Leveraging continuous glucose monitoring for personalized modeling of insulin-regulated glucose metabolism.

Continuous glucose monitoring (CGM) is a promising, minimally invasive alternative to plasma glucose measurements for calibrating physiology-based mathematical models of insulin-regulated glucose metabolism, reducing the reliance on in-clinic measurements. However, the use of CGM glucose, particularly in combination with insulin measurements, to develop personalized models of glucose regulation remains unexplored. Here, we simultaneously measured interstitial glucose concentrations using CGM as well as plasma glucose and insulin concentrations during an oral glucose tolerance test (OGTT) in in

Nocturnal Hypoglycemia in the Era of Continuous Glucose Monitoring.

Nocturnal hypoglycemia is a common acute complication of people with diabetes on insulin therapy. In particular, the inability to control glucose levels during sleep, the impact of external factors such as exercise, or alcohol and the influence of hormones are the main causes. Nocturnal hypoglycemia has several negative somatic, psychological, and social effects for people with diabetes, which are summarized in this article. With the advent of continuous glucose monitoring (CGM), it has been shown that the number of nocturnal hypoglycemic events was significantly underestimated when traditiona

[Continuous Glucose Monitoring. Which is the evidence in Children?].

Diabetes Technology refers to the software or hardware that is designed to facilitate and improve the quality of life of the patient with diabetes Mellitus. A non-systematic literature search was carried out which included articles in English and Spanish about the use of continuous glucose monitoring (CGM) in pediatric patients with Type 1 diabetes Mellitus. This review summarizes the performance of the CGM, its accuracy, and classification. A chronological synthesis of the general evidence up to June 2020 was done including both adult and pediatric studies. Aspects of metabolic control were s

Continuous Glucose Monitoring in the Management of Neonatal Hypoglycaemia.

A quality improvement project was carried out in a Level 3 Neonatal Intensive Care Unit (NICU) which aimed to successfully implement the use of continuous glucose monitoring systems (CGMS) in hypoglycaemic infants. Piloting of the device revealed several potential practical barriers to its reliably successful implementation. Five Plan-Do-Study-Act (PDSA) cycles followed, tackling these problems and other issues inductively identified throughout the project. Parents and multi-professional stakeholders were involved and consulted throughout. Change was measured on a runchart using qualitative an

Modelling intestinal glucose absorption in premature infants using continuous glucose monitoring data.

BACKGROUND: Model-based glycaemic control protocols have shown promise in neonatal intensive care units (NICUs) for reducing both hyperglycaemia and insulin-therapy driven hypoglycaemia. However, current models for the appearance of glucose from enteral feeding are based on values from adult intensive care cohorts. This study aims to determine enteral glucose appearance model parameters more reflective of premature infant physiology. METHODS: Peaks in CGM data associated with enteral milk feeds in preterm and term infants are used to fit a two compartment gut model. The first compartment desc

Empirical transition probability modeling of continuous glucose monitoring data.

INTRODUCTION: Continuous glucose monitoring (CGM) produces rich time series data. Standard metrics such as time-in-range summarize glycemia but miss the sequence of changes. We aimed to characterize glucose dynamics using empirical transition probability techniques. MATERIALS AND METHODS: Using publicly available data for 244 participants who enrolled in the Insulin-Only Bionic Pancreas (IOBP2) randomized clinical trial, we mapped each CGM reading to one of five states: < 54, 54-69, 70-180, 181-250, or >250 mg/dL. We counted state changes between consecutive readings collected every 5 minutes

Evaluation of continuous interstitial glucose monitoring in healthy calves.

In newborn calves, glucose is the primary energy source, gradually replaced by volatile fatty acids produced by the developing rumen microbiota. Although blood glucose monitoring is essential, there is currently no consensus on optimal measurement methods in calves. Current techniques rely on frequent blood sampling, which is invasive, stressful, and could itself influence glucose levels. Continuous monitoring of interstitial glucose using sensors offers a non-invasive alternative that enables tracking of glycemic fluctuations over several days. This study evaluated the feasibility of using su

Outcomes of Diabetes Management with Continuous Glucose Monitoring Technology.

Diabetes mellitus is a chronic metabolic disorder that imposes a global economic burden. Type 1 diabetes mellitus (T1DM) results from autoimmune destruction of pancreatic β-cells, while type 2 diabetes mellitus (T2DM) is characterized primarily by peripheral insulin resistance with progressive β-cell dysfunction. Despite advances in pharmacologic and self-monitoring therapies, many patients fail to achieve recommended glycemic targets. Continuous glucose monitoring (CGM) has emerged as a transformative technology with the potential to address persistent gaps in diabetes management. This review

Accuracy of continuous glucose monitoring in critically ill patients.

OBJECTIVE: Hyperglycaemia requiring insulin infusions is common among critically ill patients. Attempts to tightly control glucose levels in the intensive care unit (ICU) have had mixed results partly due to hypoglycaemia. Continuous glucose monitoring (CGM) has been widely adopted among ambulatory persons with diabetes but tested only on small numbers of patients in the ICU. This study was undertaken to address the accuracy of CGM in a group of critically ill patients. SETTING AND OUTCOMES: This observational study included critically ill ICU patients admitted between 2019 and 2023 to a tert

Integrating Continuous Dual Glucose-Ketone Monitoring into Clinical Practice.

Diabetic ketoacidosis (DKA) is a life-threatening diabetes emergency associated with mortality and severe morbidities in individuals with diabetes. DKA mainly presents with significant hyperglycemia (>250 mg/dL); however, with the increasing use of sodium-glucose transporter 2 inhibitors, we are seeing an increase in euglycemic DKA (euDKA) in both type 1 diabetes and type 2 diabetes. Although severe DKA generally requires emergency department services or inpatient hospitalization, clinicians can play an important role in helping patients prevent DKA by providing comprehensive education to thei

Nationally Subsidized Continuous Glucose Monitoring: A Cost-effectiveness Analysis.

OBJECTIVE: The Continuous Glucose Monitoring (CGM) Initiative recently introduced universal subsidized CGM funding for people with type 1 diabetes under 21 years of age in Australia. We thus aimed to evaluate the cost-effectiveness of this CGM Initiative based on national implementation data and project the economic impact of extending the subsidy to all age-groups. RESEARCH DESIGN AND METHODS: We used a patient-level Markov model to simulate disease progression for young people with type 1 diabetes and compared government-subsidized access to CGM with the previous user-funded system. Three y

Continuous glucose monitoring in cystic fibrosis - A practical guide.

Our ability to monitor blood glucose levels has become increasingly accurate over the last few decades. Continuous glucose monitoring (CGM) technology now allows providers and patients the ability to monitor glucose levels retrospectively as well as in real-time for diabetes management. CGM also provides the ability to study glucose patterns and trends for insight into the pathophysiology and natural history of disease. CGM captures a more complete picture of glucose profiles than traditional measures of glycemia such as the hemoglobin A1c or self-monitoring of blood glucose levels. This artic
