Awareness and practices regarding malaria in clinical suspects and the alarming slide positivity rate amongst them: a hospital based study from Delhi.

It has been repeatedly reported that lack of effective community participation in malaria control strategies has been partly responsible for high incidence of malaria in India. Active involvement of community in malaria control is a function of the awareness of that community. The present study was conceived with the objective to study the awareness and practices regarding malaria among fever cases clinically suspected of malaria and to assess the slide positivity rate among the same. Awareness and practices of 101 consecutive clinically suspected malaria cases (presenting with fever) attendin

Effect of monitoring the onset of calving by a calving alarm thermometer on the prevalence of dystocia, stillbirth, retained fetal membranes and clinical metritis in a Hungarian dairy farm.

The objective of the present study was to assess the effectiveness of an intravaginal thermometer in the field prediction of the second stage of labor and to determine its impact on the health of dams and newborn calves. Holstein cows (n = 241) were randomly selected about 5 (mean ± SD: 4.7 ± 2.0) days before the expected date of calving and the thermometer was inserted into the vagina. Another 113 cattle served as controls. There was no false alarm during the experiment. The risk of dystocia (Score >1) was 1.9 times higher, the prevalence of stillbirth was 19.8 times higher, the risk of retai

From cutoff to causality: regression discontinuity design for evaluating acute kidney injury e-alerts.

In 2014, acute kidney injury electronic alerts were implemented across Wales based on a predefined increase in creatinine levels. In their recent paper, Xie et al. used a regression discontinuity design to evaluate the effectiveness of these electronic alerts. Threshold-based eligibility mimics randomization, and when all assumptions are met, regression discontinuity design can estimate causal effects from observational data. Regression discontinuity design can play an important role in evaluating health and medical interventions, particularly where trials are not feasible or do not reflect re

False-positive alarms in patients with implantable loop recorder followed by remote monitoring: A systematic review.

Remote Monitoring (RM) has been shown to provide useful information about arrhythmic events in patients with implantable loop recorders (ILRs), however there is few and conflicting data about the false positive (FP) alarms burden and characteristics among ILR recipients. The aim of the present systematic review was to evaluate incidence and characteristics of FP alarms among ILR patients followed by RM. We developed a systematic research in Embase, MEDLINE and PubMed databases and selected all papers focused on false positive ILR transmissions published from June 1, 2013 to June 1, 2023. Case

Novel approach to cardiac alarm management on telemetry units.

BACKGROUND: General medical-surgical units struggle with how best to use cardiac monitor alarms to alert nursing staff to important abnormal heart rates (HRs) and rhythms while limiting inappropriate and unnecessary alarms that may undermine both patient safety and quality of care. When alarms are more often false than true, the nursing staff's sense of urgency in responding to alarms is diminished. In this syndrome of "clinical alarm fatigue," the simple burden of alarms desensitizes caregivers to alarms. Noise levels associated with frequent alarms may also heighten patient anxiety and disru

Pattern discovery in critical alarms originating from neonates under intensive care.

Patient monitoring generates a large number of alarms, the vast majority of which are false. Excessive non-actionable medical alarms lead to alarm fatigue, a well-recognized patient safety issue. While multiple approaches to reduce alarm fatigue have been explored, patterns in alarming and inter-alarm relationships, as they manifest in the clinical workspace, are largely a black-box and hamper research efforts towards reducing alarms. The aim of this study is to detect opportunities to safely reduce alarm pressure, by developing techniques to identify, capture and visualize patterns in alarms.

Adaptive threshold-based alarm strategies for continuous vital signs monitoring.

Continuous vital signs monitoring in post-surgical ward patients may support early detection of clinical deterioration, but novel alarm approaches are required to ensure timely notification of abnormalities and prevent alarm-fatigue. The current study explored the performance of classical and various adaptive threshold-based alarm strategies to warn for vital sign abnormalities observed during development of an adverse event. A classical threshold-based alarm strategy used for continuous vital signs monitoring in surgical ward patients was evaluated retrospectively. Next, (combinations of) six

Health System Redesign of Cardiac Monitoring Oversight to Optimize Alarm Management, Safety, and Staff Engagement.

OBJECTIVE: The purpose of this quality improvement project was to improve health system patient safety by creating a cardiac monitoring structure aligned with national standards. BACKGROUND: Excessive alarms pose patient safety threats and are often false or clinically insignificant. The Joint Commission identified reduction of nonactionable alarms as a National Patient Safety Goal. METHODS: The conversion to structured monitoring occurred in 4 phases: 1) defining health system monitoring structure and processes; 2) co-create sessions; 3) implementation and impact analysis; and 4) ongoing ev

The Recognizability and Localizability of Auditory Alarms: Setting Global Medical Device Standards.

Objective Four sets of eight audible alarms matching the functions specified in IEC 60601-1-8 were designed using known principles from auditory cognition with the intention that they would be more recognizable and localizable than those currently specified in the standard. Background The audible alarms associated with IEC 60601-1-8, a global medical device standard, are known to be difficult to learn and retain, and there have been many calls to update them. There are known principles of design and cognition that might form the basis of more readily recognizable alarms. There is also scope fo

Physiologic monitoring alarm load on medical/surgical floors of a community hospital.

UNLABELLED: It has been known to the public that high frequency of false and/or unnecessary alarms from patient monitoring devices causes "alarm fatigue" in critical care. But little is known about the impact to care on the less acute patients located outside the critical care areas, such as the traditional medical/surgical (med/surg) floor. METHODS: As part of a larger population management study, we initiated continuous physiological monitoring to 79 beds of floor patients in a community hospital. In order to qualify the patient monitoring alarm load for subacute medical and surgical floor

The heuristics of nurse responsiveness to critical patient monitor and ventilator alarms in a private room neonatal intensive care unit.

AIM: Alarm fatigue is a well-recognized patient safety concern in intensive care settings. Decreased nurse responsiveness and slow response times to alarms are the potentially dangerous consequences of alarm fatigue. The aim of this study was to determine the factors that modulate nurse responsiveness to critical patient monitor and ventilator alarms in the context of a private room neonatal intensive care setting. METHODS: The study design comprised of both a questionnaire and video monitoring of nurse-responsiveness to critical alarms. The Likert scale questionnaire, comprising of 50 questi

Implementation and experience of an innovative smart patient care system: a cross-sectional study.

BACKGROUND: Although a patient care system may help nurses handle patients' requests or provide timely assistance to those in need, there are a number of barriers faced by nurses in handling alarms. METHODS: The aim of the study was to describe the implementation and experience of an innovative smart patient care system (SPCS). This study applied a cross-sectional descriptive design. We recruited 82 nurses from a medical center in Taiwan, with 25 nurses from a ward that had introduced an SPCS and 57 nurses from wards that used the traditional patient care system (TPCS). The major advantages o

Could In-Home Sensors Surpass Human Observation of People with Parkinson's at High Risk of Falling? An Ethnographic Study.

Self-report underpins our understanding of falls among people with Parkinson's (PwP) as they largely happen unwitnessed at home. In this qualitative study, we used an ethnographic approach to investigate which in-home sensors, in which locations, could gather useful data about fall risk. Over six weeks, we observed five independently mobile PwP at high risk of falling, at home. We made field notes about falls (prior events and concerns) and recorded movement with video, Kinect, and wearable sensors. The three women and two men (aged 71 to 79 years) having moderate or severe Parkinson's were de

Temporal Divergence of True and False Alarms in Pediatric Nocturnal Seizure Monitoring.

Seizure detection devices can improve the safety of children with epilepsy, yet high false alarms limit their clinical applicability. Here, we investigated whether specific time periods are disproportionately prone to false alarms. We analyzed 2876 NightWatch alarms occurring between 19:00 and 09:00 in 65 children (ages 3-16, 48% female). Temporal distributions differed significantly between true and false alarms (p < 0.001), with a significant divergence in skewness (p = 0.001); notably, 30% of false alarms occurred between 06:00 and 09:00. This suggests NightWatch performance is adversely af

No More False Alert: Contrastive Learning for Predicting Health Deterioration from Imbalanced Care Records.

In this paper, we propose an outcome-based contrastive loss for imbalanced binary classification to alert to next-day health deterioration using care records and meteorological data. Long-term care facilities maintain daily care and observation records to monitor the health of older adults. Such objective records are particularly valuable when sudden deterioration occurs, enabling timely coordination with medical institutions. Predicting deterioration one day in advance could provide care staff with an actionable window to intensify observation and adjust care plans (e.g., scheduling additiona
