Concordance of molecular microbiology and conventional culture techniques for infected diabetic foot ulcer management.

AIM: The management of infected diabetic foot ulcers (DFUs) requires balancing the need for timely interventions against the desire for targeted antibiotic therapy, which relies on laboratory results. This study aimed to evaluate concordance between molecular and conventional culture and sensitivity (C&S) methods in identifying bacteria from infected DFUs. METHODS: This study was conducted alongside CODIFI2, a Phase III randomised controlled trial comparing tissue sampling with wound swabbing. It assessed C&S and metagenomic 16S rRNA gene sequencing (M16S) in DFUs with suspected mild or moder

Direct chromosome preparation and culture using chorionic villi: an evaluation of the two techniques.

We present a comparison between direct chromosome preparation and cell culture for first trimester fetal chromosome study using chorionic villi. The 2 techniques have advantages and disadvantages and are demonstrated to be appropriate for routine diagnostic work. The combined use of both methods may optimize the quality of the chromosome study and minimize the possibility of false-positive and false-negative findings.

Tissue culture techniques as an aid to prenatal diagnosis and genetic counselling in homocystinuria.

Cystathionine synthase activity was studied in skin fibroblasts from a mother with homocystinuria and her husband and newborn baby. Enzyme studies were also undertaken on a fibroblast cell line derived from amniotic fluid taken at 16 weeks' gestation. The enzyme activity was very low in the mother, within the normal range in the father, and at an intermediate level consistent with heterozygosity in the infant. The activity present in the amnion fibroblasts was similar to that found in the cell line cultured from the infant's skin biopsy.

Characterization of a Novel Human Organotypic Retinal Culture Technique.

Previous human organotypic retinal culture (HORC) models have utilized detached retinas; however, without the structural support conferred by retinal pigment epithelium-choroid (RPE-choroid) and sclera, the integrity of the fragile retina can easily be compromised. The aim of this study was to develop a novel HORC model that contains the retina, RPE-choroid and sclera to maintain retinal integrity when culturing retinal explants. After cutting circumferentially along the limbus to remove iris and lens, four deep incisions were made to flatten the eyecup. In contrast to previous HORC protocols,

Outpatient urine culture: does collection technique matter?

BACKGROUND: Dysuria is one of the most common presenting complaints of young women, and urinalysis is one of the most common laboratory tests performed. Despite the fact that the midstream clean-catch technique is commonly used for urine collection, contaminated urine cultures occur with distressing regularity. The midstream clean-catch technique is time-consuming to explain, frequently not performed correctly by patients, costly for supplies, often embarrassing for patients and staff, and of unproven benefit. Therefore, we designed a study to compare various methods of obtaining specimens for

Validation of catheter semiquantitative culture technique for nonstaphylococcal organisms.

The catheter semiquantitative culture roll tip technique has been validated as a discriminator between non-catheter-related bacteremias and catheter-related bacteremias (CRBs) caused by Staphylococcus species. However, this technique has not been specifically validated when used for the evaluation of catheters infected with organisms other than staphylococci. We reviewed catheters that had been submitted for semiquantitative roll tip culture as well as hospital records to determine clinical correlates of infection. Local infection and CRB were defined by standard criteria. Catheter-related sep

Evaluation of the MODS culture technique for the diagnosis of tuberculous meningitis.

BACKGROUND: Tuberculous meningitis (TBM) is a devastating condition. The rapid instigation of appropraite chemotherapy is vital to reduce morbidity and mortality. However rapid diagnosis remains elusive; smear microscopy has extremely low sensitivity on cerebrospinal fluid (CSF) in most laboratories and PCR requires expertise with advanced infrastructure and has sensitivity of only around 60% under optimal conditions. Neither technique allows for the microbiological isolation of M. tuberculosis and subsequent drug susceptibility testing. We evaluated the recently developed microscopic observat

A technique for the in vitro culture of human parathyroid gland tissue.

A technique for prolonged in vitro culture of human parathyroid tissue is described. Cells could be maintained in this monolayer system and were shown to continue releasing high levels of parathyroid hormone into their culture medium even after 140 days in culture. Furthermore, growth of fibroblasts, persistence of epithelial cells and parathyroid hormone release was demonstrated in cells derived from parathyroid tissue that had been cryopreserved for 2 years. The availability of viable and functioning human parathyroid tissue produced in this culture system may be of value in both auto- and a

Techniques for culturing and determining antimicrobial susceptibility of Chlamydia trachomatis.

The application of cell culture techniques to the study of chlamydiae have led to isolation of C. trachomatis from a variety of syndromes, among which are nongonococcal urethritis, post-gonococcal urethritis, cervicitis, acute salpingitis, neonatal conjunctivitis, and pneumonia of infants. The methods currently employed fo chlamydia isolation are described, as well as the conditions which affect the infectivity of these organisms to cells in vitro. Cell culture methods applicable to the study of the effect of antibacterial drugs on C. trachomatis are also presented.

Evaluation of conventional castaneda and lysis centrifugation blood culture techniques for diagnosis of human brucellosis.

We investigated the role of the lysis centrifugation blood culture technique over the conventional Castaneda technique for the diagnosis of human brucellosis. The lysis centrifugation technique has been found to be more sensitive in both acute (20% higher sensitivity; P < 0.00001) and chronic (40% higher sensitivity; P = 0.087) forms of brucellosis. The major advantage of lysis centrifugation was in the mean detection time, which was only 2.4 days in acute and 2.7 days in chronic cases, with 103 out of 110 (93.6%) and 17 out of 20 (85%) cultures from acute and chronic brucellosis, respectively

Detection of Shigellae from stools of dysentery patients by culture and polymerase chain reaction techniques.

In Bangladesh, the isolation rates of Shigella spp. range from 11% to 12% by the conventional culture technique. Since the sensitivity of this technique is low, the polymerase chain reaction (PCR) technique was used for detecting small number of Shigellae from patients' stools. Sensitivity and specificity of the two techniques were also compared. Stool samples were collected from 41 patients with dysentery who attended the Clinical Research and Service Centre of the ICDDR,B: Centre for Health and Population Research. All stool specimens were directly plated onto MacConkey, Salmonella-Shigella,

Comparison of three non-culture techniques for detection of Chlamydia trachomatis in genital tract specimens.

A direct immunofluorescence (DIF) technique (Imagen) and two enzyme immunoassay (EIA) techniques (Chlamydiazyme and IDEIA) were compared for the detection of Chlamydia trachomatis in genital specimens from 502 attenders at a genitourinary medicine clinic. Eighty-two attenders were regarded as infected: 67 with positive results by at least two of the three techniques and 15 by virtue of elementary bodies detected in stored EIA buffer samples. With a positivity criterion of greater than or equal to 6 bodies Imagen was 76% sensitive for men and 61% sensitive for women. The sensitivity of Chlamydi

Comparison of paraffin baiting and conventional culture techniques for isolation of Nocardia asteroides from sputum.

Of 1,510 sputum samples examined from 1,016 patients with bronchopulmonary disorders, Nocardia asteroides was isolated from 67 samples by paraffin baiting, as compared with only 30 isolations by the conventional technique of culturing on Sabouraud dextrose agar. This higher efficacy of the paraffin bait technique was found to be statistically significant (P less than 0.001).

A man with culture sterile cavitating lung lesions: molecular techniques can provide the key to diagnosis.

A 20-year-old male presented to the Emergency Department with pyrexia, dyspnoea, chest pain and haemoptysis. Cavitating lung lesions were noted on chest X-ray and the patient was admitted to the intensive care unit where he was intubated and ventilated. Routine investigations including serial cultures did not provide an aetiological diagnosis. As such, a CT-guided lung biopsy was carried out and 16S rDNA PCR was undertaken on the sample. This identified Fusobacterium necrophorum as the causative organism. The patient was treated for Lemierre's syndrome and successfully discharged from hospital
