BACKGROUND: Initial studies have provided a mixed perspective of the efficacy of d-cycloserine (DCS) for augmenting the efficacy of exposure-based cognitive behavioral therapy (CBT) for panic disorder. In this multicenter trial, we examine the magnitude of DCS augmentation effects for an ultra-brief program of CBT.
METHODS: We conducted a double-blind, controlled trial at three treatment sites, randomizing 180 adults with a primary diagnosis of panic disorder to five sessions of treatment, with study pill (50 mg DCS or matching placebo) administered 1 hr prior to the final three sessions. Two

OBJECTIVE: To determine the physiological impact of treatment with donepezil (Aricept) on neural circuitry supporting episodic memory encoding in patients with amnestic mild cognitive impairment (MCI) using functional magnetic resonance imaging (fMRI).
METHODS: Eighteen patients with MCI and 20 age-matched healthy controls (HC) were scanned twice while performing an event-related verbal episodic encoding task. MCI participants were scanned before treatment and after approximately 3 months on donepezil; HC were untreated but rescanned at the same interval. Voxel-level analyses assessed treatme

Cognitive impairments are core features of schizophrenia and the best predictor of functional outcome. Cholinergic system and alpha-7 nicotinic acetylcholine (α7nACh) receptors are strongly implicated in the pathophysiologic mechanisms associated with cognitive impairments in schizophrenia. Galantamine is not only a reversible, competitive inhibitor of acetylcholinesterase but also a type I positive allosteric modulator of α7nACh receptors. The objective of this meta-analysis was to examine the efficacy of galantamine for cognitive symptoms of schizophrenia. In the meta-analysis that included

The Salience Network (SN) and its interactions are important for cognitive control. We have previously shown that structural damage to the SN is associated with abnormal functional connectivity between the SN and Default Mode Network (DMN), abnormal DMN deactivation, and impaired response inhibition, which is an important aspect of cognitive control. This suggests that stimulating the SN might enhance cognitive control. Here, we tested whether non-invasive transcranial direct current stimulation (TDCS) could be used to modulate activity within the SN and enhance cognitive control. TDCS was app

Treatment for pathological gambling is in its infancy. Several cognitive and behavioral interventions have shown promise, but high attrition and relapse rates suggest that gamblers requesting treatment are not uniformly committed to change. This article describes an exploratory study with 9 severe pathological gamblers--in their majority horse race bettors--who were recruited from a community treatment center. The gamblers were treated with a hybrid intervention consisting of motivational enhancement and cognitive behavior therapy. All gamblers were retained in treatment and during a 12-month

In this chapter, we will describe what has been learned about Klotho and its potential functions in the brain. Klotho is localized in the choroid plexus and, to a lesser extent, in hippocampal neurons. Cognitive decline is a common issue in human aging affecting over 50% of the population. This cognitive decline can also be seen in animal models such as the Rhesus monkey. A long-term study undertaken by our lab demonstrated that normal brain aging in rhesus monkeys and other animal models is associated with a significant downregulation of Klotho expression. This observation substantiates data

Schizophrenia is a severe mental disorder that affects more than 1% of the population worldwide. Dopamine system dysfunction and alterations in glutamatergic neurotransmission are strongly implicated in the aetiology of schizophrenia. To date, antipsychotic drugs are the only available treatment for the symptoms of schizophrenia. These medications, which act as D2-receptor antagonist, adequately address the positive symptoms of the disease, but they fail to improve the negative symptoms and cognitive impairment. In schizophrenia, cognitive impairment is a core feature of the disorder. Therefor

INTRODUCTION: Transcranial magnetic stimulation (TMS) is a non-invasive intervention that holds promise for improving cognitive function in individuals with Alzheimer's disease (AD). However, the effectiveness of this therapy and the optimal TMS parameters has not reached a consensus. The purpose of the meta-analysis was to systematically discern the effectiveness of different components of TMS protocols on cognitive improvement in patients with mild cognitive impairment (MCI) and AD.
METHODS: The meta-analysis was preregistered on Prospero (registration number: CRD42022345482). PubMed, Web o

Alzheimer's disease (AD) is a progressive neurodegenerative disorder that impairs cognitive and psychological performance and adversely impacts daily routines and activities. We hypothesized that Binaural beat stimulation (BBS) enhances psychological performance by increasing connectivity in the $\theta $ -and $\alpha $ -bands over the fronto-parietal region. Twenty-five AD patients received either Binaural beat (BBS) or Standard Auditory beat stimulation (SAS) for 12 consecutive days. Psychometric assessments (Blessed Dementia Scale, Depression Anxiety Stress Scale, and Mini-Mental State Exam

INTRODUCTION: The gut microbiota is a matrix ecosystem vital for the maintenance of host metabolic, immune, and neurological health. Disruption of this balance, known as gut dysbiosis, is associated with numerous diseases, including diabetes and neurological disorders. Dietary polyphenols, particularly flavonols like quercetin, have shown potential in restoring microbial balance. However, their poor solubility and low bioavailability limit therapeutic outcomes.
METHODS: This review evaluates the role of dietary polyphenols in modulating gut microbiota and explores nanocarrier-based approaches

Each of three catecholamine agonist medications (clonidine, L-dopa, and ephedrine) and placebo were administered for 2-week trials to a group of amnesics with Korsakoff's psychosis following a double-blind, counterbalanced design. During the last 3 days of each treatment, patients were given a battery of neuropsychological tests to determine the effects of the drugs on memory and related cognitive processes. Only clonidine had a significant effect on measures of anterograde amnesia, and this response was restricted to the same psychometric measures that improved with clonidine in an earlier st

Schizophrenia is a psychiatric condition often characterized by cognitive deficits which precede the onset of positive symptoms and contribute significantly to enduring functional impairments and poor quality of life. Those cognitive deficits can have a profoundly negative impact on daily activities such as social relationships and work functioning. Although pharmacological interventions remain central to the treatment of schizophrenia (SZ), they have a negligible impact on cognitive functioning. Yet multiple reviews and meta-analyses support the hypothesis that Aerobic Exercise (AE) could ser

Cervical cancer ranks as the fourth most prevalent cancer among women worldwide. Early diagnosis facilitates timely intervention and treatment. Traditional colposcopy is a widely employed technique for evaluating cervical lesions. Nevertheless, the growing number of patients with cervical cancer, and the rising workload of doctors may lead to misdiagnosis and underdiagnosis. Therefore, many researchers use deep learning to make paramedical diagnoses of cervical disease. However, the current neural network models for cervical image recognition lead to poor diagnostic results due to limitations

BACKGROUND: Transcranial direct current stimulation (tDCS) can influence brain network activity and associated cognitive and behavioural functions. In addition to the extensive variety in stimulation parameters, numerous biological factors drive these effects, however these are yet poorly understood. Here, we investigate one of the major biological factors by focusing on sex-dependent effects of tDCS on a challenging cognitive control task (adaptive paced auditory serial addition task [PASAT]) in healthy humans.
METHODS: This sex-specific re-analysis was performed on data of 163 subjects who

Cognitive Stimulation Therapy (CST) is one of the best non-drug treatments for dementia patients that is based on evidence and can help them improve their cognitive and emotional performance. CST developed from reality orientation and social interaction models into a prevalent intervention that employs themed activities to stimulate memory, language, and attention functions, thereby improving participant engagement and quality of life. This paper elucidates the theoretical framework, session organisation, and therapeutic methodologies of CST. The research examines the broader implications of C

Psilocybin has been shown to improve symptoms of depression and anxiety when combined with psychotherapy or other clinician-guided interventions. To understand the neural basis for this pattern of clinical efficacy, experimental and conceptual approaches that are different than traditional laboratory models of anxiety and depression are needed. A potential novel mechanism is that acute psilocybin improves cognitive flexibility, which then enhances the impact of clinician-assisted interventions. Consistent with this idea, we find that acute psilocybin robustly improves cognitive flexibility in

Heroin addiction is a disorder that stems from maladaptive plasticity within neural circuits and produces broad cognitive deficits. Despite considerable advances in psychotherapy and pharmacotherapy for heroin relapse and addiction, effective treatments for heroin use disorder are still lacking. Increasing preclinical evidence indicates that heroin seeking behavior is persistent after withdrawal, while cognitive dysfunctions associated with chronic heroin use are an important contributing factor to risk of heroin relapse and addiction. Cognitive enhancers may be used to stimulate treatment suc

Recently, there is a great social problem that geriatric disorders, especially senile dementia, are growing rapidly with the increasing percentage of aged individuals in the total population. However, there is yet no drug that has reliable effects on senile dementia on the market. Therefore, society requires the development of new drugs that can support patients so that they can smoothly live their daily lives by themselves. In this study, we attempted to investigate the 1998 domestic state of development of cognitive enhancers by summarizing many publications and by gathering questionnaires f

BACKGROUND: This study sought to examine the trends in the prescribing of subsidized and unsubsidized cognition enhancing drugs (CEDs) in Australia over five years from 2002 to 2007. Subsidized cholinesterase inhibitor medication could be prescribed to people with mild to moderate Alzheimer's disease (AD) once a specialist physician had confirmed this diagnosis. Memantine was available for use in moderately severe AD but not subsidized.
METHODS: We analyzed the Medicare Australia and Drug Utilisation Sub-Committee databases for CED prescription data, 2002-2007, by gender, age and prescriber c
