Assessing attitudes toward laxative preparation in colorectal cancer screening and effects on future testing: potential receptivity to computed tomographic colonography.

OBJECTIVE: To investigate whether removal of the laxative preparation would improve colorectal cancer (CRC) screening rates. RESPONDENTS AND METHODS: Data are from a mixed-mode mail and telephone survey of 1636 noninstitutionalized Minnesota residents who were 18 years or older that was conducted from July 15 through October 25, 2005. The overall response rate was 49%. Analysis focused on the 759 respondents who were 50 years or older. RESULTS: One third (32%) of the respondents identified the laxative preparation as the most troublesome part of CRC screening. Overall, the self-reported likel

Determination of the positional relationship of the second Houston valve and peritoneal reflection using computed tomographic colonography and magnetic resonance imaging.

PURPOSE: The second Houston valve is used as a surrogate for estimating the position of the peritoneal reflection; however, the concordance between the positions of the valve and peritoneal reflection has not been investigated. This study aimed to clarify this positional relationship. METHODS: The second Houston valve and peritoneal reflection positions were assessed using tomographic colonography and magnetic resonance imaging. In total, 117 patients were enrolled in this study. RESULTS: The positions of the second Houston valve and peritoneal reflection were nearly concordant, although the

Prospective blinded trial comparing 45-mL and 90-mL doses of oral sodium phosphate for bowel preparation before computed tomographic colonography.

OBJECTIVE: To compare the efficacy of computed tomographic colonography (CTC) bowel preparation using single- (45 mL) versus double-dose sodium phosphate. MATERIALS AND METHODS: A hundred consecutive patients undergoing screening CTC were randomly assigned with either single (45 mL) or double dose of sodium phosphate. Stool/fluid tagging remained constant. Two radiologists with extensive CTC experience, blinded to the groups, prospectively scored the colon for residual stool and fluid by using a 4-point scale. RESULTS: There was an excellent cleansing in both groups characterized by low stoo

Clinical indications for computed tomographic colonography: European Society of Gastrointestinal Endoscopy (ESGE) and European Society of Gastrointestinal and Abdominal Radiology (ESGAR) Guideline.

This is an official guideline of the European Society of Gastrointestinal Endoscopy (ESGE) and the European Society of Gastrointestinal and Abdominal Radiology (ESGAR). It addresses the clinical indications for the use of computed tomographic colonography (CTC). A targeted literature search was performed to evaluate the evidence supporting the use of CTC. The Grading of Recommendations Assessment, Development and Evaluation (GRADE) system was adopted to define the strength of recommendations and the quality of evidence. Main recommendations 1 ESGE/ESGAR recommend computed tomographic colonogra

Computed tomographic colonography versus barium enema for diagnosis of colorectal cancer or large polyps in symptomatic patients (SIGGAR): a multicentre randomised trial.

BACKGROUND: Barium enema (BE) is widely available for diagnosis of colorectal cancer despite concerns about its accuracy and acceptability. Computed tomographic colonography (CTC) might be a more sensitive and acceptable alternative. We aimed to compare CTC and BE for diagnosis of colorectal cancer or large polyps in symptomatic patients in clinical practice. METHODS: This pragmatic multicentre randomised trial recruited patients with symptoms suggestive of colorectal cancer from 21 UK hospitals. Eligible patients were aged 55 years or older and regarded by their referring clinician as suitab

CT colonography: Why? When? How?

Although optical colonoscopy is still the gold standard for diseases of the colon, radiologic examination of the colon is now being performed by CT scan. Evaluation of the colon is enhanced by distension, which "de-folds" the intestinal wall, thus facilitating its examination for abnormalities of the mucosa, the wall as a whole, and the diameter of the bowel lumen. Water or gas (CO(2)) may be used for the distension, depending on the suspected lesions. The water enema method of colonography combines filling the bowel lumen with water and intravenous injection of a contrast medium. It is indica

Virtual colonoscopy: a new promising technique.

OBJECTIVES: Virtual colonoscopy is less invasive than conventional colonoscopy and does not require a conscious sedation. BACKGROUND: Virtual colonoscopy using the abdominal spiral computed tomography scanning allows a total colonic evaluation with minimal invasiveness. METHODS: We studied 48 patients with a virtual colonoscopy using oral iodinated contrast. Colonic lavage was achieved with an oral polyethylene glycols preparation. We examined patients who had refused a colonoscopic examinations, or patients with a stenotic processes, in which it was not possible to examine the proximal colo

The role of CT Colonography in the assessment of inflammatory bowel Diseases patients.

Inflammatory bowel disease (IBD), including Crohn's disease (CD) and Ulcerative Colitis (UC), is characterized by chronic inflammation of the gastrointestinal tract, and its long-standing course significantly elevates the risk of colorectal cancer (CRC), primarily arising from dysplastic lesions. While regular surveillance by colonoscopy is well established for UC patients, guidelines for CD remain uncertain. Computed Tomographic Colonography (CTC) offers a minimally invasive alternative for evaluating the colon, particularly in cases where colonoscopy is incomplete or contraindicated. Althoug

Colon centerline extraction in fragmented segmentations.

In virtual colonoscopy, the clinical need is a smooth centered path from the rectum to the cecum, for interactive navigation along the colonic lumen. The primary challenge is breakages in the colon, due to fecal residue, abnormalities, poor insufflation and inadequate electronic cleansing. Here we propose a method, that is a modification of the classic energy minimized geodesic, that extracts centered paths through fragmented colons. To begin, we perform electronic cleansing, automatically localize 4 points: rectum, cecum, sphlenic and hepatic flexures; followed by region growing and heuristic

[A case of Diphyllobothrium nihonkaiense identified by virtual enterography and virtual enteroscopy].

In October 2007, a woman in her early 30s presented, complaining of worm discharge in her stool. We identified it as Diphyllobothrium nihonkaiense, and identified it by virtual enterography and virtual enteroscopy. It was treated by 1500 mg/day praziquantel orally. We think these methods, of pathology as well as capsule enteroscopy and enteroscopy, are useful for the searching of the small intestinal lesions. We hope these examinations will become more widespread with reference to these cases.

Clinically validated dataset of 435 human colons segmented from CT colonography.

High-quality segmentation datasets are essential for advancing AI applications in medical imaging. However, it is challenging to generate such datasets for highly variable and complex organs like the colon. We introduce a dataset of 435 human colons, segmented from Computed Tomography Colonography (CTC) obtained from the publicly available The Cancer Imaging Archive (TCIA). Each scan includes a mask of the whole colon, including collapsed segments and the fluid, and a mask of only the gas-filled parts of the colon. The colon segmentation accuracy has been clinically validated by an expert abdo

Alternatives to colonoscopy for population-wide colorectal cancer screening.

Colorectal cancer is one of the top three cancers in the world in terms of incidence. Colonoscopy, which many regard as the gold standard in diagnosis of colonic polyps and neoplasm, is costly, invasive and labour-intensive, and deemed an unsuitable population-wide index screening tool. Alternative modalities, including guaiac and immunohistochemical faecal occult blood tests, computed tomographic colonography, colon capsule endoscopy, flexible sigmoidoscopy, and double-contrast barium enema are available. The procedures, test characteristics, and their implications are reviewed. Immunohistoch

[Virtual colonoscopy in stenosing colorectal cancer].

In 5-10% of patients it is not possible to achieve a complete endoscopic examination of the colon, because of obstructing cancer, excessive length of the colon, anatomical abnormalities or adhesions. Virtual colonoscopy is currently capable of investigating the colic lumen with a non-invasive technique, with high specificity and sensitivity. From January 2005 to July 2007 we treated 21 patients with obstructing neoplastic colorectal lesions, preventing a complete endoscopic examination. In all patients we performed a virtual colonoscopy, which revealed the presence of synchronous lesions (19%)

CT Colonography Reporting and Data System (C-RADS): Version 2023 Update.

The CT Colonography Reporting and Data System (C-RADS) has withstood the test of time and proven to be a robust classification scheme for CT colonography (CTC) findings. C-RADS version 2023 represents an update on the scheme used for colorectal and extracolonic findings at CTC. The update provides useful insights gained since the implementation of the original system in 2005. Increased experience has demonstrated confusion on how to classify the mass-like appearance of the colon consisting of soft tissue attenuation that occurs in segments with acute or chronic diverticulitis. Therefore, the u

Colorectal cancer screening: the potential role of virtual colonoscopy.

Virtual colonoscopy is a promising new technique that combines rapid spiral CT scanning of the abdomen with advanced computer programs capable of re-creating two- and three-dimensional views of the colon and rectum. Recent studies comparing this method with conventional colonoscopy show that virtual colonoscopy already is more accurate than barium enema X-ray studies for the detection of colorectal polyps, and that it approaches the accuracy of colonoscopy for diagnosing advanced lesions. Before virtual colonoscopy can be promoted for population-based screening for colorectal cancer, a number

Virtual colonoscopy-induced perforation in a patient with Crohn's disease.

We report a case of sigmoid colon perforation in a patient with Crohn's disease undergoing computed-tomographic (CT) colonography. A 70-year-old patient with Crohn's disease with terminal ileitis and sigmoid stricture underwent CT colonography after incomplete conventional colonoscopy. During the procedure, the colon was inflated by air insufflation and the patient developed abdominal pain with radiological evidence of retroperitoneal and intraperitoneal free gas. Hartmann's operation was performed. This case highlights that CT colonography is not risk-free. The risk of perforation may be high
