[Aneurysmal bone cyst of the ethmoid. Apropos of a case].

Repeated epistaxis in a 12 year old child revealed an aneurysmal bone cyst of ethmoid, an exceptional localization for this lesion. Modern imaging techniques (CT scan, NMR) provided precise data on its extent, primordial for decision as to surgical approach. NMR imaging was more effective than the CT scan for diagnosis, since blood lakes were identified, and for determination of extent, since tumoral elements were distinguished from sinusal retentions.

[Hydronephrosis due to retrograde ureteric intussusception].

Ureteric intussusception is exceptional. The authors report a case of hydronephrosis due to retrograde intussusception of the subpelvic ureter in a 3-month-old boy admitted for respiratory distress syndrome associated with fever of 39 degrees C. Imaging revealed the presence of marked dilatation of the left kidney. Surgical exploration demonstrated hydronephrosis secondary to obstruction due to ureteric intussusception. This is the first case of ureteric intussusception reported in a child.

Developmental Gerstmann syndrome: case report and review of the literature.

The tetrad of finger agnosia, dyscalculia, dysgraphia, and right-left confusion constitutes the Gerstmann syndrome (GS). A case of developmental Gerstmann syndrome (DGS) that occurred in a normal, highly intelligent child with exceptional reading skills is reported, together with a review of the literature. DGS occurs in both brain-damaged and seemingly normal children. Multiple neurological and behavioral manifestations coexisting with the Gerstmann elements suggest brain injury, whereas the occurrence of the Gerstmann tetrad (plus constructional apraxia) in an otherwise normal and intelligen

Gallbladder carcinoma in an achondroplastic boy.

Carcinoma of the gallbladder in an extremely rare tumor in childhood. We have found only five cases in the literature. Most of them were associated with an ethnic group - The Navajo Indians - who are known to have high incidence of gallbladder disease. We report here on the exceptional presence of this type of tumor in a child, who also had achondroplasia. This, as far as we can determine, is the first such case ever reported.

[Severe vitamin C deficiency and oral pathology. Apropos a case report of Barlow's disease].

Vitamin C deficiency is an exceptional findings but emphasis is placed on the need for detection of latent minimally symptomatic and intricate cases, as illustrated by a 3 1/2 year old child with a latent form of Barlow's disease. Although results of biologic tests were fairly typical, clinical signs were restricted to hypotrophy, gingivostomatitis and proctorrhagia. A literature review failed to determine whether or not specific oral manifestations could be associated with Barlow's disease.

[Herpes zoster in children: about two cases].

Herpes zoster is a rare, usually benign disease in children, only requiring symptomatic treatment. Antivirals are used under exceptional circumstances in patients with complex forms of herpes zoster, usually affecting immunocompromised persons, or with ophthalmic involvement. In newborns, it is more often due to intrauterine contamination during maternal varicella. We report two cases of herpes zoster in immunocompetent children. The first case involves a newborn while the second a 7-year old child.

[Pathology of the spleen in children--current possibilities of surgical treatment].

In the submitted paper the authors review the contemporary state of surgery of the spleen in child age. Surgical treatment is used most frequently in injuries of the spleen, in portal hypertension and in haematological diseases. In a group of patients during the last decade efforts to preserve the spleen are emphasized and experience is reported with the use of therapeutic procedures. The authors emphasize that splenectomy in children should be an exceptional operation.

HIV-1 Subtype C-Infected Children with Exceptional Neutralization Breadth Exhibit Polyclonal Responses Targeting Known Epitopes.

We have previously shown that HIV-1-infected children develop broader and more potent neutralizing antibody responses than adults. This study aimed to determine the antibody specificities in 16 HIV-1 subtype C-infected children who displayed exceptional neutralization breadth on a 22-multisubtype virus panel. All children were antiretroviral treatment (ART) naive with normal CD4 counts despite being infected for a median of 10.1 years with high viral loads. The specificity of broadly neutralizing antibodies (bNAbs) was determined using epitope-ablating mutants, chimeric constructs, and depleti

[Volvulus of the small bowel due to ascaris lumbricoides package: about a case].

We report an exceptional case of a 7 year-old patient with necrotic small bowel volvulus due to adult ascaris lumbricoides. At the admission, the child had intestinal obstruction evolving since two days with alteration of general state. Abdominal radiography without preparation showed small bowel air-fluid levels and tiger-stripe appearance evoking the diagnosis of acute intestinal obstruction associated with abdominal mass. After resuscitation, the surgical treatment consisted of laparotomy which showed necrotic volvulus of the terminal ileum containing adult ascaris lumbricoides. The patient

[Kwashiorkor after an exclusion diet for eczema].

BACKGROUND: Nutritional disorders observed in children with eczema may be aggravated by inappropriate diet, sometimes with dramatic effects. CASE REPORT: A child aged 3 years 7 months presented a state of severe malnutrition with edema (kwashiorkor). The child had chronic eczema and had been given a highly restrictive diet based on a principle of exclusion without medical advise. Cure was achieved without sequelae with a normal diet. DISCUSSION: This case illustrates the fragility of nutritional status in children with chronic widespread skin disease due to the risk of cutaneous loss of prot

[Association of trisomy 21 and gonosomal trisomy. Apropos of 2 cases].

The existence of double autosomal trisomy is exceptional in a newborn child: --Down syndrome and trisomy 18. --Down syndrome and trisomy 13. On the other hand, the association of an autosomal trisomy, generally Down syndrome with gonosomal trisomy, is less rare with an extra X (triplo X, Klinefelter) or an extra Y. The association of Down syndrome with Turner XO syndrome (autosomal gonosomal association) doesn't insert in the subject, and has been described only once in the literature.

[Malacoplakia in children].

Malakoplakia is a rare inflammatory disease, related to Enterobacteria infection in the context of a disorder of cell-mediated immunity. This disease does not have any specific clinical or laboratory signs and the diagnosis is exclusively based on histology. Malakoplakia is exceptional in children and usually involves the gastrointestinal tract. The authors report a rare case of malakoplakia of the urinary bladder in a 4-year-old child, in whom the initial diagnosis was an anomaly of the urachus.

[Bilateral transversal humeral defect accompanied by defects of lower limbs--an attempt of improvement of upper limb's function by a reconstructive procedure, a case illustration].

Motor system is the most common localization of congenital defects. Disturbances of limb formation resulting in a total defect of a limb fragment are most difficult to treat. Transversal defects within the arm are usually unilateral and rarely accompanied by other congenital mutilations. The exceptional case is presented of a child with bilateral humeral transversal defect accompanied by transversal defect of a right thigh, and bilateral hip joint dysplasion. The etiology of the mutilation is unknown. The child was able to move on a wheel-chair as well as thanks to well innervated arm-stumps a

Ectopic Cushing's syndrome due to an adrenal ganglioneuroma.

BACKGROUND: Cushing's syndrome (CS), rare in children, is due to pituitary or, less frequently, to adrenocortical tumors. Ectopic adrenocorticotropin (ACTH) secretion is exceptional. METHOD: A case of apparently ACTH-independent CS in a child is reported. RESULTS: CS was due to an adrenal ganglioneuroma where neuroendocrine cells were immunopositive for ACTH responsible for the syndrome through a paracrine effect. Cortical cell hyperplasia was observed. CONCLUSION: Benign and differentiated tumors of the neural crest such as ganglioneuromas may be responsible for CS.

Atypical cystic meningioma overexpressing AQP1 in early infancy: case report with literature review.

UNLABELLED: Meningiomas in early infancy are rare lesions, worth to be reported for their exceptional occurrence. The authors report a case of an 11-month-old female child with asymmetric macrocephaly due to the presence of a cystic atypical meningioma associated to bilateral subdural collections. CONCLUSION: This unusual and unique case of atypical cystic meningioma in early infancy showed a high positivity to immunostaining of aquaporin 1, and this pattern could correlate with the coexistence both of cysts and subdural collections.

Pulmonary involvement in tuberous sclerosis.

Tuberous sclerosis is a neurocutaneous syndrome involving many tissues. Pulmonary involvement is rare. Pulmonary cases are usually women of child-bearing age who present with spontaneous pneumothorax or progressive dyspnea. In childhood and, even more, in male patients, pulmonary involvement by tuberous sclerosis is exceptional. We report on a 4-year-old male patient diagnosed in the first months of life with tuberous sclerosis with predominantly respiratory symptoms (frequently with acute RDS criteria), in whom lung involvement was pathologically confirmed.

An ethical priority greater than life itself.

This article discusses a case in this issue of The Journal of Clinical Ethics by McCrary and colleagues, "Elective Delivery Before 39 Weeks' Gestation Reconciling Maternal, Fetal, and Family Circumstances," in which parents asked the medical team to deliver their fetus "early." The author discusses (1) the importance that parents have to a child when they are able to love the child, and how important it is for decision makers to consider this; (2) exceptional approaches that may enable parents to "change their minds"; and (3) substantive and procedural innovations that may help to prevent clin

Severe Craniocerebral Injury in a Child Caused by Repetitive Cleaver Strikes: Case Report and Review of the Literature.

Severe craniocerebral injuries are rare in the pediatric population, especially open craniocerebral injuries caused by repeated slashes with sharp instruments. Here, we report an exceptional case of a child with severe open craniocerebral injury caused by repeated chopping blows with a cleaver. The child was admitted with traumatic hemorrhagic shock, and a multidisciplinary team (MDT) was immediately organized for evaluation and treatment decisions. Preoperative imaging showed a comminuted skull fracture accompanied by extensive scalp laceration and brain tissue injury with active hemorrhage.

[Tubulo-papillary apocrine adenoma in association with syringocystadenoma papilliferum].

Tubulo-papillary apocrine adenoma (TAA) is a very rare sweat gland tumor. TAA in association with syringocystadenoma papilliferum (SCP) is exceptional. A 2-year-old Tunisian child developed a mixed tumor on the scalp: TAA in association with SCP. Histologically, the tumor consisted of dilated duct-like areas with some apocrine gland-like areas. The superficial part of the tumor was connected to the epidermis and showed the characteristics of SCP. The characteristics and differences in histopathologic and immunohistochemical findings in this mixed tumor are described.

Abdominal pregnancy: a case report and literature review.

We report a rare and unusual case of an Abdominal pregnancy that was delivered successfully at term. It is the first documented case in the local literature in almost 30 years. What makes it even more exceptional is that despite the high mortality associated with the condition, both mother and baby survived the ordeal. She subsequently delivered another child, 2 years later by an elective lower segment caesarean section and had a tubal ligation performed concomitantly. A review of the topic and its management is also presented.
