clinical trials (RCTs) comparing cefepime with other β-lactams for all-cause mortality. Cefepime-Induced Neurotoxicity in an Elderly Patient: A Case Report.

Cefepime, a fourth-generation cephalosporin, may cause neurotoxicity, especially in patients with renal impairment. We report an 81-year-old man with chronic kidney disease (CKD) treated with intravenous cefepime for pneumonia. After five days, he developed acute confusion and reduced consciousness. Laboratory results showed no metabolic abnormalities and no microbial growth in blood cultures, and chest X-ray images improved. Cefepime-induced neurotoxicity (CIN) was suspected; the drug was discontinued and replaced with piperacillin-tazobactam. Neurological status recovered fully within 48 hou

Negative myoclonus as the leading symptom in acute cefepime neurotoxicity.

An 86-year-old woman was diagnosed with hospital-acquired pneumonia with Pseudomonas aeruginosa and treated with cefepime adjusted to her renal clearance. After 4 days, she developed acute-onset negative myoclonus without signs of altered mental status. After ruling out an acute intracranial haemorrhagic or ischaemic stroke as well as other metabolic and endocrine causes of negative myoclonus, the antibiotic was switched to piperacillin/tazobactam due to a suspicion of cefepime neurotoxicity. The patient improved within 24 hours and her symptoms fully resolved within 4 days. These observations

Cefepime induced neurotoxicity: A case series and review of the literature.

Cefepime is a fourth generation cephalosporin which is bactericidal for broad spectrum of organisms. This is a case-series of three patients who presented to our hospital with confusion secondary to cefepime use to treat urinary tract infection (UTI) and health care associated pneumonia (HCAP), after excluding other common etiologies of altered mental status (AMS). Of these three patients, one had progressive expressive aphasia and the other two demonstrated asynchronous myoclonic activity of the limbs. The symptoms were seen within four to five days of initiating the treatment and resolved wi

Effect of ceftriaxone and cefepime on high-dose methotrexate clearance.

Numerous drug interactions with methotrexate have been identified, which can lead to serious life-threatening effects. Up to 90% of methotrexate is excreted unchanged in the urine with primary excretion dependent on organic anion transport in the renal proximal tubule. The two pathways responsible for methotrexate secretion are organic anion transport 1 and primarily organic anion transport 3. Penicillins undergo tubular secretion via organic anion transport, and cephalosporins are believed to also possess a similar risk when administered with methotrexate; however, there are no human studies

Neurotoxicity induced by Cefepime in a very old hemodialysis patient.

Neurotoxicity is an unusual complication of cephalosporin therapy. Only few cases of neurotoxicity induced by Cefepime have been described and probably the frequency of Cefepime-induced status epilepticus is underestimated. We report a case of an 82 year-old male, ESRD patient on chronic hemodialysis program affected by pneumonia, who received a treatment with intravenous Cefepime (1 g/day) and developed a seizure 4 days after the starting antibiotic therapy. Cefepime-induced neurotoxicity was suspected and its administration was immediately discontinued. In order to increase Cefepime clearanc
