Acute pseudo-obstruction of the colon.

Fourteen patients with acute pseudo-obstruction of the colon (Ogilvie's syndrome) were treated over a 16-year period. Ten patients (71.4%) had a recent history of mental illness and were treated with hypnotic and sedative drugs uninterruptedly for many months. The clinical picture and abdominal radiographs on admission to hospital were typical of acute mechanical obstruction of the colon; 1 patient had a perforation of the transverse colon. Obstruction was ruled out by barium enema in 9 patients, by colonoscopy in 3 and by immediate laparotomy in 2. Twelve patients were treated conservatively

[Acute pseudo-obstruction of the colon].

Acute pseudo-obstruction of the colon is a probably often misdiagnosed type of ileus, the diagnosis of which follows from the clinical pattern with the signs of a mechanical ileus, the acute course and the x-ray signs. Radiological findings cannot be clearly interpreted without knowledge of the clinical pattern and the course of the disease. Hence, the radiological correlate of the pseudo-obstruction of the colon is not specific, but it does supply a pointer to the disease of its shows dilation of the caecum, colon ascendens and colon transversum with air-pockets and reflected imaging as well

Acute pseudo-obstruction of the colon.

Nineteen patients presenting with pseudo-obstruction of the colon are described. Thirteen were female and the mean age was 65.7 years (48-82). Five cases appeared idiopathic but the remainder were associated with either intercurrent disease, surgery or trauma. Six patients underwent laparotomy, 3 for gross caecal distension and 1 subsequently died with bronchopneumonia. Thirteen patients were treated conservatively with flatus tube decompression or colonoscopy, but 3 died, 1 from a perforated caecum, 1 from a recurrent subarachnoid haemorrhage and 1 from bronchopneumonia. The high mortality of

Colonic pseudo-obstruction: the dilated colon in the ICU.

Acute colonic pseudo-obstruction is a syndrome of massive dilation of the colon without mechanical obstruction that develops in hospitalized patients with serious underlying medical and surgical conditions. Increasing age, cecal diameter, delay in decompression, and status of the bowel significantly influence mortality, which is approximately 40% when ischemia or perforation is present. Evaluation of the markedly distended colon in the intensive care unit setting involves excluding mechanical obstruction and other causes of toxic megacolon such as Clostridium difficile infection, and assessing

Gastrointestinal Mucormycosis in the Setting of Colonic Pseudo-Obstruction.

Mucormycosis is a rare opportunistic infection caused by the fungi order Mucorales that can affect multiple different organ systems. A 49-year-old man with a history of poorly controlled diabetes and hypertension presented with acute-on-chronic abdominal pain with profound worsening in the past 24 hours. He was found to have chronic intestinal pseudo-obstruction, with pneumatosis of the transverse colonic wall requiring total abdominal colectomy. Within two weeks after surgery, he developed fundal gastric injury with herniation requiring primary repair with surgical laparotomy. Tissue histopat

Colonic Pseudo-obstruction as a Rare Complication of Pheochromocytoma.

UNLABELLED: Colonic pseudo-obstruction is characterized by dilatation of the colon without a structural lesion causing the obstruction. It usually involves the caecum and right side of the colon and is commonly observed in patients with severe illness or after surgery; it is rarely caused by pheochromocytoma. The diagnosis of colonic pseudo-obstruction can be established by abdominal imaging including computed tomography (CT) of the abdomen or use of a water-soluble contrast enema. In additional to conservative or surgical treatment, alpha-blockers can be used in this setting to relieve the ps

[Acute colonic pseudo-obstruction (Ogilvie syndrome) post-renal transplant].

BACKGROUND: Acute colonic pseudo-obstruction, also known as Ogilvie syndrome, is a rare gastrointestinal syndrome in children. It is characterized by a marked dilatation of the colon evidenced by imaging and absence of mechanical obstruction. Patients typically present with abdominal pain and distended, tympanic abdomen, with peristalsis present, accompanied by nausea and vomiting. Up to 40% of patients can pass gas and/or have bowel movements. We decide to report this case because this syndrome is very rare in pediatric patients, and no cases have been reported in a post-renal transplant pedi

Acute colonic pseudo-obstruction in vascular patients (Ogilvie syndrome).

Two patients with acute colonic pseudo-obstruction are presented. Acute colonic pseudo-obstruction, first described by Ogilvie in 1948, is characterised by signs of colonic obstruction, but without mechanical obstruction to the intestinal flow. The current hypothesis states that it is caused by an imbalance between sacral parasympathetic excitatory nerves and sympathetic inhibitory nerves, thus inducing functional obstruction. It mostly develops in hospitalised patients with a variety of medical and surgical conditions. Two such patients with extensive co-morbidities, developed progressive col

Colonic pseudo-obstruction: an uncommon complication of caesarean section.

Four cases of pseudo-obstruction following lower segment Caesarean section are presented with a review of the condition. In the first patient the diagnosis was made at laparotomy, but the other 3 were diagnosed on clinical and radiolographic grounds. Two of these subsequently required surgical caecal decompression, while only 1 was successfully managed conservatively. This is typical of the spectrum of this uncommon but well recognized syndrome which may complicate Caesarean section, vaginal delivery, hysterectomy or other gynaecological operations. When diagnosed, the initial management may b

[Acute Colonic Pseudo-obstruction Induced by Neuroleptic Malignant Syndrome].

A colonic pseudo-obstruction is a disorder that causes abdominal distension and abdominal pain similar to a mechanical obstruction, but there are no structural lesions that can obstruct the gastrointestinal tract. This condition can be acute or chronic. An acute colonic pseudo-obstruction, also called Ogilvie's syndrome, is believed to be a condition induced by other causes that are different from a chronic colonic pseudo-obstruction. The pathogenesis involves abnormalities in the autonomic nervous system induced by systemic diseases or medications, and it often improves when the primary cause

Acute Colonic Pseudo-Obstruction (Ogilvie's Syndrome) after Hip Arthroplasty.

Ogilvie's syndrome is a clinical entity that occurs with signs of excessive dilatation of the colon and obstruction, despite the absence of a mechanical obstruction. Although its etiology remains uncertain, anticholinergic drugs, metabolic disorders, diabetes mellitus, hyperparathyroidism, Parkinson's disease, major orthopedic interventions, or blunt abdominal trauma are considered to be possible causes. Imbalance in sympathetic innervation plays a role in the pathogenesis. The characteristic feature of the syndrome is the presence of a major trauma or surgical history. Although this is an unc

Acute Colonic Pseudo-Obstruction Following the Use of Dexmedetomidine.

Dexmedetomidine is a preferred agent for light sedation with minimal adverse effects. We report a case of acute colonic pseudo-obstruction following dexmedetomidine use in a patient with alcohol withdrawal. He was treated with benzodiazepines first to control the withdrawal symptoms, then escalated to dexmedetomidine once delirium tremens ensued. Later on, the patient developed abdominal distension and vomiting. Imaging showed dilated bowel loops and absence of peristalsis on ultrasound. Decompression with the nasogastric (NG) tube was done, with high output from the NG tube. Dexmedetomidine i

[Acute colonic pseudo-obstruction after caesarean section; Ogilvie's syndrome].

BACKGROUND: Acute pseudo-obstruction of the colon - also known as Ogilvie's syndrome - is a rare clinical presentation in obstetrics. The syndrome is seen more often following caesarean section than vaginal delivery. CASE DESCRIPTION: We present a 38-year-old primigravida who developed Ogilvie's syndrome following secondary caesarean section. Despite conservative management, due to a caecal dilation of over 12 centimetres and impending perforation of the caecum, a hemicolectomy was necessary. CONCLUSION: Ogilvie's syndrome is potentially life-threatening, certainly if perforation of the bowe

Colonic pseudo-obstruction in a patient with Sheehan's syndrome.

A 56-year-old woman with a history of hypothyroidism and chronic constipation presented with an acute abdomen due to colonic pseudo-obstruction. Thyroid function tests were consistent with central hypothyroidism prompting intravenous administration of stress-dose glucocorticoids and levothyroxine. The patient then underwent emergency exploratory laparotomy with sigmoid resection and end-colostomy. The postoperative endocrine evaluation revealed that the patient had panhypopituitarism due to Sheehan's syndrome (SS). The diagnosis had been missed by physicians who had been treating her for sever

Acute colonic pseudo-obstruction after elective total joint arthroplasty.

A retrospective review of 31 patients who developed acute colonic pseudo-obstruction (ACPO) after total joint arthroplasty was undertaken to determine predisposing factors related to, and outcomes following, therapeutic intervention. Comparison with all patients who underwent total joint arthroplasty revealed an overall 1.2% incidence of ACPO. There was a higher incidence of ACPO in patients undergoing sequential bilateral total knee arthroplasty (3.4%) compared with unilateral total knee arthroplasty (0.3%) and a higher incidence in patients undergoing revision total hip arthroplasty (5.6%) c

Colonic manometry in children with chronic intestinal pseudo-obstruction.

Pressure changes were evaluated in the transverse, descending, and rectosigmoid colon of 30 children with chronic intestinal pseudo-obstruction. Twenty two had severe lifelong constipation and eight had symptoms suggesting a motility disorder exclusively of the upper gastrointestinal tract. Based on prior antroduodenal manometry, 24 children were diagnosed as having a neuropathic and six a myopathic form of intestinal pseudo-obstruction. On the day of study, endoscopy was used to place a manometry catheter into the transverse colon and intraluminal pressure was recorded for more than four hour
