Cesarean section by maternal request is the one performed on a pregnant woman without medical indication and without contraindication to vaginal delivery. There is great controversy over requested cesarean section. Potential risks include complications in subsequent pregnancies, such as uterine rupture, placenta previa and accreta. Potential benefits of requested cesareans include a lower risk of postpartum hemorrhage in the first cesarean and fewer surgical complications compared with vaginal delivery. Cesarean delivery has become a commonly used measure for delivery of the fetus. In the recent years incidence of Cesarean section (CS) has increased dramatically with massive pubic interest. It is called Primary Cesarean section when it is performed for the first time on a pregnant woman. This is a cross sectional study conducted on primary cesarean section from January to December 2004 in Bangabandhu Sheikh Mujib Medical University (BSMMU) Hospital, Dhaka. We report a case of monomicrobial necrotizing fasciitis affecting a healthy woman after caesarean section. On day 1 post-partum she started taking a non-steroidal anti-inflammatory drug. The patient developed an acute fulminant infection and died from sepsis. The causative bacterium was Streptococcus pyogenes.

Sensitization to latex seems to occur more frequently in women than in men. Obstetric and gynecological surgical procedures have recently been shown to be a common setting for latex anaphylaxis. We analyzed all the cases of anaphylactic reactions during 1240 cesarean sections in 2004; the patients were questioned for risk factors and underwent allergy testing for drugs and latex. Four patients had anaphylaxis under spinal anesthesia and in all cases it was due to latex allergy. We report a case of splenic rupture after cesarean delivery for abruptio placentae. The diagnosis was not suspected until a laparotomy was performed. Diagnosis and management were complicated by disseminated intravascular coagulation and renal failure. The importance of early diagnosis and management modalities are discussed.

We describe three cases in which severe hypoxemia occurred during cesarean section. The first and second patients showed low oxygen saturation (91, 90%, respectively) on their arrival at the operation room, and hypoxemia deteriorated rapidly after the induction of anesthesia. 51 cases of placenta previa among 12184 deliveries in our hospital from 1983,1,1 to 1989,2,28 were analyzed. During this period, we used conservative aggressive management to treat placenta previa. The perinatal mortality rate was 60%. Among the 51 cases, 50 were nullipara, 48 were terminated by cesarean section. The average blood loss during the operation was 743.6 ml, which was significantly higher than the average blood loss during the operation of general cesarean section. We retrospectively analyzed 194 pregnancies in women with a history of previous cesarean section (CS) who were offered a trial of labor. We offered every woman a trial of labor as long as she did not have a known previous classical scar. One hundred fifty-one women delivered vaginally (79%), 24 women had multiple uterine scars. Cecal volvulus after cesarean section is rare. Symptoms include abdominal pain, nausea, vomiting, constipation, cystic abdominal mass, and high-pitched bowel sounds. Abdominal x-ray photography is often diagnostic, revealing a dilated cecum with a single fluid level and distended loops of small bowel. The main differentiating factors in post-cesarean large bowel distention are sigmoid volvulus and pseudo-obstruction of the colon. Treatment should accomplish derotation, decompression, and anchoring to prevent recurrence.

The incidence of cesarean scar rupture varies from 0.64 to .24 per cent, with some institutions reporting no ruptures. Associated maternal mortality is extremely low, especially in low segment transverse incisions, so as to be almost nonexistent. A one-year retrospective study of cesarean section (CS) at Women and Infants Hospital of Rhode Island is reported. There were 5,467 women delivered, including 1,011 who delivered by CS, yielding a total CS rate of 18.5% and a primary rate of 13.4%. The four leading indications for primary CS were: dystocia (38.8%), breech presentation (14.9%), malposition (11.9%) and fetal distress (11.3%). Sepsis was responsible for 75% of the complications.
