Increased Adrenocorticotropic Hormone Levels Predict Recovery of Consciousness in Patients With Disorders of Consciousness.

The potential influence of pituitary-related hormones (including both pituitary gland and target gland hormones) on functional recovery after traumatic brain injury has been observed. However, the relationship between these hormones and the recovery of consciousness in patients with disorders of consciousness (DOC) remains unclear. In this retrospective and observational study, 208 patients with DOC were recruited. According to the Glasgow Outcome Scale (GOS) scores after 6 months, patients with DOC were categorized into two subgroups: a favorable prognosis subgroup (n = 38) comprising those w

White Matter Deficits Underlying the Impaired Consciousness Level in Patients with Disorders of Consciousness.

In this study, we aimed to (1) identify white matter (WM) deficits underlying the consciousness level in patients with disorders of consciousness (DOCs) using diffusion tensor imaging (DTI), and (2) evaluate the relationship between DTI metrics and clinical measures of the consciousness level in DOC patients. With a cohort of 8 comatose, 8 unresponsive wakefulness syndrome/vegetative state, and 14 minimally conscious state patients and 25 patient controls, we performed group comparisons of the DTI metrics in 48 core WM regions of interest (ROIs), and examined the clinical relevance using corre

Differential engagement of thalamic nuclei orchestrates consciousness states across anesthesia, sleep, and disorders of consciousness.

Consciousness alterations occur across various states, encompassing changes in both arousal and awareness. The contributions of thalamic nuclei of consciousness remain incompletely understood. We analyzed fMRI data across general anesthesia (propofol N = 12, sevoflurane N = 12), disorders of consciousness (DoC) (minimally conscious state N = 9, unresponsive wakefulness syndrome N = 9), and sleep stages (wakefulness N = 31, NREM1 N = 24, NREM2 N = 19), investigating thalamocortical connectivity, local fluctuation (complexity/variability), and their coupling. In our results, propofol affected pu

Coherence and Consciousness: Study of Fronto-Parietal Gamma Synchrony in Patients with Disorders of Consciousness.

Evaluation of consciousness needs to be supported by the evidence of brain activation during external stimulation in patients with unresponsive wakefulness syndrome (UWS). Assessment of patients should include techniques that do not depend on overt motor responses and allow an objective investigation of the spontaneous patterns of brain activity. In particular, electroencephalography (EEG) coherence allows to easily measure functional relationships between pairs of neocortical regions and seems to be closely correlated with cognitive or behavioral measures. Here, we show the contribution of hi

Spinal cord stimulation improves brain connectivity and consciousness level in patients with disorders of consciousness.

OBJECTIVE: Spinal cord stimulation (SCS) is an advanced neuromodulation technology in disorders of consciousness (DOC) field. However, research on the modulation effects and mechanisms of SCS is limited. METHOD: We proposed a study design (SCS and sham) to study the short-term effects of 20 minutes' SCS, in which resting state EEG and Coma Recovery Scale-Revised (CRS-R) were used to measure the changes in neural and behavioral activity caused by SCS. We used the Genuine Permutation Cross Mutual Information(G_PCMI) to analyze EEG data and study changes in cortical connectivity during SCS. Fina

Computational modelling in disorders of consciousness: Closing the gap towards personalised models for restoring consciousness.

Disorders of consciousness are complex conditions characterised by persistent loss of responsiveness due to brain injury. They present diagnostic challenges and limited options for treatment, and highlight the urgent need for a more thorough understanding of how human consciousness arises from coordinated neural activity. The increasing availability of multimodal neuroimaging data has given rise to a wide range of clinically- and scientifically-motivated modelling efforts, seeking to improve data-driven stratification of patients, to identify causal mechanisms for patient pathophysiology and l

Relationship between consciousness level and perfusion computed tomography in patients with prolonged disorders of consciousness.

PURPOSE: We assessed the relationship between consciousness level and values of cerebral blood flow (CBF), cerebral blood volume (CBV), mean transit time (MTT), and time to peak (TTP) obtained by whole-brain perfusion computed tomography (pCT) in patients with prolonged disorders of consciousness (pDOC). METHODS: This study included 29 patients in vegetative state (VS), 34 with minimally consciousness state minus (MCS-), and 13 with minimally consciousness state plus (MCS+). All patients were evaluated using the Coma Recovery Scale-Revised (CRS-R), the Glasgow Coma Scale (GCS), and the Full O

Intrathecal baclofen: effects on spasticity, pain, and consciousness in disorders of consciousness and locked-in syndrome.

Disorders of consciousness (DOCs) include coma, vegetative state (VS), and minimally conscious state (MCS). Coma is characterized by impaired wakefulness and consciousness, while VS and MCS are defined by lacking or discontinuous consciousness despite recovered wakefulness. Conversely, locked-in syndrome (LIS) is characterized by quadriplegia and lower cranial nerve paralysis with preserved consciousness. Intrathecal baclofen (ITB) is a useful treatment to improve spasticity both in patients with DOCs and LIS. Moreover, it supports the recovery of consciousness in some patients with VS or MCS.

Spasticity Management in Persons with Disorders of Consciousness.

INTRODUCTION: Spasticity is one of the most frequent neurological impairments affecting persons with disorders of consciousness (DoC). If left untreated, it can mask signs of consciousness by inhibiting one's ability to interact with the environment. The lack of information about spasticity specific to patients with DoC may result in insufficient or even inappropriate treatment. OBJECTIVE: To report spasticity characteristics and management in a large dedicated DoC rehabilitation program. DESIGN: Retrospective chart review. SETTING: An inpatient rehabilitation hospital. PARTICIPANTS: Patie

Electroencephalographic profiles for differentiation of disorders of consciousness.

BACKGROUND: Electroencephalography (EEG) is best suited for long-term monitoring of brain functions in patients with disorders of consciousness (DOC). Mathematical tools are needed to facilitate efficient interpretation of long-duration sleep-wake EEG recordings. METHODS: Starting with matching pursuit (MP) decomposition, we automatically detect and parametrize sleep spindles, slow wave activity, K-complexes and alpha, beta and theta waves present in EEG recordings, and automatically construct profiles of their time evolution, relevant to the assessment of residual brain function in patients

Pain assessment and management in disorders of consciousness.

PURPOSE OF REVIEW: Pain and suffering controversies in persons with disorders of consciousness continue to be debated by the scientific, legal and medical ethics communities. This review examines the current knowledge base for guiding decisions regarding assessment and management of pain in persons with disorders of consciousness. RECENT FINDINGS: Studies have shown that brain processing linked to pain in persons in a vegetative state is incomplete and is processed only at a primary and not higher secondary level. Therefore, such painful stimuli would not reach the threshold for conscious exp

The Dilemma of Hydrocephalus in Prolonged Disorders of Consciousness.

Prolonged disorders of consciousness (DOC) are considered to be among the most severe outcomes after acquired brain injury. Medical care for these patients is mainly focused on minimizing complications, given that treatment options for patients with unresponsive wakefulness or minimal consciousness remain scarce. The complication rate in patients with DOC is high, both in the acute hospital setting, as in the rehabilitation or long-term care phase. Hydrocephalus is one of these well-known complications and usually develops quickly after acute changes in cerebrospinal fluid (CSF) circulation af

Advances in the neurorehabilitation of severe disorder of consciousness.

INTRODUCTION: The paper describes the evolution of knowledge concerning severe brain injury which determines the Vegetative State/Unresponsive Wakefulness Syndrome. BACKGROUND: The term Vegetative State was proposed by Jennet and Plum in 1972. Later on, the Intensive Care Units progresses increased the survival of these patients and, contemporary, decreased their characteristic conditions of cachexia and severe dystonia. In 1994, the disease was conceived as a disconnection syndrome of the hemispheres from the brainstem, mainly due to a temporary or permanent deficit of the functions of the w

Medical and surgical treatments in disorders of consciousness.

Ever since a 2012 landmark study showed positive effects of amantadine in people with disorders of consciousness (DOC), there has been a shift in research efforts from merely improving diagnostics and prognostication of DOC to also include therapeutic trials, in the quest to improve consciousness recovery after brain injury. Stimulation of residual consciousness in the intensive care unit is critical because failure to do so may lead to unwarranted pessimistic prognosis and premature withdrawal of life-sustaining therapies. Similarly, it is crucial to harvest the potential of chronic DOC patie

A historical review of consciousness and its disorders.

Concepts of consciousness and its disorders begin with the realization that both reside in the brain. Then came the realization that consciousness had various components, with two principal aspects, wakefulness and awareness. Awareness has multiple interconnected components, ranging from perception to abstract thought. These require selection of certain stimuli and processing as well as judgment and motivation, colored by emotion, before a consciously directed action is produced. The brain processes information and can influence behavior at levels below conscious awareness. Deeper insights int

Kernohan's notch and misdiagnosis of disorders of consciousness.

A 69-year-old man presented with a sudden headache followed by unconsciousness. There was no head injury. The Glasgow Coma Scale (GCS) score was 3/15 and there was a left mydriasis, unreactive to light. The CT-scan showed a left acute subdural haematoma causing a remarkable mass effect. A supratentorial hemispheric craniotomy was performed. Nevertheless, after several weeks at the intensive care unit (ICU), the patient was still unresponsive to external stimuli and did not show any motor activity. A comfort care attitude was decided on with the family and the patient was extubated. However, a
