Cunninghamella bertholletiae is a rare cause of pulmonary mucormycosis. We describe a cluster of invasive pulmonary infections caused by C. bertholletiae in 4 immunocompromised patients that occurred during a 2-year period at 1 center. Three of the patients were receiving antifungal prophylaxis with itraconazole. Presenting symptoms were fever unresponsive to antibacterial chemotherapy, hemoptysis, and infiltrates on chest radiograms. Three patients were treated with liposomal amphotericin B. Only 1 patient survived.

Nonsteroidal anti-inflammatory drugs (NSAIDs) such as diclofenac (DCF), ibuprofen (IBU), and ketoprofen (KET) are commonly found in the environment and pose potential toxicological risks. This study investigates the microbial biotransformation of these NSAIDs by Cunninghamella spp. and evaluates the toxicity of the Cunninghamella-processed samples. The results demonstrate that Cunninghamella efficiently biotransformed DCF and IBU into multiple metabolites, whereas the biotransformation of KET was negligible. Microbial treatment reduced the ecotoxicity of IBU and DCF, as confirmed by Microtox®

The aim of this work is to evaluate the chemical constituents and potential biological activists of Cunninghamella blakesleeana. Three fatty acids were isolated using column chromatography and identified as palmitic acid (F1), oleic acid (F2) and stearic acid (F3) in addition to other two steroidal compounds; α-amyrin (A4), and β-sitosterol (A5). Using GC, ten fatty acids were detected the major fatty acid obtained was stearic acid (74.61%) while palmitic acid was the second high percentage (10.35%), and the least percentage obtained was arachidic acid (0.07%). Pulmonary mucormycosis occur in immunosuppressed patients with a neutropenic treatment or a malignant neoplasia. Although the histological study alone cannot enable species identification, the pathologist usually makes the diagnosis of mucormycosis and can eliminate other filamentous mycoses such as aspergillosis or scedosporiosis. Pulmonary infection caused by Cunninghamella bertholletiae is a very rare lung mucormycosis, particularly in young patients. The histological features of this filamentous mycosis are unusual making the diagnosis of mucormycosis difficult. Mycological study is then c

A 74-year-old woman was admitted for further examination because of an abnormal nodular shadow and thickening of the bronchovascular bundle in the left upper lobe on chest radiography and CT. The findings from a transbronchial biopsy specimen were not conclusive. A thoracoscopic lung biopsy specimen revealed a fungal infection, and partial resection of S1 + 2 of the left upper lobe was carried out. In the dilated bronchus, mucus of high viscosity was seen. Lymphocytes and plasma cells had infiltrated into the bronchioles and alveoli. Fungus was cultured from the mucus, and the morphological ch

Ethisterone (17α-ethynyl-17β-hydroxyandrost-4-en-3-one) (1) is a synthetic steroidal estrogen. It is extensively used as an oral contraceptive. The current study involves the structural transformation of ethisterone (1) by Aspergillus niger, and Cunninghamella blakesleeana. Fermentation of 1 with C. blakesleeana afforded two new polar metabolites, 17α-ethynyl-6β,15β,17β-trihydroxyandrost-4-en-3-one, and 17α-ethynyl-7β,15β,17β-trihydroxyandrost-4-en-3-one, while transformation of ethisterone with A. niger yielded a new metabolite, 17α-ethynyl-6α,17β-dihydroxyandrost-4-en-3-one, along with a kno

A 22-year-old Japanese woman was diagnosed with hemophagocytic lymphohistiocytosis and subsequently was treated with etoposide and cyclophosphamide. On Day 22, multiple nodular lesions appeared in the bilateral lungs. Neither the administered antibiotics nor the antifungal agent were effective, and she died suddenly of respiratory failure on Day 35. An autopsy revealed disseminated zygomycosis and a pulmonary infarction due to the embolization of an angioinvasive fungus, which was later identified as Cunninghamella bertholletiae using in situ hybridization of 18S rRNA. C. bertholletiae is aggr

Zygomycosis is an invasive mycotic disease caused by fungi of the class Zygomycetes. Infections caused by zygomycetes, are known for their difficulty of diagnosis and treatment. Cunninghamella bertholletiae (Cb), is a saprobic fungus commonly found in the soil of temperate climates. Pulmonary infections caused by Cb are being identified with increasing frequency among patients on immunosuppressive therapy, and these infections usually have a fatal outcome. We present a rare case of pulmonary zygomycosis caused by Cb in a 10 year old male child with acute lymphoblastic leukemia. In spite of int

Cunninghamella elegans was cultured from autopsy materials of the infected lung of a patient with chronic myelogenous leukemia. The histologic sections revealed extensive growth of nonseptate, broad hyphae in and about the large and small arteries and veins, occluding the vessels. This case is believed to be the second in which human infection by C. elegans has been documented.

Cunninghamella bertholletiae, a rare causative agent of mycosis among immunocompromised patients has been reported with increased frequency from western countries in recent years. We report a case of C. bertholletiae pulmonary infection in a 42-year-old male patient who developed graft-vs.-host disease following bone marrow transplantation. In spite of intensive antifungal chemotherapy following clinical diagnosis, he died of pulmonary failure. To our knowledge this is the first report from India and third report of post-BMT C. bertholletiae infection ever published.

Pediatric infections caused by Cunninghamella bertholletiae (C. bertholletiae) are exceedingly rare with only few published case reports. Herein, we describe five cases of C. bertholletiae diagnosed and treated in Pediatric Hematology-Oncology Patients. These cases indicated that pediatric C. bertholletiae infection presents with non-specific clinical features, is associated with high fatality, and frequently involves multiple anatomical sites, most commonly the lungs, demonstrating epidemiological patterns distinct from those observed in adults. From a diagnostic perspective, mNGS enables rap

Onychomycosis is a fungal infection of nails seen frequently in immune competent and immune compromised patients due to dermatophytes, Candida spp., Fusarium spp., Scopulariopsis brevicaulis, Penicillium spp., and Aspergillus spp. We report a case of onychomycosis in a young immunocompetent male who presented onycholysis of a solitary nail without inflammation. The etiological agent was diagnosed to be Cunninghamella bertholletiae, a fungus pertaining to the order Mucorales (subdivision Mucoromycotina) and known for some of the invasive lesions among immunocompromised patients. This case demon

Mucormycosis is an uncommon fungal infection, which generally develops in immunosuppressed hosts. In particular, pulmonary infection by Cunninghamella bertholletiae, a rare species of Mucor, is characterized by invasiveness and high mortality. Herein a case of pulmonary mucormycosis due to C. bertholletiae in a female patient with chronic renal insufficiency, secondary to microscopic polyarteritis, is reported. The patient survived after successful treatment with a cumulative dose of 1508 mg of amphotericin B, phased reduction of glucocorticoid therapy and chest tube drainage of a pneumothorax

Two strains of the filamentous fungus Cunninghamella elegans (ATCC 9245 and ATCC 36112) were grown in Sabouraud dextrose broth and screened for the ability to metabolize the ethanolamine-type antihistamine diphenhydramine. Based on the amount of parent drug recovered after 7 days incubation, both C. elegans strains metabolized approximately 74% of the diphenhydramine, 58% of this being identified as organic extractable metabolites. The organic extractable metabolites were isolated by reversed-phase high-performance liquid chromatography and identified by analyzing their mass and nuclear magnet

Cunninghamella bertholletiae is a saprophytic fungus found in soil. Infection with this organism is extremely rare, occurring almost exclusively in immunosuppressed hosts. There have been only three previous cases of infection with this fungus reported in solid-organ recipients. We report an unusual case of disseminated Cunninghamella infection in a woman who had received a renal transplant. A 48-year-old woman received a living-related kidney transplant for focal segmental glomerulonephritis. She was treated with plasmapheresis and muromonab-CD3 (OKT3) for two episodes of rejection. Candida species are responsible for causing invasive candidiasis with high mortality rate and their resistance to available antifungal drugs is a major clinical challenge. Biotransformation process of the labdane diterpene ent-labd-8(17)-en-15,18-dioic acid (1) carried out with Cunninghamella elegans afforded five new derivatives (compounds 2-6). Unusual regioselective hydroxylation of the methyl group at the C-20 position of labdane-type diterpene was achieved and all compounds were subjected to cytotoxicity and antifungal evaluations. Compound 1 and its derivatives were not cytotoxic to norm
