Clinical features of temporal bone cholesteatoma are miscellaneous, and sometimes misleading: signs of middle ear cholesteatoma, progressive or sudden facial palsy, sensorineural deafness as in acoustic neuroma, conductive deafness as in otosclerosis, secretory otitis media, or intracranial complications. Polytomography is the only way to pinpoint topography and extension. CT scanning is very useful in determining extension. Topography and severity of deafness are the guidelines for surgical approach. Among sixteen cases, total deafness was produced by the lesion itself in ten cases, and six h…

Cholesteatoma is abnormal epithelial tissue in the middle ear cleft and an important cause of chronically discharging ears and hearing loss. Its proximity to important middle ear structures and the brain can lead to significant complications. Cholesteatoma presents both as congenital and acquired types and can occur both in children and adults. The mainstay of treating cholesteatoma is a destructive surgery with attendant morbidity. Despite radical surgery cholesteatomas can recur, sometimes repeatedly, causing a significant impact on quality of life. The role of inflammation in cholesteatoma …

Presented clinical observation of a patient with exacerbation of left-sided chronic purulent otitis media complicated by extradural brain abscess, temporal bone osteomyelitis. The patient underwent a sanitizing operation on the left temporal bone with plastic closure of the defect in the postoperative cavity with a muscular-fascial flap. Described the postoperative management of the patient and presented the results of treatment 2 months after surgery.

External auditory canal cholesteatoma is defined as an accumulation of keratin leading to osteolytic bony erosion. It is a rare or underdiagnosed otologic entity. Our study aims to illustrate one case of external auditory canal cholesteatoma. Hypertensive diabetic patient, 65 years of age, under therapy suffering from severe right earache, insomnia, hearing loss, small purulent otorrhea and right facial paralysis grade V since 3 months. Initially the diagnosis was malignant external otitis. The patient underwent antibiotic therapy without improvement. Otologic examination showed burgeoning ulc…

Cholesteatoma in the urinary system is a rarely seen benign condition. Rosina firstly defined this condition in the year 1953. Histopathologically it is characterized with keratinization, and squamous metaplasia of urothelial epithelium associated with desquamation of keratinized layers. Flank pain is the most common symptom that is caused by elimination of keratinous material. In our case we will discuss cholesteatoma developed in an ectopic kidney which has not been described in the literature before.

Cholesteatoma is a non-neoplastic, keratinizing lesion, characterized by the proliferation of epithelium with aberrant micro-architecture into the middle ear and mastoid cavity. The exact pathogenic molecular mechanisms behind the formation and propagation of cholesteatoma remain unclear. Immunohistochemical examinations of the matrix and perimatrix have considerably improved the knowledge of cholesteatoma pathogenesis. In this review, the current concepts of cholesteatoma pathogenesis are discussed. Currently, the most widely acknowledged pathogenesis of acquired cholesteatoma is the theory t…

INTRODUCTION: The exact aetiology of congenital cholesteatoma, the less common form of this destructive disease, is still under debate.
CASE REPORT: A two-year-old boy was referred to paediatric otolaryngology with persistent, bloody, left-sided otorrhoea refractory to oral and ototopical antibiotics. Prior to its onset at age 16 months, all ear examinations on the affected side were normal. Physical examination, imaging with computed tomography and eventual tympanomastoidectomy revealed extensive cholesteatoma. The extent of the disease, age at onset of symptoms and absence of otological dis…

OBJECTIVE: To investigate the location, clinical symptom, image features, treatment and prognosis of the congenital middle ear cholesteatoma.
METHOD: Retrospective review of 9 patients treated at Department of Otolaryngology Head and Neck Surgery, the affiliated Hospital of the first Strasbourg University, France.
RESULT: The mean age was 6.1 years. Including 7 boys and 2 girls with hearing loss, and 7 of them suffered conductive hearing loss. Typical cholesteatomas were found behind posterior tympanic membrane according the otoscopic examinations in 6 cases. CT were completed in 8 of 9 case…

PURPOSE OF THE STUDY: Congenital cholesteatoma is a well-described anatomical and clinical entity. Adult forms are rare. We describe a posterosuperior encapsulated cholesteatoma and compare this case to other infantile and adult forms described in the literature.
MATERIAL AND METHODS: A 25-year-old patient with no medical history consulted for left conductive hearing loss. A flat tympanogram was obtained. The temporal bone computed tomographic scan showed a soft tissue density lesion of the middle ear and anterior stapes erosion. A congenital cholesteatoma was discovered during surgical explo…

OBJECTIVE: We report an extremely rare case of family clustering of cholesteatoma.
METHOD: Case reports and a review of the world literature concerning cholesteatoma and heredity are presented.
RESULTS: The family consists of parents and seven siblings of whom the mother and three sons have been surgically treated for cholesteatoma. All cholesteatomas in the family are acquired and all have a history of otitis media. Cholesteatomas occur with an incidence of 5/100,000 in Greenland, corresponding to two to three new cholesteatoma patients per year among the 57,000 inhabitants of Greenland. Th…

Cholesteatoma of the paranasal sinuses is a rare condition. The purpose of this paper is to present a child with a congenital maxillary sinus cholesteatoma. An 18-month-old girl presented with a 4-week history of right cheek and intraoral swelling. Examination revealed a smooth swelling of the right hard palate in association with the facial swelling in the maxillary region. An inferior meatal antrostomy revealed pultaceous debris in the right maxillary antrum and biopsy confirmed a maxillary sinus cholesteatoma. The inferior meatal antrostomy was enlarged to allow exteriorisation of the disea…

OBJECTIVE: To detect the presence of intratympanic membrane cholesteatoma in cases of chronic otitis media.
DESIGN: Two hundred forty-three temporal bones from patients with chronic otitis media were studied for the presence of intratympanic membrane cholesteatoma.
METHODS: Specimens had been previously removed at autopsy, fixed in 10% buffered formalin, decalcified in trichloroacetic acid, dehydrated in a graded series of ethanol, and embedded in celloidin. Sections were cut at a thickness of 20 microm, stained with hematoxylin eosin, and examined using light microscopy.
RESULTS: Intratymp…

The incidence of cholesteatoma and its intracranial complications in 1450 patients treated for chronic middle ear disease at the ENT Department of the General Hospital in Osijek over a 15-year period is reported. Nearly 7.5% of patients suffering from chronic middle ear disease with cholesteatoma developed intracranial complications. The most frequent complication was meningitis. The mean age of patients with intracranial complications was 34.6 years. The average duration of the disease was 11.9 years. The surgical technique depends on the type of complication as well as the general condition …

A retrospective review of 200 cases of aural polypectomy performed in Glasgow and London was assessed in previously unoperated ears and in those with mastoid cavities. In those with mastoid cavities 72% of the ears became dry whilst only 33% were dry in the previously unoperated group. Cholesteatoma was found in 33% of previously unoperated ears and in 71% with an attic or posterior marginal defect. It can be concluded from these results that aural polypectomy in the presence of a mastoid cavity is a satisfactory procedure but in a previously unoperated ear a more radical procedure should be c…
