Barrier and spermicidal contraceptive methods and risk of invasive cervical cancer.

The effects of barrier and spermicidal methods of contraception on cervical cancer risk were examined by studying 479 cases of histologically confirmed invasive cervical cancer cases and 788 random digit dialing controls. In addition to a detailed history of contraceptive practices, information was available on numerous potential confounders, including demographic characteristics, sexual behavior, reproductive factors, Pap smear screening history, and smoking. After adjustment for relevant confounders, diaphragm and condom use were found not to be significantly associated with risk of cervical

[Possible reduction of cervical cancer by the use of barrier-type contraceptives].

Forty-four cervical cancer inpatients and 44 individually age-matched controls were surveyed. As a result, the number of the cases whose partners used condoms for contraception was significantly lower than that of the controls (P less than 0.01; the odds ratio = 0.2). The same result was obtained after adjusting possible confounding variables such as the number of pregnancies and use of other contraceptive methods. Barrier-type contraceptives like condoms may reduce the incidence of sexually transmitted viral infections, which it has been suggested are associated with the disease.

The relationship of tubal infertility to barrier method and oral contraceptive use.

We studied past contraceptive use in 283 nulliparous infertile women who had a diagnosis of tubal adhesions or occlusion and in 3833 women admitted for delivery at seven collaborating hospitals from 1981 to 1983. The relative risk of tubal infertility associated with barrier contraceptive use or oral contraceptive use was calculated using multivariate logistic regression to control for confounding by region, age, religion, education, smoking, number of sexual partners, time since menarche, and use of other contraceptive methods. Women who had ever used barrier methods of contraception were at

Barriers to salpingectomy for permanent contraception: A qualitative study with obstetricians and gynaecologists.

Consideration of risk-reducing bilateral salpingectomy has been recommended instead of tubal occlusive procedures for female sterilisation due to the role of the Fallopian tubes in the aetiology of serous ovarian malignancies. This study identified barriers to performing salpingectomy for permanent contraception. Twenty-two semi-structured interviews were conducted with Australian gynaecologists, and transcripts analysed qualitatively. Barriers to performing bilateral salpingectomy included: (a) patient factors (younger age and risk of regret); (b) operative complexity and complications (parti

Provider-based barriers to provision of intrauterine contraception in general practice.

OBJECTIVES: Intrauterine contraception (IUC) is highly effective, safe and long-lasting, but is not a popular method of contraception among British women. This study examined barriers to the uptake of IUC in general practice in England. METHOD: A sequential mixed-method approach to explore the views of practitioners regarding the provision of IUC. We e-surveyed 208 practitioners from 69 practices in a region of England and subsequently interviewed 14 practitioners from eight practices. RESULTS: Just under half of general practitioners (GPs) (46.8%; 58/124), and only 8.2% (4/49) of nurses rep

Barrier contraceptive method acceptability and choice are not reliable indicators of use.

BACKGROUND: The need for safe and effective female-controlled methods that protect against sexually transmitted pathogens is widely recognized. Product effectiveness is inextricably bound to use, and, therefore, the needs and preferences of potential consumers must be considered. The degree to which measures of acceptability correlate with actual barrier method use remains unexamined. GOAL: The goal was to evaluate associations between measures of acceptability and use of existing over-the-counter barrier methods. STUDY DESIGN: In the San Francisco Bay Area, 510 females aged 15 to 30 years w

Removing Unnecessary Medical Barriers to Contraception: Celebrating a Decade of the U.S. Medical Eligibility Criteria for Contraceptive Use.

In 2010, the Centers for Disease Control and Prevention (CDC) released the U.S. Medical Eligibility Criteria for Contraceptive Use, providing recommendations for health care providers on safe use of contraception for people with certain characteristics or medical conditions. Adapted from World Health Organization guidance, the goal of the recommendations is to remove unnecessary medical barriers to contraception. Over the past decade, CDC has updated recommendations based on new evidence, collaborated with national partners to disseminate and implement the guidelines, and conducted provider su

Barriers and Facilitators to Expanding User-Administered Injectable Contraceptives in the United States.

OBJECTIVE: To identify facilitators and barriers to prescribing subcutaneous depot medroxyprogesterone acetate (DMPA) for user administration. METHODS: This mixed-methods study comprised surveys (N=422) and interviews (N=34) with family planning experts, defined as health care professionals who see at least three contraceptive visits a month, regarding their awareness, knowledge, and practice of prescribing subcutaneous DMPA for user administration. RESULTS: We found that 74.5% of professionals reported awareness of subcutaneous DMPA for user administration, but only 34.8% actively prescribe

Barriers and facilitators influencing utilization of intrauterine contraceptive device (IUCD) in Zimbabwe.

IUCD (Copper-T) is a safe, effective and long-acting reversible contraceptive and its uptake in Zimbabwe is currently less than 1%. Interventions to improve the uptake of IUCD require evidence on key factors contributing to its low uptake. The study was conducted to identify the gaps and offer opportunities for evidence-based family planning aimed at improving demand for IUCD, supply-side conditions, and influencing attitudes towards IUCD. A descriptive qualitative study design was conducted to explore and understand the perceptions of 169 women, 22 men, 16 community leaders and 20 health care

It isn't all about language: communication barriers for Latinas using contraceptives.

PURPOSE: Little is known about barriers that Latinas in the United States face in preventing unintended pregnancies beyond those of language and cost. This study examined factors inhibiting contraceptive use among 18- to 30-year-old Latinas in the Midwest. METHOD: Individual interviews (N = 31) were conducted in Spanish with Latinas residing across the state. The interview protocol included questions about contraceptives and unintended pregnancies. Interviews were recorded, transcribed, translated, and coded for themes related to barriers. RESULTS: The majority of the barriers were related t

Whose self-determination? Barriers to access to emergency hormonal contraception in Italy.

In Italy, Emergency Hormonal Contraception (EHC) is a prescription drug, available only in pharmacies. Evidence suggests that a number of doctors and pharmacists refuse to provide EHC, on grounds of conscience, although the exact frequency of this phenomenon is unknown. This creates a barrier to access to EHC for women, thus risking undermining their right to reproductive self-determination. In this article, we aim to offer a clearer empirical and theoretical understanding of the situation and to assess the force of doctors' and pharmacists' claims against providing EHC. Unlike standard discus

Barrier versus oral contraceptive use: a study of female college students.

Although they provide birth control and are easier to use, oral contraceptives (OCPs) are not the preferred approach to preventing sexually transmitted disease (STD). Do the knowledge, attitudes, and experiences of oral contraceptive users place them at greater risk for STDs than those who employ barrier methods? This study examined differences between sexually active female college students (ie, those who reported ever having had vaginal intercourse) who used OCPs and those who employed barrier methods of contraception at the time of their most recent intercourse. The authors analyzed HIV- an

The Role of Pediatric Subspecialists in Adolescent Contraceptive Care: Knowledge, Attitudes, and Barriers.

STUDY OBJECTIVE: Adolescents and young adults with chronic medical conditions (CMCs) have similar sexual behaviors to their healthy peers but are less likely to use contraception. Provision of CMC-specific sexual and reproductive health (SRH) care within pediatric subspecialty clinics may improve access for this population, although system- and provider-level barriers exist. The purpose of this cross-sectional study was to assess the attitudes, practices, and knowledge of SRH among providers in a variety of pediatric subspecialities. METHODS: Listservs were used to recruit pediatric subspecia

Barriers to accessing post-pregnancy contraception in Brazil: The impact of COVID-19.

The aim of our article is to discuss barriers associated with post-pregnancy contraception in Brazil during the SARS-CoV-2 (COVID-19) pandemic. Socioeconomic differences in gaining access to long-acting reversible contraceptive (LARC) methods became greater during the COVID-19 pandemic. The inadequate distribution of existing resources and the reduced capacity for elective care meant that healthcare providers in family planning had to be reallocated to respond to COVID-19 emergencies. In Brazil, 74% of the population depends on the national health service (Sistema Unico de Saúde) including for

Overcoming barriers to oral contraception in Italy an expert opinion to empower women.

Oral contraceptives are among the most commonly used contraceptive methods. However, Italian women underused reliable hormonal contraceptives as compared with other European women. There is a need to streamline access to oral contraception, with expected benefits on emotional well-being of women and potential savings for health system in Italy. Indeed, women can only access to oral contraceptives through mandatory medical prescription. This is one of the most important barrier experienced by women. We hereby provide an expert opinion on the potential reclassification, extending pharmacy servic

Domestic violence: a hidden barrier to contraceptive use among women in Nigeria.

BACKGROUND: The nonuse of family planning methods remains a major public health concern in the low-and-middle-income countries, especially due to its impact on unwanted pregnancy, high rate of abortion, and transmission of sexually transmitted diseases. Various demographic and socioeconomic factors have been reported to be associated with the nonuse of family planning methods. In the present study, we aimed at assessing the influence of domestic violence (DV) on contraceptive use among ever married women in Nigeria. METHODS: Data on 22,275 women aged between 15 and 49 years were collected fro

Barriers to contraceptive careseeking: the experience of Eritrean asylum-seeking women in Israel.

Objectives: In recent years, there has been a mass migration of Eritreans (many seeking political asylum) into Israel after precarious irregular movement across international borders. This study qualitatively explores the structural barriers to family planning (i.e. contraceptive services) for Eritrean women in Israel that are rooted in their temporary legal status and the patchwork of family planning services.Design/Methods: From December 2012 to September 2013, we interviewed 25 key informants (NGO workers, researchers, Eritrean community activists, International NGO representatives and Mini

Contraception for adolescents in low and middle income countries: needs, barriers, and access.

Substantial numbers of adolescents experience the negative health consequences of early, unprotected sexual activity - unintended pregnancy, unsafe abortions, pregnancy-related mortality and morbidity and Sexually Transmitted Infections including Human Immunodeficiency Virus; as well as its social and economic costs. Improving access to and use of contraceptives - including condoms - needs to be a key component of an overall strategy to preventing these problems. This paper contains a review of research evidence and programmatic experiences on needs, barriers, and approaches to access and use

Undiagnosed HIV in women attending a sexual health clinic for non-barrier methods of contraception. Two case studies.

Approximately one-third of human immunodeficiency virus (HIV) cases in the UK remain undiagnosed. More than half of new diagnoses in the last 2 years have been acquired through heterosexual sexual intercourse. Two cases of women with identified risk factors for HIV infection, who presented for contraception and were later found to be HIV-positive, are described. It is suggested that providers of contraception are well placed to identify clients at high risk of HIV and to recommend HIV testing.
