Evaluation of Galectin-3 as a Novel Biomarker for Chagas Cardiomyopathy.

OBJECTIVES: Chagas cardiomyopathy has worse long-term outcomes than other cardiomyopathies. A biomarker strategy to refer subjects for noninvasive cardiac imaging may help in the early identification of cardiac damage in subjects with Chagas disease. Galectin-3 (Gal-3) is a mediator of cardiac fibrosis shown to be upregulated in animal models of decompensated heart failure. Here we assessed the correlation of Gal-3 with myocardial fibrosis in patients with Chagas disease. METHODS: This study comprised 61 subjects with Chagas disease. All subjects underwent clinical assessments, Doppler echoca

Chagas' cardiomyopathy: the economic burden of an expensive and neglected disease.

BACKGROUND: Chagas' cardiomyopathy (CC) is a rising etiology for heart failure (HF) that previously was restricted to some countries of Latin America. The chronic CC cases cause now a profound socio-economic impact. However this issue has not been well studied if compared to other causes of HF. The objective of this study was to assess the cost burden of CC during acute decompensated HF admissions (ADHF), and compare this cost to the other etiologies of HF. METHODS AND RESULTS: By the end of 2006 we started a five year follow-up of 577 consecutive adult patients admitted at a high complexity

Early identification of patients with Chagas disease at risk of developing cardiomyopathy using 2-D speckle tracking strain: Win, Miranda prediction of Chagas cardiomyopathy.

BACKGROUND: Chagas disease is an endemic protozoan disease with high prevalence in Latin America. Of those infected, 20-30% will develop chronic Chagas cardiomyopathy (CCC) however, prediction using existing clinical criteria remains poor. In this study, we investigated the utility of left ventricular (LV) echocardiographic speckle-tracking global longitudinal strain (GLS) for early detection of CCC. METHODS AND RESULTS: 139 asymptomatic T. cruzi seropositive subjects with normal heart size and normal LV ejection fraction (EF) (stage A or B) were enrolled in this prospective observational stu

Temporal evolution of clinical event risk after primary-prevention ICD implantation: Chagas vs. non-Chagas dilated cardiomyopathy.

Chagas cardiomyopathy evolves through progressive myocardial injury that may alter the temporal distribution of arrhythmic risk compared with other dilated substrates. We analysed a two-centre registry of consecutive primary-prevention implantable cardioverter-defibrillator (ICD) recipients in Buenos Aires (2008-2025), excluding cardiomyopathies in which ICD indication is not based on ventricular dysfunction. The final cohort comprised 194 patients: 50 with Chagas cardiomyopathy (26%), 83 with ischaemic (43%) and 61 with non-ischaemic dilated cardiomyopathy (31%). Median LVEF was 30% in both g

Echocardiographic parameters associated with pulmonary congestion in outpatients with Chagas' cardiomyopathy and non-chagasic cardiomyopathy.

INTRODUCTION: Despite significant left ventricular (LV) systolic dysfunction and cardiomegaly, pulmonary congestion does not seem to be a major finding in Chagas' cardiomyopathy (CC). This study sought to identify echocardiographic parameters associated with pulmonary congestion in CC and in dilated cardiomyopathy of other etiologies, such as non-CC (NCC), and to compare pulmonary venous hypertension between the two entities. METHODS: A total of 130 consecutive patients with CC and NCC, with similar echocardiographic characteristics, were assessed using Doppler echocardiography and chest radi

Chagas Cardiomyopathy: New-Onset Biventricular Failure and Refractory Ventricular Arrhythmias.

INTRODUCTION: Chagas disease is an under-recognized cause of cardiomyopathy in the United States, and cardiac involvement may present decades after exposure with heart failure or ventricular arrhythmias. CASES: A 52-year-old man from El Salvador presented with progressive dyspnea and volume overload. Evaluation revealed severe biventricular dysfunction, elevated filling pressures, no obstructive coronary disease, and nonischemic late gadolinium enhancement. Trypanosoma cruzi serology was positive. He required inotropic support and was discharged on guideline-directed medical therapy with a we

Inspiratory Muscle Strength in Chagas Cardiomyopathy: A Systematic Scoping Review.

The increase in inflammatory markers associated with persistent chronic fibrosing myocarditis, a characteristic of chronic Chagas disease, can result in a reduction in inspiratory muscle strength (IMS) in Chagas cardiomyopathy (CC). However, literature in this field is still scarce. This review aimed to map and summarize the evidence regarding IMS in patients with CC. The inclusion criteria included reports with adult participants with a CC diagnosis, with or without heart failure (HF). The core concept examined was the maximum inspiratory pressure evaluated in the untrained and trained groups

Circulating MicroRNAs and myocardial involvement severity in chronic Chagas cardiomyopathy.

BACKGROUND: Chronic Chagas Cardiomyopathy (CCM) is characterized by a unique pathophysiology in which inflammatory, microvascular and neuroendocrine processes coalesce in the development of one of the most severe cardiomyopathies affecting humans. Despite significant advances in understanding the molecular mechanisms involved in this disease, scarce information is available regarding microRNAs and clinical parameters of disease severity. We aimed to evaluate the association between circulating levels of six microRNAs with markers of myocardial injury and prognosis in this population. METHODS:

Blood DNA methylation marks discriminate Chagas cardiomyopathy disease clinical forms.

Chagas disease is a parasitic disease from South America, affecting around 7 million people worldwide. Decades after the infection, 30% of people develop chronic forms, including Chronic Chagas Cardiomyopathy (CCC), for which no treatment exists. Two stages characterized this form: the moderate form, characterized by a heart ejection fraction (EF) ≥ 0.4, and the severe form, associated to an EF < 0.4. We propose two sets of DNA methylation biomarkers which can predict in blood CCC occurrence, and CCC stage. This analysis, based on machine learning algorithms, makes predictions with more than 9

High fat diet aggravates cardiomyopathy in murine chronic Chagas disease.

Infection with Trypanosoma cruzi, the etiologic agent in Chagas disease, may result in heart disease. Over the last decades, Chagas disease endemic areas in Latin America have seen a dietary transition from the traditional regional diet to a Western style, fat rich diet. Previously, we demonstrated that during acute infection high fat diet (HFD) protects mice from the consequences of infection-induced myocardial damage through effects on adipogenesis in adipose tissue and reduced cardiac lipidopathy. However, the effect of HFD on the subsequent stages of infection - the indeterminate and chron

Cardiomyopathy prognosis after benznidazole treatment in chronic canine Chagas' disease.

OBJECTIVES: To evaluate the effects of benznidazole on Chagas' disease cardiac prognosis using an experimental dog model of infection. METHODS: A total of 28 dogs were divided into three groups: 10 were infected with Trypanosoma cruzi and treated benznidazole during the chronic phase, 10 were infected but untreated, and 8 were non-infected/healthy. The trypanocidal efficacy was measured by parasite kDNA detection in blood and cardiac tissue samples. The effects of benznidazole in ameliorating the cardiac systolic function were evaluated by echodopplercardiogram. RESULTS: The benznidazole ini

[Adenosine induces ventricular arrythmias in hearts with chronic chagas cardiomyopathy].

Adenosine released during ischemia and hypoxia can induce ventricular arrhythmias. This phenomenon is also observed in Chagas disease. This study involved pharmacologic analysis of the arrhythmogenic properties of adenosine in healthy Sprague-Dawley rats (n=14) and in rats with chronic Chagas cardiomyopathy (n=14). Electrocardiographic and pharmacologic studies were performed on isolated hearts prepared using the Langendorff method. Adenosine increased ventricular arrhythmias in both groups of animals in a dose-dependent manner: 50% of chagasic rats developed ventricular fibrillation compared

Chagas cardiomyopathy--where do we stand after a hundred years?

A hundred years from its description, Chagas cardiomyopathy remains a challenging disease. Although successful vector-control strategies have decreased the incidence of Chagas disease in several Latin American countries, both migration to urban areas and immigration have spread the disease worldwide; and now, blood transfusion, organ transplantation, and vertical transmission are a concern. The pathogenesis of Chagas cardiomyopathy involves complex host-parasite interactions, where low-grade but incessant systemic infection and triggered autoimmune reaction are the main mechanisms for its deve

Long term outcomes post-ICD in Chagas cardiomyopathy and non-ischemic cardiomyopathy: A comparative analysis.

BACKGROUND: Chagas cardiomyopathy (CCM) is a significant cause of ventricular arrhythmias and sudden cardiac death (SCD). Although, implantable cardiac defibrillators (ICD) have been used for all forms of non-ischemic cardiomyopathy (NICM), studies on ICD efficacy in CCM are scarce. OBJECTIVE: The present study aims to compare the long-term outcomes, mortality rates, and the occurrence of tachycardia therapies after ICD implantation in patients with CCM and NICM. METHODS: The study was conducted over an 18-year period beginning in 2003. The primary outcome of this study was the difference in

Identifying Cardiac Autoantibody Biomarkers that Predict the Severity of Chagas Cardiomyopathy.

Chagas cardiomyopathy (ChCM) affects up to 30% of patients with chronic Chagas disease and lacks reliable biomarkers for severity and conduction abnormalities. We profiled cardiac autoantibodies using 2-dimensional gel electrophoresis and mass spectrometry, followed by enzyme-linked immunosorbent assay validation and machine learning analysis. Six autoantibodies linked to glycolysis, adenosine triphosphate metabolism, and sarcomere structure distinguished ChCM stages (area under the curve: 0.87; 95% CI: 0.777-0.977) and a mean classification accuracy (0.84; 95% CI: 0.705-0.935). These findings

Cardiac imaging in Chagas cardiomyopathy for phenotypic characterisation and risk stratification.

Chagas disease, caused by Trypanosoma cruzi, remains one of the leading infectious causes of non-ischemic cardiomyopathy worldwide. Although an estimated 6-7 million individuals are chronically infected, 20-30% will develop Chagas cardiomyopathy (CCM), a progressive myocardial disease characterised by conduction abnormalities, ventricular arrhythmias, focal aneurysm formation, diffuse fibrosis, and heart failure. Cardiac involvement often evolves silently over decades, and conventional clinical markers frequently underestimate early myocardial injury. Advances in cardiac imaging have fundament

Unraveling the Missing Pieces: Exploring the Gaps in Understanding Chagas Cardiomyopathy.

Chagas cardiomyopathy affects a considerable number of patients infected with the protozoan Trypanosoma cruzi (T. cruzi) and remains one of the most neglected tropical diseases despite being a significant contributor to morbidity and mortality in both endemic regions of Latin America and non-endemic countries like the United States. Since its discovery almost a century ago, knowledge gaps still exist in the mechanisms involved in the pathogenesis of Chagas cardiomyopathy, and numerous challenges exist in its diagnosis and treatment. This article reviews the main pathogenetic mechanisms involve
