Effects of cisapride on QTc interval in term neonates.

BACKGROUND: Cisapride administration for 48 hours has been shown to increase heart rate corrected QT (QTc) interval in preterm neonates. Accumulation of the drug because of liver enzyme immaturity has been suggested to be the reason. If this is correct, a longer survey of QTc interval should disclose an increase even in term neonates. OBJECTIVE: A prospective survey of the effects of cisapride on QTc interval in term neonates administered cisapride. SETTING: Neonatal Unit of the University Hospital of Dijon, France. DESIGN: QTc interval was determined just before and 48 hours, seven days, a

[Lengthening of the QT-interval in a newborn treated with cisapride].

In a 7-week-old female infant a prolonged QTc interval, up to 485 ms, was measured and attributed to the use of the prokinetic agent cisapride. The girl was born at 32 1/7 weeks of gestational age, started to use cisapride (0.8 mg/kg/day) at the age of 7 weeks because of gastro-oesophageal reflux. After cessation of cisapride therapy, the QTc interval returned to normal. A normal QTc interval had also been observed before the use of cisapride. Because of the risk of QTc prolongation and possibly cardiac arrhythmias during the use of cisapride by newborn infants, electrocardiography should be p

Cisapride and caesarean section: their role in babies with gastroschisis.

OBJECTIVE: The objective of this study was to compare the neonatal postoperative course and morbidity for patients with gastroschisis who received cisapride with those who did not receive cisapride. STUDY DESIGN: Data were obtained by review of the medical records of all the patients with gastroschisis who were admitted to Sydney Children's Hospital between January 1984 and December 1995. Data were compared between 15 babies who received cisapride with 27 who did not. The mode of delivery and outcome of babies in whom gastroschisis was diagnosed antenatally was compared with those who were di

Syncopal episodes associated with cisapride and concurrent drugs.

OBJECTIVE: To report a case of QT prolongation and syncopal episodes resulting from concomitant use of cisapride and agents known to inhibit its metabolism. CASE SUMMARY: A 53-year-old white woman was involved in two motor vehicle accidents on the same day after experiencing syncopal episodes. Cardiac and neurologic evaluations were negative; the syncopal episodes were attributed to QT prolongation associated with the concomitant use of cisapride and agents known to inhibit its metabolism. DISCUSSION: This is the first case published in the English-language literature describing QT prolongat

QT interval in children and infants receiving cisapride.

OBJECTIVES: Life-threatening arrhythmias and prolonged QT interval have been reported in adults and children using cisapride, a medication structurally similar to procainamide. Premature infants have reduced activity of cytochrome p-450, the system responsible for metabolism of cisapride, which could lead to QT prolongation. Therefore, we prospectively studied premature infants and children receiving cisapride to analyze the effect of prolonged cisapride therapy on QT interval. DESIGN: Premature infants in a neonatal intensive care unit and children seen at a pediatric gastroenterology clinic

Cisapride as a treatment for gastroparesis in traumatic brain injury.

Gastroparesis is a well-documented finding among patients with severe traumatic brain injuries. The treatment of choice has been metoclopramide, but it is less than ideal given its central dopaminergic blocking activity. Cisapride is a newer prokinetic agent without side effects on the central nervous system and might be a superior treatment. The case of a healthy 22-year-old man who suffered a severe traumatic brain injury and multiple trauma in a motor vehicle accident is reported. The patient required gastrostomy tube placement but developed recurrent aspiration pneumonia once feedings were

Cisapride compared with ranitidine in the treatment of functional dyspepsia.

OBJECTIVE: To compare the efficacy of the prokinetic drug cisapride and the antisecretory agent ranitidine in relieving symptoms of functional dyspepsia, as well as their effect on the recurrence of symptoms after the discontinuation of treatment. DESIGN: A randomized double-blind parallel-group trial of cisapride 30 mg daily and ranitidine 300 mg daily given for 2, 4 or 8 weeks, followed by a 4-week drug-free follow-up of the patients with a good or excellent response. Cisapride in the treatment of gastro-oesophageal reflux disease.

Prokinetic agents are being used increasingly in medical therapy for gastro-oesophageal reflux disease (GERD). This study examined the effect of 10 mg q.d.s., oral cisapride, or placebo, taken for 12 weeks, on 48 patients with symptoms and endoscopic evidence of GERD. Objective evaluation of benefit was obtained by endoscopy and biopsy, oesophageal manometry, acid reflux provocation test and 24-h oesophageal pH monitoring. Cisapride significantly increased lower oesophageal sphincter pressure (P = 0.003) against baseline and also against placebo, in patients (n = 9) with an hypotensive lower o

Effects of cisapride on isolated guinea pig colon.

Effects of cisapride were studied on the whole segments and muscle strips isolated from the guinea pig ascending colon. In whole segment preparations, external application of cisapride (0.4-10 microM) produced relaxation and inhibited spontaneous and evoked intraluminal pressure changes. Intraluminal application (1-10 microM) increased the pressure response. In many circular muscle strips (18 preparations out of 20), cisapride (0.3-1 microM) increased the mechanical activity, particularly when the pH was lowered, associated with an increased spike activity. However, this action was relatively

Cisapride improves nicotine-evoked antral hypomotility in smokers.

It has recently been shown that nicotine administration reduces antral motor function. The aim of this study was to test whether cisapride administration can improve nicotine-evoked antral hypomotility in humans. Eight healthy smoking volunteers were selected for the study. After an overnight fast, manometric recordings and electrogastrographic recordings were performed before and after the ingestion of a mixed solid meal. The measurements (manometric and electrogastrographic protocols) were carried out on two separate days. In each protocol, the measurements were made under three different co

Healing and prevention of relapse of reflux oesophagitis by cisapride.

Altogether, 138 patients were included in a study aimed at evaluating the effect of cisapride on healing and relapse of oesophagitis shown endoscopically. In the first phase of the study cisapride was given in an open fashion at 10 mg four times a day for 8 to 16 weeks, and healing was obtained in 69% of patients. Healing occurred later in patients with grades II to IV oesophagitis. The total score for reflux symptoms decreased by 67%. Eighty of the healed patients were included in the second phase. They were randomly assigned to double blind treatment with either cisapride 10 mg (n = 37) or p

Twenty children (age range 75 days-47 months) with reflux oesophagitis entered a random double blind trial in which they received either Cisapride (Janssen Pharmaceutical Ltd), a new prokinetic agent, or an identical placebo syrup. Diagnosis of gastro-oesophageal reflux was made by measurement of intraluminal oesophageal pH combined with manometry. Oesophagitis was assessed in all patients by histological examination of mucosal specimens taken during oesophagogastroduodenoscopy. Manometry, pH test, and endoscopy with biopsy examination were repeated at the end of the treatment period. Seventee

Fat preload delays gastric emptying: reversal by cisapride.

1. Meals empty more slowly when they contain fat. 2. This study investigated whether an intragastric fat preload in comparison with a water preload affected gastric emptying of a semisolid meal and antral motor activity, and whether cisapride reversed such effects. 3. Twelve healthy subjects were studied under three conditions each: (A), preload of 50 ml water orally; (B) and (C), preload of 50 ml cream (20 g fat). After preloads, subjects reclined right sided for 20 min. Thereafter, placebo in conditions (A) and (B) and 10 mg cisapride in (C) were administered i.v. in a random double-blind fa
