BACKGROUND: The ulnar collateral ligament (UCL) complex of the elbow plays a primary role in valgus and posteromedial stability of the elbow. The anterior oblique ligament (AOL) of the UCL is believed to provide the majority of resistance to external forces on the medial elbow. The transverse ligament (TL) of the UCL is generally thought to have minimal contribution to the elbow's overall stability. However, recent studies have suggested a more significant role for the TL. The primary aim of this study was to identify the TL's contribution to the stability of the elbow joint in determining the

Different surgical techniques have been proposed to reconstruct combined anterior cruciate (ACL) and lateral collateral ligaments (LCL). Although these surgical techniques are reliable and reproducible, the number of autologous grafts needed for the reconstruction could be a limiting factor, especially when patients present with multi-ligament knee injuries and the posterior cruciate ligament is also torn. In addition, some of these techniques are not easy to master and have a steep learning curve. We present a surgical procedure that has been used over the last 18 years to reconstruct combine

BACKGROUND AND AIMS: To study the role of MR Imaging in evaluating both normal and injured lateral collateral ligaments of the ankle.
MATERIALS AND METHODS: Twenty-four patients with clinically diagnosed inversion injury to the ankle and 20 healthy volunteers underwent Magnetic Resonance Imaging (MRI) of the ankle with special emphasis on the lateral complex ligaments.
RESULTS: The anterior talofibular (ATFL) and posterior talofibular ligaments (PTFL) were identified in 100% of the ankles of normal volunteers in the axial plane. The PTFL was identified in 100% in the coronal plane. The calca

The authors report on their experience on 176 patients with lesions of the ulnar collateral ligaments. Two samples were compared: the fresh rupture and immediate suture, and the old lesion treated by a special plasty with palmaris tendon (Bäuerle/Reill). The long term results (at least 2 years) of both collectives were nearly identical. The authors recommend immediate repair.

The percutaneous release of accessory collateral ligaments was introduced in 1986 as a safe and quick procedure to be attempted before open, more extensive joint release in the treatment of proximal interphalangeal joint flexion contracture. Our study analyzed the long-term results and patient satisfaction following a percutaneous release in 30 joints after a mean follow-up period of 34 months. In one half of cases the preoperative joint flexion deformity was reduced from 78° to 34°. The best results were observed in patients with osteoarthritis and stiff, immobilized joints. In patients with

AIM: To study the long term result of open reduction of longstanding dislocated elbows with regard to stability, avascular necrosis of the distal humerus and degenerative changes of the joint.
METHODS: Nine patients, aged between nine and 60 years (average 30 years) with longstanding posterior dislocation of the elbow underwent open reduction. The operative procedure featured a medial and lateral incision, excision of the capsule, fibrous adhesions and the collateral ligaments with no attempt to reconstruct the ligaments, as well as anterior transposition of the ulnar nerve. The procedure was

Ankle sprains are among the most commonly treated injuries that present to the emergency room, and often most inadequately diagnosed and treated. When diagnosed properly, the appropriate treatment is easily determined. When indications point to surgical repair of the lateral collateral ligaments of the ankle, the following technique, utilizing the Mini-Statak device, offers a simplified and timesaving alternative.

Combined anterior cruciate ligament/medial collateral ligament (ACL/MCL) injuries are relatively common, and multiple factors are involved in surgical decision-making, particularly when it comes to the MCL. Historically, most surgeons treated the MCL conservatively and performed staged MCL reconstruction after MCL reconstruction only if there was persistent medial instability. This was followed by a nonoperative approach for the MCL (when reconstructing the ACL) unless there was evidence of extreme (grade III or >1 cm) valgus instability, valgus malalignment, or mid-substance or tibial-sided i

The medial ulnar collateral ligament (MUCL) complex is integral for valgus elbow stability, especially in individuals engaged in repetitive overhead activities such as throwing. MUCL injuries often necessitate surgical intervention to restore elbow stability. Early studies reporting outcomes after MUCL repair demonstrated suboptimal return to play compared with ulnar collateral ligament reconstruction, prompting a shift toward reconstruction techniques. The first widely popular MUCL reconstruction technique was the Jobe technique. Many reconstruction techniques have since been described. Despi

Fibular collateral ligament (FCL) injuries can present as lateral-sided knee pain with feelings of side-to-side instability during activity. Patients with FCL injuries can have accompanying symptoms related to irritation of the common peroneal nerve. Preoperative diagnosis is imperative and should include a thorough physical examination complemented with varus stress radiographs before surgical reconstruction is indicated. In the adolescent cohort, surgical planning can be complicated by the presence of open physes, and caution must be taken to avoid drilling through or placing screw fixation

BACKGROUND: Ulnar collateral ligament (UCL) reconstruction has become an increasingly common procedure in overhead-throwing athletes because of overuse and repetitive valgus stress placed on the inside of the elbow or valgus stress during an acute traumatic event2. Athletes with a deficient UCL often report a decrease in pitch velocity and accuracy in addition to an increase in fatigue, pain, and instability4. As such, UCL reconstructive procedures have been increasing in prevalence in throwing athletes to address their symptoms and regain their throwing capabilities, particularly in young bas

BACKGROUND: Ulnar collateral ligament (UCL) reconstructions have become increasingly common, particularly in youth overhead throwing athletes. These injuries are most commonly due to overuse and repetitive trauma on the inner elbow. Throwers with a deficient UCL often report decreased pitching speeds in addition to elbow pain and instability.
INDICATION: The indications for this procedure include symptomatic valgus elbow instability during overhead throwing motions and a verified rupture of the UCL on advanced imaging.
TECHNIQUE DESCRIPTION: The ipsilateral gracilis tendon is harvested and p

BACKGROUND: This study aimed to determine the extent to which the medial collateral ligament (MCL) can be visualized during a standard posterior arthroscopic view of the elbow.
METHODS: Eight fresh human cadaveric elbows were placed in a simulated lateral decubitus position. Standard elbow arthroscopy was performed on each specimen using a standard posterior portal for visualization with a 30° arthroscope. The most distal borders of the visible part of the MCL were marked using a spinal needle and tagged using nylon sutures. Subsequently, the elbow was dissected. The overall surface area of t

Repetitive valgus forces on the throwing elbow place significant stress on that joint. This stress can cause structural damage and injury to the ulnar collateral ligament. Many acute injuries of the throwing elbow are caused by repetitive chronic wear. Although much work has been done on injury prevention in youth who are pitchers, overuse injury in throwing sports constitutes an epidemic. Failing nonsurgical management, ulnar collateral ligament reconstruction is a viable option to return the throwing athlete to competition.

The significance of collateral ligament desmitis of the tarsocrural joint is often clinically underestimated, because it is an uncommon injury with a guarded prognosis for athletic soundness. The objective of this study was to describe the clinical presentation, treatment, and outcome of 12 horses with collateral ligament desmitis, along with tarsocrural joint synovitis secondary to hemarthrosis. Criteria for inclusion in this study included clinical signs of tarsocrural joint synovitis and sonographic evidence of collateral ligament desmitis. This retrospective study evaluated horses over an

The ulnar collateral ligament (UCL), particularly the anterior portion of the anterior oblique ligament, is the primary static contributor to elbow valgus stability. UCL injuries are most common in athletes participating in overhead sports. Acute and chronic injuries to the UCL result in valgus instability, which may predispose the athlete to the development of disabling secondary elbow conditions. Provocative physical examination maneuvers include the valgus abduction test, the modified milking maneuver, and the moving valgus stress test. Plain radiographs and magnetic resonance imaging are t

The functional thumb is a necessity for successful athletic participation. It not only allows the athlete to manipulate athletic equipment but also allows the precise manipulation of objects in the athlete's hand. Injury to the thumb often negates power grip and finesse of the athletic hand. Injuries can range from fractures, to dislocations, to ligamentous injuries. The relatively unconstrained thumb metacarpophalangeal joint is particularly vulnerable to injury from an abduction moment to its distal segment. Such injuries occur from direct contact and with falls on equipment such as racquet

The bursa of the tibial collateral ligament (TCL) may be visualized at magnetic resonance (MR) imaging when it becomes distended with fluid. In the authors' experience, this finding signifies a pathologic condition either in the medial capsuloligamentous complex or in the bursa itself. Such a finding may indicate TCL bursitis. TCL bursitis can be suspected in the setting of isolated pain in the medial joint line in the absence of mechanical symptoms. Prolonged relief of symptoms after injection of steroid into the bursa is supportive of the diagnosis. Seven cases are presented in which a fluid

BACKGROUND: Concomitant anterior cruciate ligament (ACL) and medial collateral ligament (MCL) injuries are among the most commonly reported injury patterns in high-energy knee injuries. Concomitant ACL and MCL reconstruction (ACLR + MCLR) is indicated in patients with full-thickness tears and persistent valgus laxity.
PURPOSE: To systematically review the literature to identify studies reporting on patients undergoing concurrent primary ACLR + MCLR to better understand current indications, surgical techniques, complications, and outcomes.
STUDY DESIGN: Systematic review; Level of evidence, 4

Both the medial collateral ligament (MCL) and the anterior cruciate ligament (ACL) are reported to prevent valgus instability of the knee. In this study, the anatomical mechanisms by which these ligaments prevent valgus instability were experimentally investigated. The valgus rotation angle and the magnitude of the medial joint space opening were measured in six cadaveric knees, using biplanar photography before and after the MCL and/or the ACL were severed. A significant increase in the valgus rotation angle and a large medial joint space opening were observed when the MCL was severed. An inc
