Intraoperative autotransfusion was performed in 48 of 90 women with ectopic pregnancy, seen between 1990 and 1995. Autotransfusion was performed more frequently in the presence of hypovolaemic shock and more severe haemoperitoneum. The procedure appeared safe and useful. OBJECTIVE: Arthroscopic decompression (wafer procedure) of the ulnocarpal compartment in patients with ulnar impaction syndrome.
INDICATIONS: The arthroscopic wafer procedure is recommended in ulnar-plus situations with up to 3 mm length-excess. To perform this procedure the ulnar head needs to be accessible for the burr through a pre-existing, impaction-related, centroradial lesion of the triangular fibrocartilage complex (TFCC). In children with absent thumbs, some authors have stated that 'relative' contraindications of pollicization include severe fixed flexion contracture and instability of the proximal interphalangeal joint of the index finger. OBJECTIVE: To estimate the accuracy of self-screening for contraindications to combined oral contraceptive pills (COCs) and to estimate the proportion of women with contraindications to hormonal methods among those using drug shops in Tanzania.
METHODS: Trained nurses interviewed 1651 women aged 18-39 years who self-screened for contraindications to COCs with the help of a poster at drug shops in Tanzania. Nurse assessment of the women served as the gold standard for comparison with self-assessment. Blood pressure was also measured onsite.

The transjugular intrahepatic portosystemic shunt (TIPS) is an alternative therapeutic approach in the treatment of hemorrhagic portal hypertension. The use of this procedure was established in 1988 and since then, its use has extended impressively. Currently, the accepted indication for TIPS is the variceal bleeding secondary to portal hypertension refractory to medical management or sclerotherapy. In this paper the basic concepts of the TIPS procedure are reviewed, including historical perspective, technical aspects, indications, contraindications and complications.

The percutaneous transhepatic catheterisation is an important method in the management of obstructive jaundice. The results of the percutaneous transhepatic drainage - external and internal - in 14 patients are reported. The technique is described and the difficulties, complications, indications and contraindications are discussed. The value of this procedure is the temporary decompression of the obstructed biliary tract prior to radical surgery or as a palliation in patients with non-operable tumors; the risk of the procedure is low.

Before application of embolization material vasoconstriction of healthy blood vessels is achieved by intraarterial injection of norepinephrine. Endoscopic retrograde cholangio pancreatography (ERCP) combines endoscopic and radiographic techniques and is carried out to allow detailed examination of the bile ducts, pancreatic duct and gallbladder. It can serve as a diagnostic or therapeutic tool. This article provides an update on the procedure as well as indications and contraindications. The nurse's role in caring for patients undergoing ERCP--which includes preparing them for the procedure, observing them post-ERCP and providing health education for discharge and beyond--is also covered.
