[CT-findings in penetrating captive bolt injuries to the head and brain: analysis of the trauma-related CT-findings and review of the literature].

Penetrating gunshot injuries to the head and brain are rare in Germany and the rest of Western Europe. Due to the small number of cases over here no consistent diagnostic and therapeutic standards exist in this respect. Thus these kinds of injuries present a great challenge to the attending physicians. Most of these violations are a result of a suicidal attempt or an accident. Beside violations by firearms also penetrating injuries to the head and brain due to captive bolt devices, as used in slaughtery business for the "humane" killing of animals, occur from time to time. The impact on the he

[Contextual study of Airsoft gun related ocular injuries].

INTRODUCTION: Airsoft guns have become more common in France since the Decree of March 24, 1999, allowing people over 18 years of age to use weapons with an energy level below 2J. The present study aimed to assess the relationship between the context of Airsoft gun pellet related ocular injuries and their clinical consequences, in order to determine an effective prevention strategy. METHODS: A retrospective analysis of ocular injuries due to Airsoft gun pellets was conducted in two hospitals in northern France between September 2009 and August 2010. RESULTS: Six patients with a mean age of 1

Hearing loss in the trenches - a hidden morbidity of World War I.

BACKGROUND: One hundred years ago, millions of British and Allied troops were fighting in the trenches of the Great War. With a tenth of soldiers losing their lives, hearing loss seemed a low priority; however, vast numbers of troops sustained significant hearing loss. METHOD: A review was conducted of literature published between 1914 and 1925. RESULTS: Soldiers were exposed to up to 185 dB of sustained noise from new, high-energy weapons, which caused 'labyrinthine concussion'. Traumatic injuries, non-organic hearing loss and malingering were also common. One source estimated that 2.4 per

The syndrome of excited delirium.

The excited delirium syndrome (EDS) is a life-threatening condition caused by a variety of factors including drug intoxication and psychiatric illness. Fatal instances of excited delirium frequently come to the attention of the medical examiner/coroner due to the circumstances and potential causes. Excited delirium may include paranoid, aggressive, and incoherent behavior which may lead to an encounter with law enforcement. In some instances, the person may die while in the presence of law enforcement. This circumstance further broadens the potential causes of death particularly as EDS has no

Removal of an Embedded Taser Probe in the Glans Penis in the Operating Room.

Using conducted electrical weapons (CEW) to incapacitate aggressive, hostile adults as a less-than-lethal approach has become a common law enforcement practice. The most widely used CEW in law enforcement worldwide is the Taser. The target is subdued when an electric circuit is established between two probes, delivering enough energy to cause pain and muscle incapacitation. Injuries and hospitalizations are rare and mainly occur from dart penetration. Removing the dart is usually straightforward and performed by either the target or law enforcement. No previous cases of glans penis injuries fr

Exposures to conducted electrical weapons (including TASER® devices): how many and for how long are acceptable?

TASER(®) conducted electrical weapons (CEWs) are an important law-enforcement tool. The purposes of this study are a) to review recent literature regarding potential pathophysiological responses to applications of CEWs, and other related issues and b) to evaluate whether enough data exist to determine the acceptability of longer-duration (or repeated) exposures. This is a narrative review, using a multidisciplinary approach of analyzing reports from physiological, legal-medical, and police-strategy literature sources. In general, short-duration exposures to CEWs result in limited effects. Long

Does tympanic membrane perforation have a protective effect on the inner ear in blast-injured patients?

BACKGROUND: Blast-induced hearing loss is an acoustic trauma commonly caused by high-energy explosions of improvised explosive devices, and the auditory system may be affected by blast damage. This study aims to evaluate the protective effect of tympanic membrane perforation (TMP) on the inner ear against blast injury. METHODS: In this study, 43 adult patients who had suffered blast injury were divided into three subgroups: intact tympanic membranes in both ears, unilateral TMP, and bilateral TMP. Each patient underwent a comprehensive audiogram, including bone conduction, in the audiology de

Pathophysiologic changes due to TASER® devices versus excited delirium: potential relevance to deaths-in-custody?

The syndrome of excited delirium has been implicated in some deaths-in-custody which also involved the use of electronic control devices (ECDs) (including those manufactured by TASER International) on subjects. This review is an update on recent studies of pathophysiologic changes related to these two separate but parallel topics: a) first, the use of ECDs during law-enforcement activities; and b) second, the occurrence of excited delirium during such activities. This is a narrative review of elements that may be of use in generating hypotheses relating to potential similarities or differences

Survival of anesthetized Sus scrofa after cycling (7-second on/3-second off) exposures to an electronic control device for 3 minutes.

Electronic control devices (ECDs) may eventually be deployed by the military in a manner resulting in longer exposures than those encountered during law-enforcement operations. In a previous study, 18 repeated cycling (5-second on/5-second off) exposures (within a 3-minute period) of anesthetized swine to an ECD (TASER International's Advanced TASER X26 device) resulted in leg muscle contraction, acidosis, and increases in blood electrolytes. In the current study, experiments were performed to examine effects of exposures to a different cycling rate (7-second on/3-second off), from a modified

Impact of Low-Level Blast Exposure on Brain Function after a One-Day Tactile Training and the Ameliorating Effect of a Jugular Vein Compression Neck Collar Device.

Special Weapons and Tactics (SWAT) personnel who conduct breacher exercises are at risk for blast-related head trauma. We aimed to investigate the potential impact of low-level blast exposure during breacher training on the neural functioning of working memory and auditory network connectivity. We also aimed to evaluate the effects of a jugular vein compression collar, designed to internally mitigate slosh energy absorption, preserving neural functioning and connectivity, following blast exposure. A total of 23 SWAT personnel were recruited and randomly assigned to a non-collar (n = 11) and co

What Factors Are Associated With Acute Compartment Syndrome After Gunshot Injuries to the Upper Extremity?

BACKGROUND: Firearm-related injuries are a growing cause of morbidity and mortality in the United States, with varying odds of acute compartment syndrome (ACS) and other complications reported in the research. ACS is associated with higher complications after injury, including infection, need for repeat surgery, and functional loss, with the forearm being one of the most common sites for ACS. Although ACS in gunshot-related injuries has been studied, existing research documents wildly variable odds of developing ACS, with much of these data derived from military sources with high-energy weapon

Primary Care Considerations for Elderly U.S. Veterans of World War II and the Korean War: A Narrative Review.

Many of the United States' more than 18 million veterans obtain healthcare through the Department of Veterans Affairs' (VA) Veterans Health Administration system; however, recent legislative changes have expanded veterans' access to non-VA care in their communities, particularly for those who do not live near VA medical centers. Veterans are seen by physicians in outpatient practice across the United States and are admitted to non-VA hospitals; this is particularly salient for older veterans, who may require a more frequent and high level of care. We present a review of characteristics of U.S.

Sarcopenia Diagnosis and Management in Hematological Malignancies and Differences with Cachexia and Frailty.

Sarcopenia is a geriatric syndrome characterized by a progressive loss of systemic muscle mass and decreased muscle strength or physical function. Several conditions have a role in its pathogenesis, significantly impacting adverse outcomes such as falls, functional decline, frailty, disability, multiple hospitalizations, and mortality. In the oncological setting, sarcopenia is associated with an increased risk of treatment toxicity, postoperative complications, and a higher mortality rate related to other causes (e.g., pneumonia). In the hematological field, even more so, sarcopenia predicts t

Mechanisms of chemotherapy-induced behavioral toxicities.

While chemotherapeutic agents have yielded relative success in the treatment of cancer, patients are often plagued with unwanted and even debilitating side-effects from the treatment which can lead to dose reduction or even cessation of treatment. Common side effects (symptoms) of chemotherapy include (i) cognitive deficiencies such as problems with attention, memory and executive functioning; (ii) fatigue and motivational deficit; and (iii) neuropathy. These symptoms often develop during treatment but can remain even after cessation of chemotherapy, severely impacting long-term quality of lif

Cancer status in the Occupied Palestinian Territories: types; incidence; mortality; sex, age, and geography distribution; and possible causes.

Cancer is a disease in which some cells of the body grow uncontrollably and occasionally spread to other parts of the body. With a group of more than 100 different types, cancer can start almost anywhere in the body. Defective cells may form a mass called a tumor which can be cancerous (malignant), which grows and spreads to other parts of the body, or benign that can grow but not spread throughout the body. In 2021, more than 10 million people died of cancer worldwide (1 out of 6 deaths). This paper has thoroughly investigated the cancer status in the Occupied Palestinian Territories (OPT), i

Trichloroethylene: An Invisible Cause of Parkinson's Disease?

The etiologies of Parkinson's disease (PD) remain unclear. Some, such as certain genetic mutations and head trauma, are widely known or easily identified. However, these causes or risk factors do not account for the majority of cases. Other, less visible factors must be at play. Among these is a widely used industrial solvent and common environmental contaminant little recognized for its likely role in PD: trichloroethylene (TCE). TCE is a simple, six-atom molecule that can decaffeinate coffee, degrease metal parts, and dry clean clothes. The colorless chemical was first linked to parkinsonism
