Cell therapy as a treatment of secondary lymphedema: a systematic review and meta-analysis.

BACKGROUND: Lymphedema, the accumulation of interstitial fluid caused by poor lymphatic drainage, is a progressive and permanent disease with no curative treatment. Several studies have evaluated cell-based therapies in secondary lymphedema, but no meta-analysis has been performed to assess their efficacy. METHODS: We conducted a systematic review and meta-analysis of all available preclinical and clinical studies, with assessment of their quality and risk of bias. RESULTS: A total of 20 articles using diverse cell types were selected for analysis, including six clinical trials and 14 pre-cl

The German National Strategy for Gene- and Cell-Based Therapies: Generating Impact by Employing a Novel Multi-Stakeholder Approach.

Gene- and cell-based therapies (GCTs) represent a disruptive and transformative class of biomedical innovations. They address diseases by adding, removing, repairing, or replacing genes and/or by endowing distinct living cells with additional biological functions. Through this plethora of options, numerous conditions-including genetic disorders, cancers, and degenerative diseases-have become potential targets for a curative therapy. Thus, GCTs are considered the "Future of Medicine" as they (i) offer a potential cure, particularly for rare and severe disorders previously considered untreatable

Value‑Based Contracting for Cell and Gene Therapies: A Scoping Review of Payer-Manufacturer Agreements and Treatment Center Considerations in the United States.

Cell and gene therapies (CGTs) offer the potential for durable or curative outcomes but come with high upfront costs and uncertainty about long-term safety and effectiveness. To address these challenges, manufacturers and payers have expressed sustained interest in value-based contracts (VBCs), which tie reimbursement to real-world outcomes. While publicly disclosed VBCs have been most visible around earlier CGT launches, details on their structure, assessed outcomes, and operational execution remain limited. For treatment centers, understanding this landscape is important because they are wel

Current Strategies of Muscular Dystrophy Therapeutics: An Overview.

Muscular dystrophies are a group of genetic disorders characterized by varying degrees of progressive muscle weakness and degeneration. They are clinically and genetically heterogeneous but share the common histological features of dystrophic muscle. There is currently no cure for muscular dystrophies, which is of particular concern for the more disabling and/or lethal forms of the disease. Through the years, several therapies have encouragingly been developed for muscular dystrophies and include genetic, cellular, and pharmacological approaches. In this chapter, we undertake a comprehensive e

Challenges of Treatment Methodologies and the Future of Gene Therapy and Stem Cell Therapy to Treat Retinitis Pigmentosa.

Retinitis pigmentosa (RP) is a heterogeneous group of hereditary retinal degenerations for which there is currently no cure. Studies investigating the use of gene therapy, gene editing, and stem cells as potential treatment strategies have shown promising results in animal models and some early clinical trials. Even still, major barriers still exist, including the ability to develop therapies that can target the wide range of mutational etiologies and phenotypic presentations that encompass RP. Additionally, effective screening and early diagnosis are crucial for maximum therapeutic potential,

Challenges of translating a cell therapy to GMP.

Over the last decade, cell and gene therapies have contributed remarkably to the array of novel therapies combating diseases that did not have any hope for an effective treatment or, let alone, a cure. This remarkable achievement was underlined by the marketing approval of CAR T cell therapies in 2017 in the United States, followed by many other countries, world-wide. Since then, thousands of patients have benefited from this autologous, gene modified cell therapy (Abou-El-Enein et al., 2021). Rare diseases, particularly innate neurological diseases such as Huntington's disease have also been

Looking into the future: Gene and cell therapies for glaucoma.

Glaucoma is a complex group of optic neuropathies that affects both humans and animals. Intraocular pressure (IOP) elevation is a major risk factor that results in the loss of retinal ganglion cells (RGCs) and their axons. Currently, lowering IOP by medical and surgical methods is the only approved treatment for primary glaucoma, but there is no cure, and vision loss often progresses despite therapy. Recent technologic advances provide us with a better understanding of disease mechanisms and risk factors; this will permit earlier diagnosis of glaucoma and initiation of therapy sooner and more

Democratizing the Cure: Indigenous Innovations and the Future of Pediatric Cell and Gene Therapy in India.

Cell and gene therapy (CGT) represents a paradigm shift in the treatment of diseases once regarded incurable. This is particularly true for pediatric cancers and inherited disorders which often have a monogenic basis. While the global landscape of CGT is rapidly evolving with numerous approved products, India faces unique challenges in adopting high-cost, complex therapies. This perspective offers a comprehensive overview of the current status of pediatric CGT in India, emphasizing the urgent need for affordable and indigenous alternatives. We examined the spectrum of conditions for which CGT

Target product profile for cell-based and gene-based therapies to achieve a cure for HIV.

This target product profile (TPP) highlights the minimal and optimal characteristics for ex-vivo and in-vivo cell and gene therapy-based products aimed at achieving an HIV cure (ie, durable antiretroviral-free viral control). The need for an effective, safe, scalable, affordable, accessible, and acceptable cure for HIV infection remains a major global priority. The possibilities for cell and gene therapy-based products for an HIV cure are rapidly expanding. In a multi-stakeholder consensus process of clinical experts and civil society, including representatives from low-income and middle-incom

Remyelination: cellular and gene therapy.

Dysfunctional myelination or oligodendroglial abnormalities play a prominent role in a vast array of pediatric neurological diseases of genetic, inflammatory, immunological, traumatic, ischemic, developmental, metabolic, and infectious causes. Recent advances in glial cell biology have suggested that effective remyelination strategies may, indeed, be feasible. Evidence for myelin repair is accumulating in various experimental models of dysmyelinating and demyelinating disease. Attempts at remyelination have either been directed towards creating myelin de novo from exogenous sources of myelin-e

Various Cell Therapy Approaches for Bone Diseases in the Controlled Clinical Trials: A Systematic Review and Meta-analysis Study.

The objective of this review was to answer two critical questions in the cell-based bone defect therapies that were as follows: 1) does cell therapy associate with the increase in the occurrence of adverse events in the patients compared to control groups, 2) does the use of various cell therapy approaches, including More-than Minimal Manipulation (MMM) and Minimal Manipulation (MM), affect the occurrence of adverse events? An electronic literature search was performed in five databases. The controlled clinical trial studies were selected according to the eligibility criteria. Then, they were

Assessing the therapeutic potential of lab-made hepatocytes.

Hepatocyte transplantation has potential as a bridge or even alternative to whole-organ liver transplantation. Because donor livers are scarce, realizing this potential requires the development of alternative cell sources. To be therapeutically effective, surrogate hepatocytes must replicate the complex function and ability to proliferate of primary human hepatocytes. Ideally, they are also autologous to eliminate the need for immune suppression, which can have severe side effects and may not be sufficient to prevent rejection long term. In the past decade, several methods have been developed

Advancing Cell Therapies for Solid Tumors: A Pathway to Overcome Biological, Operational, and Regulatory Hurdles.

Cellular therapies have revolutionized the treatment of hematologic malignancies and are now emerging as potentially promising interventions for solid tumors. While considerable learnings can be leveraged from hematologic applications to inform the development of cell therapies in solid tumors, a number of biological and operational challenges-such as target-antigen heterogeneity; off-tumor target-mediated toxicity; immunosuppressive microenvironment; limited trafficking and persistence; tissue accessibility; and significantly larger patient populations-necessitate innovative clinical, manufac

Next-generation therapies for osteoarthritis: the evolving role of cell therapy products.

Osteoarthritis (OA) remains a major cause of disability worldwide; however, current non-surgical treatments offer transient symptom relief without altering disease course. This leaves a therapeutic gap for patients with early-to-moderate disease who are not candidates for surgery but continue to experience pain and functional limitation. Intra-articular interventions such as non-steroidal anti-inflammatory drugs, hyaluronic acid, and platelet-rich plasma may ease symptoms, but do not modify disease progression. By contrast, cell therapy products hold promise as regenerative approaches that may

Cell-Based Therapies for Spinocerebellar Degenerations: A Systematic Review of Human Clinical Evidence.

Spinocerebellar degenerations (SCDs) are progressive hereditary and selected sporadic cerebellar neurodegenerative disorders with limited disease-modifying options. Cell-based therapies particularly, mesenchymal stromal/stem cells (MSCs), neural stem/progenitor cells, olfactory ensheathing cells (OECs), cord blood mononuclear cells (CBMCs), and hematopoietic stem cell transplantation (HSCT) are biologically plausible candidates, but their clinical efficacy and safety remain uncertain. We searched MEDLINE/PubMed, Embase, Scopus, Web of Science, and Cochrane Library from inception to 10 October
