Smoking and coronary artery disease assessed by routine coronary arteriography.

The association between extent and duration of smoking habit and severity of coronary atheroma was examined in 387 patients undergoing routine coronary ateriography before valve replacement surgery. Total number of cigarettes smoked in life correlated significantly with severity of coronary artery disease (p less than 0.001) and number of coronary arteries with stenoses of 50% or more (p less than 0.001). Severity of coronary artery disease in current smokers was similar to that in former smokers. Multiple regression analysis showed diastolic blood pressure, cigarette consumption, age, ratio o

Left coronary artery transit time in latent coronary heart disease.

In a group of 104 cases with possible silent coronary heart disease, 36 had normal angiographic findings and 68 significant coronary artery disease. The transit time of the contrast medium in the left coronary artery was significantly shorter in cases with abnormalities of the artery than in non-afflicted cases. The extent of arterial disease seemed to influence the transit time inversely, whereas the location of the abnormalities did not influence the transit time.

Prevalence of Coronary Microvascular Disease and Coronary Vasospasm in Patients With Nonobstructive Coronary Artery Disease: Systematic Review and Meta-Analysis.

Background A relevant proportion of patients with suspected coronary artery disease undergo invasive coronary angiography showing normal or nonobstructive coronary arteries. However, the prevalence of coronary microvascular disease (CMD) and coronary spasm in patients with nonobstructive coronary artery disease remains to be determined. The objective of this study was to determine the prevalence of coronary CMD and coronary vasospastic angina in patients with no obstructive coronary artery disease. Methods and Results A systematic review and meta-analysis of studies assessing the prevalence of

Colchicine for symptomatic coronary artery disease after percutaneous coronary intervention.

BACKGROUND: Percutaneous coronary intervention (PCI), the preferred coronary reperfusion strategy, induces endothelial trauma which may mount an inflammatory response. This has been shown to increase the likelihood of further major adverse cardiovascular events (MACE). Colchicine, a cheap and widely used anti-inflammatory has shown promise in improving cardiovascular outcomes. We aimed to perform a systematic review and meta-analysis to study the effects of colchicine in patients with symptomatic coronary artery disease (CAD) who have undergone PCI. METHOD: We systematically reviewed and meta

Invasive Coronary Physiology Assessment for Patients With Stable Coronary Disease.

Visual assessment of coronary stenosis severity using conventional coronary angiography is associated with wide interoperator variability and a weak relationship with hemodynamics. Invasive coronary physiology assessment using fractional flow reserve (FFR) has been shown to be safe and beneficial. Large multicenter randomized trials have demonstrated the superiority of FFR-guided percutaneous coronary intervention in reducing the risk of major cardiac adverse events, number of stents used, and total cost in patients with multivessel coronary disease. FFR requires vasodilatory agents for the mi

Multivessel coronary artery vasospasm masquerading as surgical coronary atherosclerotic disease.

BACKGROUND AND AIM: We report a case of a 39-year-old lady presenting with worsening angina. MATERIALS AND METHODS: This is a case report study. Clinical case data was retrieved from hospital paper and electronic records. RESULTS: Invasive coronary angiography revealed disease in the left main stem, proximal left anterior descending (LAD) artery and circumflex artery. The patient proceeded to on-pump coronary artery bypass grafting. Intraoperatively, there were multiple unsuccessful attempts to wean off cardiopulmonary bypass. An on-table angiogram-which initially triggered asystole requirin

Prognosis of asymptomatic coronary artery disease after percutaneous coronary intervention.

The appropriate treatment for asymptomatic patients with obstructive coronary stenoses is controversial. The effect of percutaneous coronary intervention (PCI) on the prognosis of such patients is unknown. The present study compared the 1-year outcomes of patients with stable coronary artery disease (CAD) with regard to the presence or absence of symptoms after elective PCI. A total of 1,944 consecutive patients with stable CAD who had undergone elective PCI were studied. They were divided into 2 groups according to symptom status: asymptomatic (n = 1,052) and symptomatic (n = 892). A 1-year f

Polycystic kidney disease with coronary aneurysm and acute coronary syndrome.

Autosomal dominant polycystic kidney disease (ADPKD) is a common disorder occurring in approximately 1 out of every 400 to 1,000 live births. An extremely rare association between ADPKD and coronary aneurysms has been determined. No connection between this association and acute coronary syndromes has been identified; however, a few reports have appeared in the literature. We present and demonstrate the very rare association of ADPKD and coronary aneurysm, and briefly review this unusual relationship.

Association Between Coronary Artery Anomalies, Obstructive Coronary Disease, and Procedural Complexity During Coronary Angiography.

BACKGROUND: The clinical and procedural implications of coronary artery anomalies (CAAs) remain incompletely defined. AIMS: To evaluate the association between anomalous coronary anatomy, obstructive coronary artery disease (CAD), and procedural complexity during coronary angiography and percutaneous coronary intervention (PCI). METHODS: In this retrospective single-center study, all patients who underwent coronary angiography between 2014 and 2025 were screened. Among 9567 angiograms, 134 patients (1.4%) had CAAs of origin/course or termination (excluding myocardial bridges). A vessel-level

Coronary microvascular disease in patients referred to coronary angiography following coronary computed tomography angiography.

BACKGROUND: Ischaemia without obstructive epicardial coronary artery disease (CAD) is common and is often related to coronary microvascular disease (CMD). Previous studies primarily focused on functional assessment in patients with established ischaemia without obstructive epicardial CAD. AIMS: We sought to assess the prevalence of CMD and compare clinical and procedural characteristics including myocardial perfusion imaging, as derived from rubidium-82 positron emission tomography (82Rb-PET), and health status according to CAD classification. [Coronary microvascular disease, vasospastic angina].

Angina and myocardial ischemia as detected by non-invasive modalities frequently occur in individuals without significant (obstructive) epicardial coronary artery disease. These are caused by microvascular coronary artery disease, coronary vasospasm, or a combination thereof. Patients suffering from these conditions have a poor quality of life, and prognosis, and frequently need rehospitalization which poses a significant burden on the healthcare system. It is now clear that the pathomechanism of these conditions can be objectively and quantitatively elucidated by invasive coronary thermodilut

Exercise and Coronary Heart Disease.

Coronary artery disease (CAD) can be obstructive or nonobstructive. Patients with nonobstructive and stable angina pectoris are usually women. Nonobstructive CAD is caused by endothelial dysfunction at the microvascular level, such as cardiac syndrome X and coronary slow flow syndrome. Even if coronary anatomy is nonobstructive, the presence of myocardial ischemia is a major determinant for the exercise program. CAD is a chronic inflammatory disease, and the progression of the disease can lead to a rapid change in the functional capacity of CAD patients. Exercise training is a major component

Exercise Benefits Coronary Heart Disease.

Coronary heart disease (CHD) is a group of diseases that include: no symptoms, angina, myocardial infarction, ischemia cardiomyopathy and sudden cardiac death. And it results from multiple risks factors consisting of invariable factors (e.g. age, gender, etc.) and variable factors (e.g. dyslipidemia, hypertension, diabetes, smoking, etc.). Meanwhile, CHD could cause impact not only localized in the heart, but also on pulmonary function, whole-body skeletal muscle function, activity ability, psychological status, etc. Nowadays, CHD has been the leading cause of death in the world. However, many

Infectious and coronary artery disease.

BACKGROUND: Atherosclerotic event is one of the most causes of death in the world. Coronary artery disease (CAD) is one manifestation of atherosclerosis. It is well-known that several risk factors, such as diabetes mellitus (DM), smoking, hypertension (HTN), have effects on it. It is proposed that infection can lead to atherosclerosis or even make its process faster. Here, we discuss about the effect of some of infectious agents on the atherosclerosis and CAD. METHODS: In this study, first we did a comprehensive search in PubMed, Scopus, and Science Direct using some related keywords such as

Genetics of coronary artery disease.

There is almost no data on the genetics of acute coronary syndromes, so this review discusses primarily the 50 genetic risk variants associated with coronary artery disease that are of genome-wide significance in the discovery population and replicated in an independent population. All of these risk variants are extremely common with more than half occurring in >50% of the general population. They increased only minimally the relative risk for coronary artery disease. The most striking finding is that 35 of the 50 risk variants act independently of known risk factors, indicating there are seve

Coronary heart disease in men.

Elimination of key risk factors such as dyslipidemia and hypertension is important for reducing cardiovascular events later in life. A medical history, physical examination, and laboratory determination of lipid and glycosylated hemoglobin levels provide a good assessment of cardiovascular risk. A statin is first-line therapy for reducing LDL-C, which is the primary lipid target in most patients. High-dose statin therapy may be required to reach desired target levels. The choice of initial antihypertensive therapy is based on patient comorbidities and drug side effects; however, most patients

[Rheology and coronary heart disease].

More than 25 years ago it was demonstrated that an improvement in various parameters of blood flow in patients who have an hyperviscosity syndrome improves coronary blood flow. Hyperfibrinogenemia with resulting increase in plasma viscosity and erythrocyte aggregation has been demonstrated in patients with coronary heart disease. Poiseuille's law, which describes the interaction between vascular resistance, vessel geometry and blood viscosity, indicates--when applied to coronary artery disease--that an increase in the viscosity of blood, especially of plasma, can in the poststenotic microcircu
