[Relationship between crowding and community acquired skin and soft tissues infections].

The association of the spread of community infections with poverty and overcrowding is well known. In our hospital, located in José C. Paz, we assist sporadic cases of post-cesarean infections caused by community acquired methicillin-resistant Staphylococcus aureus (CaMRSA). In a prospective study of families treated at our hospital, we investigated the relationship between a history of skin and soft tissue infections (SSTI) and extreme overcrowding defined as households with unsatisfied basic needs type 3 (NBI 3). A total of 264 households were included in the study; 109 (41.3%) had a history

The impact of viral infections in children with community-acquired pneumonia.

For many years viral causes of community-acquired pneumonia were often been given limited attention. The number of published studies on influenza alone increased fivefold from 2001 to 2010. Specifically several studies have underlined that the involvement of viruses in community-acquired pneumonia has been underestimated, and this underestimation has been attributed to a lack of appropriate diagnostic methods. Now, with the advent of modern molecular assays, it is well recognized that viruses account for the largest proportion of community-acquired pneumonia in preschool children in both devel

[Etiological factors of community acquired urinary tract infections in hospitalized patients].

INTRODUCTION: Etiological factors of community-acquired urinary tract infections (UTI) are specific for age, gender, season, complication of UTI and type of UTI. Their prevalence and susceptibility to antimicrobial agents shows geographic and time variability. PURPOSE: To evaluate etiological characteristics of acute community-acquired UTI in hospitalised patients. PATIENTS AND METHODS: This retrospective-prospective study included 200 adult patients with community-acquired UTI who were, in view of the serious clinical picture and unsuccessful ambulatory treatment, hospitalised in the Clinic

Common community acquired infections and subsequent risk of chronic lymphocytic leukaemia.

Emerging evidence supports a role for immune-related factors in the causation of chronic lymphocytic leukaemia (CLL). Using the population-based U.S. Surveillance Epidemiology and End Results-Medicare database, 10,171 elderly CLL patients and 122,531 frequency-matched controls were identified in order to evaluate several community acquired infections associated with subsequent CLL risk. Odds ratios (ORs) were adjusted for gender, age, race, calendar year and number of physician claims. CLL risk was increased following Medicare claims for sinusitis (OR = 1.11; 95% CI = 1.05-1.17), pharyngitis (

Healthy children with invasive community-acquired methicillin-resistant Staphylococcus aureus infections.

Reports of community-acquired methicillin-resistant Staphylococcus aureus (CA-MRSA) infections in the pediatric community have exploded during the past decade. These infections typically result in mild skin and soft tissue infections that can be managed simply with oral antimicrobials. Recently, there have been reports of invasive CA-MRSA infecting children without risk factors, with isolated cases of life-threatening disease. We report 2 atypical cases of invasive CA-MRSA infecting previously healthy children.

Multidrug-resistant Escherichia coli clonal groups causing community-acquired bloodstream infections.

OBJECTIVES: A multidrug-resistant Escherichia coli clonal group (designated CgA) has been isolated from women with cystitis and pyelonephritis in several communities. This study was designed to determine if CgA can cause community-acquired bloodstream infections. METHODS: All community-acquired bloodstream infections caused by E. coli identified at the San Francisco General Hospital between May 2001 and May 2003 were included. The diagnosis of septicemia was based on admission diagnosis. E. coli isolates were characterized by antibiotic susceptibility profile, enterobacterial repetitive inter

[Role of thiamphenicol in the treatment of community-acquired lung infections].

Streptococcus pneumoniae and Haemophilus influenzae are the two main pathogens responsible for bacterial respiratory tract infections. Their antimicrobial susceptibility to antibiotics like beta-lactams, macrolides or fluoroquinolones has been largely studied, while it remains less known to other antibiotics like thiamphenicol, erythromycin, cotrimoxazole or tetracycline, often used in developing countries due to their availability. In this study, the activity of chloramphenicol and thiamphenicol on different respiratory tract pathogens was found to be equivalent. However, thiamphenicol was be

Chlamydia pneumoniae infections in patients with community-acquired pneumonia in Slovenia.

It is well known that Chlamydia pneumoniae is an important respiratory pathogen. In this study, the prevalence of specific antibodies to C. pneumoniae in hospitalized patients with community-acquired pneumonia (943 adult females, 990 adult males and 185 children) was evaluated over a period of 7 y (1993-99). Two serum samples were obtained from all of the patients: 1 on admission and the other 3 weeks later. The specimens were tested for C. pneumoniae IgG, IgM and IgA antibodies by means of a microimmunofluorescence test. Acute infection with C. pneumoniae was determined in 9.4% of females and

Cost of care for inpatients with community-acquired intra-abdominal infections.

Few studies have assessed the actual costs associated with failure of initial empiric antibiotic therapy administered to patients with community-acquired intra-abdominal infections. The goals of this study were (i) to determine the frequency of unsuccessful initial empiric therapy in a real-world setting and (ii) to determine the associated impact on medical costs. Thus, a retrospective chart review was performed at four acute-care university hospitals in France. A total of 292 patients hospitalized for community-acquired intra-abdominal infection were included. The mean age of the cohort was

The role of fluoroquinolones in respiratory tract infections: community acquired pneumonia.

Newer fluoroquinolones may play an important role in the management of community acquired pneumonia. They retain activity similar to older fluoroquinolones against Gram-negative bacteria and are significantly more active against Gram-positive bacteria, especially pneumococci. They are also active against bacteria causing atypical pneumonia, penicillin-sensitive and -resistant and macrolide-sensitive and -resistant pneumococci and against beta-lactamase producing and non-producing Haemophilus influenzae. They have similar or slightly lower activity than ciprofloxacin against other Gram-negative

Community acquired bacterial infections and their antimicrobial susceptibility in Nairobi, Kenya.

The purpose of the study was to determine the pattern and antimicrobial sensitivity on community acquired bacterial strains in Nairobi, Kenya. Clinical specimens collected from out-patient clinics at the Kenyatta National Hospital were cultured on appropriate media and identified according to Cowen and Steel's manual. The antimicrobial sensitivity was determined using comparative disc diffusion techniques. Between 1991 and 1995, there were a total of 1659 positive cultures comprising 30 different bacterial species. Out of the overall gram negative isolates (61.9%), E.coli and Klebsiella spp fo

The emergence of severe, community-acquired methicillin-resistant Staphylococcus aureus infections.

The number of community acquired methicillin-resistant Staphylococcus aureus (CA-MRSA) infections is rapidly increasing. Most CA-MRSA infections are localized soft tissue infections; however, severe life-threatening infections have been occasionally described. This report serves to increase the awareness of severe CA-MRSA infections by presenting a fulminant CA-MRSA infection with sepsis, endocarditis, septic pulmonary emboli, and extensive soft tissue and bone destruction. A review of the literature revealed 14 cases of severe CA-MRSA infections with a median age of 13 y; 93% had no underlyin

Evaluation and cost assessment of fluoroquinolones in community-acquired respiratory infections.

Several new fluoroquinolones have been marketed since the late 1990s. Fluoroquinolones are an effective treatment for most community-acquired respiratory tract infections, including acute sinusitis, acute exacerbations of chronic bronchitis and community-acquired pneumonia. However, other antibiotics, including beta-lactams, macrolides, tetracyclines and trimethoprim-sulfamethoxazole, are also effective against these respiratory infections. From a managed care perspective, it is the subtle differences between the drugs in the eradication of bacterial pathogens, adverse effects, dose regimens,

Novel cost-effective approaches to the treatment of community-acquired infections.

OBJECTIVE: To review approaches to the diagnosis and treatment of patients with community-acquired infections. INTRODUCTION: Dramatic changes in the antibiotic susceptibility of pathogens commonly associated with community-acquired infections have occurred during the past decade. DISCUSSION: Changes in the antibiotic sensitivity profile of Streptococcus pneumoniae, Haemophilus influenzae, and Moraxella catarrhalis that have occurred over the past several years have required modifications in empiric antibiotic selections for infections due to these pathogens. The most profound changes have oc

The prevalence of nosocomial and community-acquired infections in Australian hospitals.

The first national survey of the prevalence of nosocomial and community-acquired infections in Australian hospitals was carried out during July 1984. Data were collected on 28,643 patients in rural and metropolitan, public and private acute-care hospitals. The over-all adjusted prevalence of nosocomial infections was 6.3%; the prevalence of community-acquired infection was 9.7%. A total of 5940 infections occurred; 39% were hospital-acquired infections and 61% were community-acquired infections. These occurred at the following sites: the respiratory tract, 35.4% (2100 infections; contributing

Community-acquired lower respiratory tract infections. Prevention and cost-control strategies.

The treatment of patients with community-acquired pneumonia can be expensive, particularly if care is hospital-based. Cost control begins with prevention. Current influenza vaccines are about 80 percent protective, but grossly underused. Amantadine and rimantadine are effective chemoprophylactic agents against influenza A, but also underused. Use of pneumococcal vaccine is controversial, but patients who are thought to be at increased risk should be immunized. Management decisions in patients with pneumonia that have major cost implications include the need for hospitalization and choice of di
