Two hundred unselected patients with chronic idiopathic urticaria were treated with adequate doses of conventional antihistamines given at frequent intervals; 98.5% were completely freed of disease while on therapy. In three cases, the lesions subsided but did not clear completely. The addition of 800 mg of cimetidine in divided doses cleared the lesions in two cases, and in the third case the dose of H1 blocker was increased, which resulted in complete clearance of wheals and symptoms. Addition of H2 blockers play a role, but only in a very small percentage of patients with chronic idiopathic

A 31-year-old black male experienced severe recurrent urticaria and angioedema during meals. An abnormal sialographic pattern indicated submandibular sialoadenitis, and surgical excision of the gland resulted in complete resolution of the urticaria. This report describes the association of sialoadenitis and urticaria and possible mechanisms involved.

Results in the treatment with Ketotifen of 36 cases of chronic Urticaria are exposed. Patients between ten and fifty years old, and with symptoms chronicity during from two until ten years, were studied. Chronic cases of unknown etiology, cholinergic urticaria, salicylates urticaria, urticaria by pressure, local heat urticaria and chronic dermographism were treated. Results were hopeful. Especially in patients with cholinergic urticaria, urticaria by heat, salicylates

Chronic urticaria is differentiated from the acute type by its persistence and by its causative agents. It can result from histamine, and other chemical mediators, released from damaged mast cells either by an antigen-antibody reaction or some other mechanism. The cause, often elusive, may be common foods or medications. External temperature changes can be manifested by urticaria through various mechanisms. The endogenous causes include chronic foci of infections such as in teeth, sinuses or tonsils, endocrine changes, parasitic infestations. Chronic urticaria may be a symptom of underlying mo

Urticaria is defined as the sudden appearance of erythematous, itchy wheals of variable size, with or without angioedema (AE) (swelling of the deeper layers of the skin). Its classification depends on time course of symptoms and the presence of eliciting factors. When it lasts less than 6 weeks it is classified as acute urticaria (AU), and if the symptoms persist for more than 6 weeks, it is classified as chronic urticaria (CU). Current International Guidelines also classify CU as chronic spontaneous urticaria (CSU) and inducible urticarial, according to the absence or presence of environmenta

OBJECTIVE: Chronic urticaria (CU) is a common, heterogeneous, and debilitating disease. Antihistamines and omalizumab are the mainstay therapies of CU. Additional treatment options are needed. Here, we review the off and beyond label use of licensed drugs, novel treatments that are currently under development, and promising new targets.
DATA SOURCES: MEDLINE was searched for recent reports of the successful use of treatments in CU and promising targets for the development of novel treatment options. We also searched ClinicalTrials.gov for recent and ongoing randomized clinical trials in CU.

Histamine is a key inflammatory player in the pathogenesis of urticaria, a mast-cell-driven disease characterized clinically by the development of wheals, angioedema, or both. Changes to the management of chronic spontaneous urticaria have recently been adopted due to increasing literature surrounding the efficacy and safety of up-dosing modern second-generation H1-antihistamines and the use of omalizamub, a biologic agent, as a third-line treatment. Given the prevalence of chronic urticaria and its impact on quality of life, this editorial aims to provide a summary of the proposed updated gui

GOAL: Idiopathic chronic urticaria may be associated to other auto-immune diseases, in particular thyroiditis. The goal of our study is to show that this association is higher than the incidence of auto-immune thyroiditis in the general population.
METHODS: It is a retrospective observational study including 90 patients with chronic urticaria. We calculated the proportion of patients having associated auto-immune thyroiditis based on a level of anti-TPO >220 mUI/ml.
RESULTS: The mean age of patients was 36.7 years with a sex ratio F/H of 2.9/1. The anti-TPO antibodies were positive in 16 pat

BACKGROUND: Chronic urticaria is a common condition encountered in general practice and a frequent source of referral to the clinical immunologist, allergist and dermatologist.
OBJECTIVE: This article discusses the assessment and management of chronic urticaria in the general practice setting.
DISCUSSION: Chronic urticaria is defined as the occurrence of transient wheals lasting more than 6 weeks in duration. In 80% of cases, a cause is not identified and this is classified as chronic idiopathic urticaria. A physical trigger, vasculitis or systemic disease account for a smaller proportion of

BACKGROUND: In 1769, William Cullen introduced the word "urticaria" (transient edematous papules, plaque with itching). Urticaria affects 15-25% of people at least once in their life time. It is a clinical reaction pattern triggered by many factors causing the liberation of vasoactive substances such as histamine, prostaglandins and kinins. Urticaria is classified according to its duration into acute (< 6 weeks duration) and chronic (>6 weeks duration). Various clinical investigations may be initiated to diagnosis the cause.
AIMS: To evaluate the types of chronic urticaria with reference to e

Chronic urticaria (CU) is a common condition faced by the practicing allergist. There is a considerable degree of variation in the evaluation of patients with CU. The intent of this article is to provide the practicing allergist with an appropriate, evidence-based strategy for diagnosis and initial management of patients with CU. A review was performed of pertinent literature of CU pertaining to its natural history, impact on quality of life, subtypes, utility of diagnostic tests, and initial treatment recommendations. The reported duration of CU varies based on the nature of the referring cen

Chronic urticaria is a disease consisting of spontaneous pruritic welts, present on all or most days for more than 6 weeks. It is commonly supposed to be allergic in origin, although allergy is not the cause in the majority of cases, and it has therefore been termed 'chronic idiopathic urticaria'. Recent evidence indicates that at least a subset of patients in whom no extrinsic or internal cause can be identified are in fact autoimmune in origin. This is based mainly on the detection of pathogenic autoantibodies to the high-affinity immunoglobulin E receptor FcepsilonR1, which are thought to a

BACKGROUND: Chronic idiopathic urticaria (CIU) is a common and frustrating disorder defined as a regular recurrence of hives for a period of more than 6 weeks, and has a major impact on patients' daily quality of life--H1 receptor antagonists are considered to be particularly effective in reducing pruritus, and are recommended as first-line treatment in patients with CIU. However, the efficacy of the treatment is insufficient.
OBJECTIVE: To evaluate the efficacy of desloratadine in CIU in Israeli patients.
METHODS: An open label multicenter study in 10 centers in Israel. Upon completion of 3
