Current and future status of adjuvant therapy for breast cancer.

Adjuvant systemic treatments have greatly improved the prognosis of women with early breast cancer. Combination chemotherapy and, for patients with oestrogen receptor-positive (ER+) tumours, endocrine treatment has been found to reduce the frequency of relapse and improve survival. New adjuvant strategies include the introduction of taxanes into adjuvant chemotherapy schedules, the use of aromatase inhibitors in place of, or in addition to, tamoxifen, and the use of adjuvant bisphosphonates. Combination chemotherapy has been found to reduce the annual odds of recurrence and death in pre- and p

Clodronate: The Influence on ATP Purinergic Signaling.

ATP is involved in numerous physiological functions, such as neurotransmission, modulation, and secretion, as well as in cell proliferation, differentiation, and death. While ATP serves an essential intracellular role as a source of energy, it behaves differently in the extracellular environment, where it acts as a signaling molecule capable of activating specific purinergic receptors (P2YRs and P2XRs) that modulate the response to ATP. Extracellular ATP signaling is a dynamic area of research, with particular interest in ATP's effects on inflammatory conditions and pain modulation. Clodronate

Plasma PTH and PTH-rP levels and clodronate therapy in cancer patients with hypercalcemia.

To investigate the roles of parathyroid hormone (PTH) and parathyroid hormone-related peptide (PTH-rP) in hypercalcemic patients with various malignancies, we measured the plasma levels of intact PTH and intact PTH-rP in five hypercalcemic patients with hematologic malignancies and 29 hypercalcemic patients with solid cancers. Ten eucalcemic cancer patients with either hepatocellular carcinoma or squamous cancer of the lung served as controls. The results showed a suppressed PTH level in all of the 34 hypercalcemic cancer patients. The PTH-rP levels were within normal limits (< 2.5 pg/mL) for

Non-purulent myositis caused by direct invasion of skeletal muscle tissue by Leptospira in a hamster model.

Myalgia is a common symptom of Leptospira infection in humans. Autopsies have reported that muscle tissue shows degeneration and necrosis of the myofibers and infiltration of inflammatory cells composed mainly of macrophages and lymphocytes. It remains unclear whether Leptospira directly infects the muscle and how the infiltrating inflammatory cells are involved in muscle fiber destruction. This study evaluated the relationship between histopathological changes and leptospiral localization in the muscle tissue of a hamster model. The influence of macrophages in skeletal muscle injury was also

Should bisphosphonates be standard therapy for bone pain?

We have been studying bisphosphonates since the early 1980s, initially investigating etidronate in the management of hypercalcaemia and, since the mid-1980s, clodronate in the management of hypercalcaemia, bone pain, and skeletal complications in patients with bone metastases. We have also recently reported that bone metastases can be prevented or delayed in patients without evidence of bone disease but with recurrent disease at other sites. Bisphosphonates are now the standard therapy for hypercalcaemia after rehydration. For patients with bone metastases and bone pain, a trial of clodronate

[[Clodronate therapy of hypercalcemia caused by hyperparathyroidism].

The diphosphonates, powerful agents which inhibit osteolysis, have been used parenterally with success to treat malignant hypercalcemia. 4 cases of anatomically confirmed primary hyperparathyroidism (age of patients 58-78 years) were admitted with a serum calcium of over 4 mmol/l in one case, over 3,5 mmol/l in a second case and over 3 mmol/l in the other two. The patients received an infusion of isotonic saline and dichloromethylene-diphosphonate (clodronate) parenterally in a dose of 500 mg a day. Serum calcium began to fall in all four patients 48 hours after the treatment was started. Intr

An evidence-based review of risk-reductive strategies for osteonecrosis of the jaws among cancer patients.

PURPOSE: Bone antiresorptive treatment is associated with osteonecrosis of the jaws. Interventions used to treat this complication are diverse, controversial, and largely empirical but certain risk factors could help in its avoidance. The aim of this evidence-based review is to elucidate any interventions that are effective in reducing the risk for development of ONJ in cancer patients receiving bone antiresorptive therapy and to quantify the effectiveness of such interventions. MATERIALS & METHODS: We searched MEDLINE, EMBASE, the Cochrane Central Register of Controlled Trials (CENTRAL), and

Secondary hyperparathyroidism associated with dichloromethane diphosphonate treatment of Paget's disease.

Six patients with extensive polyostotic Paget's disease were treated for 3 months with dichloromethane diphosphonate (CL2MDP; 1600 mg/day, orally). Serum alkaline phosphatase and urinary hydroxyproline excretion decreased 60-80% in each patient. Blood ionized calcium (ca++) and immunoreactive PTH (iPTH) were measured weekly during the first month of therapy and monthly thereafter. As an index of parathyroid function, PTH secretory reserve was assessed by EDTA infusions before treatment, at the end of treatment, and 3 months after Cl2 MDP therapy was stopped. Before therapy, iPTH and Ca++ were

Macrophage depletion with clodronate-containing liposomes affects the incidence and development of rheumatoid arthritis.

OBJECTIVE: This study investigated the effect of macrophage depletion with clodronate-containing liposomes (Clo-lip) on the incidence and development of rheumatoid arthritis (RA). METHODS: The effect of macrophage depletion with Clo-lip in the spleen was assessed by HE (haematoxylin and eosin) staining and immunohistochemistry (IHC). Thirty BALB/c mice were randomly divided into three groups, which were administered PBS-lip, Clo-lip, or normal saline. RA model mice were then created and the appearance of the paws was observed. Expression of CD68 by macrophages was examined by immunofluorescen

Pharmacokinetics of clodronate in haemodialysis patients.

BACKGROUND: Clodronate is a bisphosphonate used in the treatment of hypercalcaemia of various aetiologies. The major route of elimination of clodronate is renal excretion. The aim of the study was to derive data for the adjustment of dosage in haemodialysis patients. METHODS: The pharmacokinetic parameters describing the fate of an intravenous infusion of 300 mg clodronate disodium were studied in 10 haemodialysis patients. Clodronate disodium in serum, urine and dialysate samples was analysed by capillary gas chromatography with mass-selective detection. RESULTS: Of the 300 mg clodronate in

[Dichloromethylenbiphosphonic acid in the therapy of myositis ossificans progressive (MOP)].

The authors report 3 cases of myositis ossificans progressiva (MOP) treated with i.v. dichloromethylenbiphosphonic acid. Two of these patients presented a severe development of the disease so that they could not walk anymore. After a few months of treatment with dichloromethylenbiphosphonic acid (300 mg/die for 10 continual days every month) sufficient recovery in motor capacity was achieved that walking was possible again. The favourable effect of therapy has been shown only by clinical features because there is no biological marker to monitor in the follow-up. During the treatment no side ef

Algodystrophy in conjunction with van der Hoeve's syndrome.

Van der Hoeve's syndrome is the all-round variant of Lobstein's disease (late osteogenesis imperfecta) and is characterized by the concurrent presence of osteoporosis, blue sclerae, and deafness. The literature has reported sporadic cases of a combination of algodystrophic syndrome and Lobstein's disease. Our case represents a classic example: a 50-year-old woman with blue sclerae since birth, bilateral deafness, major dental alterations, vitreous osteoporosis and a history of pathological fractures who, two years from the onset of menopause, complained of pain involving the right ankle. After

Toll-like receptor-4 signaling and Kupffer cells play pivotal roles in the pathogenesis of non-alcoholic steatohepatitis.

BACKGROUND/AIMS: Studies in animal models and humans suggest a link between endotoxemia and non-alcoholic steatohepatitis. Since Kupffer cells are responsible for clearing endotoxin and are activated via endotoxin interaction with Toll-like receptor 4 (TLR-4), we examined the relationship between hepatic TLR-4 expression and Kupffer cell content during the genesis of steatohepatitis. METHODS: Male C57BL/6, C3H/HouJ and TLR-4 mutant C3H/HeJ mice were fed control or methionine/choline-deficient diet (MCDD). In one group of C57BL/6 mice, Kupffer cells were depleted by weekly intraperitoneal inje

Etidronic acid. A review of its pharmacological properties and therapeutic efficacy in resorptive bone disease.

Etidronic acid is an orally and intravenously active bisphosphonate, which is believed to inhibit resorption of bone via a number of cellular mechanisms, including alteration of osteoclastic activity. In studies of patients with symptomatic Paget's disease, etidronic acid 5 to 20 mg/kg/day administered orally rapidly decreased the biochemical indices of bone turnover. Mineralisation defects in forming bone may be avoided by the use of an initial dosage of 5 mg/kg/day for up to 6 months; dosages above 10 mg/kg/day should be limited to 3 months' duration, and dosages greater than 20 mg/kg/day sh

Identification and preliminary validation of biomarkers associated with mitochondrial and programmed cell death in pre-eclampsia.

BACKGROUND: The involvement of mitochondrial and programmed cell death (mtPCD)-related genes in the pathogenesis of pre-eclampsia (PE) remains inadequately characterized. METHODS: This study explores the role of mtPCD genes in PE through bioinformatics and experimental approaches. Differentially expressed mtPCD genes were identified as potential biomarkers from the GSE10588 and GSE98224 datasets and subsequently validated. Hub genes were determined using support vector machine, least absolute shrinkage and selection operator, and Boruta based on consistent expression profiles. Their performan

Characterization of the early ultrastructural and biochemical events occurring in dichloromethane diphosphonate nephrotoxicity.

A chelator, dichloromethane diphosphonate (Cl2MDP), used to treat for malignancy-induced hypercalcemia, has nephrotoxic potential. An acute animal model developed to examine the mechanism was used to further characterize the renal effects. NAG enzymuria appears to be an early premonitor of injury. Ultrastructurally, an increase in size and number of protein-containing phagolysosomal reabsorption droplets in proximal convoluted tubules associated with proteinuria precedes advent of tubular cell necrosis indicating these organelles to be a potential target site for Cl2MDP in the kidney. In vitro
