Effect of cost sharing on adherence to evidence-based medications in patients with acute coronary syndrome.

OBJECTIVES: Cost-sharing scheme for pharmaceuticals in Spain changed in July 2012. Our aim was to assess the impact of this change on adherence to essential medication in patients with acute coronary syndrome (ACS) in the region of Valencia. METHODS: Population-based retrospective cohort of 10 563 patients discharged alive after an ACS in 2009-2011. We examined a control group (low-income working population) that did not change their coinsurance status, and two intervention groups: pensioners who moved from full coverage to 10% coinsurance and middle-income to high-income working population,

Market share and costs of biologic therapies for inflammatory bowel disease in the USA.

BACKGROUND: Real-world data quantifying the costs of increasing use of biologics in inflammatory bowel disease (IBD) are unknown. AIM: To determine the outpatient IBD drug utilization trends, relative market share, and costs in the USA during a 9-year period. METHODS: The Truven MarketScan® Database was analysed for patients with Crohn's disease (CD) and ulcerative colitis (UC) during 2007-2015. National drug codes were used to identify prescription drugs; Healthcare Common Procedure Coding System J-codes were used to capture biologic out-patient infusions. Proportion of drug usage, relative

Sharing the benefits and costs of conservation: public preferences for transboundary lynx policy.

Transnational conservation is becoming ever-more important, yet reaching consensus on international conservation agreements remains challenging when decision-makers prioritise national benefits. Whether such approaches align with public preferences, however, remains unclear. We conduct a Discrete Choice Experiment to understand preferences for Iberian lynx conservation in Spain and Portugal, with a focus on the distribution of costs and benefits across countries. Results indicate that respondents favour an equal sharing of conservation costs and benefits over alternative allocation schemes. We

Sharing the costs and benefits of climate change mitigation via Shapley value.

This paper examines the free rider problem that exists in the joint effort to mitigate climate change. There is a need to develop a model that is stable and that provides evidence of an objective burden sharing rule so that the environmental agreement is more acceptable. This study approaches this problem via a cooperative game at the global level to make International Environmental Agreements (IEA) more stable. For this purpose, we apply the Shapley value transfer mechanism and find that under the commitment scenario, some regions attain the maximum benefits by joining the coalition. Shapley

Payers should cover COVID-19 vaccines for all ages and risk groups with no patient cost sharing.

The COVID-19 vaccine saved millions of lives during the pandemic. Clinical trials at the time of authorization and approval showed remarkable effectiveness. However, the effectiveness of the COVID-19 vaccine has changed now that people are no longer naive to the virus. The benefit of vaccination is incrementally lower for some people, raising questions about who should receive the vaccine, what the vaccines should cost, and which subpopulations would be cost-effective to vaccinate. The Institute for Clinical and Economic Review (ICER) recently released its assessment of the COVID-19 vaccine. I

Modelling the impact of changes to prescription medicine cost-sharing schemes among middle aged and older adults.

OBJECTIVE: To assess impacts of government changes to prescription medicine co-payments on individuals' out-of-pocket expenditure. METHODS: Participants were community-dwelling adults primarily aged ≥56 years from The Irish Longitudinal Study on Ageing study. Ireland has two prescription cost-sharing schemes: the General Medical Services (GMS) scheme (primarily low-income populations), involving low monthly payment caps and co-payments, and the Drugs Payment Scheme (DPS) (for others), with higher caps and no co-payment limit. We modelled changes to these schemes implemented between 2016 and 2

Vial Sharing of High-Cost Drugs to Decrease Leftovers and Costs: A Retrospective Observational Study on Patisiran Administration in Bologna, Italy.

Waste of high-cost medicines, such as orphan drugs, is a major problem in healthcare, which leads to excessive costs for treatments. The main objective of this study was to evaluate the impact of a vial-sharing strategy for patisiran, an orphan drug used for the treatment of hereditary transthyretin-mediated amyloidosis, in terms of a reduction in the discarded drug amount and cost savings. The retrospective observational study was conducted in a tertiary referral center (Emilia-Romagna, Italy), between February 2021 and November 2022. Data on drug waste were calculated as "(mg used-mg prescri

Examining the Distinct Effects of Structural, Financial, and Information-Sharing Integration on Hospital Costs.

OBJECTIVE: Hospitals are pursuing integration strategies to contain costs, but evidence remains limited. This study examined the independent associations of 3 forms of integration-structural, financial, and information sharing-with hospital costs. STUDY DESIGN: Hospital fixed-effects panel regression using data from multiple sources-CMS Hospital Compare, Let's Get Healthy California, and the American Hospital Association Annual Surveys and Information Technology Supplements-for years 2014-2016. METHODS: Three integration dimensions were measured: (1) structural integration (health maintenanc

It's who you know: patient-sharing, quality, and costs of cancer survivorship care.

PURPOSE: Cancer survivors frequently receive care from a large number of physicians, creating challenges for coordination. We sought to explore whether cancer survivors whose providers have more patients in common (e.g., shared patients) tend to have higher quality and lower cost care. METHODS: We performed a retrospective cohort study of 8,661 patients diagnosed with loco-regional breast, prostate, or colorectal cancer. We examined survivorship care from days 366 to 1,095 following their cancer diagnosis. Our primary independent variable was "care density," a novel metric of the extent to wh

CHS-DRG 2.0 and insurance-specific patient cost sharing in gestational diabetes admissions: a regression discontinuity in time study.

OBJECTIVES: Diagnosis-related group reform aims to restrain inpatient spending, but obstetric evaluations commonly emphasize average hospital costs rather than insurance-specific patient cost sharing. We evaluated early discontinuities associated with CHS-DRG 2.0 in hospitalization costs, out-of-pocket (OOP) payment, self-pay ratio, and selected perinatal outcomes among gestational diabetes mellitus (GDM) admissions. METHODS: We analyzed 8,690 singleton GDM delivery admissions at a tertiary maternal and child health hospital in Fujian, China. The main cohort excluded eight stays exceeding 60

Does Cost-Effectiveness Analysis Overvalue Potential Cures? Exploring Alternative Methods for Applying a "Shared Savings" Approach to Cost Offsets.

OBJECTIVES: To evaluate alternative methods to calculate and/or attribute economic surplus in the cost-effectiveness analysis of single or short-term therapies. METHODS: We performed a systematic literature review of articles describing alternative methods for cost-effectiveness analysis of potentially curative therapies whose assessment using traditional methods may suggest unaffordable valuations owing to the magnitude of estimated long-term quality-adjusted life-year (QALY) gains or cost offsets. Through internal deliberation and discussion with staff at the Health Technology Assessment bo

Coordinating extreme-heat adaptation at mega-sporting events: early warning and cost sharing in a multi-actor differential game.

INTRODUCTION: Coordinating extreme-heat adaptation at mega-sporting events is a time-dependent public health challenge because public authorities, event organizers, and participating teams control different protective measures while heat risk evolves over a finite competition horizon. Existing sport-heat research describes exposure and prevention yet provides limited guidance on how adaptation should be timed, coordinated, and financed across responsible institutions. METHODS: We develop a three-actor linear-quadratic differential game with gross event heat-risk pressure and accumulated adapt

Patient Cost-Sharing and Utilization of Breast Cancer Diagnostic Imaging by Patients Undergoing Subsequent Testing After a Screening Mammogram.

IMPORTANCE: Out-of-pocket costs (OOPCs) have been largely eliminated for screening mammography. However, patients still face OOPCs when undergoing subsequent diagnostic tests after the initial screening, which represents a potential barrier to those who require follow-up testing after initial testing. OBJECTIVE: To examine the association between the degree of patient cost-sharing and the use of diagnostic breast cancer imaging after undergoing a screening mammogram. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study used medical claims from Optum's deidentified Clinformatics

Medicaid work requirements and cost-sharing under H.R.1: Implications for structurally vulnerable and medically complex populations in New Orleans.

BACKGROUND: Medicaid serves as the cornerstone of healthcare access for low-income and medically complex individuals in the United States. H.R.1, also known as the One Big Beautiful Bill Act, introduces sweeping federal Medicaid reforms, including $1 trillion in cuts, mandatory 80-hour monthly work requirements, and $35 co-pays per clinical service, that threaten to disenroll millions of beneficiaries. The implications of these changes for structurally vulnerable populations and student-run free clinics (SRFCs) remain poorly understood. METHODS: We conducted a retrospective cross-sectional st

Liver Transplant Costs and Activity After United Network for Organ Sharing Allocation Policy Changes.

IMPORTANCE: A new liver allocation policy was implemented by United Network for Organ Sharing (UNOS) in February 2020 with the stated intent of improving access to liver transplant (LT). There are growing concerns nationally regarding the implications this new system may have on LT costs, as well as access to a chance for LT, which have not been captured at a multicenter level. OBJECTIVE: To characterize LT volume and cost changes across the US and within specific center groups and demographics after the policy implementation. Positive density-dependent growth supports costs sharing hypothesis and population density sensing in a manipulative parasite.

Parasites manipulate their hosts' phenotype to increase their own fitness. Like any evolutionary adaptation, parasitic manipulations should be costly. Though it is difficult to measure costs of the manipulation directly, they can be evaluated using an indirect approach. For instance, theory suggests that as the parasite infrapopulation grows, the investment of individual parasites in host manipulation decreases, because of cost sharing. Another assumption is that in environments where manipulation does not pay off for the parasite, it can decrease its investment in the manipulation to save res

Implementation of a shared care guideline on the lumbosacral radicular syndrome: effects on unnecessary referrals, waiting time and costs

Purpose To determine the effects of the implementation of the LRS-guideline in two regions in The Netherlands with regard to reduction of unnecessary referrals and duration of total diagnostic procedure. The effects were related to the cost of implementing the guideline. Theory The guideline Although in The Netherlands GPs, physiotherapists and neurologists share the same guidelines regarding the conservative treatment of LRS, adherence turns out to be difficult. The main reasons are that patients exert pressure on the GP for referral, and that there is no collaboration between careproviders o
