Role of an outpatient clinic in screening chronic complications of diabetes: a model for diabetes managed care.

The purpose of this study was to evaluate the need for an outpatient clinic for screening chronic complications of diabetes mellitus and to explore the major risk factors for such complications. A total of 558 patients (293 men and 265 women, aged 61.4 +/- 10.0 yr) with non-insulin-dependent diabetes mellitus were recruited. All examinations were performed in all patients except for those with previously known complications. A nonmydriatic fundus camera was used to detect retinopathy. Microalbuminuria was detected with a semiquantitative method. A monofilament, semiquantitative tuning fork and

An evidence map of systematic reviews on models of outpatient care for patients with chronic heart diseases.

BACKGROUND: Chronic heart disease affects millions of people worldwide and the prevalence is increasing. By now, there is an extensive literature on outpatient care of people with chronic heart disease. We aimed to systematically identify and map models of outpatient care for people with chronic heart disease in terms of the interventions included and the outcomes measured and reported to determine areas in need of further research. METHODS: We created an evidence map of published systematic reviews. PubMed, Cochrane Library (Wiley), Web of Science, and Scopus were searched to identify all re

Therapeutic innovation in high-prevalence chronic diseases: Challenges and opportunities for specialist care models.

Therapeutic innovation is expected to change if not disrupt present care models for several chronic diseases in the coming years, as suggested by recent clinical trials. New drugs that anticipate and possibly delay the full expression of a disease will likely face some common challenges, such as the need of designing and implementing large scale interventions; the necessary engagement of multiple specialties for both diagnosis and treatment; the shift from specialist to non-specialist interventions and secondary prevention. Building on the case of HCV and other innovation in hepatology, we dis

The Chronic Kidney Disease Care Model: Quantifying Management Strategies for Improving Patient Outcomes in the USA.

PURPOSE: While chronic kidney disease (CKD) afflicts an estimated 14% of US adults, the majority remain unaware of the ailment until later stages. CKD progression can lead to reduced quality of life, cardiovascular disease (CVD), and end-stage renal disease (ESRD). Consensus guidelines highlight novel interventions for the early recognition and management of CKD. Here, we describe the rationale and structure of a policy model for CKD that provides contemporary estimates of long-term effectiveness and cost-effectiveness of medical interventions. The utility of the model is illustrated by assess

Navigating diabetes care inequities: an observational study linking chronic care model's structural elements to process and outcomes of type 2 diabetes care in Belgium.

BACKGROUND: Although the Chronic Care Model (CCM) provides the essential structural components of practice organisation to deliver high-quality type 2 diabetes (T2D) care, little is known about which of its elements are most important, and the extent to which it may reduce social inequities in the quality of T2D care. This study aims to assess the association between the implementation of CCM's structural elements and the quality of T2D care processes and outcomes in Flanders (Belgium), paying specific attention to differences by patients' socioeconomic vulnerability. METHODS: We developed a

Economic sustainability of a comprehensive care health service delivery model for medicaid eligible children with chronic conditions.

Healthcare programs often begin with funds from federal grants that carry with them the expectation that successful programs will become economically self-sustaining. This article describes one such program for lower income children and adolescents with chronic medical conditions that began as a demonstration program developed with federal funds and shows ways in which it became economically sustainable through following principles based in the literature. In addition, we show that partnerships with hospitals and health care systems are possible ways to sustain programs. We hope that the illus

Development of a collaborative chronic care model for management of cardiometabolic disease in low- and middle-income countries.

INTRODUCTION: Cardiometabolic diseases (CMD) which include cardiovascular disease (CVD), diabetes, hypertension, and other metabolic syndromes represent a significant global health burden. Three quarters of global CVD deaths occur in low-and-middle-income countries (LMICs) and CMD account for approximately 35 percent of deaths in the Sub-Saharan Africa (SSA) region. The COVID-19 Pandemic significantly accelerated the transformation of the landscape in the management of patients with multiple long-term conditions, prompting innovation in healthcare delivery and highlighting the importance of mo

Global nurse navigation models in chronic disease care: A scoping review and conceptual framework development.

BACKGROUND: Nurse Navigator (NN) models have expanded globally to address rising chronic disease burdens and patients' unmet psychosocial and sociocultural needs. Variations in health system structures have produced diverse NN models across countries. PURPOSE: This scoping review examined how NN models are implemented in chronic disease care and identified shared and divergent features. METHODS: Using JBI methodology and PRISMA-ScR reporting, a systematic search of five databases was conducted in March 2025. Guided by Vanasse's multidimensional care-trajectory model (Vanasse et al., 2018), 3

From targets to real world impact: a review of models of care for chronic HBV and HCV in Europe.

Hepatitis B virus (HBV) and hepatitis C virus (HCV) remain public health threats in the WHO European region, where an estimated 29 million people live with chronic infection and viral hepatitis-related deaths now surpass those from HIV/AIDS and tuberculosis combined. Although effective prevention tools and antiviral treatments reduce the risk of complications, overall mortality has not declined. This Series paper reviews models of care (MoC) implemented between 2015 and 2025, drawing on scientific literature and policy documents to assess regional progress. Simplified testing and treatment, ch

Leveraging large language models to inform paediatric chronic condition care: a cross-sectional study.

This study assessed how ChatGPT 3.5, ChatGPT 4.0 and Google Gemini perform in providing educational content about coeliac disease and type 1 diabetes mellitus. We analysed 76 frequently asked questions for accuracy, comprehensiveness, readability and consistency. The models delivered highly accurate and comprehensive responses across the board. While ChatGPT 4.0 offered the most readable content, all models struggled with overall readability. Each model maintained consistent performance throughout testing. These results indicate that large language models show promise as supplementary tools fo

Effectiveness of Models Used to Deliver Multimodal Care for Chronic Musculoskeletal Pain: a Rapid Evidence Review.

BACKGROUND: Primary care providers (PCPs) face many system- and patient-level challenges in providing multimodal care for patients with complex chronic pain as recommended in some pain management guidelines. Several models have been developed to improve the delivery of multimodal chronic pain care. These models vary in their key components, and work is needed to identify which have the strongest evidence of clinically-important improvements in pain and function. Our objective was to determine which primary care-based multimodal chronic pain care models provide clinically relevant benefits, def

Prophylactic antibiotics in chronic pediatric lung disease: balancing benefit, resistance, and future care models.

INTRODUCTION: Children with chronic suppurative or inflammatory lung diseases, including cystic fibrosis (CF), non-CF bronchiectasis (NCFB), chronic suppurative lung disease (CSLD), and primary ciliary dyskinesia (PCD), experience recurrent infection and persistent neutrophilic airway inflammation that together drive progressive lung injury. Prophylactic antibiotic strategies have therefore been explored to interrupt this infection - inflammation cycle, encompassing both pathogen-specific approaches and longer-term anti-exacerbation or immunomodulatory therapies. AREAS COVERED: We review clin

Rethinking Environmentally Sustainable Chronic Care: A Canadian Case Study of Funded Small House Models.

Canada's healthcare system faces increasing strain as patients who could be managed in alternative levels of care (ALCs) occupy acute hospital beds. The small house model (SHM) offers an opportunity to alleviate the growing burden on hospitals and long-term care facilities while supporting environmentally sustainable care. This case study quantifies the greenhouse gas (GHG) footprint of two facilities in Hamilton, Ontario. Annual emissions were 1,612 kg CO2e/bed and 512 kg CO2e/bed, respectively, with travel contributing significantly to emissions. Our study provides a baseline GHG footprint o

The Dutch lymphedema guidelines based on the International Classification of Functioning, Disability, and Health and the chronic care model.

Lymphedema is a chronic and progressive condition due to an imbalance between lymphatic filtration and transport capacity. A relative overload of fluid is caused by lymphatic impairment (afterload impairment); an absolute overload is caused by increased filtration (preload raised). Lymphedema can result in considerable disability and loss of quality of life. The Dutch Society of Dermatology organized a task force to create guidelines using the International Classification of Functioning, Disability, and Health. As lymphedema is a chronic condition, a new approach was chosen according to the ch

Hospital Readmission in Stroke Survivors in Social Vulnerability: Predictive Modeling with Machine Learning from the Perspective of the Chronic Conditions Care Model.

Hospital readmission among stroke survivors is frequent, especially in contexts of social vulnerability, compromising recovery and overburdening health services. This study aimed to develop a predictive model of hospital readmission among socially vulnerable stroke survivors, based on the Chronic Conditions Care Model (CCCM). Machine learning algorithms were applied, specifically decision tree and logistic regression, with data split into training (70% and 80%) and testing (30% and 20%) sets. Analyses were conducted using Python, with accuracy evaluated through ROC curves, AUC, and the confusi

Factors associated with self-care in people with advanced chronic kidney disease and their experience of care in Catalonia: an application of Leventhal's self-regulatory model.

BACKGROUND: Although evidence has demonstrated the influence of perceptions of chronic kidney disease on health outcomes in patients, there are still gaps in our knowledge regarding how such perceptions manifest themselves in specific populations with advanced chronic kidney disease (ACKD). This study aims to investigate the cognitive and emotional processes of people with ACKD in relation to their illness, as well as to examine their coping strategies and experiences of care in the context of outpatient care in the southern metropolitan area of Barcelona (Spain).
