State-Level Variation in Medicaid Managed Care Enrollment and Specialty Care for Publicly Insured Children.

IMPORTANCE: Medicaid and Children's Health Insurance Program cover almost 50% of children with special health care needs (CSHCN). CSHCN often require specialty services and have been increasingly enrolled in Medicaid managed care (MMC) plans, but there is a dearth of recent national studies on specialty care access among publicly insured children and particularly CSHCN. OBJECTIVE: To provide recent, nationwide evidence on the association of MMC penetration with specialty care access among publicly insured children, with a special focus on CSHCN. DESIGN, SETTING, AND PARTICIPANTS: This cross-

Strategies for improving rural trauma health systems.

Rural Americans face persistent and widening disparities in trauma and emergency care driven by geographic isolation, limited workforce capacity, longer transport times, and fragile hospital finances. These challenges are poised to intensify under the 2025 tax and spending reconciliation law, which substantially reduces federal Medicaid and Children's Health Insurance Program funding and is projected to increase uninsurance, placing already vulnerable rural hospitals at heightened risk of service reduction or closure. This article examines the structural pressures confronting rural trauma syst

American Academy of Pediatrics Community Access to Child Health (CATCH) Program: a model for supporting community pediatricians.

Advocacy on behalf of children who are medically underserved and the pediatricians who care for them has been a long-standing core commitment of the Royal College of Paediatrics and Child Health and the American Academy of Pediatrics. Although different in etiology, barriers to adequate health care exist in both nations. In the United States, almost 18 million children have either no health insurance or inadequate coverage, whereas in the United Kingdom, parents can, in most cases, readily enroll their youngsters in a universal health insurance program that is not dependent on employers or emp

Vaccination Coverage Among Children Aged 19-35 Months - United States, 2017.

The Advisory Committee on Immunization Practices (ACIP) recommends routine vaccination by age 24 months against 14 potentially serious illnesses (1). CDC used data from the 2017 National Immunization Survey-Child (NIS-Child) to assess vaccination coverage at national, state, territorial, and selected local levels among children aged 19-35 months in the United States. Coverage remained high and stable overall, exceeding 90% for ≥3 doses of poliovirus vaccine, ≥1 dose of measles, mumps, and rubella vaccine (MMR), ≥3 doses of hepatitis B vaccine (HepB), and ≥1 dose of varicella vaccine. The challenge of caring for indigent children with rheumatologic diseases.

Poverty and lack of insurance prevent complete access to tertiary care for many children with rheumatologic diseases. Long-term solutions to provide community based support for local teams and other services are needed. Physicians need to work with colleagues in health care systems and government to make the health care system fully available to all families. Medical schools can act as catalysts in helping government agencies redefine policies to support outreach and other health care programs for the indigent. Governmental agencies must collaborate with insurance companies to change policies

Health Services Use Among Children With Prenatal Substance Exposure or Neonatal Abstinence Syndrome.

OBJECTIVE: Infants with prenatal substance exposure or neonatal abstinence syndrome (NAS) use health services more often than other children; however, little is known about their use of mental health services and psychotropic medication. METHODS: The sample (N=1,004,085) consisted of infants born in 2016 in 38 states who were followed through the fifth year of life and enrolled each year in Medicaid or the Children's Health Insurance Program. Infants with prenatal substance exposure or NAS were identified with ICD-10 diagnosis codes; procedure and revenue codes documented their service use.

A longitudinal evaluation of fissure sealants applied in dental practices.

Sealants are highly effective in preventing dental caries in the pits and fissures of teeth when applied by trained operators in clinical trials and public health programs. The effectiveness of fissure sealants when applied in dental practices is still not known. The purpose of this longitudinal study was to evaluate the effectiveness of fissure sealants applied in dental practices in preventing dental caries on occlusal surfaces of first permanent molars. In 1990, on the Island of Montréal, 911 randomly selected children, from 6 to 9 years of age, were examined; out of those, 816 and 733 were

Imputed food insecurity as a predictor of disease and mental health in Taiwanese elementary school children.

This study investigated the association between food insecurity and Taiwanese children's ambulatory medical care use for treating eighteen disease types linked to endocrine and metabolic disorders, nutrition, immunity, infections, asthma, mental health, injury, and poisoning. We used longitudinal data in the Taiwan National Health Insurance scheme (NHI) for 764,526 elementary children, and employed approximate NHI data to construct three indicators imputed to food insecurity: low birth weight status, economic status (poverty versus non-poverty), and time of year (summer break time versus semes

Marital disruption and health insurance.

Despite the high levels of marital disruption in the United States and the fact that a significant portion of health insurance coverage for those less than age 65 is based on family membership, surprisingly little research is available on the consequences of marital disruption for the health insurance coverage of men, women, and children. We address this shortfall by examining patterns of coverage surrounding marital disruption for men, women, and children, further subset by educational level. Using the 1996, 2001, and 2004 panels of the Survey of Income and Program Participation (SIPP), we fi

Health Care Setting and Minimally Adequate Depression Treatment Among Publicly Insured Children.

IMPORTANCE: The prevalence of depression among children and adolescents has increased, yet many do not receive adequate treatment. The rise of telemental health care highlights the need to examine how delivery setting changes are associated with depression care. OBJECTIVE: To estimate the association between service delivery setting (telehealth vs clinic-only care) and the receipt of mental health treatment among publicly insured children and adolescents with depression. State Public Coverage of Pregnant Undocumented Immigrants and Prenatal Insurance Uptake.

UNLABELLED: Policy Points Twenty-four states and the District of Columbia offer public insurance to pregnant undocumented immigrants who are income eligible for Medicaid. We found that residing in a state with public coverage of pregnant undocumented immigrants was associated with increased prenatal Medicaid coverage among immigrants and was not associated with corresponding reductions in private or other insurance coverage. Offering state public insurance to pregnant undocumented immigrants could increase immigrants' access to insurance coverage and recommended care during pregnancy. CONTEXT
