BACKGROUND AND AIMS: A direct causal association between inflammatory bowel disease (IBD) and appendiceal neoplasm (AN) is unclear.
METHODS: Patients with IBD and AN were identified from 1992 to 2023 using bioinformatics and natural language processing tools.
RESULTS: Thirty-one patients were identified. The most common type of AN was appendiceal mucinous neoplasm (83.9 %). Three patients with ulcerative colitis (9.7 %) had recurrence after surgical resection due to peritoneal seeding.
CONCLUSIONS: Incidence and recurrence of AN in patients with IBD is low. Further studies to compare AN in

Colitis cystica profunda (CCP) is a rare, uncommon and nonneoplastic condition that can occur anywhere in gastrointestinal tract, but its main occurrence is in the rectum and sigmoid colon. It is characterized by the presence of mucin filled cysts, lined by benign epithelium, beneath the muscularis mucosae, usually confined to the submucosa, and it can clinically and radiologically mimic a neoplasm. Here we report a rare case of CCP in a patient with a 2-months history of abdominal pain and severe anemia, associated with diverticulosis. The knowledge of this entity and its differential diagnos

Despite advances in medical therapy, surgery continues to play a vital role in the management of Crohn's disease and its complications. Continuing from Part I of this series (small intestine/ileal disease), we focus next on colonic Crohn's disease and associated neoplasms. We will first review the surgical management of medical-refractory Crohn's colitis and its complications and then examine cancer risk, surveillance, and surgical management of Crohn's-associated colorectal dysplasia and malignancy. We conclude with a discussion of restoration of gastrointestinal continuity following colonic

Colitis-associated colorectal cancer (CAC) is a serious condition driven by chronic inflammation in the colon, representing a significant challenge in both preventative and therapeutic contexts. Apis mellifera intermissa venom has shown promising therapeutic potential in various disease models, particularly those involving inflammation and tumorigenesis. This study evaluates the therapeutic effects of venom derived from honeybees native to Algeria on the progression of CAC in azoxymethane (AOM)-treated mice. A total of 28 male mice were randomly allocated into four groups (n=7 per group): a co

Colorectal cancer is a known complication of chronic inflammation of the colon ("colitis-associated colon cancer"). Inflammatory bowel disease (IBD) is a chronic inflammatory disorder of the gastrointestinal tract. Patients with IBD are at increased risk of colon cancer compared to the general population. Kinase signaling pathways play critical roles in both the inflammation and regulating cellular processes such as proliferation and survival that contribute to cancer development. Here we review the interplay of kinase signaling pathways (mitogen-activated protein kinases, cyclin-dependent kin

BACKGROUND: Ulcerative colitis (UC) is a progressive disorder that elevates the risk of colon cancer development through a colitis-dysplasia-carcinoma sequence. Gene expression profiling of colitis-associated lesions obtained from patients with varied extents of UC can be mined to define molecular panels associated with colon cancer development.
METHODS: Differential gene expression profiles of 3 UC clinical subtypes and healthy controls were developed for the GSE47908 microarray data set of healthy controls, left-sided colitis, pancolitis, and colitis-associated dysplasia (CAD) using limma R

Colorectal cancer (CRC) is the third most prevalent malignancy worldwide. Inflammatory bowel diseases (IBD) encompass a group of chronic intestinal inflammatory disorders, including ulcerative colitis (UC) and Crohn's disease (CD). As a chronic inflammatory bowel disease, UC may persist and elevate the risk of malignancy, thereby contributing to the development of colorectal cancer, known as colitis-associated colorectal cancer (CAC). Chronic intestinal inflammation is a significant risk factor for colorectal cancer, and the incidence of colitis-associated colorectal cancer continues to rise.

Colitis-associated colorectal cancer (CAC) develops in the setting of long-standing intestinal inflammation and remains a major complication of inflammatory bowel disease (IBD). Although effective control of inflammation may reduce cancer risk, approaches that specifically prevent the progression from chronic colitis to neoplasia remain limited. Disruption of the gut microbiota-metabolism-immunity axis is increasingly implicated in this process, as microbial dysbiosis and altered metabolite production can compromise epithelial barrier integrity, sustain inflammatory signaling, and disturb muco

The case histories of 1961 patients with inflammatory bowel disease (IBD), 1227 with Crohn's disease (CD) and 734 with ulcerative colitis (UC), have been studied for the incidence of extraintestinal malignant neoplasms. There were 54 extraintestinal cancers in 51 patients: 28 patients with CD and 23 with UC; 25 men and 26 women. There were 9 breast, 7 skin, 15 reticuloendothelial, 11 genitourinary, 3 lung, 3 perianal, 2 pancreatic islet cell, and several miscellaneous cancers. The number of patient-years from the onset of disease to the last date of follow-up was calculated for men and women w

A 37-year-old male with long-standing and extensive ulcerative pancolitis developed a rapidly lethal poorly differentiated neuroendocrine carcinoma (NEC) in the sigmoid colon. Prior biopsies obtained from multiple sites of the colon during endoscopic surveillance showed minimal inflammatory changes and no sign of dysplasia. Patients with inflammatory bowel disease (IBD) are at increased risk of colorectal malignancies, and adenocarcinoma is the most common type of colorectal neoplasm associated with ulcerative colitis and Crohn's disease, but other types of epithelial and nonepithelial tumors

Colorectal cancer (CRC) linked to colitis is a major cause of cancer-related deaths, highlighting the need for effective treatment options. This study aimed to investigate the effects of berberine (BRR) on gut microbiota and the endocannabinoid system (ECS) in a mouse model of colitis-associated CRC, which was induced using azoxymethane (AOM) and dextran sulfate sodium (DSS). Through a combination of animal experiments, microbial sequencing, and biochemical assays, we discovered that BRR significantly inhibited tumor development. This was demonstrated by a dose-dependent increase in body weigh

Emerging evidence has linked high dietary fructose intake with adverse health outcomes, yet its role in colitis-associated colorectal cancer (CAC) remains underexplored. Here, we demonstrate that high fructose consumption aggravates intestinal inflammation and significantly promotes tumorigenesis in an AOM/DSS-induced CAC model via activation of the EREG-EGFR signaling axis. We first developed the Food∩TCM database, which encompasses 112 medicine and food homologous TCM, along with their chemical compositions. Using the network-based pharmacological intelligence platform, we identified Laminar

BACKGROUND/AIM: Cluster of differentiation (CD) 44 is a primary surface marker of cancer stem cells present in colorectal cancer (CRC); however, the role of CD44 expression in CRC prognosis remains elusive. Hence, this study aimed to investigate the association between CD44 localization and tumor regions using a colitis-associated CRC mouse model.
MATERIALS AND METHODS: Herein, a mouse model of colitis-associated CRC was established via azoxymethane treatment, followed by dextran sulfate sodium.
RESULTS: Notably, the mice presented with a decrease in body weight, diarrhea, and bloody stools,

BACKGROUND: Patients with a long history of ulcerative colitis (UC) are at risk of developing a serious complication known as UC-associated neoplasia (UCAN). Because the treatment strategy for UCAN greatly differs from that for sporadic tumors, UCAN needs to be distinguished from sporadic tumors. This article provides an overview of the current status and future challenges regarding the surveillance colonoscopy (SC) and endoscopic submucosal dissection (ESD) of neoplastic lesions in patients with UC.
SUMMARY: To reduce the risk of associated mortality, the current guidelines recommend initiat

A man aged above 70 years old with a medical history of ulcerative colitis presented with unintentional weight loss. A pancreatic mass associated with pancreatic duct dilatation was detected on imaging procedures. Initial investigations including fine needle aspiration and cytology examination were inconclusive. A diagnosis of intraductal tubulopapillary neoplasm (ITPN) was made with histopathology and immunohistochemistry examination on a surgically resected specimen. Two years after surgery, the patient remained well with no radiological evidence of recurrence.ITPN is a rare pancreatic duct

Six patients with Crohn's disease had colonic intraluminal multilobulated masses detected on barium enema. Four had Crohn's colitis and two ileocolitis. The mean duration of disease was four and eight tenths years. Three patients underwent surgery and three had colonoscopy with multiple biopsies and cytology. Pseudopolypoid inflammatory tissue was found in each case. One patient died after a prolonged postoperative course due to sepsis and abscess formation. No surgery was performed in three patients and follow-up colonoscopic examinations at four and a half years revealed no change in these f

OBJECTIVES: The risk of colitis-associated cancer increases with disease duration in ulcerative colitis (UC), yet surveillance colonoscopy protocols generally stratify risk uniformly for patients with disease lasting over 8 years. This study evaluated whether shorter surveillance intervals might enhance lesion detection rates in patients with extended disease duration.
METHODS: This retrospective observational study analyzed patients diagnosed with UC-associated neoplasms between 2010 and 2023. Colonoscopies before lesion detection were retrospectively reviewed, and risk stratification was ap

BACKGROUND: Parasutterella excrementihominis (P. excrementihominis), a Betaproteobacteria species enriched in ulcerative colitis (UC) patients, is implicated in chronic inflammation. However, its mechanistic role in UC progression and colitis-associated colorectal cancer (CAC) remains unclear.
OBJECTIVE: This study investigates the pathogenic role of P. excrementihominis in UC and CAC, focusing on its induction of neutrophil extracellular traps (NETs) and underlying mechanisms.
DESIGN: Clinical stool samples from UC patients and healthy controls were analysed for P. excrementihominis abundan
