Antimicrobial Resistance of Enteric Salmonella in Bangui, Central African Republic.

Introduction. The number of Salmonella isolated from clinical samples that are resistant to multiple antibiotics has increased worldwide. The aim of this study was to determine the prevalence of resistant Salmonella enterica isolated in Bangui. Methods. All enteric Salmonella strains isolated from patients in 2008 were identified and serotyped, and the phenotypes of resistance were determined by using the disk diffusion method. Nine resistance-associated genes, bla TEM , bla OXA , bla SHV , tetA, aadA1, catA1, dhfrA1, sul I, and sul II, were sought by genic amplification in seven S.e. Typhimur

Investigation of a mpox outbreak in Central African Republic, 2021-2022.

Human monkeypox virus is spreading globally, and more information is required about its epidemiological and clinical disease characteristics in endemic countries. We report the investigation of an outbreak in November 2021 in Central African Republic (CAR). The primary case, a hunter, fell ill after contact with a non-human primate at the frontier between forest and savannah. The ensuing investigation in a small nearby town concerned two families and four waves of inter-human transmission, with 14 confirmed cases, 11 suspected cases and 17 non-infected contacts, and a secondary attack rate of …

Prospecting for yaws in the Mbaïki Health District in the Central African Republic.

INTRODUCTION: yaws is endemic in the Central African Republic. The last cases of yaws notified by CAR to WHO date back to 2012. The objective of this study was to measure the prevalence of yaws in the health district of Mbaïki and to describe its clinical and epidemiological characteristics. METHODS: this is a descriptive cross-sectional study, conducted from April 10 to 18, 2020 in the Mbaïki health district. Yaws cases were sought in 570 households in the 38 selected villages of the district. Any consenting individual over the age of one year with yaws-like skin lesions was a suspected case…

The molecular epidemiology of type 1 poliovirus in Central African Republic.

An increase in the incidence of acute flaccid paralysis cases associated with wild-type 1 poliovirus occurred in children in the city of Bangui, Central African Republic (CAR), in 1993 and 1994. Genetic relationships of 33 isolates were analysed by restriction fragment length polymorphism and by sequencing the VP1/2A junction region (150 nucleotides) of the viral genome. Two distinct genotypes, A and B, were co-circulating in 1993, while in 1994 only a third genotype, C, was observed. Comparison of the sequences found, with those of the sequences from isolates from neighbouring and other endem…

Seroprevalence of five arboviruses in Zebu cattle in the Central African Republic.

Sequential blood samples taken from 237 adult and 147 young Zebu cattle in the Central African Republic (CAR) were examined by enzyme-linked immunosorbent assay for the presence of antibodies to Rift Valley Fever (RVF), Dugbe (DUG), Crimean-Congo haemorrhagic fever (CCHF), Chikungunya (CHIK) and Wesselsbron (WSL) viruses. Antibodies to DUG and CCHF viruses were detected in 70% and 61% respectively, of the sera from adult cattle, probably due to common antigens. This hypothesis is supported by the fact that CCHF virus was isolated only 3 times from ticks, whereas DUG virus was isolated from alm…

[Status of human African trypanosomiasis in the Nola-Salo-Bilolo in the Central African Republic in 2005].

In November-December 2005, the technical staff of the Organisation for Coordination of the Control of Endemic Diseases in Central Africa (OCEAC) and the National Program for Control of Human African Trypanosomiasis (PNLTHA) undertook screening for human African trypanosomiasis in the historical focus of Nola-Salo-Biolo in Central Africa. A total of 31 new cases of typanosomiasis were detected for a prevalence of 0.44%. This study also provided insight into the limits of this old focus and showed that circulation of the parasite is still heavy. The endemia of sleeping sickness has now been cont

Rotavirus prevalence and genotypes in the Central African Republic, 2011-2021.

BACKGROUND: Rotavirus gastroenteritis is one of major causes of death in infants, particularly in sub-Saharan Africa. In the Central African Republic (CAR), sentinel surveillance of rotavirus gastroenteritis was established in 2011. In this study, we assessed the burden of rotavirus gastroenteritis and identified rotavirus strains circulating in CAR during 2011-2021. METHODS: Stool samples were collected from < 5-year-old children with diarrhoea according to WHO criteria, at the sentinel site in Bangui, CAR. Samples were screened for group A rotavirus antigen by EIA. RNA was extracted from al…

Mectizan delivery systems and cost recovery in the Central African Republic.

The Central African Republic (CAR) has a serious onchocerciasis problem. The disease is endemic in three quarters of the country and there is considerable onchocercal blindness in the north-west. The low population density and extreme poverty (the CAR being one of the 20 poorest countries in the world) combine to make mass treatment with Mectizan (ivermectin, MSD) a challenge. Although planned, primary health care (PHC) is not widely developed in the country. Mectizan distribution was carried out in 1993-1994 by mobile teams in order to address the most urgent need, particularly in the north-w…

[Monkey pox virus infection in humans in the Central African Republic].

A human monkeypox outbreak is reported which occurred in January 1984 in the extreme south-west areas of the Central African Republic. Six persons were found to be affected in a Pygmy camp with an estimated population of 50 residents. In the two affected families, out of 11 members, only unvaccinated children and a 22 year old unvaccinated woman contracted the disease. The disease was of moderate severity in two patients and very mild in the other four. The clinical diagnosis was confirmed by virus isolation from skin lesions of 4 patients and by sero-immunologic tests in all of them. The clin…

Central African Republic is part of the West-African hepatitis B virus genotype E crescent.

BACKGROUND: Recent studies have shown that Hepatitis B virus (HBV) genotype E predominates in a vast crescent in West-Africa spanning from Senegal to Angola. OBJECTIVES: To determine whether HBV strains in the Central African Republic (CAR) belong predominately to the homogeneous West-African genotype E or whether they are more closely related to genotypes found in East Africa. STUDY DESIGN: Serum samples were randomly collected from 196 patients admitted with symptoms of acute or chronic hepatitis to the Central Hospital in Bangui. Thirty complete and 36 partial sequences of HBV strains wer…

Focus of Ongoing Onchocerciasis Transmission Close to Bangui, Central African Republic.

Recently, there were anecdotal reports of a high number of persons with epilepsy, including children with nodding seizures in the Landja Mboko area located about 9 km from the capital city Bangui, Central African Republic. We suspected the area to be endemic for onchocerciasis, and that the alleged increase in the number of epilepsy cases was due to ongoing Onchocerca volvulus transmission. However, ivermectin mass drug distribution (MDA) had never been implemented in the area. Therefore we performed an Ov16 antibody prevalence study among children, aged 6-9 years, using the biplex rapid diagn…

Outbreak of Pneumococcal Meningitis, Paoua Subprefecture, Central African Republic, 2016-2017.

We report a pneumococcal meningitis outbreak in the Central African Republic (251 suspected cases; 60 confirmed by latex agglutination test) in 2016-2017. Case-fatality rates (10% for confirmed case-patients) were low. In areas where a recent pneumococcal conjugate vaccine campaign was conducted, a smaller proportion of cases was seen in youngest children.

Spectrum of female commercial sex work in Bangui, Central African Republic.

Classification of professional and non-professional female sex workers (FSWs) into different categories, never previously reported in the Central African Republic (CAR), may be useful to assess the dynamics of the human immunodeficiency virus (HIV) epidemic, design operational intervention programmes to combat HIV and other sexually transmitted infections (STIs) and to adapt these programmes to the broad spectrum of sexual transactions in the CAR. Our study proposes a socio-behavioural classification of FSWs living in the CAR and engaged in transactional and commercial sex. Thus, the aims of t…

Distribution and antibiotic susceptibility of Shigella isolates in Bangui, Central African Republic.

We conducted a prospective study in four urban health care centres between January 2004 and November 2005 to determine the distribution and susceptibility patterns of Shigella strains causing invasive diarrhoea in Bangui, Central African Republic. Of the 155 Shigella isolated, Shigella flexneri (51%) and Shigella dysenteriae (30%) were the most common and the most resistant to usual antibiotics, including amoxicillin, sulphamethoxazole-trimethoprim and chloramphenicol. Though multi-drug resistance was common, no strains were resistant to quinolone and fluoroquinolones. We conclude that short-c…

Sexually transmitted infections in young pregnant women in Bangui, Central African Republic.

In early 1996, 481 women visiting the antenatal services of the 3 major governmental health centres in the capital city of the Central African Republic (CAR) were included in the study. All study participants underwent the health centre's routine gynaecological examination, including laboratory diagnosis of trichomoniasis, candidiasis, gonorrhoea, syphilis and bacterial vaginosis. Cervical secretions and blood samples from study participants were sent to the National STD Reference Centre for diagnosis of Chlamydia trachomatis, Neisseria gonorrhoeae, Candida albicans, Treponema pallidum, and HI…

A rapid increase in coverage of COVID-19 vaccination, Central African Republic.

PROBLEM: In 2021, Central African Republic was facing multiple challenges in vaccinating its population against coronavirus disease 2019 (COVID-19), including inadequate infrastructure and funding, a shortage of health workers and vaccine hesitancy among the population. APPROACH: To increase COVID-19 vaccination coverage, the health ministry used three main approaches: (i) task shifting to train and equip existing community health workers (CHWs) to deliver COVID-19 vaccination; (ii) evidence gathering to understand people's reluctance to be vaccinated; and (iii) bundling of COVID-19 vaccinati…
