An audit of emergency contraception: a look at patient characteristics and the effects of a consultation proforma.

The aim of this study was to examine the characteristics of patients requesting emergency postcoital contraception at a genitourinary medicine (GUM) clinic. We also compared the quality of information obtained during the consultation, before and after a proforma was introduced. A retrospective review of all clinical notes of patients who attended for postcoital contraception between January and December 1994 and April to June 1995 was performed. Eighty-three per cent of patients were aged 17-29 years, 68.8% were in relationship, 41.3% were not using regular contraception, 33.8% accepted a sexu

Reasons for pregnancy termination: negligence or failure of contraception?

BACKGROUND: The aim of the study was to analyze the reasons for the failure of contraception and the reasons for not using any contraception among women seeking a legal abortion on social grounds. The women were also asked about their knowledge of contraception methods, including postcoital contraception. METHODS: We interviewed 200 women applying for a legal abortion within the first trimester of pregnancy about contraception, the contraceptive methods used, and the possible reasons for failure of contraception. RESULTS: Of all the women interviewed, 93% claimed to have adequate knowledge o

Efficacy and side effects of immediate postcoital levonorgestrel used repeatedly for contraception. United Nations Development Programme/ United Nations Population Fund/World Health Organization/World Bank Special Programme of Research, Development and Research Training in Human Reproduction, Task Force on Post-Ovulatory Methods of Fertility Regulation. vonhertzenh@who.ch.

We evaluated the efficacy and side effects of immediate postcoital administration of levonorgestrel 0.75 mg used repeatedly for contraception. A total of 295 healthy women with infrequent coitus were enrolled at 6 study sites. Each woman took levonorgestrel 0.75 mg by mouth immediately after intercourse during 6 months as her only method of contraception. We collected data on side effects and acceptability and calculated the Pearl index failure rates over 133 woman-years of use by standard methods. The Pearl index failure rate was 6.8 (95% CI 3.1-12.9) pregnancies per 100 woman-years of use. T

Emergency contraception: a review.

In the Netherlands, many women use a postcoital method of contraception in "emergency" situations. Postcoital contraception started in the 1960's with the administration of large doses of estrogens: 50 mg diethylstilbestrol for 5 days or 5 mg ethinylestradiol for 5 days. In the eighties, a double-blind study compared the original hormonal therapy of 5 mg ethinylestradiol for 5 days with a combination pill containing just 0.1 mg in combination with 1 mg d1-norgestrel, of which two doses are give, the second 12 hours after the first. This method was as effective in preventing pregnancy as the or

The anti-reproductive pharmacology of LH-RH and agonistic analogues.

LH-RH and three particular ("super") analogues were evaluated for agonistic (ovulation-induction and short-term uterotrophic properties) and postcoital contraceptive activity in rodents. Additionally, LH-RH and/or a representative analogue (D-Ala6-des Gly10-Pro9-LH-RH ethylamide) were tested for postcoital contraceptive/vaginal smear/return to fertility effects, precoital contraceptive activity, and effects on puberty in the immature female. All compounds induced ovulation and uterotrophic effects and terminated pregnancy when administered either pre- or post-implantation. LH-RH and the repres

[Emergency contraception--current trends, possibilities and limitations].

INTRODUCTION: Emergency contraception has been used for over three decades. Indications for emergency contraception are intercourse without contraceptive protection or inadequate application of other contraceptive means. Also, this method is the only way out in situations when sexual intercourse has proceeded not only without protection, but also without voluntary agreement of both partners. CONTRACEPTION MEANS: Despite of their proven efficiency, it is thought that application of contraception means, even in countries with highly developed systems of health care, is far below optimal. Emergency contraception.

Emergency contraception means preventing pregnancy after unprotected sexual intercourse. This is also called postcoital contraception (PCC) or the 'morning-after pill'. High doses of oestrogen or progestogen or a combination of both may be used as PCC up to 72 hours after unprotected intercourse. The use of mifepristone as emergency contraception has also proved promising. Some women use emergency contraception, but there are many who do not know much about it. Users, providers and other health professionals need to be educated about this method. Emergency contraception does not fall within th

Contraception in the adolescent: current concepts for the pediatrician.

An overview is presented of the major methods of contraception available to the sexually active adolescent. Emphasis is given to the combined birth control pill, while the literature describing absolute and relative contraindications to oral contraception is reviewed. It is noted that adolescents with chronic illness must also be evaluated for contraceptive needs. Other methods covered include the intrauterine device, barrier methods (diaphragm, condom, and vaginal contraceptives), injectable contraceptives, postcoital contraception, and methods under current investigation. The approach to eac

Free access to emergency oral contraceptives (EOCs) in the European Union: A regulatory vacuum to be filled.

BACKGROUND: According to the World Health Organization (WHO), any woman in childbearing age who have decided not to take permanently an oral contraceptive but who are sexually active has the right to access emergency contraception (EC). Despite this, in many European countries there are no specific laws governing the criteria for access to emergency oral contraceptives (EOCs) for girls under 18, especially about the need for third party consent. This normative vacuum is dangerous as it risks creating confusion and entrusting the management of the fundamental right of self-determination to the

Dynamics of Emergency Contraceptive Use in Accra, Ghana.

Recent data suggest increasing rates of emergency contraception (EC) use in Ghana, particularly in urban areas. In 2018, we collected survey data from 3,703 sexually experienced women aged 16-44 years living in low-income settlements of Accra. We estimated the prevalence of lifetime and current EC use. Multivariate logistic regression was used to assess factors associated with current use of EC. Retrospective monthly calendar data on contraceptive use were analyzed to identify patterns of EC use, including repeat and continuous use, and uptake of other contraceptive methods in the months follo

Practice Bulletin Summary No. 152: Emergency Contraception.

Emergency contraception, also known as postcoital contraception, is therapy used to prevent pregnancy after an unprotected or inadequately protected act of sexual intercourse. Common indications for emergency contraception include contraceptive failure (eg, condom breakage or missed doses of oral contraceptives) and failure to use any form of contraception (1-3). Although oral emergency contraception was first described in the medical literature in the 1960s, the U.S. Food and Drug Administration (FDA) approved the first dedicated product for emergency contraception in 1998. Interception by post-coital IUD insertion.

After the development of postcoital hormonal interception, the postcoital use of IUDs to prevent pregnancy was proposed by Tatum. The major advantage of this treatment is that it avoids the use of the usual massive dose of estrogens (a very effective postcoital method) which is associated with nausea and vomiting. This is the 1st method to be effective up to 5 days after exposure, 3 days more than the estrogen treatment. It may also provide long-term contraception in women who wish to continue using the IUD. A major disadvantage of postcoital IUD use is that IUD insertion can produce serious c

Safety of repeated use of emergency contraceptive pills in the same menstrual cycle: a systematic review.

BACKGROUND: Emergency contraception refers to contraceptive methods used following unprotected sexual intercourse. OBJECTIVE: To review the literature on safety of repeated use of emergency contraceptive pills (ECPs) in the same menstrual cycle. METHODS: We searched multiple databases from inception through February 2024 for articles on repeated ECP use. We assessed the risk of bias for each study and certainty of evidence for all outcomes. RESULTS: From 4565 articles identified, six met the inclusion criteria. Four studies of repeated levonorgestrel (LNG)-ECP use provided very low certaint

Ectopic pregnancy after levonorgestrel emergency contraception.

BACKGROUND: Although the possibility of ectopic pregnancy after intake of levonorgestrel (LNG) as an emergency contraceptive (EE) pill is well-known, the causality has not been well established. CASE: A 27-year-old nulliparous woman with regular menstrual periods took 1.5-mg LNG EE midcyclic 5 h after an unprotected intercourse. She had prolonged vaginal bleeding at the expected time. She consulted the general practitioner because of continuous vaginal bleeding for 4 weeks and lower abdominal pain. The pregnancy test was positive, and her symptoms and clinical findings suggested an ectopic pr

Sometimes You Do Get a Second Chance: Emergency Contraception for Adolescents.

Unplanned or unintended pregnancy remains a significant challenge for adolescents; many teens who plan ahead but opt not to choose long-acting reversible contraceptive methods have high failure rates with condom usage, oral contraceptives, and other less long-acting methods. Emergency contraception (EC) remains a necessity for those adolescents seeking a second chance to prevent the unintended consequences of unplanned sexual activity. At present, 5 postcoital methods remain available as EC globally: intrauterine devices, ulipristal acetate, a selective progesterone modulator, mifepristone; le

Pharmaceutical Quality and Accessibility of Emergency Contraceptive Levonorgestrel Tablets via Personal Importation in Japan.

INTRODUCTION: Levonorgestrel (LNG) tablets are used for emergency contraception and must be taken within 72 h after unprotected intercourse. In Japan, access had historically been restricted, although it has improved with the introduction of over-the-counter LNG tablets. However, barriers such as cost and convenience might still lead some individuals to obtain LNG through personal importation websites. We assessed the accessibility, regulatory compliance, and pharmaceutical quality-specifically, content assay and content uniformity-of LNG products obtained through these unregulated online path
