Central sleep apnea reversed by continuous positive airway pressure.

Continuous positive airway pressure (CPAP) is an accepted and highly effective method used in the treatment of obstructive sleep apnea (OSA). We have explored the potential utility of CPAP in a patient with sleep apnea without a major contribution of airway obstruction as measured by the usual criteria. The patient showed dramatic clinical and laboratory improvement in central apnea when treated with CPAP. A low level of CPAP (7.5 cm H2O) resulted in the reduction of central apneas from 281 to 32 per night with no significant change in obstructive apneas, which increased from 43 to 59 per nigh

Decreasing Continuous Positive Airway Pressure Failure in Preterm Infants.

BACKGROUND AND OBJECTIVES: Many preterm infants stabilized on continuous positive airway pressure (CPAP) at birth require mechanical ventilation (MV) during the first 72 hours of life, which is defined as CPAP failure. Our objective was to decrease CPAP failure in infants ≤29 weeks' gestational age (GA). METHODS: A quality improvement bundle named OPTISURF was implemented for infants ≤29 weeks' GA admitted on CPAP, consisting of stepwise escalation of CPAP and less invasive surfactant administration guided by fractional inspired oxygen concentration ≥0.3. The CPAP failure rate was tracked by

Facial deformations during nasal continuous positive airway pressure therapy.

Obstructive Sleep Apnoea (OSA), characterised by repeated collapse of the pharyngeal airway during sleep, causes cessation of breathing followed by arousal, restoring normality. Continuous Positive Airway Pressure (CPAP) is a non-invasive, effective treatment for OSA where positive pressure is applied to the airway through a mask, maintaining patency. Nasal masks are commonly used, contacting the face across the upper lip, sides of the nose and the nasal bridge. Despite health benefits, therapy compliance is sub-optimal, often due to poor mask fit and discomfort. Masks have been designed to co

Continuous Positive Airway Pressure: To Bubble or Not to Bubble?

Continuous positive airway pressure (CPAP) systems can be broadly grouped into continuous flow or variable flow devices. Bubble CPAP (bCPAP) is a continuous flow device and has physiologic properties that could facilitate gas exchange. Its efficacy has been reported to be similar to variable flow CPAP systems when used as a primary mode of respiratory support. Post-extubation bCPAP is reported to significantly reduce extubation failure rates among preterm infants ventilated for less than 2 week when compared to Infant flow driver CPAP (variable flow). bCPAP has been successfully used in resour

A randomized cross-over study of auto-continuous positive airway pressure versus fixed-continuous positive airway pressure in patients with obstructive sleep apnoea.

BACKGROUND AND OBJECTIVES: To compare the efficacy of auto-CPAP (AutoSet Spirit, ResMed) versus fixed-CPAP (S6 Elite, ResMed) in improving daytime sleepiness, health status, objective compliance and the ultimate treatment preference in patients with severe OSA. METHODS: The study recruited 43 subjects aged 18-65 years with newly diagnosed severe OSA (AHI >30/h). Patients were initially treated with either auto-CPAP or fixed-CPAP for 2 months and then crossed over after a washout period of 1 week for another 2 months. RESULTS: The study was completed by 41 patients. Results are presented as m

Prediction of upper airway dryness and optimal continuous positive airway pressure conditions.

Continuous positive airway pressure is the most effective long-term treatment for obstructive sleep apnoea, which is a sleeping disorder characterized by pauses in breathing during sleep. It introduces pressurized atmospheric air into the respiratory system in order to maintain open airways without blockage. Some continuous positive airway pressure devices incorporate a convective heat transfer humidifier to overcome dryness. However, many side effects, including the unacceptable excess of water droplets in the air supply line, have been reported and improvements are essential for better patie

The Pressure Is On!: Neonatal Skin and Nasal Continuous Positive Airway Pressure.

BACKGROUND: Premature infants have an increased risk for developing skin breakdown. Perceivably noninvasive interventions may have detrimental effects on the infant's skin. PURPOSE: This case presentation describes an extreme case of nasal breakdown associated with nasal continuous positive airway pressure (nCPAP). METHODS: Highlighted is the vital importance of proper placement, along with appropriate apparatus size, in the efforts to prevent skin breakdown. Ensuring that pressure points are avoided is imperative to prevent nCPAP-related injuries, along with treating the area in the event o

Neural drive during continuous positive airway pressure (CPAP) and pressure relief CPAP.

BACKGROUND: Pressure release continuous positive airway pressure (CPAP) is an evolution of CPAP that has been reported to improve patient comfort. We hypothesised the pressure release would lead to unloading of the inspiratory muscles and therefore conducted a prospective double-blind cross-over physiological study of autotitrating CPAP (APAP) against autotitrating pressure relief CPAP (PR-APAP). METHODS: Eleven patients with severe obstructive sleep apnoea (OSA; mean AHI 74.5+/-14.4/h) were studied. We assessed neural drive by recording the oesophageal pressure, gastric pressure, transdiaphr

Early nasal intermittent positive pressure ventilation versus continuous positive airway pressure for respiratory distress syndrome.

AIM: To determine whether early nasal intermittent positive pressure ventilation (NIPPV), in comparison to early continuous positive airway pressure (CPAP), can reduce the need for intubation and mechanical ventilation in preterm neonates with suspected respiratory distress syndrome (RDS). METHODS: In this stratified open-label randomized controlled trial, neonates (28-34 weeks gestation) with respiratory distress within 6 h of birth and Downe's score >or= 4 were eligible. Subjects were randomly allocated to 'early-NIPPV' or 'early-CPAP' after stratifying for gestation (28-30 weeks, 31-34 wee

Altitude-Associated Central Apnea in Continuous Positive Airway Pressure Therapy.

Introduction  At high altitudes, hypobaric hypoxia leads to central apneas in both healthy adults and patients with obstructive sleep apnea. This condition correlates with intermittent desaturation and disruption of sleep. Materials and Methods  The clinical case of a patient evaluated during a sleep medicine outpatient consultation at the Fundación Neumológica Colombiana (Bogotá, Colombia, 2,600 masl) is reported. Case Study  We present a 64-year-old male patient living in a high-altitude city, who was an aircrew member and was on continuous positive airway pressure (CPAP) therapy for severe

Continuous positive airway pressure increases CSF flow and glymphatic transport.

Respiration can positively influence cerebrospinal fluid (CSF) flow in the brain, yet its effects on central nervous system (CNS) fluid homeostasis, including waste clearance function via glymphatic and meningeal lymphatic systems, remain unclear. Here, we investigated the effect of supporting respiratory function via continuous positive airway pressure (CPAP) on glymphatic-lymphatic function in spontaneously breathing anesthetized rodents. To do this, we used a systems approach combining engineering, MRI, computational fluid dynamics analysis, and physiological testing. We first designed a na

Continuous positive airway pressure (CPAP) for acute bronchiolitis in children.

BACKGROUND: Acute bronchiolitis is one of the most frequent causes of emergency department visits and hospitalisation in children up to three years of age. There is no specific treatment for bronchiolitis except for supportive treatment, which includes ensuring adequate hydration and oxygen supplementation. Continuous positive airway pressure (CPAP) aims to widen the lungs' peripheral airways, enabling deflation of overdistended lungs in bronchiolitis. Increased airway pressure also prevents the collapse of poorly supported peripheral small airways during expiration. Observational studies repo

High continuous positive airway pressure in neonates: A physiological study.

OBJECTIVE: We sought to evaluate physiological cardiorespiratory implications of high pressures (>8 cmH2 O) on continuous positive airway pressure (CPAP) in preterm neonates. METHODS: Fifteen preterm neonates at postmenstrual age ≥32 weeks on CPAP 5 cmH2 O were enrolled. Pressures were increased by 2 cmH 2 O increments until 13 cmH 2 O. At each increment, cardiac output, electrical diaphragmatic (Edi) activity, and clinical cardiorespiratory parameters were measured. Predefined cut-off values for changes in cardiorespiratory parameters were used as termination criteria. Data, presented as mea
