Child Maltreatment, Trauma, and Physical Health Outcomes: The Role of Abuse Type and Placement Moves on Health Conditions and Service Use for Youth in Foster Care.

OBJECTIVE: The purpose of the study was to investigate the relations between abuse types, non-maltreatment-related trauma, and health service utilization in a sample of youth in foster care with and without chronic medical conditions. METHOD: A total of 213 youth, aged 8-21 years, provided self-report of general trauma and abuse exposure. Medicaid claims for each child were collected from official state databases. RESULTS: Exposure to sexual abuse, neglect, or general trauma but not exposure to physical abuse or psychological abuse increased the rates of medical visits, while only general tr

Protocol for a prospective multicenter longitudinal randomized controlled trial (CALIN) of sensory-tonic stimulation to foster parent child interactions and social cognition in very premature infants.

INTRODUCTION: Premature birth is associated with long-term somatic and neurological disorders, including cognitive, social and behavioral impairments. Moreover, the mothers of infants born preterm exhibit a higher prevalence of anxiety and depressive symptoms after birth. Early rehabilitation, developmental care, and parenting support have already been shown to have a positive impact on neurological outcome. However, no randomized controlled study has so far assessed the effects on parenting and long-term neurological outcomes of proprioceptive stimulation to trigger positive brain plasticity

Management of Vertebral Defects, Anal Atresia, Cardiac Defects, Tracheoesophageal Fistula or Atresia, Renal Anomalies, and Limb Abnormalities (VACTERL) in a Child With Complex Medical Needs in the Foster Care System: A Pediatric Case Study.

Vertebral defects, anal atresia, cardiac defects, tracheoesophageal fistula or atresia, renal anomalies, and limb abnormalities (VACTERL) association is a complex congenital condition characterized by the presence of malformations that affect various organ systems. Most children born with VACTERL association require surgery shortly after birth, often undergoing multiple procedures during infancy, which can lead to a wide range of physical challenges. The unique combination of malformations in these children in addition to having complex care needs that need to be met can result in physical and

Rethinking the role of the pretend play mode in mentalisation-focused psychoanalytic child therapy for adopted and foster children with attachment trauma: from a pre-mentalising mode to a developmental achievement and a milestone on the trajectory towards mentalisation.

Pretend play is considered to be of great importance in both normal and problematic child development. However, the developmental and therapeutic value of pretend play has perhaps been somewhat overshadowed by the classification of the pretend mode as a non-mentalising mode, a perspective from literature on mentalisation-based psychotherapy with personality disordered adults. In this article, we highlight this difference in emphasis. After having situated this subtle and not seldom unacknowledged difference in meaning surrounding the concept of pretend mode, we focus on psychodynamic child psy

Neglect subtypes in relation to rereport and foster care entry outcomes.

BACKGROUND: There have been mixed findings on whether neglect increases the risk of re-report and foster care entry compared with other child maltreatment types. Studies suggest that child neglect can be broken into subtypes which, in turn, may lead to different child welfare outcomes. OBJECTIVE: The purpose of this study was to examine whether the re-report and foster care entry outcomes varied by child maltreatment type, specifically the subtypes of neglect. PARTICIPANTS AND SETTING: Data were derived from a regional longitudinal study with linked CPS and state-level administrative data. P

Safety and stability for foster children: a developmental perspective.

Children in foster care face a challenging journey through childhood. In addition to the troubling family circumstances that bring them into state care, they face additional difficulties within the child welfare system that may further compromise their healthy development. This article discusses the importance of safety and stability to healthy child development and reviews the research on the risks associated with maltreatment and the foster care experience. It finds: Family stability is best viewed as a process of caregiving practices that, when present, can greatly facilitate healthy child

The foster care system and health status of foster children.

The foster home care system is the most commonly used alternative for the care of dependent children in the United States. However, it often fails to achieve its objective of returning the child to his home or providing a permanent substitute family. Children entering foster care have frequently suffered social, emotional, and medical neglect, and physical abuse. They have a high rate of chronic medical problems, educational handicaps, and severe emotional impairment. In many cases the health care that foster children receive fails to recognize and/or adequately address their disabilities. In

"Nothing but a pair of shorts and a lifetime of trauma": Trauma-related concerns of US foster caregivers.

BACKGROUND: Caring for a child who has experienced trauma is a common expectation for foster caregivers; it is well-recognized that youth entering care will have experienced at least some level of trauma, often needing trauma-sensitive caregiving and mental health supports while in care. Foster caregivers receive varying degrees of trauma-related trainings, and have a range of experience providing care for this population. OBJECTIVE: This study sought to understand the themes relating to trauma that foster caregivers raise when discussing their experiences with fostering, and how this relates

Parent reported melatonin use characteristics, effectiveness, and adverse effects among children in foster care compared to children in the community.

PURPOSE: To examine characteristics of child melatonin use including parent perceptions of safety and effectiveness among children in foster care versus the community. We also sought to understand how prior adverse child experiences (ACEs), child sleep problems, and bedtime routines/interactions might predict melatonin effectiveness. METHODS: This online survey-based study collected data from N = 292 parents of children (4-12 years) currently in foster care (n = 128) and the general community (n = 164) who used melatonin at least once in the past 30 days. RESULTS: Children in foster care wer

Fostering health in the foster care maze.

The fastest growing population of children in foster care today is quite young, and many of these children have significant health care needs. The General Accounting Office (GAO) reported that children in foster care "are among the most vulnerable individuals in the welfare population" (GAO, 1995, p. 1). Poverty, increased homelessness, substance abuse, and a rise in the incidence of persons with HIV all contribute to the problems faced by these children. The Caring Communities for Children in Foster Care Project, funded by the Maternal Child Health Bureau Integrated Services Medical Home Init

Adverse Childhood Experience histories in foster parents: Connections to foster children's emotional and behavioral difficulties.

BACKGROUND: Adverse childhood experiences (ACEs) are known risk factors for negative health and behavioral outcomes, including caregiving. Foster parents are key to supporting foster children, yet there is limited research on their ACE history and how it might relate to their foster children's behavior. OBJECTIVE: This study addressed three research questions: (1) What are ACE totals in this sample of foster parents and how do they compare with the original CDC-Kaiser study? (2) Does foster parents' ACE exposure relate to foster child behavior? (3) Is the relation between foster parents' ACEs

Developmental Challenges of Young Children in Foster Care: Clinical Report.

This clinical report and its accompanying policy statement review the developmental challenges of young children in foster care under the age of 5 years, including recent research on prenatal and postnatal environmental influences on early brain development, and the effects of severe or chronic stress on child development secondary to neglect, physical and sexual abuse. This report also highlights the impact of family disruption and child-parent attachment that occurs with foster care placement, the effects of structural and interpersonal racism, multigenerational poverty, and the more pervasi

Working toward reunification as a case goal: does family structure matter?

Despite an increase in single father-headed households, few studies have examined how single fathers fare in child welfare. This study examined the relationship between family structure and reunification as a case goal, and the moderating role of child race/ethnicity. Data came from the 2019 Adoption and Foster Care Analysis and Reporting System, a national database of all children in foster care in the United States during any point in the fiscal year. The analytic sample included 234,075 children who entered foster care in FY 2019. Logistic regression analyses were conducted to examine wheth

Permanency and the Foster Care System.

Each year over 20,000 youth age out of the child welfare system without reaching a permanent placement in a family. Certain children, such as those spending extended time in foster care, with a diagnosed disability, or adolescents, are at the highest risk for aging out. As young adults, this population is at and increased risk of incarceration; food, housing, and income insecurity; unemployment; educational deficits; receipt of public assistance; and mental health disorders. We reviewed the literature on foster care legislation, permanency, outcomes, and interventions. The outcomes of children

Mental health service utilization by children in foster care in California.

An examination of Medi-Cal-paid claims was undertaken to assess the utilization of mental health services by children in California's foster care system. Using unduplicate counts of service use and diagnoses, it was determined that children in foster care account for 41% of all users of mental health services even though they represent less than 4% of Medi-Cal-eligible children. When partitioned into specific service categories, children in foster care account for 53% of all psychologist visits, 47% of psychiatry visits, 43% of Short Doyle/Medi-Cal inpatient hospitalization in public hospitals

Predicting risk of entry into foster care from early childhood experiences: A survival analysis using LONGSCAN data.

This study examined whether a multi-domain model of maltreatment informed by an ecological framework-including factors related to the child, caregiver, family, neighborhood, and dimensions of maltreatment experience-predicted entry into foster care between the ages of 4 and 18 among children with no prior foster care experience. To determine which factors predict entry into foster care, secondary data analyses were conducted utilizing a sub-sample from LONGSCAN (Longitudinal Studies of Child Abuse and Neglect) of 942 children and their primary caregivers. Results demonstrate that there are imp

What does it mean to be a 'foster parent'? -exploring Foster parent narratives using ideal-type analysis.

OBJECTIVE: Foster care services in Denmark, as in many other countries, face challenges with recruitment, assessment, and retention of foster families. It is essential to understand how foster parents understand their role and how this might relate to child outcomes. This paper develops a typology of foster parent types through an ideal-type analysis of interviews with foster parents. PARTICIPANTS AND SETTING: The sample consist of 14 Danish foster parents who had participated in a quasi-randomized trial exploring the effects of Mentalization Based Therapy for foster families. METHODS: Foste

Interaction microanalysis of foster care research using THEME.

Environmental stress is a key element to the understanding of the psychopathology of children in foster care. Such children often present a wide range of symptoms from anxiety to depression, including abnormal behaviors in their interactions with adults that can be related to experience suffered in their family of origin (e.g., abandonment, abuse, etc.). Foster care should provide a safe environment, both to protect children from abuse and to help them build a well-adjusted developmental trajectory. The relationships with the family of origin may also be maintained. How do children in foster c

Mandates are not enough: placement provider and systems factors that drive ambulatory healthcare use for children in foster care.

INTRODUCTION: In the United States, children are mandated to receive healthcare when they enter foster care and regularly thereafter; states may require more frequent visits. Despite regulations and public insurance to support access to healthcare for young people in foster care, health outcomes for this population remain poor. This study seeks to understand the extent to which caregiver/placement characteristics, child characteristics, and an information-sharing intervention are associated with receipt of mandated healthcare visits for youth in foster care, which may help the field to better

Differential Risks: How Disability Shapes Risk in the Transition to Adulthood for Youth who Age Out of Foster Care.

OBJECTIVE: This study examines how disability is associated with risk during the transition to adulthood for youth who age out of foster care and considers how experiences in the child welfare system contribute to these associations. BACKGROUND: The transition to adulthood is important for later socioeconomic standing, health, and wellbeing. Youth who age out of foster care with disabilities may require a high level of support during this transition yet may lack support. METHOD: This study employs linear probability models to estimate the association between disability and incarceration, hom
