Changing prescribing patterns in an Irish community mental health service.

OBJECTIVES: Medication is an important component of the treatment of many mental illnesses. Very little information is available about the particular medications that are being prescribed by community mental health services and how this has changed over time. We set out to obtain details of psychiatric medications being prescribed by one Irish community mental health service. METHOD: All prescribing by the Cluain Mhuire Community Mental Health Service became electronic during 2004. Using Business Intelligence software, we obtained details of all psychiatric medications prescribed from 2005 to

Strategies for adapting under pressure: an interview study in community mental health services.

BACKGROUND: Mental health services are operating under significant and sustained pressures. Healthcare professionals and managers are continually making adjustments, often improvised and inconsistent across teams. This study aimed to identify and describe the everyday pressures experienced in community mental health settings and the adaptive strategies used by senior leadership and management staff to respond. The longer-term goal is to inform more coordinated, practical approaches to managing services under pressure. METHODS: We conducted qualitative semi-structured interviews with 22 senior

Increasing chronic disease preventive care in community mental health services: clinician-generated strategies.

BACKGROUND: People with a mental health condition experience a high prevalence of chronic disease risk behaviours e.g., tobacco smoking and physical inactivity. Recommended 'preventive care' to address these risks is infrequently provided by community mental health services. This study aimed to elucidate, among community mental health managers and clinicians, suggestions for strategies to support provision of preventive care. METHODS: Three qualitative focus groups (n = 14 clinicians) were undertaken in one regional community mental health service to gather perspectives of barriers to prevent

A community survey on maternal and child health services utilization in rural Ethiopia.

A household health interview survey on MCH services utilization was carried out in 22 villages of a rural district of Arsi region, Ethiopia, before the launching of an integrated MCH programme. Coverage of antenatal services was 26%, and 61% of the women who received antenatal care reported having had 3 or more visits. Antenatal care was positively associated with living within 10 km of the Health Centre. Twenty-eight percent of the mothers attended the under-5 clinic and most returned for 3 or more visits. In addition, 99% reported having breast-fed their last child but more than 25% started

Community health nurses' knowledge of, attitudes toward, and involvement with adolescent contraceptive services.

A national sample of community health nurses were surveyed to assess their knowledge of, attitudes toward, and involvement with reproductive health services. Results indicated that slightly more than half of the nurses provided or administered contraceptive services to adolescents, although 73% worked in settings where contraceptive services were provided to adolescents. However, only 3.6% indicated that adolescents were their primary client population. The majority (64.3%) stated that they felt as prepared to work with adolescents as they did with adult clients. While 98% were aware that most

Depressive symptoms and health service utilization among the community elderly.

The relationship between depression and complaints of physical disease in the elderly is often discussed but has received little documentation. The authors administered questionnaires to 88 elderly volunteers to assess two common psychologic diagnoses, affective disorders and organic brain syndrome, and three common cardiovascular symptoms, pain or numbness in the jaw or arm, pressure in the chest, and difficulty in breathing. Data on utilization of health care professionals and demographic information were also obtained. A significant correlation between reports of depression and complaints o

Rethinking health care cultures in community services: Towards an operational five-criteria framework.

Calls for cultural change in health services often remain confined to managerial or ethical discourse, lacking critical depth and operational clarity. This article develops the concept of health care cultures (HCCs) as an analytical category in its own right: dynamic configurations of power, knowledge and practice that actively shape how care is conceived, delivered and governed. Drawing on a Gramscian framework, feminist care theory, and the literature on structural competency, I conceptualise HCCs as sites of tension between hegemonic and subaltern forms of knowledge and practice. This refra

Complex PTSD among persons with serious mental illness receiving community mental health services.

BACKGROUND: Complex PTSD (CPTSD) is often associated with prolonged or repeated trauma exposure and the experience of intimate partner and childhood abuse. CPTSD includes the criteria for PTSD (re-experiencing, avoidance, and sense of threat) in addition to three criteria for self-organization disturbances (affective dysregulation, negative self-concept, and relational disturbance). METHODS: This study aimed to assess profiles of CPTSD symptoms and their association with psychiatric distress among people with co-occurring Serious Mental Illness (SMI; schizophrenia/schizoaffective, bipolar, an

Communication Over Incarceration: Improving Care Coordination Between Correctional and Community Mental Health Services.

Millions of people are incarcerated every year in the United States, many of whom have mental disorders and substance use disorders. Alongside the considerable churn of people into and out of U.S. jails and prisons, numerous barriers often impede information exchange between correctional and community mental health services, disrupting continuity of care and threatening the well-being of incarcerated and formerly incarcerated people. This Open Forum examines barriers to information sharing, a critical component of care coordination, between correctional and community mental health services and

Correlates for Suicidality Among At-risk Youth Receiving Community-Based Mental Health Services.

"At-risk" adolescents are at high risk of unsuccessfully transitioning into adulthood and are also at elevated risk for suicidal behavior. Though much research has been conducted on risk factors for suicidality among the general adolescent population, research on suicidality among "at-risk" adolescents is lacking. This is a notable gap in the literature given that "at-risk" adolescents may be three times more likely to exhibit suicidality. The present study addressed this research gap by examining correlates for suicidality among "at-risk" adolescents receiving mental health services in the co

Further Inspection: Integrating Housing Code Enforcement and Social Services to Improve Community Health.

As a result of working inside homes, city housing inspectors witness hidden and serious threats to public health. However, systems to respond to the range of problems they encounter are lacking. In this study, we describe the impact and enabling environment for integrating a novel Social Service Referral Program within the Inspectional Services Department in Chelsea, MA. To evaluate the first eight months of the program, we used a mixed-methods approach combining quantitative data from 15 referrals and qualitative interviews with six key informants (inspectors, a case manager, and city leaders

Partnering for Vaccine Equity: A Public Health-Community Action Model to Advance Delivery of Essential Health Services.

As part of CDC's Partnering for Vaccine Equity Program, the Association of State and Territorial Health Officials worked with the National Community Action Partnership and five community action agencies (CAAs) to address disparities in adult immunization among racial and ethnic minority populations. CAAs leveraged partnerships with public health, healthcare, and other local entities to increase uptake of COVID-19 and other vaccines, while simultaneously addressing related social determinants of health. With over 1000 agencies across the United States, including state associations, CAAs are acc

Abortion care services delivered from a community sexual and reproductive health setting: views of health care professionals.

BACKGROUND: Abortion services should provide high-quality contraceptive care. The community sexual and reproductive health (SRH) services may be well placed to deliver more abortion care in the UK. We wished to determine the views of health professionals working in SRH regarding their attitudes towards providing more abortion services and also the views of staff within one community SRH centre in Scotland where a service providing early medical abortion (EMA) was due to commence. METHODS: An anonymous questionnaire distributed to attendees at a UK SRH scientific meeting collected data on demo

Savoring the service: A qualitative study of Latine community health workers' health promotion efforts.

Latine communities have long faced stark health inequities and systemic barriers to accessing healthcare services. Community health workers (CHWs) serve as a key link between patients and the often-daunting healthcare system, providing crucial health education and support. Despite this important yet demanding role, little is known about CHWs' personal experiences of community health promotion and how they might draw upon their work for strength and endurance. The present study explored the potential for relational savoring, a procedure that elicits feelings of closeness and connection, to capt

The relationship between community public health, behavioral health service accessibility, and mass incarceration.

BACKGROUND: The relationship between healthcare service accessibility in the community and incarceration is an important, yet not widely understood, phenomenon. Community behavioral health and the criminal legal systems are treated separately, which creates a competing demand to confront mass incarceration and expand available services. As a result, the relationship between behavioral health services, demographics and community factors, and incarceration rate has not been well addressed. Understanding potential drivers of incarceration, including access to community-based services, is necessar

Community health volunteers could help improve access to and use of essential health services by communities in LMICs: an umbrella review.

A number of primary studies and systematic reviews focused on the contribution of community health workers (CHWs) in the delivery of essential health services. In many countries, a cadre of informal health workers also provide services on a volunteer basis [community health volunteers (CHV)], but there has been no synthesis of studies investigating their role and potential contribution across a range of health conditions; most existing studies are narrowly focused on a single condition. As this cadre grows in importance, there is a need to examine the evidence on whether and how CHVs can impro
