A Case of Pediatric Myasthenia Revealed by Marked Eyelid Ptosis After Using Cyclopentolate Eye Drops.

Pediatric myasthenia gravis (PMG), while uncommon, poses significant diagnostic challenges due to its potential impact on development and its often subtle initial symptoms, such as ptosis and diplopia. This case report details an unusual presentation of PMG in a three-year-old female, initially diagnosed with a chalazion, highlighting the complexity of diagnosing this autoimmune neuromuscular disorder in children. Despite treatment for the assumed chalazion, the child's ptosis persisted, and disparities in visual acuity between her eyes became evident. The turning point in her diagnosis came d

Possible allergic reactions to cyclopentolate hydrochloride: case reports with literature review of uses and adverse reactions.

Cyclopentolate has been widely used as a cycloplegic and mydriatic agent for over 30 years. It has gained widespread use as the cycloplegic drug of first choice for most children over the age of 1 year and allows many optometrists and ophthalmologists to carry out quick, successful cycloplegic refractions with few complications. During this time very few side-effects have been reported with the most commonly used 1% solution. This paper outlines two cases in which a possible allergic-type reaction occurred shortly after the instillation of 1% cyclopentolate hydrochloride in 'Minims' form (Smit

Spectroscopic quantification of cyclopentolate using an erythrosine-based resonance rayleigh scattering strategy: application to ophthalmic formulations.

Cyclopentolate (CYP), an antimuscarinic (anticholinergic) agent, is pharmacologically employed in ophthalmology to induce cycloplegia (temporary paralysis of the ciliary muscle) and mydriasis (pupil dilation) for diagnostic and therapeutic procedures, including ocular examinations and surgeries. This study establishes a spectrofluorimetric method for quantifying CYP in ophthalmic solutions based on Resonance Rayleigh Scattering (RRS) signal modulation. The approach exploits the formation of an ion-associate complex between CYP and the dye erythrosine, which induces a measurable RRS enhancement

A Clinical Randomized Trial Comparing the Cycloplegic Effect of Cyclopentolate Drops Applied to Closed Eyelids Versus Open Eyelids.

BACKGROUND AND PURPOSE: Adequate cycloplegia and dilation are required for refraction and fundus exam in children. Standard practice is to instill cycloplegic drops in the inferior cul-de-sac, and this is often traumatic for children. Our study assesses the use of cyclopentolate on closed lids as a method of instillation for ensuring complete cycloplegia. PATIENTS AND METHOD: Ninety children presenting for annual refraction were enrolled. Three were excluded as they did not finish the testing. One drop of Alcaine® and one drop of cyclopentolate HCL 1% were used in each eye. Cyclopentolate dro

A cross-over study of the cycloplegic effects of a single topical application of cyclopentolate-phenylephrine and routine atropinization for 3.5 days.

Static refraction in children after the topical instillation of atropine twice a day during 3.5 days was compared with the refraction after a single instillation of one drop of 0.85% cyclopentolate + 1.5% phenylephrine in a randomized cross-over study. Atropine eye drops were applied by the parents at home, while the combination drop was applied by personnel at the eye clinic. Refraction was determined by retinoscopy in 40 children (3-6 years) in a single blind manner. No statistically significant difference in cycloplegic effect was found between the 2 methods. The results imply that in clini

Comparison of cycloplegia at 20- and 30-minutes following proxymetacaine and cyclopentolate instillation in white 12-13-year-olds.

CLINICAL RELEVANCE: Reducing the time between drop instillation and refraction reduces the time paediatric patients and young adults spend in practice, facilitating more eye examinations daily. BACKGROUND: The current procedure for paediatric cycloplegic refraction is to wait for at least 30-minutes post-instillation of a cycloplegic before measuring spherical equivalent refraction. This study compared cycloplegia at 20- and 30-minutes following 0.5% proxymetacaine and 1.0% cyclopentolate in 12-13-year-olds. METHODS: Participants were 99 white 12-13-year-olds.
