The authors report a case of a late-onset spontaneous choroidal hemorrhage in a patient with a corneal transplant that resulted in rupture of the keratoplasty wound and expulsion of ocular contents. This case is important because it highlights the predisposing factors of a delayed-onset choroidal hemorrhage including anticoagulant therapy, advanced age, glaucoma, and hypertension.

UNLABELLED: The authors describe a 79-year-old man with Osler-Rendu-Weber syndrome, who developed intraoperative choroidal hemorrhage in left eye. During phacoemulsification under local anesthesia, shallowing of the anterior chamber was observed. Acetazolamide was administered and peribulbar block performed. Phacoemulsification was continued and intraocular lens implanted. Five months postoperatively resolution of the choroidal hemorrhage was observed. Best corrected visual acuity was 0.5. Patient developed retinal embolism in his right eye 16 years earlier.
CONCLUSIONS: (1) Choroidal hemorrh

PURPOSE: To determine the cause of spontaneous choroidal hemorrhage in a 67-year-old man after a myocardial infarction and administration of tissue plasminogen activator.
METHODS: The patient underwent ocular examination.
RESULTS: The patient retained excellent visual acuity and the choroidal hemorrhage resolved completely within two months.
CONCLUSION: The administration of tissue plasminogen activator was responsible for the large extent of hemorrhage and should be considered in the differential diagnosis of hemorrhagic choroidal detachment.

Uncomplicated posterior chamber intraocular lens implantation was performed in two eyes that had been salvaged after expulsive choroidal hemorrhage during a previous cataract operation and in one eye after loss of the fellow eye from expulsive choroidal hemorrhage. The procedure was performed in a 72-year-old woman three weeks after and in an 81-year-old man four months after the successful management of expulsive choroidal hemorrhage. A third patient, an 84-year-old woman, had posterior chamber intraocular lens implantation in her remaining eye six years after the loss of other eye. All three

PURPOSE: This work describes a stepwise surgical approach to draining choroidal detachments and 2 cases for which this approach was used.
METHODS: The first step involves insertion of an anterior chamber maintainer and a nonvalved 23- or 25-gauge trocar cannula at the highest peak of hemorrhagic choroidal detachment (as determined using B-scan ultrasonography), 6 to 8 mm from and angled 20° to 30° toward the limbus. The second step involves removal of the trocar to expose the sclerotomy. Alternatively, the second step can be insertion of a second trocar. The third step involves the creation o

PURPOSE: To describe two cases of medium-sized uveal melanoma presenting with hemorrhagic choroidal detachments.
OBSERVATIONS: The first case is a 39-year-old man who presented with choroidal hemorrhage and angle closure glaucoma. The second case is a 42-year-old man who presented with choroidal hemorrhage and posterior scleritis. Vitrectomy with transvitreous fine needle aspiration biopsy was ultimately required to diagnose malignant uveal melanoma in each case.
CONCLUSIONS AND IMPORTANCE: Intraocular hemorrhage is a rare presenting sign of uveal melanoma. When it does occur, it is typicall

PURPOSE: To describe a patient with Osler-Rendu-Weber syndrome who developed a nonsimultaneous intraoperative choroidal hemorrhage in each eye.
METHOD: Interventional case report. A 65-year-old Caucasian woman with Osler-Rendu-Weber syndrome developed a choroidal hemorrhage in the left eye during vitrectomy for a complicated retinal detachment with a poor visual outcome. Fifteen years later, she developed a macula on retinal detachment in the right eye, which also had a dense cataract. Immediately after uncomplicated phacoemulsification and intraocular lens implantation, under monitored anest

Limited choroidal hemorrhage was observed in eight (2.2%) of 368 eyes subjected to extracapsular cataract extraction, compared with a rate of 3.07% in our previous study, in which the technique employed was intracapsular extraction. The difference was found to be significant only for eyes with high myopia. Myopia was found to be the main risk factor for the development of limited choroidal hemorrhage, especially when general anesthesia was used. The presence of glaucoma or the combination of extracapsular cataract extraction and trabeculectomy may increase the susceptibility to this complicati

In the elderly patient with a massive vitreous hemorrhage in one eye and senile macular choroidal degenerative changes in the fellow eye, a hemorrhagic disciform detachment causing this massive vitreous hemorrhage must be considered in the differential diagnosis. Ultrasonography selectively combined with radioactive phosphorus (32P) testing differentiates between a hemorrhagic disciform detachment or disciform scar and a malignant melanoma of the choroid. The ultrasonographic findings of a hemorrhagic disciform detachment may vary, but usually the posterior pole lesion is relatively flat and b

A 45-year-old white man had a bulbar subconjunctival mass, eyelid edema, and choroidal indentation. Biopsy of the subconjunctival mass revealed acute and chronic inflammation. One year later a spontaneous vitreous hemorrhage, choroidal folds, increased choroidal indentation, and serous retinal detachment were present. B-mode ultrasonography, operative findings, and biopsy revealed a chronic inflammatory mass with fibrosis confined to Tenon's capsule. Initial treatment with systemic corticosteroids resulted in no improvement, but mild progression occurred when the corticosteroids were tapered.

PURPOSE: To describe the intraoperative management of choroidal effusion (CE) or suprachoroidal hemorrhage (SCH) during cataract surgery with the phacoemulsification technique.
METHODS: The study is a retrospective interventional study through which we describe the intraoperative management adopted in 6 cases of CE or SCH during cataract surgery. The study involved 6,400 eyes (phacoemulsification) in 6 years observational time (incidence rate 0.094%). The surgical time at which these complications happened differed: nucleolus phacoemulsification in 2 eyes, cortex removal by bimanual irrigatio

Intraventricular hemorrhage (IVH), which afflicts thousands of people of all ages every year, frequently results in the development of communicating hydrocephalus. Classically, IVH-induced hydrocephalus has been attributed to reduced resorption of cerebrospinal fluid (CSF) due to dysfunction of arachnoid granulations, but this explanation may be incomplete. We hypothesized that IVH would cause inflammation of the choroid plexus and of the ependymal lining of the ventricles, resulting in dysfunction of these barrier cells. Barrier dysfunction, in turn, would be expected to cause an increase in

Purpose: To report a rare case of spontaneous suprachoroidal hemorrhage in polypoidal choroidal vasculopathy (PCV) and propose a possible pathophysiologic cascade with associated clinical warning signs. Methods: A single observational case was reviewed. Results: A 66-year-old man with PCV developed sequential massive submacular hemorrhage, breakthrough vitreous hemorrhage, non-appositional choroidal elevation, and ultimately appositional suprachoroidal hemorrhage with angle closure and ocular hypertension. Despite treatment with posterior sub-Tenon triamcinolone and conservative management, th
