Triplicated middle cerebral arteries (duplicated and ipsilateral accessory) associated with triplicated anterior cerebral arteries (accessory) diagnosed by magnetic resonance angiography.

PURPOSE: To describe a case of duplicated middle cerebral artery (MCA) combined with ipsilateral accessory MCA, forming a triplicated MCA, associated with the accessory anterior cerebral artery (ACA), forming a triplicated A2 segment of the ACA detected incidentally on magnetic resonance (MR) angiography. METHODS: A 70-year-old woman with internal carotid artery (ICA) stenosis at the origin, which was detected by ultrasound, underwent cranial MR imaging and MR angiography of the intracranial region for an evaluation of brain and cerebral arterial lesions. The MR machine was a 3-Tesla scanner.

Cerebral dissecting aneurysm and intimal fibroelastic thickening of cerebral arteries. Case report.

A 19-year-old white man developed aphasia and right hemiplegia after several falls while waterskiing. Cerebral angiography displayed a ripple appearance and a "string of beads" sign along the left middle cerebral artery, with occlusion or stenosis of most of its branches. The patient died after 6 days, of transtentorial herniation due to massive left cerebral infarction. At necropsy, the infarct was found to be due to a subintimal dissecting aneurysm of the left middle cerebral artery. Multifocal areas of intimal fibroelastic thickening (IFT) were found not only at the site of dissection, but

Repeated loading and damage progression in cerebral arteries.

Traumatic brain injury poses a major public health challenge with significant immediate and long-term effects. Repetitive head trauma is an ongoing area of research, and little is known about the response of cerebral blood vessels to such loading. This study investigated the mechanical response of cerebral arteries to repetitive overstretch, hypothesizing that repeated overstretch leads to cumulative damage. To test this hypothesis, middle cerebral artery segments from twelve piglets were subjected to sub-yield, high-rate overstretch of varying severities, with up to 10 repetitions. The stress

Bilateral, hyperdense middle cerebral arteries predict bihemispheric stroke.

The hyperdense vessel sign is a debated topic in terms of its sensitivity and specificity for acute cerebral ischemia. Bilateral, hyperdense middle cerebral artery signs are rare presentations of bilateral cerebral infarction. We describe a 17-year-old boy with a history of cerebral palsy and a repaired atrial septal defect, presenting with lethargy and respiratory failure. Noncontrast computed tomography of the brain revealed bilateral, hyperdense middle cerebral artery signs, and he subsequently demonstrated bilateral cerebral infarctions in the distributions of the middle cerebral arteries.

[Dissection of four cerebral arteries after protracted birth].

A 37-year-old woman suffered from middle cerebral artery infarction secondary to dissection of the left internal carotid artery. Nine days before, a cesarean section had been performed on her after 20 h of unsuccessful labor. Cerebral angiography at admission revealed no further vascular abnormalities. A few days later, however, the patient developed additional dissections of the right internal carotid artery and both vertebral arteries. Pregnancy, childbirth, and a history of rheumatoid arthritis in this patient may have contributed to the dissections; however, due to the unknown etiology of

Ruptured dissecting aneurysms of both anterior cerebral arteries.

A case of bilateral dissecting aneurysms of the anterior cerebral arteries presenting with subarachnoid haemorrhage is reported. Dissection involved the proximal A2 portions of both anterior cerebral arteries. Dissection of the left anterior cerebral artery was responsible for the subarachnoid haemorrhage. Both aneurysms were treated by coating with cotton and fibrin glue.

[Evaluating the extracranial cerebral arteries with ultrasound angiography].

AIM: Ultrasound angiography is a new method of colour-coded vessel imaging. The present study aimed at giving further information on its clinical utility in neurosonology. METHOD: The extracranial arteries of 53 patients and 5 normal persons were investigated by ultrasound angiography in comparison with colour-coded duplex sonography. RESULTS: We found a sharper imaging of stenoses and plaques by ultrasound angiography compared with colour Doppler. The imaging of the vessels can be achieved more continuously, avoiding dropouts of the colour. Small vessels and branches can be detected more re

Contribution of meningeal arteries to cerebral arteriovenous malformations.

We studied 52 patients with cerebral arteriovenous malformations (AVMs) by selective angiography and found that half received supply from meningeal branches of the external carotid artery. The frequency of meningeal arterial contribution was significantly higher in superficial AVMs, especially in the temporal, parietal and occipital regions. Larger lesions and higher shunt with steal phenomena on angiography were other factors favouring a meningeal arterial contribution. Diffuse AVMs with pial arterial networks around the nidus commonly received meningeal blood supply. In the past, meningeal f

Type III collagen mutations cause fragile cerebral arteries.

Premature vascular aneurysms and fragility of cerebral arteries are commonly associated with type III collagen mutations and physical signs suggesting a generalized abnormality of connective tissue. Sometimes these traits are clearly genetically transmitted. Here we present seven examples of early cerebrovascular aneurysms or fragility including five examples of carotid cavernous sinus aneurysms. With one exception in which we suspect the mutation is too small to be detected, all of them had easily visible abnormalities of their type III collagen proteins. Further work in progress will eventua

Nicotine-induced relaxation in isolated canine cerebral arteries.

Nicotine in concentrations ranging from 5 times 10- minus 6 to 10- minus 4 M caused a transient relaxation in strips of isolated canine basilar, middle cerebral and posterior cerebral arteries contracted with prostaglandin F2a (PGF). Transmural neural stimulation did not produce changes in the tension of these arteries. Tyramine contracted the arterial strips. Relaxation induced by nicotine was markedly attenuated by hexamethonium (10- minus 5 M), cocaine (10- minus 5M), bretylium (2 times 10- minus 5 M), sotalol (5 times 10- minus 5 M), propranolol (10- minus 6 M) and removal of Ca-++ from th

Alcohol Metabolism into Acetaldehyde in Developing Cerebral Arteries.

Alcohol exposure during pregnancy leads to fetal alcohol spectrum disorders (FASD), yet the mechanisms through which alcohol disrupts the developing cerebrovasculature remain poorly defined. Acetaldehyde, the first oxidative metabolite of alcohol, can alter vascular function, but whether developing cerebral arteries possess intrinsic capacity to generate acetaldehyde is unknown. Alcohol is primarily oxidized by alcohol dehydrogenase (ADH), cytochrome P450 2E1 (CYP2E1), and catalase (CAT), and their local metabolic activity may contribute to cerebrovascular vulnerability. In this study, cerebra

Doppler examination of cerebral arteries in uremic children.

Transcranial Doppler ultrasonography is a useful method for the estimation and monitoring of cerebral circulation in dialyzed patients. The aim of this study was to evaluate the effect of disease and treatment on cerebral circulation in children on maintenance hemodialysis (HD) and children prior to renal replacement therapy. We demonstrated that in uremic children blood flow velocities of the internal carotid artery (ICA), anterior cerebral artery (ACA), middle cerebral artery (MCA), and posterior cerebral artery (PCA) 120 min and 240 min from the beginning of an HD session were significantly

Shear stress in cerebral arteries supplying arteriovenous malformations.

Arteries supplying cerebral arteriovenous malformations (AVMs) are known to dilate with time. These changes are reversible, and the feeders have been shown to slowly decrease in calibre after removal of the AMV. There is evidence that arteries alter their internal diameters in response to sustained changes of blood flow so that shear stress is kept constant. This implies that blood flow-induced shear stress might be the driving force for remodelling of the cerebral vascular network in the presence of an AVM, and for reversion of these changes after radical operation. The objective of this stud

The effect of carbon dioxide on cerebral arteries.

The constancy of cerebral blood flow and volume relies heavily upon the cerebral arteries' intrinsic ability to respond to changes in the partial pressure of arterial CO2. The physiologic mechanisms underlying these responses have not been determined, although changes in extracellular and intracellular pH, mediation by prostanoids and neural activity have been suggested. CO2 reactivity can be influenced by oxygen status and blood pressure and can vary according to age and brain region. In certain pathological conditions or diseases, it can be severely altered. Modern techniques, which measure

Ultrastructural study of cerebral arteries in Moyamoya disease.

The cerebral arteries in seven cases of Moyamoya disease, demonstrated by angiography were observed by light, transmission and scanning electron microscopy. In two cases, the lumen of the terminal portion of the internal carotid artery was completely occluded by proliferated connective tissue. The characteristic pathologic change was extensive destruction of the smooth muscle cells and the elastica interna. Degeneration and necrosis of smooth muscle cells were obvious in the media. Many condensed organelles and other elements of cellular destruction were dispersed throughout the interstitium.

Cerebral arteries in hypertension: structural and hemodynamic aspects.

Chronic hypertension is associated with structural alterations in the cerebrovascular bed, with increased vessel wall thickness and media/lumen ratio over a large range of arterial sizes. In the relaxed cortical surface arteries of spontaneously hypertensive rats and in some segments of the basal arteries, the enlarged media encroaches on the lumen. The altered vessel geometry increases the steepness of the resistance curve and enables the blood vessels to withstand higher intraluminal pressure. On the other hand, the risk for ischemic complications is increased because of an elevated minimal
