Association of decrease in insula fMRI activation with changes in trait anxiety in patients with craniomandibular disorder (CMD).

Patients with chronic pain and especially with craniomandibular disorder (CMD) show specific psychopathology in trait anxiety. In a previous longitudinal functional imaging study on CMD we found that the anterior insula was modulated by successful therapy intervention and pain relief. We here intended to investigate possible associations between anterior insula fMRI-activation during occlusal movements and trait anxiety over a splint therapy approach in patients with CMD. Three fMRI-investigations of a craniomandibular occlusion task were performed together with pain score evaluations and scor

Correlation between dento-skeletal characteristics and craniomandibular disorders in growing children and adolescent orthodontic patients: retrospective case-control study.

PURPOSE: The aim of this retrospective case-control study was to identify, in a group of growing children and adolescents affected by malocclusion, specific dento-skeletal characteristics which could be correlated to the onset, in the above-mentioned subjects, of craniomandibular disorders (CMD). MATERIALS AND METHODS: Among the patients treated at the Paedodontics and Orthodontics department of Bari Dental School, we recruited a group of patients with malocclusion and symptoms of temporomandibular disorders, as an experimental group. We considered as controls those patients who, match-paired

The recognition of craniomandibular disorders--a comparison between clinical, tomographical, and dental panoramic radiographical findings in thirty-one subjects.

A comparison between clinical, tomographical, and dental panoramic radiographical findings was made in thirty-one female patients suffering from craniomandibular disorders (CMD). In general it was found that condylar sclerosis was more common in these patients than in earlier studies. After clinical separation of the material into two groups based upon the origin of pain, myogenous versus arthrogenous, no radiographical confirmation of the differential diagnostics could be made. Regarding vertical condylar asymmetry measured on the Orthopantomogram, it was found that 74% of the patients with C

Assessing, categorizing and proposing the therapeutic effect of complete dentures on temporomandibular disorders analyzed by craniomandibular index: a prospective interventional study.

BACKGROUND: Edentulism is a devastating condition that is often irreversible and considered a marker of a diseased state of oral health. The prevalence of edentulism globally is 22%, which is quite high. The long-term absence of complete dentures is thought to cause joint problems. OBJECTIVES: To evaluate and categorize the effect of complete dentures on signs and symptoms of temporomandibular disorders (TMD) analyzed by the Craniomandibular Index (CMI) in completely edentulous patients. METHODS: This interventional and prospective study was conducted. Preliminary screening of completely ede

[Dissertations 25 years after date 45. Prevalence and etiology of craniomandibular dysfunction in the Netherlands].

The disorders temporomandibular dysfunction and craniomandibular dysfunction are still being discussed intensely in the literature 25 year after the publication of the dissertation 'Prevalence and etiology of craniomandibular dysfunction. An epidemiological study of the Dutch adult population'. Attention is especially being devoted to occlusion and its relationship with this disorder; the conclusions reached are often contradictory. In addition to the definitions of temporomandibular and craniomandibular dysfunction and of occlusion, a possible explanation for this controversy can be found in

Functional disturbances of the masticatory system in relation to articulatory disorders of speech in a group of 6-8-year-old children.

In the sample of 208 children, clinical signs and symptoms of craniomandibular disorder were mild, somewhat more common in boys than in girls, and clearly more common in children referred for speech therapy than in the control group. Of variables measured in millimetres, the differences indicate sexual dimorphism rather than differences between boys and girls in the occurrence of craniomandibular disorders. The differences may also reflect later maturing of neuromuscular control in boys rather than true differences between boys and girls in the prevalence of craniomandibular disorder. Logistic

[Head posture in orthodontics: physiopathology and clinical aspects 2].

The Authors review in orthodontic respects present knowledges about head posture involvement in craniofacial morphogenesis and pathology. Relationships between craniofacial morphology, craniocervical posture, craniomandibular posture, cervical spine curvature, hyoid bone position and posture of whole body in space are shown, in attempt to explain conditions such as "forward head posture", mouth breathing and some occlusal disorders. Main methods to evaluate craniocervical relations on lateral skull radiographs are analysed. Pathogenesis of pain syndromes associated with abnormal craniocervical

[Evaluation of the posture of patients before and after orthodontic surgery].

INTRODUCTION AND AIMS: The term "craniomandibular disorder" is used to describe a series of symptoms and signs that directly affect the stomatognathic apparatus with possible repercussions on the otovestibular and oculomotor apparatus and on the cervical spine that may condition the entire body posture. The aim of this study was to evaluate a series of parameters correlated to the occlusal situation and to verify how these factors are affected by a change following the correction of occulusal ratios. METHODS: The paper reports a series of 15 patients suffering from facial dysmorphia treated s

[The role of occlusion and articulation in craniomandibular dysfunction].

Historically, occlusal interferences are considered to be a major etiologic factor of craniomandibular disorders. Recent evidence suggests however that occlusal and articular parameters only play a minor etiologic role, if any. This controversy will only be solved after studies investigating well-defined homogenous patient groups. Concerning treatment, appliance therapy has proven to be a valuable part of the conservative reversible treatment procedure which allows to manage the majority of craniomandibular patients. Irreversible procedures e.g. complex orthodontic or prosthetic therapy, occlu

Craniomandibular stiffness in myogenous and arthrogenous CMD patients, and control subjects: a clinical and experimental investigation.

The purpose of this study was to compare active maximum mouth opening, endfeel distance, and craniomandibular stiffness values of three craniomandibular disorder (CMD) patient subgroups and a control group. The CMD patient subgroups consisted of myogenous pain patients and arthrogenous pain patients with a 'closed lock' and arthrogenous pain patients without a 'closed lock'. Both myogenous pain patients and the 'closed lock' patients showed great differences on all parameters (P < 0.05-P < 0.001). However, the arthrogenous pain patients without a 'closed lock' revealed no statistical differenc

[Dental and pharmacological treatment options for bruxism].

Bruxism has a multifactorial etiology. The consequences of bruxism, abrasion of teeth and a possible craniomandibular disorder, present themselves periferal. So, treatment can have divers points of action. Counseling seems to be useful in all cases. Treatment with a hard acrylic occlusal appliance (oral splint) is especially useful in those situations where the patient has a lot of abrasion or suffers from a craniomandibular disorder with pain. Pharmacological treatment will have a more central effect. Some drugs are effective on bruxism. However, the real pharmacological mechanisms have not y

Chronic craniomandibular pain after craniotomy: A long-term clinical study.

OBJECTIVE: Chronic craniomandibular/cervical pain and temporomandibular disorders have not been studied in patients who had a craniotomy several years previously. The aim of the current clinical work was to address these issues. METHODS: A total group of 150 ambulant patients who had a previous craniotomy was subclassified according to whether or not the temporalis muscle was manipulated. RESULTS: The average incidence of multiple subsite regional head and neck pain was 69.3% a number of years after a craniotomy. Evidence of internal derangement of the temporomandibular joint was significant

[Temporomandibular joint diseases].

Why are patients liable to suffer from craniomandibular disorders? What are the various local and global consequences? What is the part played by bruxism and craniofacial traumas in the aetiology of cranio-mandibular disorders? What is the contribution that can be expected from MI in this diagnosis? What are the various treatments that can be taken into consideration? These are all questions that the GPs could ask themselves concerning TMJ and related disorders.

Virtual surgical planning in craniomaxillofacial surgery: a structured review.

Craniomaxillofacial (CMF) surgery is a challenging and very demanding field that involves the treatment of congenital and acquired conditions of the face and head. Due to the complexity of the head and facial region, various tools and techniques were developed and utilized to aid surgical procedures and optimize results. Virtual Surgical Planning (VSP) has revolutionized the way craniomaxillofacial surgeries are planned and executed. It uses 3D imaging computer software to visualize and simulate a surgical procedure. Numerous studies were published on the usage of VSP in craniomaxillofacial su
