OBJECTIVE: To assess the effects of clinical chorioamnionitis and labor complications on short-term neonatal morbidity, including seizures.
METHODS: This was a retrospective cohort study of all live-born term infants who weighed more than 2500 g delivered between 1988 and 1997 at Parkland Memorial Hospital, Dallas, Texas. Infant outcomes were compared between women with and without clinical diagnoses of chorioamnionitis. Chorioamnionitis was based on maternal fever of 38C or greater with supporting clinical evidence including fetal tachycardia, uterine tenderness, and malodorous infant.

BACKGROUND: Fusobacterium nucleatum and Capnocytophaga species are common oral pathogens and infrequent causes of systemic infection in patients with compromised immunity or disrupted mucosal integrity. The isolation of both organisms from a clinical specimen suggests an oral source of infection.
CASE: A 23-year-old black woman was admitted at 24 weeks' gestation in preterm labor. She subsequently developed signs of clinical chorioamnionitis, including fever, fetal tachycardia, and uterine tenderness. Bacteriologic studies of the amniotic fluid and subchorionic placental cultures yielded F nu

Effects of imipenem/cilastatin (IPM/CS) therapy, flomoxef (FMOX) therapy and combined ceftazidime + aspoxicillin (CAZ/ASPC) therapy as initial therapies for chorioamnionitis were assessed clinically. 1. The subjects were 49 women with threatened abortion and 29 with premature rupture of membranes (PROM), complicated in all cases by chorioamnionitis. The inflammation was treated with IPM/CS in 19 patients, FMOX in 39, CAZ in 11, and CAZ/ASPC in 9. 2. The response rate to therapy for chorioamnionitis was 95.9% (47/49) in the threatened abortion group. Of the 49 patients in this group, 16 (32.7%)

Amniotic fluid infections have been strongly associated with coitus during pregnancy. If the relationship is causal it may be possible to identify a time sequence between coitus, penetration of bacteria through the mucous plug in the cervix, the development of infection in the extraplacental membranes near the cervical os, spread of the infection to the amniotic fluid and complications of the infection. The present study looked for such a sequence in an analysis of 541 pregnancies. A peak frequency of chorioamnionitis limited to the extraplacental membranes was present when labor and delivery

Acute chorioamnionitis complicates 1-2% of all pregnancies and might increase the prevalence of endometritis that can cause Asherman syndrome or adhesions, but little is known about the direct effects of chorioamnionitis on future fertility. We aimed to evaluate the effect of chorioamnionitis on future fertility and obstetrics complications in patients diagnosed with chorioamnionitis during their pregnancy. We performed an observational, case-control retrospective study of pregnant women aged 18-40 years old, hospitalized with a diagnosis of chorioamnionitis between January 2013 and December 2

A histologic response of histologic chorioamnionitis (HCA) is defined as an intrauterine inflammatory condition characterized by acute granulocyte infiltration into the fetal-maternal or the fetal tissues. Prevalence of HCA is inversely correlated with gestational age, occurring in 50% of preterm birth and in up to 20% of deliveries at term. Regardless of these standard definitions, understanding HCA is challenging as it reflects a heterogeneous condition. A histologic response of HCA from term placentas often does not correspond to a clinical presentation; in this context, the present review

Mid-trimester pregnancy loss defined as miscarriage at 14-23 weeks' gestation and preterm birth between 24 and 28 weeks are in essence clinical manifestations of the same disease process. The pathogenic and socio-biologic risk factors are the same, but the timing of onset of uterine activity and cervical dilatation may be delayed in the case of preterm birth. The overwhelming majority of cases are associated with ascending infection from the lower genital tract. Women with a prior history of late miscarriage are at increased risk of preterm delivery and vice versa. The risk of preterm delivery

OBJECTIVE: To examine the effect of antenatal steroid exposure and in utero inflammation on the development of severe intraventricular haemorrhage, periventricular leukomalacia and long-term neurological outcome in infants less than 30 completed weeks gestation.
METHOD: Infants less than 30 completed weeks gestation from January 1996 to July 2001 were identified from a prospectively managed database. Placental pathology was reviewed for the presence or absence of chorioamnionitis and funisitis. Infants were divided into three groups depending on the degree of exposure to fetal inflammation (n

BACKGROUND: Premature rupture of membrane (PROM), especially when preterm or prolonged is associated with an increased risk of chorioamnionitis with its attendant feto-maternal complications.
AIM: The study was aimed to determine the association of clinical signs of chorioamnionitis with histological chorioamnionitis and neonatal outcomes in women with PROM.
MATERIALS AND METHODS: Eligible participants with clinical diagnosis of PROM at gestational age of ≥28 weeks managed between December 2018 and June 2019 were consecutively recruited. Their sociodemographic characteristics, obstetrics his

Fungal chorioamnionitis is a rare but potentially life-threatening complication of pregnancy with significant maternal and fetal consequences. We describe a severe case of Candida glabrata chorioamnionitis in a 19-week twin pregnancy conceived through in vitro fertilization (IVF), resulting in maternal septic shock and fetal demise. The patient was successfully treated with anidulafungin after the identification of C. glabrata in blood and placental cultures. This case highlights the growing association between assisted reproductive technologies and fungal chorioamnionitis, emphasizing the nee

OBJECTIVE: To examine the relationship of endocervical pathogens and cervicitis with the development of chorioamnionitis, premature rupture of membranes (PROM) and prematurity.
METHODS: Three groups of pregnant women were included in the study: group A, controls with no evidence of cervicitis; group B, women with cervicitis due to pathogenic bacteria, who had been treated; and group C, women with untreated cervicitis who had not followed the prescribed therapy. Endocervical samples were cultured and aerobic and anaerobic bacteria isolated to examine the correlation of cervicitis with chorioam

Intraamniotic infection is a significant cause of perinatal morbidity. Capnocytophaga, a gram-negative anaerobe found in the oral cavity, has been implicated as an unusual cause of neonatal sepsis. We report a patient in whom this unusual organism was identified and describe the postpartum and neonatal courses.

A 21 year old primigravida with a twin pregnancy developed Pasteurella multocida chorioamnionitis. Infection occurred at 27 weeks gestational age after prolonged rupture of membranes. The twin in the separate sac presenting proximal to the cervix suffered infection and died shortly after birth whereas the other twin was not infected. The bacterium is believed to have caused ascending infection from asymptomatic colonization of the vaginal tract.

The purpose of this study was to relate histologic chorioamnionitis to the isolation of microorganisms from the freshly separated chorioamnion in women who had early preterm delivery (before 35 weeks' gestation) following spontaneous labor. Histologic chorioamnionitis was identified in 51 of 95 study subjects. It was more common in the second trimester (72%) than from 27-34 weeks' gestation (33%) (P less than .001). Culture specimens were obtained for aerobic and anaerobic bacteria, yeasts, mycoplasmas, and Chlamydia. Microorganisms were recovered from 38 subjects; all culture reports were neg

The use of diagnostic amniocentesis has been proposed for the evaluation of patients with clinical suspicion of chorioamnionitis, such as those with premature rupture of membranes and premature labor. We describe a patient in whom the diagnosis of Candida chorioamnionitis was made after diagnostic amniocentesis with the assistance of a simple and rapidly performed potassium hydroxide smear.

Background and Objectives: Intrapartum clinical chorioamnionitis at term is a complication associated with adverse maternal and neonatal outcomes. We aimed to identify factors independently associated with severe histological chorioamnionitis in women with clinical chorioamnionitis at term and with severe histological chorioamnionitis accompanied by neonatal intensive care unit (NICU) admission, considered the most severe end of the spectrum. Materials and Methods: We retrospectively identified all women with a diagnosis of clinical chorioamnionitis during labor at term between 2017 and 2022.
