Clinical Outcomes of Ulnar Collateral Ligament Surgery in Nonthrowing Athletes.

BACKGROUND: Ulnar collateral ligament (UCL) surgery continues to demonstrate excellent clinical outcomes and a high return-to-play (RTP) rate with a low complication rate. Recent studies have demonstrated similar clinical outcomes for baseball players who have undergone either UCL reconstruction or UCL repair. In comparison, few studies have assessed the clinical outcomes of UCL surgery for nonthrowing athletes. PURPOSE/HYPOTHESIS: The primary objective of this study is to provide clinical outcomes of UCL surgery performed in nonthrowing athletes at a single institution with a minimum 2-year

Cadaveric Biomechanical Analysis of 3 Lateral Ulnar Collateral Ligament Reconstructions.

OBJECTIVE: To biomechanically assess the angular stability provided by 3 techniques for reconstruction of the lateral ulnar collateral ligament. METHODS: Eight cadaveric elbows were tested with the lateral collateral ligament complex intact, disrupted from the origin at the lateral epicondyle, and reconstructed with 3 different techniques using ulnar bone tunnels: a suture "lasso" or palmaris longus tendon "lasso" both docked with a metal button at the origin, and a doubled-over palmaris longus tendon docked with metal buttons at both the origin and ulnar insertion. Elbows were tested with a

Ulnar collateral ligament injury in baseball pitchers: MR imaging evaluation.

The ulnar collateral ligament (UCL) provides stability to the medial aspect of the elbow during valgus stress. Trauma to this ligament may result from repetitive forceful throwing. Diagnosis of UCL injury has been based on clinical findings of medial joint pain and valgus instability, as direct imaging of this structure has not been available. Eleven baseball pitchers with clinical evidence of UCL injury were evaluated with magnetic resonance (MR) imaging. Surgical correlation was obtained in six patients, four of whom underwent UCL reconstruction. MR imaging findings in UCL injury included la

Return to sport after ulnar collateral ligament repair and reconstruction.

BACKGROUND: Given the rising incidence of elbow ulnar collateral ligament (UCL) injury among young athletes in overhead sports, it is important to identify optimal treatments. The historic gold standard is UCL reconstruction. However, suture-augmented UCL repair techniques have become increasingly popular given their high rates of return to play in partial or full thickness avulsion UCL tears. The purpose of this study was to determine complications, clinical outcomes, and return to sport in UCL repair and reconstruction. We hypothesized that there would be a similar rate of return to sport in

Lateral Ulnar Collateral Ligament Reconstruction With Positive Pivot Shift Test.

BACKGROUND: Posterolateral rotatory instability of the elbow (PLRI) results from incompetence of the lateral ulnar collateral ligament (LUCL), part of the lateral elbow ligamentous complex. A pivot shift test is a sensitive and specific intraoperative maneuver to assess for PLRI. While several LUCL repair and reconstruction techniques have been described, ligament reconstruction remains the gold standard treatment. INDICATIONS: LUCL reconstruction is indicated for active patients with symptomatic PLRI whose conservative treatment has failed. Patients classically present with pain and mechanic

Biomechanical Comparison of 3 Thumb Ulnar Collateral Ligament Repair Methods.

PURPOSE: Differences in range of motion, pinch strength, biomechanical strength, or joint angulation have previously been investigated for various means of treatment of ulnar collateral ligament (UCL) tears. We sought to address a gap in the literature by comparing thumb metacarpophalangeal (MCP) joint angle measurements and biomechanical strength before complete, acute UCL tear and after repair with suture anchors (SA), suture tape (ST) anchor augmentation, or reconstruction with palmaris longus graft (PL). METHODS: Thumbs and, if present, the PL tendon were harvested from 15 fresh-frozen ca

Impact of Ulnar Collateral Ligament Tear on Posteromedial Elbow Biomechanics.

Ulnar collateral ligament insufficiency has been shown to result in changes in contact pressure and contact area in the posteromedial elbow. This study used new digital technology to assess the effect of a complete ulnar collateral ligament tear on ulnohumeral contact area, contact pressure, and valgus laxity throughout the throwing motion. Nine elbow cadaveric specimens were tested at 90° and 30° of elbow flexion to simulate the late cocking/early acceleration and deceleration phases of throwing, respectively. A digital sensor was placed in the posteromedial elbow. Each specimen was tested wi

Optimal management of ulnar collateral ligament injury in baseball pitchers.

The ulnar collateral ligament stabilizes the elbow joint from valgus stress associated with the throwing motion. During baseball pitching, this ligament is subjected to tremendous stress and injury if the force on the ulnar collateral ligament during pitching exceeds the physiological limits of the ligament. Injuries to the throwing elbow in baseball pitchers result in significant time loss and typically surgical intervention. The purpose of this paper is to provide a review of current information to sports medicine clinicians on injury epidemiology, injury mechanics, injury risk factors, inju

Effect of the medial collateral ligament and the lateral ulnar collateral ligament injury on elbow stability: a finite element analysis.

Ligaments are the most important stabilizer of elbow. However, the stress of ligaments is hard to measure because of the complex biomechanical environment in the elbow. Our objective was to develop a human elbow finite element model and to validate it by a comparison with previous experimental data. Then several different ligaments injury conditions and elbow flexion were simulated to analyse the elbow instability and to stress the biomechanical consequences. The computational investigation of different effects of ligament constraints of elbow was studied by means of finite element analysis. T

Biomechanical Comparison of Anatomic Restoration of the Ulnar Footprint vs Traditional Ulnar Tunnels in Ulnar Collateral Ligament Reconstruction.

BACKGROUND: Current techniques for ulnar collateral ligament (UCL) reconstruction do not reproduce the anatomic ulnar footprint of the UCL. The purpose of this study was to describe a novel UCL reconstruction technique that utilizes proximal-to-distal ulnar bone tunnels to better re-create the anatomy of the UCL and to compare the biomechanical profile at time zero among this technique, the native UCL, and the traditional docking technique. HYPOTHESIS: The biomechanical profile of the anatomic technique is similar to the native UCL and traditional docking technique. STUDY DESIGN: Controlled

Repair of the Thumb Ulnar Collateral Ligament With Suture Tape Augmentation.

One of the most commonly injured structures of the thumb metacarpophalangeal (MCP) joint is the ulnar collateral ligament (UCL). An acute injury of this ligament is often amenable to primary repair. Despite the favorable outcomes of primary repair, the thumb is often restricted during initial healing of the ligament with immobilization and delayed rehabilitation. We present a novel technique to augment the repair of the UCL with suture tape to provide immediate biomechanical support and strength during the critical time of ligament healing. We describe the surgical technique of suture tape aug

[Sprain of the thumb metacarpal-phalangeal with lesion of the ulnar collateral ligament].

The sprain of the thumb metacarpal-phalangeal joint must be treated in an adapted way in order to avoid chronic laxity. The comparative and bilateral clinical exam is the main element that will allow to detect laxity. In the case of non laxity, a conservative treatment with a splint makes sense. In the case of laxity with Stener effect, a surgical procedure is recommended in order to reintegrate the collateral ligament. In the case of laxity without Stener effect, the choice of treatment is contentious. A US or MRI would be useful if the Stener effect could be underlying. For whichever treatme

The appearance of the lateral ulnar collateral ligament on magnetic resonance imaging.

The lateral ulnar collateral ligament is an important element of the elbow's lateral capsuloligamentous complex, and loss of integrity contributes to posterolateral rotatory instability. However, the normal appearance on magnetic resonance imaging is poorly defined. The purpose of this study was to assess the appearance of the lateral ulnar collateral ligament of asymptomatic elbows on magnetic resonance imaging. We performed magnetic resonance imaging on 20 asymptomatic elbows. The lateral ulnar collateral ligament was identified as a hypointense structure originating from the lateral epicond
