Is there a link between nickel allergy and coronary stent restenosis?

Although many patients with coronary artery disease are being treated by coronary stents, in-stent restenosis is the major limitation of percutaneous coronary stenting procedures. Most stents are made of stainless steel, and that, allergic reactions to nickel ions released from coronary stainless-steel stents may be one of the triggering mechanisms for in-stent restenosis. We aimed to evaluate the relationship between in-stent restenosis and nickel allergy in a prospective study. For this purpose, we applied epicutaneous patch test for nickel in 43 patients who had undergone elective intracoro

The variable role of inflammation in acute coronary syndromes and in restenosis.

Inflammation has recently been shown to be an important pathogenetic component of atherosclerosis in general and of acute coronary syndromes in particular. Not only activated inflammatory cells have been found in the plaques, but, more interestingly, also activated circulating inflammatory cells as well as elevated levels of systemic markers of inflammation have been described. Among these, C-reactive protein is of clinical value, as its levels are associated with the outcome. Inflammation is important also in triggering the mechanisms of restenosis and CRP has been recently described as a use

Lipoprotein(a) and coronary thrombosis and restenosis after stent placement.

OBJECTIVES: The objective of this prospective study was to evaluate the relation between high lipoprotein(a) levels and thrombotic and restenotic events after coronary stent implantation. BACKGROUND: Lipoprotein(a) may promote atherogenesis, coronary thrombosis and restenosis after balloon angioplasty, but the clinical significance remains unclear. METHODS: The study included 2,223 consecutive patients with successful coronary stent placement. According to the serum level of lipoprotein(a), patients were divided in two groups: 457 patients of the highest quintile formed the high lipoprotein(

Swine model of coronary restenosis: effect of a second injury.

Looking for a coronary artery restenosis model closer to human pathology, a protocol of balloon injury/reinjury (plaque of dilatation) in swine coronary artery was designed. Pig coronary arteries (n = 24) were dilated for this study: 12, group 1, once (sacrifice at 10.0 +/- 2.2 weeks); 6, group 2, twice at 2-wk intervals (sacrifice at 5.2 +/- 0.2 wk); 6, group 3, twice at 4-wk intervals (sacrifice at 9.3 +/- 1.9 wk). A single overdilatation resulted in an eccentric neointimal hyperplasia representing half of the wall area (group 1, 45.6 +/- 5.1%). In animals (groups 2 and 3) subjected to redil

Sirolimus- or paclitaxel-eluting stents to prevent coronary artery restenosis.

The restenosis rate is lower with stent implantation than with balloon angioplasty. Nevertheless, even with the use of stenting, restenosis still occurs in approximately one-third of patients with diabetes, small coronary vessels, and long lesions. The two drugs commonly used in eluting stents are sirolimus and paclitaxel. Systemically administered sirolimus decreased vascular proliferation in animal models. After preliminary trials showing benefit with sirolimus-eluting stents in de novo coronary lesions, the large-scale SIRIUS (Sirolomus-coated Bx Velocity balloon-expandable stent in the tre

Clinical restenosis after coronary stenting: perspectives from multicenter clinical trials.

OBJECTIVES: We sought to evaluate clinical restenosis in a large population of patients who had undergone coronary stent placement. BACKGROUND: One-year success after coronary stenting is limited mainly by restenosis of and requirement for repeat revascularization of the treated lesion. We studied 6,186 patients (6,219 lesions) pooled from several recently completed coronary stent trials. Clinical restenosis was defined using three different definitions: target lesion revascularization (TLR) beyond 30 days, target vessel revascularization (TVR) beyond 30 days, and target vessel failure (TVF),

Effects of low-dose aspirin on restenosis after coronary angioplasty.

After angioplasty of a previously untreated native coronary artery and after 2 weeks of aspirin therapy, 216 subjects (aged less than 70 years without acute infarction) were randomized to treatment with soluble aspirin, 100 mg/day, or placebo to study the effect on restenosis. Follow-up, defined as angiography at 6 months, earlier angiographic restenosis or coronary bypass surgery was completed by 108 aspirin- and 104 placebo-treated patients. Restenosis (stenosis greater than or equal to 50% plus loss of greater than or equal to 50% of gain, or surgery) occurred in 38 (35%) aspirin- and 45 (4

Intravascular Radiation Delays Recurrence of Recalcitrant Coronary In-Stent Restenosis.

PURPOSE: Intravascular brachytherapy (IVBT) has a well-proven activity against coronary restenosis of a native vessel or a single bare metal stent. However, its effectiveness for multiple recurrent same-vessel in-stent restenoses (ISRs) is unproven. The objective of this study was to determine if IVBT prolongs the ISR-free interval in recalcitrant lesions. METHODS AND MATERIALS: ISR-free intervals before and after IVBT for individual patients were compared. T1 was the ISR-free interval prior to IVBT, and T2 was the ISR-free interval after IVBT. Accrual was sought from patients with a prior IS

Exercise After Coronary Stenting: Potential Effects on in-Stent Restenosis.

PURPOSE OF REVIEW: Structured exercise is a cornerstone of secondary prevention after percutaneous coronary intervention (PCI), yet its specific relationship with in-stent restenosis (ISR) is less well characterized. This narrative review summarizes the biological rationale, clinical evidence, safety, and practical prescription of exercise training in patients with coronary stents, with a focus on its potential impact on ISR. RECENT FINDINGS: Exercise training improves endothelial nitric oxide bioavailability, reduces systemic inflammation, and favorably modulates neointimal proliferation in

Postprocedure chest pain after coronary stenting: implications on clinical restenosis.

OBJECTIVES: The goal of this study was to analyze the incidence and predictors of postprocedure chest pain (PPCP) after percutaneous coronary intervention (PCI) and its correlation with clinical restenosis. BACKGROUND: Chest pain after PCI occurs frequently even in the absence of procedural events and is considered to be due to vasospasm or coronary artery stretch. The short- and long-term significance of PPCP after otherwise successful stenting is not clear. METHODS: We analyzed 1,362 patients undergoing coronary stenting for PPCP, procedural and in-hospital events, 30-day major adverse car

Prevention of restenosis after percutaneous coronary interventions: the medical approach.

Restenosis following successful percutaneous coronary revascularization continues to represent a major problem limiting the clinical efficacy of this procedure. The underlying mechanisms of restenosis are comprised of a combination of effects from vessel recoil, negative vascular remodeling, thrombus formation and neointimal hyperplasia. Indeed, there are important interactions among all of these mechanisms. For example, neointimal hyperplasia is stimulated by growth factors, which are released by local thrombi and the injured arterial segment itself, and act to enhance the expression of other

Intravascular ultrasound predictors of restenosis after percutaneous transcatheter coronary revascularization.

OBJECTIVES: This study sought to evaluate preintervention and postintervention intravascular ultrasound studies for potential predictors of angiographic restenosis and to use ultrasound predictors of restenosis to enhance our understanding of the pathophysiology of the restenosis disease process. BACKGROUND: Restenosis remains the major limitation of percutaneous transcatheter coronary revascularization. Although its mechanisms remain incompletely understood, numerous studies have identified some of the clinical, anatomic and procedural risk factors for restenosis. Intravascular ultrasound im

Long-term results of directional coronary atherectomy: predictors of restenosis.

OBJECTIVES: This study was performed to obtain better understanding of the long-term clinical efficacy of directional coronary atherectomy. BACKGROUND: Although this procedure yields favorable acute results, its acceptance has been limited by the perception that late results (that is, freedom from restenosis) are no better than those of conventional angioplasty. METHODS: A total of 225 atherectomies performed in 190 patients between August 1988 and July 1991 were examined. Minimal lumen diameter of the treated segments was measured on angiograms obtained before, after and 6 months after inte

Blood Pressure Levels at the Time of Percutaneous Coronary Revascularization and Risk of Coronary In-Stent Restenosis.

BACKGROUND: High blood pressure (BP) levels expose patients treated with percutaneous coronary interventions (PCI) to very high risk of 10-year cardiovascular morbidity and mortality. OBJECTIVE: To investigate the role of BP levels at the time of PCI on the risk of in-stent restenosis (ISR). METHODS: We retrospectively included 796 patients previously treated with PCI, who underwent repeated angiography for recurrent angina or reversible myocardial ischemia. Patients were stratified into either case (n = 354) and control (n = 442) groups in the presence or absence of ISR (defined as in-stent

Restenosis of Coronary Arteries in Patients with Coronavirus Infection: Case Series.

INTRODUCTION: Coronavirus infection is a risk factor for vascular thrombosis. This is of particular importance for patients undergoing myocardial revascularization since this infection can be a trigger for the formation of restenosis in the area of a previously implanted coronary stent. Understanding the risk factors for stent thrombosis and restenosis is of particular importance in individuals at risk for adverse outcomes. The rarity of such situations makes the present study unique. OBJECTIVE: Studying the peculiarities of restenosis and thrombosis of the coronary arteries in patients after

Coronary artery stent fracture with aneurysm formation and in-stent restenosis.

Coronary stent fracture is a relatively rare but potentially serious complication of coronary artery stenting. It has been recognised as a cause of in-stent restenosis as well as acute stent thrombosis. Most fractures occur in stents after aggressive post-dilatation, stents implanted in tortuous and calcified lesions, and after cardiac trauma [M.S. Lee, D. Jurewitz, J. Aragon, J. Forrester, R.R. Nickel and molybdenum contact allergies in patients with coronary in-stent restenosis.

BACKGROUND: Coronary in-stent restenosis might be triggered by contact allergy to nickel, chromate, or molybdenum ions released from stainless-steel stents. We investigated the association between allergic reactions to stent components and the occurrence of in-stent restenosis. METHODS: Patients with coronary stainless-steel stents who underwent angiography for suspected restenosis were consecutively included in this study. Quantitative coronary angiography for analysis of percentage diameter stenosis was done on 131 patients (mean age 62 years [SD 9]) with 171 stents 6.1 months (2.7) after s

Pharmacological approaches for the prevention of restenosis after percutaneous coronary intervention.

A large number of drug trials for prevention of restenosis have been conducted with many showing little or conflicting benefit. Antiplatelets such as aspirin, ticlopidine and thromboxane A2 receptor inhibitors have not shown a clear benefit. Similarly, antithrombotics, either acting indirectly such as heparin, or as direct thrombin inhibitors such as hirudin and hirulog, do not prevent restenosis. Trials with ACE inhibitors, HMG-CoA reductase inhibitors and fish-oil supplements have yielded inconclusive results. The antiproliferatives, angiopeptin, trapidil and tranilast have shown some benefi
