Anxieties of the Group Psychotherapist: A Psychodynamic Perspective on Countertransference, Group Phases, and the Internal Work of the Therapist.

While patient anxiety is well-documented in psychotherapy literature, the anxieties of the therapist, particularly in group settings, receive considerably less attention. This article explores the specific anxieties experienced by group therapists, their origins, their impact on the therapeutic process, and strategies for their management. It is a theoretical review integrating psychodynamic (e.g. Freud, Bion), relational (e.g. Mitchell), and neurobiological perspectives, illustrated with clinical case material. Group therapists' anxieties are often rooted in early group experiences, narcissis

Striving toward useful interpretation while managing countertransference enactments: encounters with a thick-skinned narcissistic person.

Narcissistic patients tend to push the analyst to work harder than usual to contain, understand, translate, and utilize their countertransference states. This is because of the unusually extreme reliance on denial, devaluation, projective identification, and control that these individuals exhibit. Defenses against loss, envy, greed, and dependence create difficult transference states in which symbolic or creative material is flattened, stripped, and neutralized. Feelings are out of the question. This clinical paper explores the narcissistic lack of connection to self and other that endures in

Navigating Countertransference in Inpatient Settings: Optimizing Interventions for Patients with Borderline Personality Disorder and Repeated Acute Hospitalizations.

Over the years, the psychiatric inpatient treatment paradigm has shifted to more brief stays focused on acute stabilization and psychopharmacologic-focused interventions, rather than individual psychotherapeutic engagement. Unfortunately, this has allowed patients with complex interpersonal dynamics, particularly borderline personality disorder, to slip through the cracks of effective treatment. This can contribute to repeated inpatient admissions, where both patients and clinicians feel trapped in a maladaptive, unhelpful cycle. In this article, we examine the evolution of inpatient treatment

Telling a Complicated Grief: A Psychodynamic Study on Mental Health Nurses' Countertransference Reactions to Patients' Suicidal Behavior.

A key element of suicide education, training and clinical supervision is enhancing emotional awareness about mental health professionals' countertransference reactions, as emotional responses to patients' suicidal behavior (SB) that may be unbeneficial to care. This study aimed to explore emotional responses to patients' SB in mental health nurses (MHNs) according to a psychodynamic perspective. Twenty-eight Italian MHNs (61% females, Mage = 52 years) were interviewed to examine their deep feelings about attempted suicides or deaths by suicide of patients with mental illness. Mental states as part of countertransference responses in psychotherapists facing reports of traumatic events of mourning and sexual violence.

The study aims to compare the mental states and countertransference responses of 92 psychodynamically oriented psychotherapists, male and female, experienced and inexperienced, facing written reports of real patients who experienced traumatic events. Two vignettes were presented: one of a sexual violence, the other the sudden death of a significant person. The Mental States Rating System (MSRS; Bouchard, Picard, Audet, Brisson, & Carrier, 1998), the MSRS Self-Report (Goldfeld & Bouchard, 2004), and the Inventory of Countertransference Behavior (ICB; Friedman & Gelso, 2000) were used. Changes in mentalization in patients with personality disorders during sequential brief-Adlerian psychodynamic psychotherapy: The role of therapists' technique and countertransference.

UNLABELLED: Mentalization, that is the capacity to understand our and others' behaviors in terms of intentional mental states, represents one of the core features of personality disorders (PDs) and can be related to therapists' countertransference (CT) and interventions. AIMS: The aim of the present work was to study the relationship between therapists' technique, therapists' CT, and patients' mentalization, in a sample of patients with PDs undergoing a 40-session program of sequential brief-adlerian psychodynamic psychotherapy, a psychodynamic therapeutic model specifically developed for the

When the light shed by God is dimmer than the light shed upon God: countertransference illumination of latent religious object representations of a Jewish patient in psychoanalysis.

A report is offered of the psychoanalysis of an emotionally constricted, religiously devout ko'hen, a member of the Jewish priestly class, undertaken with the author, himself a religiously committed Jew. While the analysand brought a great deal of material to his hours and was deeply committed to the analysis, the sessions were emotionally vapid, obsessively repetitious and ruminative. For more than three years, the patient's steadfast production of a plethora of dreams did not much alter the brittle quality of work and the restricted range of the transference. At first, though religious theme

Clinicians' Emotional Reactions toward Patients with Depressive Symptoms in Mood Disorders: A Narrative Scoping Review of Empirical Research.

The purpose of this article is to narratively review the empirical literature on clinicians' emotional, cognitive, and behavioral responses (i.e., countertransference) to depressive and other symptoms of patients with mood disorders. Therapist subjective responses (countertransference) can negatively affect both diagnostic and therapeutic processes, especially when they are not recognized and managed promptly. However, at the same time, countertransference recognition, processing, and management can help inform the diagnostic process and improve the therapy process and outcome. In the last cou

The perverse subject of analysis.

In this paper I suggest that the analysis of perversion necessarily involves the elaboration and analysis of a perverse transference-countertransference. Both analyst and analysand contribute to and participate in the perverse transference-countertransference which intersubjective construction is powerfully shaped by the perverse structure of the patient's unconscious internal object world. In the fragment of an analysis that is presented, I illustrate the way in which the analyst makes use of his experience in (of) the transference-countertransference in gaining understanding of the perverse

Exploring the outcomes of psychotherapy sessions: how do therapists' responsiveness and emotional responses to patients with personality disorders affect the depth of elaboration?

BACKGROUND: The impact of depth of elaboration in individual psychotherapy sessions on overall treatment effectiveness was found in the empirical literature. In the best sessions, relevant content is processed with greater depth; in contrast, in the shallower sessions, the emerging content is more superficial. Evidence suggests that achieving a high level of depth is closely related to specific therapist characteristics and relational dimensions (including clinicians' emotional responses to patients). The present study aimed to (a) compare therapist responsiveness and countertransference patte

"Are We Safe Analysts?" Cisgender Countertransferential Fantasies in the Treatment of Transgender Patients.

The authors examine the impact of countertransference in two clinical cases of transgender patients treated by two cisgender analysts who are accustomed to receiving nonconforming gender patients in France and Brazil. The context is that of contemporary views of transphobic countertransference reactions, specifically the work of Griffin Hansbury, who describes these reactions in terms of "unthinkable anxieties." Like other theorists with expanding notions of countertransference, the authors view transphobia in analysis as an "instrument of research" and consider how taking responsibility for t

Who's Afraid of Murderous Rage? When Euthanasia Colludes with Self-Destructiveness.

The impact of intense countertransference affects in working with patients experiencing complex trauma can have a critical effect on decisions about euthanasia, especially when such decisions are made solely on the grounds of a psychiatric condition. These countertransference dynamics become particularly significant in the context of the rising number of euthanasia requests by psychiatric patients in the Netherlands. We contend that for a subgroup of patients with complex trauma, attachment trauma, and personality disorders, the label "treatment-resistant" may be applied prematurely and incorr

Drawing trauma: the therapeutic potential of witnessing the child's visual testimony of war.

Countertransference plays an often neglected role in witnessing children's testimony of war and trauma. A dual notion of countertransference, based on the work of Winnicott and Klein, is offered that involves both internal conflict related to early life experience and socially mediated notions of childhood, war, and trauma circulating in a given time and place. A drawing by a thirteen-year-old boy living in the refugee camps in Darfur is used to show how countertransference affects our interpretation of the image, even while its symbolization in language establishes the conditions for a potent

Is stuttering a contraindication to psychotherapy?

This is a case report of a patient who stutters and was treated for problems of self-esteem with focused brief psychotherapy. The literature by and large is against the use of individual psychotherapy for neurotic problems in patients who stutter. The countertransference may account for the unwarranted pessimism in the literature about such patients. In this case the successful resolution of countertransference feelings facilitated the achievement of the goals of therapy. Further reports on such treatment attempts are warranted. Speech therapy may be facilitated by brief focused psychotherapy

A Psychodynamic Approach to Co-occurring Borderline Personality and Substance Use Disorders in the Emergency Department.

Patients with co-occurring substance use disorders and borderline personality disorder have high rates of morbidity, mortality, and utilization of medical services. Their acute symptoms present complex challenges to clinical staff in the medical emergency department related to both logistics and management of countertransference. This article examines patterns in countertransference and proposes application of psychodynamically informed principles and strategies to facilitate safety and enhance communication during fraught clinical encounters.

Psychiatrists' emotional reactions: Useful for precise diagnosis in adolescence?

Therapist response (countertransference) can be defined as the range of emotions a clinician experiences when interacting with a patient. Previous research has shown that this reaction plays an important role in the establishment of the therapeutic relationship, and that certain psychiatric disorders are systematically related to strong countertransference reactions in clinicians. For this study, 30 videotaped sessions were retrospectively analyzed by one observer using the Therapist Response Questionnaire (TRQ). The authors aimed to investigate the differences in therapist response between an

Patient personality and therapist response: an empirical investigation.

OBJECTIVE: The aim of this study was to examine the relationship between therapists' emotional responses and patients' personality disorders and level of psychological functioning. METHOD: A random national sample of psychiatrists and clinical psychologists (N=203) completed the Therapist Response Questionnaire to identify patterns of therapists' emotional response, and the Shedler-Westen Assessment Procedure-200 to assess personality disorders and level of psychological functioning in a randomly selected patient currently in their care and with whom they had worked for a minimum of eight ses

Real and unreal elements within the psychotherapy relationship.

This article honors Charles Gelso by examining the ways in which he embodied intellectual curiosity, clarity, and precision in theoretical writings and scientifically rigorous research with regard to two central aspects of the psychotherapy relationship: countertransference and the real relationship. We offer a behind-the-scenes perspective of what it was like to work closely with Charlie, with the associated demands and joys. Finally, we offer some future directions for scholarship on countertransference and the real relationship. (PsycInfo Database Record (c) 2026 APA, all rights reserved).

Variations in the analytic holding environment.

The concept of the holding environment has been used previously to characterize the treatment needs of severely regressed patients (Winnicott, 1963b). This holding of dependence is contrasted with the holding needs of narcissistic, apparently disengaged patients (Modell, 1976). The present paper considers the type of holding required by difficult, borderline patients. It is suggested that these borderline patients need a holding of their ruthlessness and rage, and also of analytic self blame. Clinical vignettes describe the holding of an anxiously demanding and of an overtly hateful borderline
