Head Trauma in Elderly Patients: Mechanisms of Injuries and CT Findings.

BACKGROUND: Head injuries in elderly people are a common cause of hospitalization at emergency departments. This group of patients is at high risk of post-traumatic intracranial pathology, which is diagnosed by computed tomography (CT) scanning of the head. OBJECTIVES: The aim of this study was to determine the incidence and outcomes of head trauma in older people in different scenarios, on the basis of CT scan findings. MATERIAL AND METHODS: The study involved a retrospective analysis of medical records of patients treated in the Emergency Department of Copernicus Memorial Hospital in Lodz,

Propofol sedation in severe head injury fails to control high ICP, but reduces brain metabolism.

We have compared the effects of an intravenous infusion of propofol with those of morphine and midazolam on global brain metabolism (AVDO2) and brain perfusion following severe head injury. Fifteen patients were sedated with either a continuous infusion of propofol (mean rate 232 mg/h, range 150-400 mg/h) or infusions of morphine (mean rate 2.3 mg/h, range 0-4 mg/h) and midazolam (mean rate 2.8 mg/h, range 0-5 mg/h). Both groups were well matched for sex, age and level of coma (Glasgow coma scale) prior to sedation. Continuous data collection of AVDO2, mean arterial blood pressure (MABP), intr

[A guide to initial management of minor head injury].

We reviewed the records of 1,335 minor head injury patients with initial Glasgow Coma Scale (GCS) scores of 15 treated by our neurosurgery service between January 1998 and December 2000. Skull X-ray was performed in 945 patients (71%), and Computed tomography (CT) was performed in 590 patients (44%). Skull fracture was shown radiographically in 24 patients (2.5%), and abnormalities on the initial CT were seen in 29 patients (4.9%). The most frequent intracranial lesion on CT was acute epidural hematoma with skull fracture. Significantly more intracranial lesions were found in those with a frac

Head injuries in children under 36 months of age. Demography and outcome.

Head injuries in children under the age of 3 years have not been extensively studied, due in part to the lack of an objective tool for neurological assessment. We have developed a Children's Coma Scale (CCS) by modifying the verbal response subscore of the Glasgow Coma Scale (GCS) to overcome this limitation. When applied prospectively to children under 3 years of age, we found the CCS to be useful in predicting outcome. During the 5-year study period from 1981 to 1985, there were 738 patients with head injuries (0-16 years) admitted to the Children's Memorial Hospital in Chicago, including 31

[The neurological and ophthalmic symptoms in the child after the serious head injury due to the fall from the fifth storey. A case report].

INTRODUCTION: The prognosis in the case of head injury may be different, Serious head injures are the leading causes of the death. In the remaining cases, the child recovers entirely, or disturbances of the cognitive and motor activities remain. The aim of the work is to show neurological and ophthalmological complications at the child after the fali down from the fifth storey. CASE REPORTS: A four-year-old girl with severe injuries of the body as result of the fall down from the fifth storey has been described. She had serious changes within head--the bruise of the brain with typical symptom

White matter damage following acute head injury.

The study of a series of brains from patients who had a severe head injury and died within 72 h without a lucid interval showed that there was a step-wise progression in the development of retraction balls. At 2 h after injury sinusoidal enlargement of the axons was evident. This progressed over 16 h when the lesions appeared as retraction balls which were fully developed at 72 h. There was a similar increase of staining with an immunoperoxidase method for glial fibrillary acid protein (GFAP) initially around blood vessels spreading diffusely into the white matter. The number of reactive astro

Prognostic implications of hyperglycaemia in paediatric head injury.

Fifty children with head injury were evaluated in an attempt to establish a correlation between post-traumatic hyperglycaemia and long-term outcome. In all the patients, the blood glucose level was measured on admission and on the days following the trauma (threshold of normal value set at 150 mg/dl). Hyperglycaemia was seen more frequently in children with severe head injury than in those with mild and moderate head injury. It was present in 87.5% of the patients with a Glasgow Coma Score (GCS) < or =8 (the average blood glucose level on admission was 237.8+/-92 mg/dl), in 60% of the patients

Urgent airways after trauma: who gets pneumonia?

BACKGROUND: Several risk factors, including emergent intubation, severe head injury, shock, blunt trauma, and high severity of injury, have been identified as risk factors for the development of pneumonia after trauma. This study assesses the contribution of emergent intubation to the development of pneumonia after injury. METHODS: A retrospective review of all trauma patients requiring intubation or cricothyroidotomy in the Emergency Department (ED) or in the pre-hospital area (field) over a 41/2 year period. RESULTS: 571 patients comprised the study population. Of these, 80% had airways es

Management of raised intracranial pressure after severe head injury.

The mainstay of early management following severe head injury (less than 8 on the Glasgow coma scale [GCS]) remains urgent cardiopulmonary resuscitation, triage, transfer to a neurosurgical centre, emergency CT scanning and appropriate surgical intervention (Rose et al, 1977; Seelig et al, 1981; Langfitt and Gennarelli, 1982; Bricolo and Pasut, 1984). Subsequent therapy usually includes a period of intensive care with or without artificial ventilation (Becker et al, 1977; Bruce et al, 1978; Marshall et al, 1979). Intracranial pressure (ICP) monitoring is employed in many centres during the fir

Prevalence of post-concussion syndrome and associated factors among patients with traumatic brain injury at Debre Tabor Comprehensive Hospital, North Central Ethiopia.

INTRODUCTION: The occurrence of three or more of the following signs and symptoms, such as headache, dizziness, exhaustion, irritability, sleeplessness, difficulties in concentrating, or memory problems, following a head injury is referred to as post-concussion syndrome (PCS). Even though post-concussion syndrome has not been studied in Ethiopia, the productive age group is frequently affected by health issues related to head trauma, which either directly or indirectly affect the growth of the nation. OBJECTIVE: To assess the prevalence and associated factors of post-concussion syndrome among

Functional and Structural Integrity of Frontoparietal Connectivity in Traumatic and Anoxic Coma.

OBJECTIVES: Recovery from coma might critically depend on the structural and functional integrity of frontoparietal networks. We aimed to measure this integrity in traumatic brain injury and anoxo-ischemic (cardiac arrest) coma patients by using an original multimodal MRI protocol. DESIGN: Prospective cohort study. SETTING: Three Intensive Critical Care Units affiliated to the University in Toulouse (France). PATIENTS: We longitudinally recruited 43 coma patients (Glasgow Coma Scale at the admission < 8; 29 cardiac arrest and 14 traumatic brain injury) and 34 age-matched healthy volunteers.

Neuro-ophthalmic manifestations in traumatic brain injury: a hospital-based study from Nepal.

OBJECTIVE: Neuro-ophthalmic manifestations occur frequently in patients with a traumatic head injury which are often ignored, leading to late presentation with permanent visual disability and visual field defects. This study aimed to find the various neuro-ophthalmic manifestations of traumatic brain injury and correlate them with the neurological status and neuroimaging abnormality. METHODS: A hospital-based cross-sectional study was carried out among patients admitted to our centre with a diagnosis of traumatic brain injury for a period of six months from August 2020 to January 2021. A successful initial management of a penetrating head trauma in a rural district hospital: Case report.

Penetrating head trauma is the most fatal form of head injury. Although many cases of penetrating head trauma have been reported in the literature, its management remains complex, requiring a multidisciplinary team, which makes it a challenge in district hospitals where human resources and technical equipment are limited. We aim to present a successful initial management of a case of penetrating head trauma in a rural district hospital before a transfer to a first category hospital. A 27-year-old man, smoker with no pathology history was involved in a road accident, falling from a motorbike wi

Risk Factors Associated with the Outcome of Post-Traumatic Hydrocephalus.

BACKGROUND AND AIM: Post-traumatic hydrocephalus is a common complication that arises after head injury. However, risk factors associated with the outcome of post-traumatic hydrocephalus have seldom been addressed. Therefore, we performed this clinical study to analyze the risk factors affecting the outcome of post-traumatic hydrocephalus in patients with head injuries. METHODS: A total of 116 post-traumatic hydrocephalus patients, admitted in our hospital between March 2012 and October 2017 were reviewed. The related factors assessed were age, gender, Glasgow Coma Score on admission, platele

Clinical Signs of Basilar Skull Fracture and Their Predictive Value in Diagnosis of This Injury.

Although clinical signs for the diagnosis of basilar skull fracture (BSF) are ambiguous, they are widely used to make decisions on initial interventions involving trauma patients. We aimed to assess the performance of early and late (within 48 hr posttrauma) signs for BSF diagnosis and to verify the correlation between the presence of these signs and head injury severity. We conducted a prospectively designed follow-up study at a referral hospital for trauma care in Sao Paulo, Brazil, and performed structured observations for 48 hr post-blunt head injury in patients aged 12 years or older. Basilar artery vasospasm after severe head injury: a preliminary transcranial Doppler ultrasound study.

BACKGROUND: Post-traumatic vasospasm after severe head injury is now a well known entity. However, all studies available in the literature have evaluated only the anterior cerebral circulation. We evaluated the incidence of basilar artery vasospasm in patients with severe head injury. METHODS: Basilar artery mean blood flow velocity was measured in 16 patients with severe closed head injury (Glasgow Coma Scale 8 or less) using transcranial Doppler ultrasonography. Ten normal subjects also underwent the same investigation. The patients' age ranged from 5 to 65 years. The study group included 1

Comparative analysis of plasma and salivary S100B as predictors of traumatic brain injury severity according to GCS and MAIS/head.

BACKGROUND: Accurate assessment of traumatic brain injury (TBI) severity is essential for clinical management and outcome prediction. Traditional tools, like the Glasgow Coma Scale (GCS) and Maximum Abbreviated Injury Scale for head injuries (MAIS/Head), have limitations, particularly in cases involving sedation, complex injuries, or diffuse brain injuries. This study evaluates the diagnostic accuracy of plasma and salivary S100B levels as biomarkers for TBI severity, based on GCS and MAIS/Head, with emphasis on emergency settings. METHODS: Multicenter prospective cohort study enrolled 57 adu

The burden of head trauma in rural and remote North Queensland, Australia.

BACKGROUND: Head trauma is a leading cause of death and disability worldwide. Young males, Indigenous people, and rural/remote residents have been identified as high-risk populations for head trauma, however, Australian research is limited. Our aim was to define and describe the incidence, demographics, causes, prehospital interventions, and outcomes of head trauma patients transported by aeromedical services within North Queensland, Australia. We hypothesized that young, Indigenous males living remotely would be disproportionately affected by head trauma. METHODS: We conducted a retrospectiv

Assessment of mortality associated with mild head injury in the pediatric age group.

Reducing mortality among accident and trauma patients requires careful attention to monitoring those regarded as being at low risk. We hospitalized almost 1600 head-injured patients in the period between 1979 and 1992 at the Neurosurgery Department of Gazi University Medical School, Ankara, Turkey. These patients were selected from among the numerous patients admitted to our emergency unit and treated with the same protocol in the same department. Among the hospitalized children, there were three patients defined as having a mild head injury on the basis of Glasgow Coma Scale scores of 15 who
