Bilateral predominant radial nerve crutch palsy. A case report.

Axillary crutch palsy, although well described in the literature, is rarely seen in clinical practice. Little has been written in the contemporary orthopaedic literature. The purpose of this case report is to report on a well-documented axillary crutch palsy with bilateral, predominantly radial, nerve dysfunction that resolved spontaneously in eight weeks.

[Indication and results of an alloarthroplastic hip joint replacement with the "crutch" endoprosthesis].

After discussing the relevant literature concerning indication, results and complications of total-hip-replacement by special prothesis of the upper end of the femur in cases of tumors or serious traumatic bone destructions of the intertrochanteric region of elder patients we representate our own 30 patients (from the years 1973 to 1982). Often, this special prothesis could be only a palliative regional solution in the cases of local removal of the tumor because of the advanced disease. But this treatment is important for temporary reconstruction; even in cases of aged patients with serious fr

The mechanical performance of ambulation using spring-loaded axillary crutches. A preliminary report.

Crutches with the extension post replaced by a spring-loaded post were compared with standard axillary crutches with respect to stresses developed during the stance phase of locomotion. Seven subjects were tested using both standard and spring-loaded crutches. The crutches were fitted with strain gauges; crutch length and grip location were adjusted for each subject using accepted standards. Subjects ambulated using a three-point "swing-through" gait pattern while strain gauge data were recorded. Spring-loaded axillary crutches had 22% smaller shock waves upon initial contact and 24% smaller p

The influence of a polymer damper on swing-through crutch gait biomechanics.

Forearm crutch technology has evolved slowly compared to other assistive mobility devices, despite the highly repetitive nature of forearm crutch gait and the high incidence of overuse injuries. Using 13 able-bodied volunteers between the ages of 19 and 27, we compared the ground reaction forces of a novel crutch design featuring an elastomeric polymer situated below the handle to an identical design without a damper system and to a commercially available generic rigid forearm crutch model. There were no differences in peak vertical force or impulse between crutches. The crutch with the damper

Upper extremity biomechanical model of crutch-assisted gait in children.

A 3D biomechanical model with a novel instrumented Lofstrand crutch system is presented. The novel Lofstrand crutch system consists of two six-axis load cells incorporated in the crutch to study the reaction forces occurring at the crutch handle and the cuff. The goal of this study is to quantify the effect of the cuff forces with the help of this improved crutch system. The kinematic model developed is verified based on previous studies. The kinetic model, consisting of the forces, is derived using the kinematic data, anthropometric data and the reaction forces generated from the load cells.

Upper extremity kinetics during Lofstrand crutch-assisted gait.

A three-dimensional (3D) biomechanical model was developed to determine upper extremity kinematics and kinetics of persons walking with forearm crutches. Six-component load cells and strain gauges were installed in the crutches to determine crutch forces. A six-camera VICON motion system was used to acquire coordinate data from 24 reflective markers attached to the upper extremities and crutches. Joint axes for the wrist, elbow, and glenohumeral joints were defined and joint forces and moments were determined using inverse dynamics. Accuracy of the crutch instrumentation was established by sim

Energy cost of three-point crutch ambulation in fracture patients.

The energy consumption associated with unilateral nonweight-bearing (three-point) crutch ambulation was evaluated in a group of 25 newly injured fracture patients immobilized in a long or short leg cast. After 5 min of crutch ambulation, the rate of oxygen uptake was 32% greater than the value for normal walking (averaging 15.7 ml/kg-min); the heart rate was 53% greater than normal (averaging 153 beats/min); and the respiratory quotient was markedly elevated (averaging 1.03). In those subjects able to ambulate for 10 min, these values averaged 17.6 ml/kg-min and 173 beats/min, approaching the

Unilateral Palmar Callus and Irritant Hand Eczema - Underreported Signs of Dependency on Crutches.

Leg amputees who can't use prostheses and patients with arthritis are often dependent on crutches. Their chronic use can exert significant friction forces. The palmar skin will respond by forming a hyperkeratotic callus. We report for the first time unilateral palmar callus formation caused by friction from using crutches. Another possible adverse effect is the triggering of irritant contact dermatitis by the handholes of crutches. We report two cases with hand dermatitis due to the chronic dependence on crutches and discuss treatment options.

Comparative Analysis of Predetermined Axillary Crutch Length Settings to Individualized Fittings.

Axillary crutches are the most common assistive devices given to individuals with musculoskeletal injuries in an acute care setting. Nurses are frequently the care provider fitting injured individuals with crutches. This study compared the crutch length determined by the crutch manufacturer's height setting with the crutch length attained after applying a standard clinical protocol for crutch fitting. A total of 116 adults with lower extremity injuries were enrolled. Self-reported height was documented as well as initial crutch length as indicated by the numbers on the push-button feature of t

Biomechanical Analysis of the Unaffected Limb While Using a Hands-Free Crutch.

Basic human ambulation relies on a bipedal gait, which has been reported to be directly related to quality of life. However, injuries to the lower limb can cause an inability to walk and require non-weightbearing periods to heal. Among the many ambulatory aids, standard axillary crutches are prescribed. However, due to the disadvantages of having to use both hands, a slow gait, pain, nerve damage, and gait patterns that differ from that of healthy subjects, currently, a new generation of ambulatory aids has emerged. Among such aids, hands-free crutches (HFCs) are of particular interest due to

Persistent anterior interosseous nerve palsy following forearm crutch use.

A 43-year-old man presented with weakness of the interphalageal joint of his right thumb following the use of forearm crutches. On examination he was unable to oppose his thumb and index finger to form the 'ok' sign. Nerve conduction showed anterior interosseous nerve (AIN) damage along its path to the flexor pollicis longus. The patient was managed conservatively with little clinical improvement seen at 4 months. AIN palsies are very rare and account for <1% of all upper limb lesions. Although AIN palsies resulting from other causes such as surgery and blunt trauma are more common, we report

Combination ptosis crutch and moisture chamber for management of progressive external ophthalmoplegia.

BACKGROUND: A 46-year-old woman with a long-standing history of chronic progressive external ophthalmoplegia manifested a primary visual symptom of bilateral ptosis. Previous lid surgery was unsuccessful because of severe sight-threatening exposure keratitis. CASE REPORT: The patient's problem was managed by fabricating a spectacle-mounted combination ptosis crutch and moisture chamber. The custom device successfully provided both cosmetic and visual relief for the patient while maintaining corneal integrity. CONCLUSIONS: This case demonstrates the effectiveness of a dual-purpose appliance t

EMG Activity With Use of a Hands-Free Single Crutch vs a Knee Scooter.

BACKGROUND: Foot and ankle injuries frequently require a period of nonweightbearing, resulting in muscle atrophy. Our previous study compared a hands-free single crutch (HFSC) to standard axillary crutches and found increased muscle recruitment and intensity while using the HFSC. Knee scooters are another commonly prescribed nonweightbearing device. The purpose of this study is to examine the electromyographic (EMG) differences between an HFSC and knee scooter, in conjunction with device preference and perceived exertion. METHODS: A randomized crossover study was performed using 30 noninjured

Experimental characterization of axillary/underarm interface pressure in swing-through crutch walking.

Supporting weight on the upper support of crutches is not recommended because it can lead to axillary nerve damage. Despite this warning, improper axillary loadings may still occur because of a lack of arm strength or fatigue. It is generally accepted that improper use of conventional axillary crutches contributes to axillary nerve damage, but surprisingly there are no studies characterizing axillary support/underarm configurations. In this study, we compared traditional and horizontal axillary support designs by measuring various biomechanical parameters on the axillary support during a swing

Recurrent upper limb ischaemia due to a crutch-induced brachial artery aneurysm.

An 83-year old man who had used bilateral axillary crutches for 67 years was referred to our hospital for acute left upper limb ischaemia. He underwent successful recanalization through emergent catheter thromboembolectomy. However, a crutch-induced left brachial artery aneurysm was subsequently detected by computed tomography. Therefore, we performed aneurysm exclusion and subsequent saphenous vein bypass grafting. When a crutch user presents with upper limb ischaemia, a high index of suspicion and early identification of the crutch induced vascular injury are mandatory for appropriate treatm

Assessing digital crutch-assisted walking in people with Parkinson's Disease: an exploratory study.

Parkinson's disease (PD) is a neurodegenerative disorder marked by motor symptoms such as bradykinesia, tremor, rigidity, and postural instability, which impair gait and balance. As PD progresses, gait disturbances-including reduced speed, shorter strides, and freezing of gait (FoG)-increase the risk of falls and limit functional independence. While wearable sensors are commonly used to monitor gait in PD, there has been limited research on technological devices designed to assist mobility in this population. This study explored the feasibility of mCrutch, a sensorized crutch system, in suppor

Acute upper limb ischemia secondary to crutch-induced brachial artery aneurysm.

Presentation of upper limb ischemia in patients with long-term crutch use warrants suspicion of proximal aneurysmal degeneration with distal thromboembolization. We present a case of a 63-year-old man with a history of chronic axillary crutch use due to childhood poliomyelitis who presented with acute left-hand ischemia. Computed tomography angiography identified a left brachial artery aneurysm, which was determined to be the etiology of the thromboembolic event. The aneurysm was successfully excluded with an axillary-to-brachial artery bypass using the greater saphenous vein. He recovered wel
