Self-measurement and ambulatory monitoring of blood pressure: a subject's chronobiological perspective.

Population studies show the direct benefit from self-measurement for guiding the treatment of patients with severe hypertension; they also separate groups of children with a positive vs. negative family history of high blood pressure, despite the difficulty of obtaining valid readings during the rest/sleep span, which are needed for a reliable estimation of the circadian parameters. From the viewpoint of preventive maintenance, a 67-year-old woman monitored her blood pressure and heart rate around the clock for 7 days, using concomitantly an ambulatory monitor and a manual instrument. Self-mea

Biology of mammalian photoperiodism and the critical role of the pineal gland and melatonin.

In mammals, photoperiodic information is transformed into a melatonin secretory rhythm in the pineal gland (high levels at night, low levels during the day). Melatonin exerts its effects in discrete hypothalamic areas, most likely through MT1 melatonin receptors. Whether melatonin is brought to the hypothalamus from the cerebrospinal fluid or the blood is still unclear. The final action of this indoleamine at the level of the central nervous system is a modulation of GnRH secretion but it does not act directly on GnRH neurones; rather, its action involves a complex neural circuit of interneuro

[Biological rhythm and thromboembolic disease. Physiological, epidemiological and pharmacological aspects].

Arterial and venous thromboembolic disorders are the leading cause of death in most of the advanced nations. The study of physiologic, epidemiologic and pharmacologic relationships of these disorders to biological rhythms, may lead to a better understanding and perhaps a better treatment. Chronophysiologic studies have shown that hemostatic variables follow circadian rhythms. The level of platelets aggregation and of blood coagulation has been found to be increased in the morning and decreased at night, whereas fibrinolytic activity is lower in the morning than in the evening. Chronoepidemiolo

Circadian pacemaker interferes with sleep onset at specific times each day: role in insomnia.

The human circadian pacemaker modulates our desire and ability to fall asleep at different times of day. To study this circadian component of sleep tendency, we have analyzed the sleep-wake patterns recorded from 15 free-running subjects in whom the sleep-wake cycle spontaneously desynchronized from the circadian rhythm of body temperature. The analysis indicates that the distribution of sleep onsets during free run is bimodal, with one peak at the temperature trough and, contrary to previous reports, a second peak 9-10 h later. Furthermore, there are two consistent zones in the circadian temp

[Structural alterations during the course of glaucoma disease].

BACKGROUND: Structural changes in the course of glaucoma disease affect the trabecular meshwork and ciliary body in addition to the optic disc as the primary site of glaucoma damage. OBJECTIVES: Latest results from experimental studies, animal models and measurements in human eyes are presented and discussed. RESULTS: The presenting scenario is complex with age, biochemical and mechanical stress factors leading to subsequent, irreversible tissue change in the trabecular meshwork and cribriform plate of the optic nerve, resulting in neuronal tissue loss. CONCLUSION: Knowledge of these change

Chronobiology of Natriuretic Peptides and Blood Pressure in Lean and Obese Individuals.

BACKGROUND: Diurnal variation of natriuretic peptide (NP) levels and its relationship with 24-h blood pressure (BP) rhythm has not been established. Obese individuals have a relative NP deficiency and disturbed BP rhythmicity. OBJECTIVES: This clinical trial evaluated the diurnal rhythmicity of NPs (B-type natriuretic peptide [BNP], mid-regional pro-atrial natriuretic peptide [MR-proANP], N-terminal pro-B-type natriuretic peptide [NT-proBNP]) and the relationship of NP rhythm with 24-h BP rhythm in healthy lean and obese individuals. METHODS: On the background of a standardized diet, healthy

The Early Worm Catches the Bird? Productivity and Patterns of Trichobilharzia szidati Cercarial Emission from Lymnaea stagnalis.

Digenean trematodes are common and abundant in aquatic habitats and their free-living larvae, the cercariae, have recently been recognized as important components of ecosystems in terms of comprising a significant proportion of biomass and in having a potentially strong influence on food web dynamics. One strategy to enhance their transmission success is to produce high numbers of cercariae which are available during the activity peak of the next host. In laboratory experiments with 13 Lymnaea stagnalis snails infected with Trichobilharzia szidati the average daily emergence rate per snail was

[Basic mechanisms of chronopharmacology].

Many animal and human studies have demonstrated time-dependent changes in the effects and kinetics of drugs that can be explained in part by parallel changes in their absorption, metabolism and excretion. Although food influences drug absorption, its presence in the stomach does not explain the circadian rhythms in the absorption of xenobiotics. Circadian rhythms in blood flow, biliary secretion and the physico-chemical properties of drugs seem to be very important factors involved in the circadian rhythms of drug absorption. Time-dependent changes have been reported in the activity of hepatic

Seasonal variation in occurrence of pulmonary embolism: analysis of the database of the Emilia-Romagna region, Italy.

Seasonal variation in the occurrence of cardiovascular and cerebrovascular events, including pulmonary embolism (PE), has been reported; however, recent large-scale, population-based studies conducted in the United States did not confirm such seasonality. The aim of this large-scale population study was to determine whether a temporal pattern in the occurrence of PE exists. The analysis considered all consecutive cases of PE in the database of all hospital admissions of the Emilia Romagna region in Italy at the Center for Health Statistics between January 1998 and December 2005. PE cases were

Melatonin in the multi-oscillatory mammalian circadian world.

In mammals, the complex interaction of neural, hormonal, and behavioral outputs from the suprachiasmatic nucleus (SCN) drives circadian expression of events, either directly or through coordination of the timing of peripheral oscillators. Melatonin, one of the endocrine output signals of the clock, provides the organism with circadian information and can be considered as an endogenous synchronizer, able to stabilize and reinforce circadian rhythms and to maintain their mutual phase-relationship at the different levels of the circadian network. Moreover, exogenous melatonin, through an action o

Melatonin as a chronobiotic.

Melatonin, hormone of the pineal gland, is concerned with biological timing. It is secreted at night in all species and in ourselves is thereby associated with sleep, lowered core body temperature, and other night time events. The period of melatonin secretion has been described as 'biological night'. Its main function in mammals is to 'transduce' information about the length of the night, for the organisation of daylength dependent changes, such as reproductive competence. Exogenous melatonin has acute sleepiness-inducing and temperature-lowering effects during 'biological daytime', and when

Temporal organization of the brain: Neurocognitive mechanisms and clinical implications.

The synchrony between the individual brain and its environment is maintained by a system of internal clocks that together reflect the temporal organization of the organism. Extending the theoretical work of Edelman and others, the temporal organization of the brain is posited as functioning through "re-entry" and "temporal tagging" and binds the wide range of possible times to a unified cognitive experience which is held in unison with the outside world. Dysfunction in this system is reflected in the temporal discord seen in cases of aging, sleep disorder, jet-lag, and shift-work, as well as i

[Sub-circadian rhythms as an instrument for the determination of and prediction for the state of the organism].

The paper deals with the analysis of literature concerning the role and hierarchy of circadian rhythms in the structure of rhythmic processes in organism. Substantiation is given to the concept according to which the diagnostic and prognostic value of subcircadian rhythms is predetermined by their wide representation at different levels of the living system organization, high sensitivity to external impacts, and responsiveness to all sorts of shifts in the state of organism. The above considerations are used in the physiology of labor, experimental and clinical medicine. Illustration can be de

Chronobiological approaches to Alzheimer's disease.

Dynamic circadian rhythms contribute to memory formation, and the hormonal and neurochemical changes that follow circadian patterns are frequently dysregulated with aging. The effect of aging on circadian rhythms is a double-edged sword; on one hand, poor sleep quality compromises neuronal structure and function in regions that support cognition, and on the other hand, perturbation of central and peripheral oscillators changes the hormonal milieu, with consequences for neuroplasticity. In the current review, recent developments surrounding the circadian regulation of memory formation are descr

[Functional disorders in the chronological progression of glaucoma].

BACKGROUND: Glaucomatous optic neuropathy is a complex process, which not only leads to destruction of the retinal ganglion cells but also negatively influences the third and fourth neurons. Therefore, in histological studies of glaucoma a significant cell loss in both the lateral geniculate nucleus and in the visual cortex could be detected. OBJECTIVE: The involvement of the entire visual pathway in glaucoma leads to a variety of functional disturbances, which are presented in this overview. RESULTS: Besides the well-known visual field defects in glaucomatous optic neuropathy, additional di

Breaking heart: chronobiologic insights into takotsubo cardiomyopathy.

A considerable amount of evidence has shown that the major acute cardiovascular diseases, ie, myocardial infarction, sudden cardiac death, stroke, pulmonary embolism, and rupture or dissection of aortic aneurysms do not occur randomly in time, but exhibit specific temporal patterns in their onset, according to time of day, month or season, and day of the week. This contributes to the definition of "chronorisk", where several factors, not harmful if taken alone, are capable of triggering unfavorable events when presenting all together within the same temporal window. This article reviews the ac

[The role of the epiphysis in the pathogenesis of depressions].

On the pinealectomized rats shifts of main parameters of forced swimming and its time course with increase of rhythmical depression index were observed. Pinealectomy also potentiated depressogenic effect of reserpine which was attenuated by melatonin. At the same time on this model pineal compounds (melatonin, peptides) after acute and chronic administration demonstrated their own antidepressant activity.

Is there symmetry in motor imagery? Exploring different versions of the mental chronometry paradigm.

Motor imagery and motor execution share similar processes. However, only some factors that affect motor execution affect motor imagery in the same way. We investigated whether bimanual coordination constraints (parallel movements are performed slower than symmetric movements) are observed in motor imagery and whether the way of implementing the mental chronometry paradigm, which is used to investigate motor imagery, influences the results. Participants imagined and executed repetitive symmetric and parallel bimanual movements in three different tasks. Participants performed a certain number of

[The chronobiological peculiarities of the hepatobiliary system function in chronic cholecystitis with dysfunction of the gallbladder].

Study of biological cycles in chronic cholecystitis with dysfunction of gallbladder has a great practical importance. The results of chronoanalysis testify a dominance of 24-hours cycle in structure of cycles of functional activity of organs of hepatobiliary system. The comparative analysis of trust intervals reveals the intensification of internal connections, expressed intensification of synchronization of cycles of functional activity of system and a breach of synchronism of central regulation and self-regulation, testifying overstrain of proper adaptive possibilities and unconcordance in s
