The physiologic effects of extended wear soft contact lenses.

The major physiologic effect to the eyes of soft contact lenses in general and continuously worn hydrogel lenses in particular is a decrease in the amount of oxygen normally available to the cornea. This reduction in oxygen supply produces changes as minimal as clinically undetectable epithelial edema and as severe as the anoxic overwear syndrome that produces pain and decreased vision. Induced corneal edema can produce refractive error changes and endothelial cell changes. On a long-term basis the lenses produce physical trauma to the anterior surface of the cornea as well as certain metaboli

Extended-wear contact lenses for the treatment of pediatric aphakia.

The practicality of extended-wear contact lenses in the refractive correction of pediatric aphakia was assessed with 240 eyes in 184 patients. Patient ages at the time of contact lens fitting ranged from 18 days to 9.8 years. One hundred forty-one eyes were fit from 1 day to 55 months postoperatively and then followed for 6 months to 5.7 years (average, 29 months). Only five patients lost more than five lenses. The overall loss rate was less than one lens per year of follow-up. No patient had contact lens-related complications with permanent visual sequelae. Only 14% of patients had contact le

Prospective trial of daily and extended wear disposable contact lenses.

PURPOSE: The current study prospectively evaluates the response to daily and extended wear of disposable lenses with each patient acting as his/her own control. This paradigm allowed for elimination of individual variation as a confounding factor, as well as determining whether an intrinsic factor, other than lens wear, predisposed patients to complications. METHODS: One hundred thirty-four patients were randomly assigned to contralateral daily wear (DW) or extended wear (EW) with Acuvue lenses for 1 year. All lenses were disposed of on a weekly basis. Bilateral bacteriology of lids, conjunct

Nocardia asteroides keratitis associated with extended-wear soft contact lenses.

Chronic unilateral keratitis developed in a 34-year-old woman who wore extended-wear contact lenses. A clinical response was not obtained until Nocardia asteroides was correctly identified as the causal agent. The response to 30% sulfacetamide was dramatic. We review the clinical presentation of Nocardia keratitis and recommendations for management. Use of corticosteroids should be avoided in Nocardia keratitis. This opportunistic organism should be considered in patients who wear contact lenses in whom infectious keratitis develops.

Extended-wear soft contact lenses for vision studies in monkeys.

The authors have designed and produced extended-wear contact lenses for rhesus monkeys. High-plus lenses to correct neonatal aphakia as well as lenses dyed black for use as occluders to treat amblyopia have been evaluated. Four infant monkeys fitted with soft lenses have successfully worn these extended-wear lenses almost continuously throughout their first year of life. These experiments demonstrate that rhesus monkeys tolerate extended-wear contact lenses well when these lenses are correctly designed, comfortably fitted, and their wear carefully monitored. Furthermore, the results demonstrat

Corneal ulcers in patients with cosmetic extended-wear contact lenses.

Although cosmetic extended-wear contact lenses are generally safe, they can be associated with a number of complications. A review of 124 cases of corneal ulcers treated between Jan. 1, 1982, and June 1, 1983, disclosed six cases in otherwise healthy patients (four women and two men, ranging in age from 15 to 41 years) who wore extended-wear contact lenses for correction of myopia. In five of the six, the contact lenses had undergone recent manipulation. Two patients had changed their contact lenses without proper disinfection procedures. Five of the six had been treated with antibiotics befor

Extended wear soft contact lenses in the correction of aphakia.

This paper reports the study over a 2-year period of forty patients with uniocular aphakia fitted with spherical extended wear hydrophilic lenses: Sauflon 85 per cent and Duragel 72 per cent. The patients' ages ranged from 17 to 87 years (mean 52). They were unable to handle a daily wear soft lens or tolerate a hard lens. The success rate was 85 per cent.

Corneal ulcers associated with cosmetic extended wear soft contact lenses.

In 52 patients with a history of using cosmetic extended wear soft contact lenses, corneal ulcers developed requiring admission to Wills Eye Hospital between January 1, 1982, and June 30, 1986. A retrospective study was conducted to determine the contact lens history, clinical presentation, culture and sensitivity results, and final visual acuity of these patients. Pseudomonas organisms were isolated in 75% of culture-positive cases. The final visual acuity was 6/60 or less in 12% of all cases and 25% of the Pseudomonas ulcers. Patients and practitioners must be aware of this vision-threatenin

Bilateral simultaneous Pseudomonas keratitis with myopic extended-wear contact lenses.

Bacterial keratitis is reported more frequently in the literature with the use of extended-wear contact lenses. This report describes a case of bilateral simultaneous pseudomonas keratitis in an elderly patient wearing extended-wear contact lenses for correction of myopia. The lenses were not properly disinfected prior to the development of the corneal ulcers. When bacterial keratitis is suspected with the use of extended-wear contact lenses, prompt diagnosis and treatment are necessary. Appropriate microbiologic testing of the contact lenses, lens cases, and lens solutions should also be unde

Persistent corneal edema in aphakic eyes from daily-wear and extended-wear contact lenses.

Corneal edema developed in seven aphakic eyes of six patients (three men and three women ranging in age from 60 to 88 years) who used daily- or extended-wear hydrophilic and silicone contact lenses. This edema, which developed suddenly, persisted despite discontinuation of contact lens wear and treatment with topical corticosteroids. The onset of the edema occurred in two patterns. In three eyes, edema occurred soon (30 minutes to 30 days) after the contact lens was first worn and persisted during follow-up periods averaging 15 months. In the other four eyes, the edema developed late, after 20

Intraocular lenses versus extended-wear contact lenses in aphakic rehabilitation. A controlled clinical study.

A controlled clinical study with random allocation of method of rehabilitation (lens implant or contact lenses) was designed for comparative outcome assessments. Sixty-one patients were included in the study: 31 patients randomized to lens implantation and 30 patients to extended-wear contact lens rehabilitation with early post-operative fitting. Complication rates were low in both groups with slight accumulation of intraocular complications in the implant group and of external complications in the contact lens group. We found no significant differences between the groups in final visual acuit

[Complications of disposable extended-wear contact lenses. Apropos of 6 cases].

We report six cases of corneal complications observed in disposable extended-wear contact lens users: two corneal ulcers, one superficial infiltrate, two punctate keratitis with subepithelial infiltrates and one superficial punctate keratitis. In four cases a bacterial etiology was confirmed. Disposable contact lenses are not complication-free. In our series, complications were favoured or provoked by incorrect use in most cases. Other possible risk factors include poor hygiene during lens manipulation, damage to the corneal epithelium during lens exchange or lens wear, use of contaminated lub

Extended-wear contact lenses in patients with corneal grafts and aphakia.

Extended-wear contact lenses (EWCL) were fitted successfully on 61 aphakic eyes with penetrating corneal grafts. In two- to 32-months follow-up there were 26 eyes that had varying amounts of superficial neovascularization of the grafts and one Staphylococcus epidermidis corneal graft ulcer. There were five homograft rejections, all occurring in severely diseased eyes. Variable vision and visual acuity less than that with hard contact lenses or aphakic spectacles were the most frequent problems. Both were due to the high corneal astigmatism present in many patients. The results indicate that EW

Clinical evaluation of extended-wear lidofilcon B contact lenses in aphakia.

A total of 138 aphakic eyes in 109 patients were fitted with lidofilcon B contact lenses for extended wear. These patients were observed for six months to more than three years. Their problems with fit, comfort, infection, lens loss, and visual acuity were reviewed. A comparison between patients in whom the lenses were used successfully and patients in whom the lenses were used unsuccessfully was made, and the causes of failure discussed.

Pseudomonas corneal ulcer with extended-wear soft contact lenses for myopia.

Extended-wear soft contact lenses for myopic correction are becoming a more frequent alternative to spectacles. The risk of corneal ulceration with their use exists. In our patient, a 29-year-old woman, a severe Pseudomonas aeruginosa corneal ulcer developed after the use of extended-wear contact lenses for myopia.

Which lens: contact lenses currently available for extended wear in aphakia.

To date, a number of specific materials have been used for extended wear lenses in aphakia. While none of the materials are ideal, reasonable results have been achieved with three types of materials: thin hydrophilics, silicone, and high water content hydrophilics. The various available lenses are described, and their physical properties are related to the process of extended wear. The authors' techniques for fitting, follow-up, evaluation, and prevention of complications are presented.

Corneal response to extended wear of CAB contact lenses in aphakia.

We studied 41 aphakic eyes in 30 patients who had been wearing CAB (cellulose acetate butyrate) contact lenses on an extended basis. Patients were 51 to 81 years old and averaged 14.3 months of wearing time. Visual acuity was very good. Pachometry measurements ranged from 0.42 mm to 0.66 mm and averaged 0.550 mm. This compares favorably with data in the literature on corneal thickness seen with routine use of PMMA or hydrogel lenses or with extended wear of hydrogel lenses. Corneal thickness was also compared with unoperated fellow eyes and eyes using CAB lenses on a daily basis only.

Serious corneal complications associated with extended-wear soft contact lenses for myopia.

Three patients wearing extended wear contact lenses for correction of myopia presented with severe corneal complications including corneal ulcers and corneal infiltrates. All cases were assembled over a four-month period, reflecting the alarming rise in severe corneal complications related to increased usage of these lenses. We feel indiscriminate dispensing and lack of proper follow-up care are primary contributing factors in the near epidemic rise in severe corneal problems related to this usage. Two of our three patients had significant permanent visual loss following resolution of their co
